Anterior Shoulder Laxity Is Not Correlated with Medial Elbow Laxity in High School Baseball Players

Context:Uninjured baseball players have been shown to have increased anterior glenohumeral joint laxity, which may result in adaptive changes at the medial elbow.Objective:To determine the relationship between anterior shoulder laxity and medial elbow laxity in both arms of baseball and nonbaseball high school athletes and compare the laxity of dominant and nondominant shoulders and elbows of high school baseball and nonbaseball players.Design:Cohort design.Setting:Local high schools.Participants:Thirty healthy high school male athletes.Outcome Measures:Anterior shoulder and medial elbow laxity measurements were taken bilaterally with the Ligmaster™.Results:Dominant and nondominant shoulder laxity was significantly greater in the nonbaseball players than the baseball players. No other significant relationships existed.Conclusions:High school baseball players exhibit less anterior shoulder laxity than do nonbaseball players. No relationship exists between anterior shoulder and medial elbow laxity in high school baseball players.

Context:Subacromial impingement is a common injury in baseball players and has been linked to a reduction in the subacromial space. In addition, it has been suggested that decreases in scapular upward rotation will lead to decreases in the subacromial space and ultimately impingement syndrome.Objective:The objective of this study was to evaluate the relationship between acromiohumeral distance and scapular upward rotation in healthy college baseball players.Design:Posttest-only study design.Setting:Controlled laboratory setting.Participants:24 healthy college baseball players.Intervention:Participants were measured for all dependent variables at preseason.Main Outcome Measures:Acromiohumeral distance at rest and 90° of abduction was measured with a diagnostic ultrasound unit. Scapular upward rotation at rest and 90° of abduction was measured with a digital inclinometer.Results:Dominant-arm acromiohumeral distance at rest and 90° of abduction (P = .694, P = .840) was not significantly different than in the nondominant arm. In addition, there was not a significant correlation between acromiohumeral distance and scapular upward rotation at rest and 90° of abduction for either the dominant or the nondominant arm.Conclusions:These results indicate that the acromiohumeral distance is not adapting in the dominant arm in healthy throwing athletes. In addition, a relationship was not identified between acromiohumeral distance and scapular upward rotation, which was previously suggested. These results may suggest that changes that are typically seen in an injured population may be occurring due to the injury and are not preexisting. In addition, scapular upward rotation may not be the only contributing factor to acromiohumeral distance.

The purpose of this study was to examine the mediating role of mindful engagement in the relationship between male high school athletes’ motivational climate perceptions on their teams (i.e., caring, task-, and ego-involving climate) to athlete coachability. Athletes (N = 164, Mage = 15.58 years) from multiple sports completed measures assessing mindful engagement in sport (Cognitive and Affective Mindfulness Scale—Revised), Caring Climate Scale, task- and ego-involving climate perceptions (Perceived Motivational Climate in Sport Questionnaire), and coachability (Athletic Coping Skills Inventory). Initial bivariate correlations linked mindful engagement and coachability positively with perceptions of a caring and task-involving climate and negatively with ego-involving climate perceptions. Structural equation modeling analyses then revealed mindful engagement mediated the relationship between climate and coachability. Encouraging coaches and players to foster a caring/task-involving climate might assist in enhancing athletes’ mindful engagement in sport, which may positively influence the degree to which they are coachable.

