Using Solution-Focused Brief Counseling to Provide Injury Support

Athletes at all levels of competition run the risk of becoming injured through their sport participation, and individuals who work closely with athletes during the rehabilitation process (e.g., coaches, athletic trainers, sport psychology consultants) may be called upon to provide mental and emotional support. One approach that seems well suited as a theoretical framework for injury support is solution-focused brief counseling (SFBC). In this paper, athletes’ responses to injury are outlined, and several roles of the consultant are identified. Then the basic tenets of SFBC are discussed, and an example showing how SFBC can be used in an injury support setting is provided.

Context: Few validation studies of sport injury-surveillance systems are available. Objective: To determine the validity of a Web-based system for surveillance of collegiate sport injuries, the Injury Surveillance System (ISS) of the National Collegiate Athletic Association's (NCAA). Design: Validation study comparing NCAA ISS data from 2 fall collegiate sports (men's and women's soccer) with other types of clinical records maintained by certified athletic trainers. Setting: A purposive sample of 15 NCAA colleges and universities that provided NCAA ISS data on both men's and women's soccer for at least 2 years during 2005–2007, stratified by playing division. Patients or Other Participants: A total of 737 men's and women's soccer athletes and 37 athletic trainers at these 15 institutions. Main Outcome Measure(s): The proportion of injuries captured by the NCAA ISS (capture rate) was estimated by comparing NCAA ISS data with the other clinical records on the same athletes maintained by the athletic trainers. We reviewed all athletic injury events resulting from participation in NCAA collegiate sports that resulted in 1 day or more of restricted activity in games or practices and necessitated medical care. A capture-recapture analysis estimated the proportion of injury events captured by the NCAA ISS. Agreement for key data fields was also measured. Results: We analyzed 664 injury events. The NCAA ISS captured 88.3% (95% confidence interval = 85.9%, 90.8%) of all time-lost medical-attention injury events. The proportion of injury events captured by the NCAA ISS was higher in Division I (93.8%) and Division II (89.6%) than in Division III (82.3%) schools. Agreement between the NCAA ISS data and the non–NCAA ISS data was good for the majority of data fields but low for date of full return and days lost from sport participation. Conclusions: The overall capture rate of the NCAA ISS was very good (88%) in men's and women's soccer for this period.

The motivation for youth sport involvement may differ for single-sport (non-specialists), multi-sport, and single-sport specialized athletes. To investigate differences between adolescent single-sport athletes (NSSA), multi-sport athletes (MSA), and single-sport specialized athletes (SSSA) on measures of sport enjoyment and motivation. A secondary aim was to compare these variables between age groups. Adolescent sport participants in 6th, 7th, and 8th grade from the Western United States (n=306, age=13.0±1.0 yrs) completing the Sources of Enjoyment in Youth Sport Questionnaire (SEYSQ) that assesses sport enjoyment in the subscales of self-referenced competency (SRC), other-referenced competency and recognition (ORCR), effort expenditure (EE), competitive excitement (CE), affiliation with peers (AP), and positive parental involvement (PPI). The participants also reported their age, gender, grade, years of sport participation and sport status (NSSA, MSA, SSSA). SSSA reported significantly more enjoyment in all subscales except PPI when compared to NSSA (p<0.05; ES 0.4 – 0.99). MSA and SSSA showed significant differences in the subscales of SRC and EE; however, effect size was small (0.37 and 0.33, respectively). Overall scores for intrinsic and extrinsic motivation were significantly lower among NSSA compared to MSA and SSSA (p<0.01) with no differences between MSA and SSSA (p>0.05). Results revealed no significant differences in the SEYSQ’s subscales for age (p>0.05). Within the parameters of this study, adolescents that specialize in a single sport or those who compete in multiple sport both rely on intrinsic and extrinsic sources of enjoyment for motivation in very similar ways; whereas adolescent NSSA are less motivated and experience less enjoyment from sport participation compared to MSA and SSSA. Objectives To help athletic trainers promote a “food-first” philosophy to support health and performance, understand federal and sport governing body rules and regulations regarding dietary supplements and banned substances, and become familiar with reliable resources for evaluating the safety, purity, and efficacy of dietary supplements. Background The dietary supplement industry is poorly regulated and takes in billions of dollars per year. Uneducated athletes need to gain a better understanding of the safety, eligibility, and efficacy concerns associated with choosing to take dietary supplements. The athletic trainer is a valuable athletic team member who can help in the educational process. In many cases, athletic trainers are asked to help evaluate the legality, safety, and efficacy of dietary supplements. For this position statement, our mission is to provide the athletic trainer with the necessary resources for these tasks. Recommendations Proper nutrition and changes in the athlete's habitual diet should be considered first when improved performance is the goal. Athletes need to understand the level of regulation (or lack thereof) governing the dietary supplement industry at the international, federal, state, and individual sport-participation levels. Athletes should not assume a product is safe simply because it is marketed over the counter. All products athletes are considering using should be evaluated for purity (ie, truth in labeling), safety, and efficacy.

