Data and HIT systems in primary care settings: an analysis of perceptions and use

PurposeElectronic Health Records (EHRs) and other Health Information Technologies (HITs) pose significant challenges for clinicians, administrators and managers in the field of primary care. While there is an abundance of literature on the challenges of HIT systems in primary care, there are also practices where HITs are well-integrated and useful for care delivery. This study aims to (1) understand how exemplary primary care practices conceptualized data and HIT system use in their care delivery and (2) describe components that support and promote data and HIT system use in care delivery.Design/methodology/approachThis paper is a sub-analysis of a larger qualitative data set on exemplary primary care in which data was collected using in-depth interviews, observations, field notes and primary source documents from week-long site visits at each organization. Using a combination of qualitative analysis methods including elements of thematic analysis, discourse analysis, and qualitative comparison analysis, we examined HIT-related data across six exemplary primary care organizations.FindingsThree key components were identified that underlie engagement with data and HIT systems: data audience identification, defined data purpose and structures for participation in both data design and maintenance.Originality/valueWithin the context of primary care, these findings have implications for effective integration of HIT systems into primary care delivery.

Purpose The purpose of this paper is to acknowledge the value of joint educational entrepreneurship programs: universities impact on economic growth by building collaborative networks in order to encourage innovation through interdisciplinary training schemes. The case of the master’s degree in Entrepreneurial Innovation Management, set up jointly by the University of Salerno and the Universidad Católica de Pereira, is presented. Design/methodology/approach In order to conduct a competitive market analysis, the latest data on master’s degrees in economics and information technologies management have been collected by means of a scraping procedure in order to build a data set for analysis. The authors have considered the masters’ degrees offered in Bogotá, where most universities of Colombia are located. Findings The data point out that current master’s degree courses do not recognize the importance of interdisciplinary training, which is in great demand in the world of work: economics and computer science never run together and rarely do universities collaborate within a network to set up joint programs. Practical implications The entrepreneurial culture could yield economic and social benefits by training students for a dynamic, global and increasingly digital job market. The case study represents a first step in building a network, which could be extended to other countries in the future. Originality/value The originality of the study lies in the proposal of a joint Italy–Colombia master’s degree, which is set up within a higher education network and may prove useful in creating job opportunities in both countries involved. Moreover, the learning path balances two traditionally separated disciplinary fields: economics and computer engineering.

Abstract Background There is a compelling rationale that effective communication between hospital allied health professionals and primary care practitioners could improve quality and continuity of patient care. It is not known which methods of communication are used, nor how effectively they facilitate the transition of care when a patient is discharged home from hospital. Our systematic review aims to investigate the methods and effectiveness of communication between hospital allied health professionals and primary care practitioners. Method Systematic review of quantitative and qualitative studies with narrative synthesis. Medline, CINAHL, EMBASE, PsycInfo and Proquest Nursing and Allied Health Sources were searched from January 2003 until January 2020 for studies that examined hospital allied health professionals communicating with primary care practitioners. Risk of bias in the different study designs were appraised using recognised tools and a content analysis conducted of the methodologies used. Results From the located 12,281 papers (duplicates removed), 24 studies met the inclusion criteria with hospital allied health professionals communicating in some form with primary care practitioners. There was, however, limited literature investigating the methods and/or the effectiveness of communication between hospital allied health professionals and primary care practitioners.Conclusion There is currently no 'gold standard' method or measure of communication between hospital allied health professionals and primary care practitioners. There is an urgent need to develop and evaluate multidisciplinary communication with enhanced health information technologies to improve collaboration across healthcare settings and facilitate continuity of integrated people-centred care.Registration: www.crd.york.uk PROSPERO CRD42019120410

Purpose The access to the rare originals of the early Spanish colonial imprints of the Philippines remains problematic. The reference librarians often are restricted to directing the students and scholars to the secondary resources that are available both in print and as a part of the digital assets within the North American academic libraries. This paper aims to focus on the select primary source editions including select Spanish language colonial imprints that are available electronically on the Web along the Open Access. These Web-based resources serve as the reference tools for the early history of the Philippines and Southeast Asia. As many of these publications are rare and extremely expensive for most libraries, the Open Access resources serve as an aid to building a virtual collection of these items. Design/methodology/approach The author had to create a data set of the early imprints of the Spanish Philippines using several bibliographic resources. The data set will be submitted as an Appendix for this research paper. The author did both qualitative and quantitative analysis of the data set along with the voyant-based digital humanities approach for topic modeling. Findings The goals of this paper were to not only survey the early Spanish printing of the Philippines but also provide the reader with a somewhat complete picture of how the printing began in the Spanish Philippines, what kind of the first books were printed and how one can access them given their rarity and fragility. The collection building paradigms are undergoing significant shifts, and the focus of many academic libraries is shifting toward providing access to these items. As these items high-value low-use items continue to be part of the Special Collections, the access to these is problematic. The virtual collections thus serve as a viable alternative that enables further research and access. While the creators of these works are long gone, the legacy of the Spanish colonial domination, printing and the religious orders in the Philippines remain alive through these works. Research limitations/implications As this is an introductory paper, the author focused on the critical editions rather than providing a comprehensive bibliographic landscape of the presses that produced these editions. He also did not take into consideration many pamphlets that were published in the same period. He also did not consider the Chinese language publications of the Islands. The Chinese had been block printing since medieval times (Little, 1996). In the context of the Spanish Philippines, the Chinese migration and trade have been studied in detail by Chia (2006), Bjork (1998) and Gebhardt (2017). The scope of this paper also was centered toward building a virtual collection of these rare books. Practical implications Rare books are often expensive and out-of-reach for many libraries; the virtual collection of the same along the Open Access model represents an alternative to collect and curate these collections. The stewardship of these collections also acquires a new meaning in the digital milieu. Social implications This research paper will allow scholars to see past the analog editions and help them focus on curating a virtual collection. The questions of electronic access are often ignored when it comes to visiting and using them in a controlled environment of the reading room in the Special Collections. The author argues that one way to enable access to these rare and expensive books is to provide access to their digital counterparts. These digital/virtual surrogates of the originals will facilitate further research. Originality/value The author could not find similar research on the publications of the early Spanish colony of the Philippines.

