Detectability of residual Epiphany and gutta-percha after root canal retreatment using a dental operating microscope and radiographs - an ex vivo study

Objectives: This study aimed to evaluate the efficacy of ProTaper, Mtwo, and WaveOne retreatment files and Hedstrom files for removal of gutta-percha from the straight root canals using cone-beam computed tomography (CBCT). Materials and Methods: Forty freshly extracted single-rooted and single-canal teeth were selected for this study. The teeth were decoronated, and biomechanical preparation was performed up to #30 K-file. The root canals were obturated using lateral compaction technique with gutta-percha and Resilon sealer. The teeth were then randomly divided into 4 groups, and CBCT images were obtained. All the canals were then retreated with either ProTaper retreatment files, Mtwo retreatment files, WaveOne files, or Hedstrom files. The surface area of the remaining filling material after the retreatment procedure was quantified by CBCT. Statistical analysis was performed via one-way ANOVA and the Tukey-Kramer multiple comparisons test. Results: None of the file systems could completely remove the filling material from the canals. Data analysis revealed significant differences between the groups in the apical and middle thirds (P<0.05). Conclusion: All the file systems left some filling material in the canals. Mtwo retreatment files had maximum efficacy for removal of filling materials in comparison with other files. WaveOne files can also be used for root canal retreatment.

The purpose of this ex vivo study was to determine, in "open" and "closed" systems, whether the design has an influence on the penetration length of sodium hypochlorite mixed with a radiopaque contrast medium, measured in millimeters, when delivered using positive pressure (PP) and using sonic (SI) or passive ultrasonic (PUI) activation. Sixty single-rooted teeth were divided into two groups: open and closed systems (n=30). Root canal shaping was performed to a working length of 17 mm. The samples were divided into three sub-groups (n=10) according to irrigant delivery and activation: PP, and SI or PUI activation. By using radiographs, penetration length was measured, and vapor lock was assessed. For the closed group, the penetration distance means were: PP 15.715 (±0.898) mm, SI 16.299 (±0.738) mm and PUI 16.813 (±0.465) mm, with vapor lock occurring in 53.3% of the specimens. In the open group, penetration to 17 mm occurred in 97.6% of the samples, and no vapor lock occurred. Irrigant penetration and distribution evaluation using open and closed systems provide significantly different results. For closed systems, PUI is the most effective in delivering the irrigant to working length, followed by SI.

The aim of this ex vivo was to investigate if two radiopaque root canal sealers with different formulations would influence the radiographic perception of root canal fillings. The root canals of 48 extracted maxillary canines were prepared and randomly assigned to 3 groups of 16 specimens each. In each group, the root canals were filled by lateral condensation of gutta-percha and one of the tested sealers: Endométhasone, Sealer 26, or a non-radiopaque sealer. A through-and-through void was simulated in half of the specimens from each group (n=8). The buccolingual radiographic images obtained were randomly interpreted for voids existence by a radiologist and an endodontist. The differences in sensitivity and specificity between groups and examiners were compared using, respectively, Fisher's Exact and McNemar tests at 5% significance level. Both radiopaque sealers caused a significant decrease in sensitivity at the coronal part of fillings. The use of Endométhasone increased specificity values for both coronal and apical portions of the root canal fillings. In conclusion, the tested sealers influenced the radiographic perceptions of laterally condensed root canal fillings in a different way.
