Population, Policy Development and Implementation in Sub-Saharan Africa

Educational psychology in Africa has a rich and colorful history. In sub-Saharan Africa educational psychology, as both a profession and a scientific field, is particularly vibrant. The emergence of educational psychology in sub-Saharan Africa shows how the science and the profession has pirouetted in ways that could support mental health and learning in African contexts in innovative ways. While emanating within Western cultures, educational psychology has been adapted and, perhaps, been deeply enriched in the African context. After the initial establishment of educational psychology in sub-Saharan Africa, three broad eras of theoretical development are evident: (a) the era of ecosystems and community, (b) the era of inclusion, and (c) the era of strength-based and positive approaches. During the era of ecosystems and community, emergent theories challenged the dominance of the individualist paradigms in educational psychology and provided broadened conceptualizations of the factors that impact mental health and effective learning. The role of communities was also given prominence. During the era of inclusion, the medical model was challenged as the primary foundation for legitimizing educational psychological assessments and interventions. Educational psychologists moved toward rights-based approaches that championed the rights of vulnerable populations and the creation of inclusive learning environments. The inclusion of children with disabilities influenced policy development in multiple sub-Saharan countries and expanded the dialogues on how best to support learning for all children. During the era of strength-based and positive approaches, theoretical and pragmatic approaches that forefront strengths, capacities, and possibilities started to develop. This era signified yet another departure from previous hegemonic paradigms in that educational psychology moved beyond the individual level, toward more systemic approaches, but then also used approaches that focused more on strengths and the mobilization of resources within these systems to address challenges and to optimize educational psychological support. These eras in the development of educational psychology in sub-Saharan Africa created optimal opportunities to respond to the United Nation’s Sustainable Development Goals (SDGs). In terms of SDGs, educational psychology responds primarily to Global Goal 3 (health and well-being) and Global Goal 4 (quality education). At the same time it supports the Global Goals of no poverty (1), gender equality (5), decent work and economic growth (8), reduced inequalities (10), sustainable cities and communities (11), and building partnerships for the goals (17).

Africa region remains the continent with the highest total fertility rate among other major regions of the world such as Europe, North America, Asia and Latin America and Oceania. This paper examines the determinants of high fertility in sub-Saharan Africa; it also determines the policy implication for reaping and optimizing demographic dividend. Secondary data sources were employed in achieving the set objectives. This paper submitted that determinants such as age at first marriage; high child mortality; low female education; gender preference; and limited birth spacing were the determinants of high fertility in Africa. For Africa to harness the demographic dividend, certain policy implications such as investment in child survival and health programmes; investment in quantity and quality of education; multi-sectoral approaches and meeting infrastructural development; enhance job market and enact and enforce laws to prevent early marriage among other policy programmes must be embraced. The paper concludes that there is high fertility in sub-Saharan Africa because of the in-built population momentum of the populace. Also, fertility must be reduced significantly if sub-Saharan Africa must reap and optimize the promising dividend. This paper, therefore, recommends that all government in Africa continent should come up with and implement effective population policy that will help to reduce high fertility level.

Background: Stroke is a leading cause of death and disability in sub-Saharan Africa with increasing incidence. In Kenya, it is a neglected condition with a paucity of evidence despite its need for urgent care and hefty economic burden. Therefore, we reviewed studies on stroke epidemiology, care, and outcomes in Kenya to highlight existing evidence and gaps on stroke in Kenya.Methods: We reviewed all published studies on epidemiology, care, and outcomes of stroke in Kenya between 1 January 1990 to 31 December 2020 from PubMed, Web of Science, EBSCOhost, Scopus, and African journal online. We excluded case reports, reviews, and commentaries. We used the Newcastle-Ottawa scale adapted for cross-sectional studies to assess the quality of included studies.Results: Twelve articles were reviewed after excluding 111 duplicates and 94 articles that did not meet the inclusion criteria. Five studies were of low quality, two of medium quality, and five of high quality. All studies were hospital-based and conducted between 2003 and 2017. Of the included studies, six were prospective and five were single-center. Stroke patients in the studies were predominantly female, in their seventh decade with systemic hypertension. The mortality rate ranged from 5 to 27% in-hospital and 23.4 to 26.7% in 1 month.Conclusions: Our study highlights that stroke is a significant problem in Kenya, but current evidence is of low quality and limited in guiding policy development and improving stroke care. There is thus a need for increased investment in hospital- and community-based stroke care and research.

