The Effects of Physical Therapists’ Guidance on Improvement in a Strength-Training Program for the Frail Elderly

Elderly participants experiencing difficulty in chair rising and with a maximum knee-extensor torque below 87.5 N · m were randomized to different versions of a strength-training program for the knee-extensors: to a high-guidance group (HG; two group sessions supervised by a physical therapist and one unsupervised home session per week, n = 17), a medium-guidance group (MG; one supervised group session and two unsupervised home sessions per week, n = 16), or a control group (C; no exercise, n = 16). Maximal isometric knee strength increased more in HG than in C (p = .03) and with increasing guidance (p = .03). The effect was mainly the result of participants with low initial strength. Walking speed increased more for HG than for C (p = .02) and than for MG (p = .06). No statistically significant improvements were seen on other functional tests. In summary, the study shows a trend toward better results with more supervision, but more and larger studies are needed to confirm this.

This study’s purpose was to investigate whether a 10-week low-intensity strength-training program could improve strength of the knee extensors and functional ability. Participants 65 years and older with low knee-extensor muscle strength were randomized into an exercise (n = 11) and a control group (n = 10). Knee-extensor strength and functional ability were measured before and after the program and again 6 months later. Knee-extensor strength (Nm) increased by 54% (13% in the control) by the end of the training program (F = 13.02, p = .01), and most of this improvement was still present 6 months later. The program had a beneficial effect on functional tasks, especially the time taken to rise from a chair in combination with a 3-m walk (F = 3.99, p = .03) and self-reported ability related to lower extremity performance (F = 6.97, p = .02). It seems that this program could contribute to improving functional ability in frail older people.

Methods This study was 37 patients who had significant postoperative complications after cardiovascular surgery. Participants were randomly - NMES group, n = 18; control, n = 19. It was not possible to blind the investigator. Analyzed basic clinical data. The dynamometry of the muscles upper and lower extremities was carried out, as well as a 6-minute walk test (6MWT). Also, the thickness of the quadriceps was measured using ultrasound.The patients underwent NMES on the quadriceps femoris muscle, daily from the third postoperative day, until discharge. The duration session was 90 minutes. Outcomes No differences were found in the baseline characteristics of the groups, including the results of laboratory and instrumental studies. Groups were comparable in the surgery and perioperative parameteres. The initial strength indicators also had no significant differences in the groups. At discharge knee extensors strength (KES) was significantly higher in the NMES group. The knee flexor strength (KFS) and handgrip strength (HF) increased the same in both groups. The quadriceps crosssectional area (CSA) muscle increased more in the NEMS group than in the control to the time of discharge. Average KES increased to a greater extent in the NMES group. At the same time, average and maximum KFS increased equally in both groups. A 6MWT before discharge did not show a difference between groups (P=.166). The NMES course did not affect the duration of hospitalisation (P=.429). Discussion This pilot study show beneficial effects of NMES on muscle strength in patients with complications after cardiovascular surgery. Purpose: The intensity of exercise-induced pain (EIP) reflects the metabolic environment in the exercising muscle, so during endurance exercise this may inform the intelligent regulation of work rate. Conversely, the acute debilitating effects of EIP on motor unit recruitment could impair the estimation of force produced by the muscle and impair judgement of current exercise intensity. This study investigated whether muscle pain that feels like EIP, administered via intramuscular injection of hypertonic saline, interferes with the ability to accurately reproduce torque in a muscle group relevant to locomotive exercise. Methods: On separate days, fourteen participants completed an isometric torque reproduction task of the knee extensors. Participants were required to produce torque at 15 and 20% maximal voluntary torque (MVIT), without visual feedback before (Baseline), during (Pain/No Pain), and after (Recovery) an injection of 0.9% isotonic saline (Control) or 5.8% hypertonic saline (Experimental) into the vastus lateralis of the right leg. Results: An elevated reported intensity of pain, and a significantly increased variance in mean contraction torque at both 15% (P=0.049) and 20% (P=0.002) MVIT was observed in the Experimental compared to the Control condition. Both 15 and 20% target torques were performed at a similar pain intensity in the Experimental condition (15% MVIT, 4.2 ± 1.9; 20% MVIT, 4.5 ± 2.2; P>0.05). Conclusion: These findings demonstrate that the increased muscle pain from the injection of hypertonic saline impeded accurate reproduction of knee extensor torque. These findings have implications for the detrimental impact of EIP on exercise regulation and endurance performance.