Context: Humeral torsion (HT) has been linked to various injuries and benefits. However, the exact interplay between HT, shoulder range of motion (ROM), competition level differences, and injury risk is unclear. Objective: To determine the relationship between HT, ROM, and injury risk in baseball players. Secondarily, to determine HT based on competition level. Data Sources: PubMed, Embase, Web of Science, CINAHL, and Cochrane databases were searched from inception until November 4, 2018. Study Selection: Inclusion criteria consisted of (1) HT measurements and (2) arm injury or shoulder ROM. Study Design: Systematic review. Level of Evidence: Level 3. Data Extraction: Two reviewers recorded patient demographics, competition level, HT, shoulder ROM, and injury data. Results: A total of 32 studies were included. There was no difference between baseball players with shoulder and elbow injuries and noninjured players (side-to-side HT difference: mean difference [MD], 1.75 [95% CI, –1.83 to 2.18]; dominant arm: MD, 0.17 [95% CI, –1.83 to 2.18]). Meta-regression determined that for every 1° increase in shoulder internal rotation (IR), there was a subsequent increase of 0.65° in HT (95% CI, 0.28 to 1.02). HT did not explain external rotation (ER ROM: 0.19 [95% CI, –0.24 to 0.61]) or horizontal adduction (HA ROM: 0.18 [95% CI, –0.46 to 0.82]). There were no differences between HT at the high school, college, or professional levels. Conclusion: No relationship was found between HT and injury risk. However, HT explained 65% of IR ROM but did not explain ER ROM or HA ROM. There were no differences in HT pertaining to competition level. The majority of IR may be nonmodifiable. Treatment to restore and maintain clinical IR may be important, especially in players with naturally greater torsion. HT adaptation may occur prior to high school, which can assist in decisions regarding adolescent baseball participation.

The purpose of this study was to evaluate the effectiveness of repetitions to fatigue (RTF) for estimating one-repetition maximum (1-RM) bench press performance in male high school athletes. Members of high school athletic teams (N = 213, age = 16.3 ± 1.1 yrs, weight = 79.9 ± 16.7 kg) from four states were tested for 1-RM bench press and RTF after completing 4–6 weeks of resistance training. A new equation for use with male high school athletes was developed from a random sample of 180 participants; it appears to have excellent predictive potential (r = 0.96, SEE = 4.5 kg) and cross-validated well on a subsample (n = 33) from this population (r = 0.98, t = 0.64). Therefore, RTF can be used with acceptable accuracy to estimate maximal strength in the majority of adolescent male athletes who need to handle excessively heavy weights.

Objective:To determine the relationship between lumbopelvic (LP) stabilization strength and pelvic motion during running.Design:Runners were assessed for pelvic motion and undertook an LP stabilization strength test.Participants:Sixteen elite male middle- and long-distance runners.interventions:Pelvis kinematics were assessed while subjects ran at 5 m/s on a treadmill.Main Outcome Measures:Angular pelvis displacement was divided into 3 axes of rotation: pelvic tilt, obliquity, and rotation. LP stabilization strength was the capacity to resist increasing static loads applied to each leg and maintain a neutral LP zone. Intercorrelations were calculated for all measures of pelvic motion and LP stabilization strength.Results:There were no significant relationships found among any of the variables (P> .05). However, the LP stabilization strength test possessed good interday reliability.Conclusions:The relationship between pelvic motion and muscle function should be studied under a variety of other conditions.

Context:The Upper Quarter Y-Balance Test (YBT-UQ) is a unique movement test where individuals perform at the limits of their stability, requiring the coordination of balance, proprioception, range of motion, and stabilization. It is not yet clear if performance on the YBT-UQ differs between sports with dissimilar emphasis on upper-extremity performance.Objective:To compare performance on the YBT-UQ between wrestlers, whose sport requires some degree of closed-chain activity, and baseball players, whose sport is primarily open kinetic chain in nature.Design:Cross-sectional.Setting:High school preparticipation physical assessment.Participants:24 healthy high school male wrestlers (mean age 16.12 ± 1.24 y) and 24 healthy high school male baseball players (mean age 15.79 ± 1.25 y).Interventions:All subjects performed the YBT-UQ, which requires reaching in 3 directions while maintaining a push-up position.Main Outcome Measures:The variables of interest include the maximum reach in each direction, as well as the composite score. In addition, asymmetries between limbs for each reach direction were compared.Results:Wrestlers performed significantly better than baseball players in the medial direction, inferolateral direction, and in composite scores. In the medial direction, wrestlers exhibited greater scores (P < .01) on both left and right limbs, 10.5 ± 10.2%LL and 9.95 ± 10.2%LL, respectively. Significant differences (P < .01) were also observed in the inferolateral direction, with a difference of 11.3 ± 12.0%LL on the left and 8.7 ± 11.0%LL on the right. Composite scores were higher (P < .01) for the wrestlers, with a difference of 7.0% on the left and 7.1% on the right.Conclusions:This study suggests that wrestlers perform better on the YBT-UQ than baseball players. The findings may suggest sport-specific normative data for the YBT-UQ in high school athletes.