PurposeNeuroscience data are spread across a variety of sources, typically provisioned through ad-hoc and non-standard approaches and formats and often have no connection to the related data sources. These make it difficult for researchers to understand, integrate and reuse brain-related data. The aim of this study is to show that a graph-based approach offers an effective mean for representing, analysing and accessing brain-related data, which is highly interconnected, evolving over time and often needed in combination.Design/methodology/approachThe authors present an approach for organising brain-related data in a graph model. The approach is exemplified in the case of a unique data set of quantitative neuroanatomical data about the murine basal ganglia––a group of nuclei in the brain essential for processing information related to movement. Specifically, the murine basal ganglia data set is modelled as a graph, integrated with relevant data from third-party repositories, published through a Web-based user interface and API, analysed from exploratory and confirmatory perspectives using popular graph algorithms to extract new insights.FindingsThe evaluation of the graph model and the results of the graph data analysis and usability study of the user interface suggest that graph-based data management in the neuroscience domain is a promising approach, since it enables integration of various disparate data sources and improves understanding and usability of data.Originality/valueThe study provides a practical and generic approach for representing, integrating, analysing and provisioning brain-related data and a set of software tools to support the proposed approach.

Purpose Globally, in 1980, approximately 5.8 per cent of the world population was 65 years old and older. By 2050, this number will more than triple to 16 per cent. From a leadership perspective, there is at least one challenge (among many others challenges) to consider. This paper (viewpoint) aims to provide support for the growing need for academically prepared managers. Design/methodology/approach This paper is a viewpoint which presents several characteristics of the long-term care (LTC) field that support the need for academically trained leaders. Findings LTC leaders in all countries must be sufficiently versed in numerous management areas to provide leadership when called on by those assigned to their care. Given local area variations in population needs present across all countries, it may be unwise to advocate for national, countrywide standardization of requirements. Yet, older adults accessing LTC services should expect a minimum level of knowledge from all of their providers – not just those who provide direct, hands-on care. However, similar to those who provide direct care, leaders should receive competency-based education with specific attention to effective communication skills, team-based approaches to care delivery, information technologies and population health. Originality/value Although much of the extant literature focuses on the delivery of care to older persons, there is a dearth of literature addressing the role of LTC leaders in light of global aging. Establishing a minimum level of academic training and increasing transparency focused on the positive experiences of elders residing in LTC facilities should help dispel the notion that placement in an LTC facility reflects filial failure.

Health information technologies (HITs) have become increasingly used in the NHS and offer prescribers the opportunity to prescribe in a more consistent and reliable way. There is a growing use of electronic prescribing systems, especially in primary care. This will likely reduce prescription errors, but evidence is unclear if it will improve patient outcomes. Clinical decision support systems can reduce variability and alert clinicians when prescriptions could cause patients harm; however, automation bias can create new errors to prescribers who over-rely on the system. HITs can better communication by improving discharge letters, facilitating telehealth appointments and supporting those working in remote settings. Mobile apps offer a way to engage patients in their own care and allow remote monitoring of chronic conditions in primary care, and acute conditions in emergency care settings. There are challenges in realising these benefits, with inconsistent infrastructure and a 10-year delay in realising predicted efficiency savings.

Abstract Recent advances in biomedical research have generated a large volume of drug-related data. To effectively handle this flood of data, many initiatives have been taken to help researchers make good use of them. As the results of these initiatives, many drug knowledge bases have been constructed. They range from simple ones with specific focuses to comprehensive ones that contain information on almost every aspect of a drug. These curated drug knowledge bases have made significant contributions to the development of efficient and effective health information technologies for better health-care service delivery. Understanding and comparing existing drug knowledge bases and how they are applied in various biomedical studies will help us recognize the state of the art and design better knowledge bases in the future. In addition, researchers can get insights on novel applications of the drug knowledge bases through a review of successful use cases. In this study, we provide a review of existing popular drug knowledge bases and their applications in drug-related studies. We discuss challenges in constructing and using drug knowledge bases as well as future research directions toward a better ecosystem of drug knowledge bases.