AbstractThis study investigated the effectiveness of a specialized strength training program on maximal handgrip strength in young wrestlers. 72 young wrestlers (36 children: 8–10 years-old and 36 adolescents:13–15 years-old) participated in the present study. Both age-categories were assigned into a training group (18 children and 18 adolescents) and a control group (18 children and 18 adolescents). The training groups, in conjunction with the wrestling training performed a 4-month (2 sessions/week) specialized handgrip training program. Maximal handgrip strength was evaluated pre, at the intermediate (2 months) and at the completion of the program (4 months). Maximal handgrip strength values increased during the intermediate and post-training measurements compared to pre-training measurement in training and control groups (p<0.001). No significant differences were observed on pre-training and intermediate measurements between groups, while significant differences were observed during the post-training measurement. Training group exhibited significantly (p<0.01) greater maximal handgrip strength values than the control group irrespective of age-category and hand-preference. A 4-month handgrip strength training program, incorporated into the conventional wrestling training, provokes greater adaptations in maximal handgrip strength than the wrestling training per se. For greater handgrip training adaptations are required more than 14 specialized handgrip training-sessions.

Falls are one of the major problems for elderly people and proprioceptive exercises have been suggested as an alternative in rehabilitation and preventive programs. The purpose of this study was to investigate the influence of a proprioceptive neuromuscular facilitation (PNF) exercise program on balance, knee extension and flexion isometric torque, and knee extension rate of force development (RFD). Fourteen older faller subjects (>60 years) were equally assigned into two groups: a control group (CG: n=7) and a training group (TG: n=7). The PNF training program was performed for 10 weeks on TG, with a frequency of three times per week. Patients were assessed before and after the PNF program, with respect to balance (Berg Balance Scale score—BBS), knee maximal isometric extension and flexion torque, knee extensor RFD, and knee extensors and flexors neuromuscular activation level and coactivation level around the knee. After 10 weeks, balance (P<0.001) and knee extension torque (P=0.05) were improved in TG while no differences were found for CG. These improvements were mainly attributed to central nervous system adaptations, since no differences were found for neuromuscular activation level and coactivation.

Background: Resistance training studies in multiple sclerosis (MS) often use short intervention periods. Furthermore, training efficiency could be optimized by unilateral training and/or electrical stimulation. Objective: To examine the effect(s) of unilateral long-term (20 weeks) standardized resistance training with and without simultaneous electro-stimulation on leg muscle strength and overall functional mobility. Methods: A randomized controlled trial involving 36 persons with MS. At baseline (PRE) and after 10 (MID) and 20 (POST) weeks of standardized (ACSM) light to moderately intense unilateral leg resistance training (RESO, n = 11) only or resistance training with simultaneous electro-stimulation (RESE, n = 11, 100 Hz, biphasic symmetrical wave, 400 µs), maximal isometric strength of the knee extensors and flexors (45°, 90° knee angle) and dynamic (60–180°/s) knee-extensor strength was measured and compared with a control group (CON, n = 14). Functional mobility was evaluated using the Timed Get Up and Go, Timed 25 Foot Walk, Two-Minute Walk Test, Functional Reach and Rivermead Mobility Index. Results: Maximal isometric knee extensor (90°, MID: +10 ± 3%, POST: +10 ± 4%) in RESO and knee flexor (45°, POST: +7 ± 4%; 90°, POST: +9 ± 5%) in RESE strength increased ( p < 0.05) compared with CON but RESO and RESE did not differ. Also, impaired legs responded positively to resistance training (unilateral leg strength analysis) and functional reaching increased significantly in RESO (+18%) compared with CON. Dynamic muscle strength and the remaining functional mobility tests did not change. Conclusion: Long-term light to moderately intense resistance training improves muscle strength in persons with MS but simultaneous electro-stimulation does not further improve training outcome.