Context:Chronic ankle instability (CAI) describes the residual symptoms present after repetitive ankle sprains. Current rehabilitation programs in the high school population focus on a multistation approach or general lower-extremity injury-prevention program. Specific rehabilitation techniques for CAI have not been established.Objective:To determine the effectiveness of a 4-wk biomechanical ankle platform system (BAPS) board protocol on the balance of high school athletes with CAI.Design:Randomized control trial.Setting:Athletic training facility.Patients:Twenty-two high school athletes with “giving way” and a history of ankle sprains (ie, CAI) were randomized into a rehabilitation (REH) (166.23 ± 0.93 cm, 67.0 ± 9.47 kg, 16.45 ± 0.93 y) or control (CON) (173.86 ± 8.88 cm, 84.51 ± 21.28 kg, 16.55 ± 1.29 y) group.Interventions:After baseline measures, the REH group completed a progressive BAPS rehabilitation program (3 times/wk for 4 wk), whereas the CON group had no intervention. Each session consisted of 5 trials of clockwise/counterclockwise rotations changing direction every 10 s during each 40-s trial. After 4 wk, baseline measurements were repeated.Main Outcome Measures:Dependent measures included longest time (time-in-balance test), average number of errors (foot lift test), average reach distance (cm) normalized to leg length for each reach direction (Star Excursion Balance Test [SEBT]), and fastest time (side hop test [SHT]).Results:Significant group-by-time interactions were found for TIB (F1,20 = 9.89, P = .005), FLT (F1,20 = 41.18, P < .001), SEBT-anteromedial (F1,20 = 5.34, P = .032), SEBT-medial (F1,20 = 7.51, P = .013), SEBT-posteromedial (F1,20 = 12.84, P = .002), and SHT (F1,20 = 7.50, P = .013). Post hoc testing showed that the REH group improved performance on all measures at posttest, whereas the CON group did not.Conclusion:A 4-wk BAPS rehabilitation protocol improved balance in high school athletes suffering from CAI. These results can allow clinicians to rehabilitate in a focused manner by using 1 rehabilitation tool that allows benefits to be accomplished in a shorter time.

Context:Numerous studies have documented reduced glenohumeral joint range of motion (ROM) of the dominant extremity among baseball players, which may be caused by throwing-related factors that also affect the radioulnar joint.Objective:To measure pronation and supination ROM in competitive baseball pitchers and position players.Participants:Asymptomatic professional and collegiate baseball pitchers (N = 59) and position players (N = 43).Outcome Measures:Bilateral radioulnar pronation and supination ROM.Results:Pitchers demonstrated significantly less pronation and total ROM in the throwing arm than the nondominant arm. The position players demonstrated significantly less total ROM in the throwing arm than the nondominant arm.Conclusions:The reported normative values may provide useful standards for interpretation of radioulnar ROM measured in both asymptomatic and symptomatic baseball players.

Context:Elbow injuries are widely reported among baseball players. The elbow is susceptible to injury when elbow-flexor and -extensor forces are imbalanced during pitching or throwing. Assessment of muscle-strength ratios may prove useful for diagnosing elbow injury.Objective:The purpose of this study was to assess the relationship between the elbow-flexor and -extensor functional isokinetic ratios and elbow injury in baseball players.Design:Retrospective study.Setting:Biomechanics laboratory.Participants:College baseball players with (n = 9) and without (n = 12) self-reported elbow pain or loss of strength were recruited.Intervention and Main Outcome Measures:Trials were conducted using a dynamometer to assess dominant-arm flexor and extensor concentric and eccentric strength at angular velocities of 60° and 240°/s. Functional isokinetic ratios were calculated and compared between groups.Results:Regression analysis revealed that a ratio of biceps concentric to triceps concentric strength greater than 0.76 (the median value) significantly predicted elbow injury (P = .01, odds ratio of injury = 24). No other ratios or variables (including position played) were predictive of injury status.Conclusions:These findings suggest that the ratio of biceps concentric to triceps concentric functional strength strongly predicts elbow-injury status in baseball players. Assessment of this ratio may prove useful in a practical setting for training purposes and both injury diagnosis and rehabilitation.
