Investigating the cause of chest pain relies much upon history, and in particular a clear description of the nature of the pain itself. An intubated ventilated patient is usually unable to provide a precise description of chest pain and the physician depends more on ancillary aids including a chest X-ray and electrocardiogram (ECG). Echocardiography can add vital information in a rapid and non-invasive manner, either contributing directly to the diagnosis or else directing the physician to the most appropriate investigation for which the patient requires transfer beyond the intensive care unit. Underlying coronary artery lesions leading to myocardial ischaemia, pericardial pathology, and thoracic aorta anomalies usually require prompt attention but other conditions including pathologies of the lung, oesophagus, and chest wall may also be identified.