The purpose of the study was to determine the effect of bed rest with or without strength training on torque fluctuations and activation strategy of the muscles. Twelve young men participated in a 20-day bed rest study. Subjects were divided into a non-training group (BRCon) and a strength-training group (BRTr). The training comprised dynamic calf-raise and leg-press exercises. Before and after bed rest, subjects performed maximal contractions and steady submaximal isometric contractions of the ankle extensor muscles and of the knee extensor muscles (2.5-10% of maximal torque). Maximal torque decreased for both the ankle extensors (9%, P < 0.05) and knee extensors (16%, P < 0.05) in BRCon but not in BRTr. For the ankle extensors, the coefficient of variation (CV) for torque increased in both groups ( P < 0.05), with a greater amount ( P < 0.05) in BRCon (88%) compared with BRTr (41%). For the knee extensors, an increase in the CV for torque was observed only in BRCon (22%). The increase in the CV for torque in BRCon accompanied the greater changes in electromyogram amplitude of medial gastrocnemius (122%) and vastus lateralis (59%) compared with BRTr ( P < 0.05). The results indicate that fluctuations in torque during submaximal contractions of the extensor muscles in the leg increase after bed rest and that strength training counteracted the decline in performance. The response varied across muscle groups. Alterations in muscle activation may lead to an increase in fluctuations in motor output after bed rest.

Introduction: Fibromyalgia is a disease characterized by chronic pain; it is a syndrome with an unknown cause and has no cure. Objective: Examine the feasibility of a strength training program in patients with FM. Methods: Fortyseven patients from general community were allocated into two groups: strength training group and control group. The patients underwent strength training performed three times per week for eight weeks. We used the Fibromyalgia Impact Questionnaire and Perceived Stress Scale to obtain data. Results: After eight weeks of strength training, there was a significant reduction in pain (p = 0.00) and stress (p = 0.02). No changes were found in the variables analyzed in the control group pre- and post-test. Conclusions: The practice of strength training is a safe and effective alternative for the treatment of fibromyalgia patients.

Introduction: The aim of the study was to compare the impact of a 6-week program combined with football training and strength training on strength and power parameters in highly trained soccer players. It was hypothesized that 6 weeks would be enough for the experimental group to improve their jumping ability and pushing power. Material and methods: The study involved 34 female footballers playing in Ekstraliga (age: 22 ± 5 years; body height: 167 ± 5 cm; body weight: 60 ± 8 kg). The contestants were divided into an experimental group and a control group of 17 people in each group. The control group performed football trainings combined with 2 additional strength units (50% 1RM load) 3 times a week. The experimental group, apart from football trainings, additionally performed 2 strength trainings per week (50–85% 1RM load). Results: The results of the experiment show a significant improvement in the height of the vertical jump and bench press while sitting in the experimental group CMJ (p <0.05; p = 0.000029; ES: 1.00; 11%), CMJA (p <0.05; p = 0 00003; ES = 1.09; 13%), and LP (p <0.05; p = 0.027681; ES = 0.48; 6%), while in the control group the changes in the results after the training program were statistically insignificant CMJ (p> 0.05; p = 0.274000; ES: 0.07, 1%), CMJA (p> 0.05; p = 0.350958 ES = 0.27, 3%), and LP (p> 0.05; p = 0.130153; ES = 0.27; 4%). Conclusion: The presented research results suggest that a short, six-week training program including general football training and strength training performed twice a week with loads gradually increasing during the preparatory period may significantly improve the strength and jumping parameters of the lower limbs in soccer players. Such information can be valuable at the stage of training in the field of special skills training in football.

The effectiveness of a twice-a-week strength training program on children was evaluated in 14 boys and girls (mean age 10.8 yrs) who participated in a biweekly training program for 8 weeks. Each subject performed three sets of 10 to 15 repetitions on five exercises with intensities ranging between 50 and 100% of a given 10-repetition maximum (RM). All subjects were pre- and posttested on the following measures: 10-RM strength, sit and reach flexibility, vertical jump, seated ball put, resting blood pressure, and body composition parameters. The subjects were compared to a similar group of boys and girls (n = 9; mean age 9.9 yrs) who were randomly selected to serve as controls. Following the training period, the experimental group made greater gains in strength (74.3%) as compared to the control group (13.0%) (p < 0.001), and differences in the sum of seven skinfolds were noted (−2.3% vs. +1.7%, respectively, p < 0.05). Training did not significantly affect other variables. These results suggest that participation in a short-term, twice-a-week strength training program can increase the strength and improve the body composition of young boys and girls.
