Background: Acute hepatitis caused by viral organisms which are typically implicated in the cases of encephalitis are rarely reported in literature. The presentation of enteroviral meningoencephalitis predominated with the clinical picture of hepatitis has rarely been recorded in populations other than neonates. Case Presentation: A male 2-year-old, presenting with a fever of 38 to 40 °C with chills and rigors, lethargy and drowsiness for 6 days. Along with the nausea, vomiting and watery diarrhea, he developed mouth ulcers (peri-oral vesicles) during the stay at the hospital. After extensive workup he was later diagnosed with meningoencephalitis and acute hepatitis due to Coxsackie virus A. After a stay of more than a month he was successfully treated and discharge. Conclusion: A more thorough evaluation should be carried out for atypical viral infections presenting with clinical presentation of hepatitis and meningoencephalitis.

Toxic thyroid adenoma is the third leading cause of hyperthyroidism (following Graves-Basedow disease and toxic multinodular goiter). It consists in the presence of a single nodule that autonomously produces thyroid hormones, causing – in time – outlining of characteristic clinical picture of hyperthyroidism. Paraclinical diagnosis involves thyroid hormones assesment and thyroid ultrasound and scintigraphy that show characteristic changes. There are several treatment options and, with correct management, the prognosis is very good.

Introduction: Whipple’s disease is a chronic systemic infectious disorder with Tropheryma whipplei as an etiologic agent, occurring rarely and affecting numerous organs and systems. The variety of symptoms and a non-typical course make it difficult to establish a proper diagnosis. Purpose: In this study, etiopathogenesis, diagnostics and treatment of Whipple’s disease were presented based on the case report of 60-year-old man diagnosed with Whipple’s disease. Case presentation: Persistent diarrhoea with weight loss, lymphadenopathy in the abdominal cavity and moderate microcytic anemia predominated in the clinical picture. Diagnosis was put based on the clinical picture and macroscopic assessment of the small intestine and the presence of macrophages filled with a PAS-positive substance in the lamina propria. To deepen diagnostics, samples collected were assessed showing macrophages with the damaged mucosa, containing numerous elongated micro-organisms whose ultrastructure corresponded to Tropheryma whipplei. The patient’s clinical conditions improved after antibiotic therapy. Conclusions: It is vital to remember about Whipple’s disease in patients with chronic diseases due to a non-specific clinical picture and difficulties in establishing a proper diagnosis. When the disease is diagnosed unequivocally, proper and effective antibiotic therapy should be instituted immediately.

The authors describe the case of a patient treated with several cycles of chemotherapy due to an advanced stage non-Hodgkin lymphoma. One daafter the last cycle, he was admitted to our Intensive Care Unit with a septic shock-like clinical picture which didn’t respond to the aggressive treatment and the patient died a few hours later. The autoptical findings cast some doubts on the diagnosis, and demonstrated the presence of other factors imitating its symptoms. In this article, the mimickers of septic shock are reviewed and discussed, as some of them require an aggressive immunosuppression instead of the recommended treatment for septic shock.

Abstract Introduction : Covid-19 infection has particularly affected older adults. Clinical observations in this population highlight major respiratory impairment associated with development or aggravation of frailty state.Case Presentation : Mr P is a 93 years old frail patient, hospitalized after a COVID-19 infection. The assessment process of this patient has been supported by an innovative multi-systemic tool developed in view of the COVID-19 clinical consequences and a systemic evaluation of motor functions by the Frail BESTest. This process allowed presenting a mixed clinical picture associating an important respiratory distress (linked with the acute respiratory distress syndrome) and an evident motor frailty. The care plan was developed accordingly, and four assessment were done in the same manner until the return home of Mr PConclusion : This case report allows seeing holistically a COVID-19 clinical picture, showing the different axis of clinical reasoning to enhance the rehabilitation process. Furthermore, this case report illustrate the importance of rehabilitation in the Covid-19 context.

Abstract Background Vertical transmission of coronavirus disease 2019 (COVID-19) from mother to newborn infant is doubtful, and very little is known about disease severity and neonatal outcome. Case presentation We present a preterm Iranian infant born to a Persian mother with severe COVID-19 pneumonia. The mother underwent cesarean delivery, and amniotic fluid yielded a positive result for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) by real-time reverse transcription polymerase chain reaction (RT-PCR). The newborn infant showed early-onset infection with SARS-CoV-2 confirmed on pharyngeal swabs by RT-PCR assay within 24 hours after birth, suggesting vertical transmission. Unfortunately, the mother died 14 days after delivery. We describe the clinical course and outcome of the infant up to 7 months of age. Conclusion COVID-19 infection in pregnant women may increase maternal morbidity, mortality and possibly vertical transmission in severe cases. However, it does not seem to progress to serious early or late neonatal complications.

A case of hereditary nonspherocytic hemolytic disease is reported. The patient presented the clinical picture of hemolytic disease of the newborn infant at birth; however, laboratory studies confirmed the correct diagnosis. Family study revealed the mother to have a similar but milder hemolytic anemia. Splenectomy in the patient at 10 months of age was not beneficial. Differential diagnosis is briefly discussed. The nature of the red cell defect responsible for the condition is unknown. This study points out that hereditary nonspherocytic hemolytic anemia should be considered in patients with the clinical picture of erythroblastosis fetalis who fail to have the expected laboratory findings of that disease.