AbstractInformality is a defining characteristic of cities in the global South and most especially across the region of sub-Saharan Africa. Policy responses by governments towards the informal economy impact the livelihoods of informal entrepreneurs. In South Africa the informal economy is a critical source of livelihoods in urban areas. Many participants in the informal economy of South Africa’s major urban centres are international migrants, mostly drawn to the country from other parts of sub-Saharan Africa. The objective in this paper is to examine the challenges faced by international migrant entrepreneurs in relation to policy development for the informal economy of the City of Cape Town. The analysis uses qualitative interviews with key stakeholders, documentary sources and secondary surveys. It is revealed that in Cape Town despite a pro-development rhetoric in the inner city there is evidence of a subtle but systematic exclusion of street traders, including of migrant entrepreneurs. Little evidence exists of a coherent analysis by city policy makers to understand and foreground the contributions made by migrant entrepreneurs for the urban economy.

Chronic kidney disease (CKD) is a global public health problem, seemingly affecting individuals from low-income and-middle-income countries (LMICs) disproportionately, especially in sub-Saharan Africa. Despite the growing evidence pointing to an increasing prevalence of CKD across Africa, there has not been an Africa-wide concerted effort to provide reliable estimates that could adequately inform health services planning and policy development to address the consequences of CKD. Therefore, we established the CKD in Africa (CKD-Africa) Collaboration. To date, the network has curated data from 39 studies conducted in 12 African countries, totalling 35 747 participants, of which most are from sub-Saharan Africa. We are, however, continuously seeking further collaborations with other groups who have suitable data to grow the network. Although many successful research consortia exist, few papers have been published (with none from Africa) detailing the challenges faced and lessons learnt in setting up and managing a research consortium. Drawing on our experience, we describe the steps taken and the key factors required to establish a functional collaborative consortium among researchers in Africa. In addition, we present the challenges we encountered in building our network, how we managed those challenges and the benefit of such a collaboration for Africa. Although the CKD-Africa Collaboration is focused primarily on CKD research, many of the lessons learnt can be applied more widely in public health research in LMICs.

Abstract A common explanation for the high fertility prevailing in sub-Saharan Africa (SSA) is a widespread desire for large families. This situation poses a challenge to population policy-makers in the continent. If the desired family size is high, then presumably family planning programs can only have a limited effect on fertility because these programs aim to assist women in achieving their reproductive goals. But this conclusion is based on the assumption that family planning programs do not affect the desired family size, which is questionable and is investigated here. This study examines the determinants of trends wanted and unwanted fertility in SSA using fixed-effects regressions of country-level data. The dependent variables include the total fertility rate, and its wanted and unwanted components. Explanatory variables include a family planning program score and four socioeconomic variables (women’s educational attainment, child mortality, GNI per capita, and percent urban). Data come from 103 DHS surveys in 25 countries in SSA with at least two DHS surveys between 1989 and 2019. Women’s education and family planning programs are found to be the dominant determinants of fertility decline and their effects operate by reducing both wanted and unwanted fertility. The effects of education are not surprising but the finding that family planning programs can reduce wanted fertility implies that their impact can be larger than conventional wisdom suggests. Indeed, in a few poor countries, the implementation of high-quality programs has been associated with substantial declines in wanted fertility (e.g., Ethiopia, Malawi, Rwanda). The mechanism through which this effect operates is unclear but likely involves media programs that diffuse knowledge about the benefits of smaller families.

Water and land are vital resources essential to ensuring sustainable and productive rural economies. They are also essential for safeguarding food security and socio-economic development. In spite of this, the concept of water-land-food (WLF) security nexus has generally been examined from a top-down manner with women mostly disenfranchised in the access and management of water and land in particular. Concurrently, risks linked with climate crisis aggravate gender inequalities. The limited access to resources, restricted formal rights through top-down management exacerbates the vulnerability of poor rural women. Furthermore, policy development procedures follow a sectoral approach with no account of interrelationship and interdependence between the sectors. This, directly and indirectly limits the stewardship of natural resources and mitigation of the effects of climate change. This study, therefore, examines gender roles and their implications for water, land, and food security in a changing climate through an integrative systematic review of literature in Sub-Saharan Africa. It further explains the importance to consider the nexus in adaptation. Food security and sustainable livelihoods could be ensured if women can freely and adequately access land and participate in decision-making processes.