Solution-focused brief therapy (SFBT) has become a popular methodology used in business, counseling, and other fields to help individuals problem-solve. In education, this approach is known as solution-focus brief counseling (SFBC). This chapter will review the origins of the solution-focused approach to counseling and problem-solving, present the major tenants, and discuss challenges. The chapter will also present interview data from current school counselors to examine how school counselors use SFBC in schools and provide strategies for implementation.

The solution-focused approach is a form of brief therapy that has been successfully adapted to business coaching and school counseling. The purpose of this article is to give an introduction to solution-focused counseling and how to use it in the field of sport psychology. This article highlights key issues in solution-focused counseling. It also describes the relationship between the athlete and the counselor, where it is common practice to distinguish between three types of relationship: the visitor type, the complainer type, and the customer type. In a solution-focused process of counseling, the introductory conversation usually has the following structure: (a) description of the problem, (b) development of well-formulated goals, (c) exploration for expectation, and (d) end-of-session feedback. The solution-focused process and a number of techniques are described, together with a case example from sport.

Issues related to sexuality, sexual orientation, and romantic relationships have received attention in the sport psychology literature. An area that has not been addressed, however, is that of romantic relationships among sport teammates. Such intrateam romantic relationships may have certain benefits but can also be disruptive to teams and team functioning. The purpose of this manuscript is to (a) address issues related to intrateam romantic relationships, and (b) to propose strategies for sport psychology consultants to consider and use when working with teams when intrateam romantic relationships develop. Specifically, sport psychology consultants who encounter intrateam romantic relationships may find it valuable to consider family system models as a theoretical framework for intervention, clearly identify the client, determine the willingness of those involved to consult, and assess their own abilities to effectively intervene and to receive supervision for such interventions. A well-defined, credible approach may help sport psychology consultants to succeed in complex circumstances and gain the trust, respect, and cooperation of the coaches, teams, and athletes with whom they work.

The real world practice of sport and performance psychology will inevitably present the practitioner with performers for whom spirituality is the supreme motivator (Balague, 1999). Spirituality and sport, despite its practical relevance, is an underdeveloped and sometimes misunderstood combination (Nesti, 2007). The aim of this study was to reveal the individual experiences of athletes as they integrate spirituality and their sport participation. Five high level athletes (1 female, 4 male), each holding a Christian worldview were interviewed. A strategically designed interview guide illuminated several key themes, which Watson and Nesti (2005) suggested, would contribute much to the existing literature. Out of these themes, practical implications have been considered for the delivery of sport and performance psychology services to Christian athletes. Evidenced by this study, is the reality that some Christian athletes present a unique perspective through which appropriately tailored sport psychology services can readily precipitate personal excellence in sport and life.

Context: Certified athletic trainers (ATs) are responsible for integrating relevant professionals into the rehabilitation team to assist with the holistic care of injured athletes. Objective: To explore National Collegiate Athletic Association Division I (DI) ATs' experience with sport psychology consultants (SPCs), willingness to encourage athletes to use SPCs for injury rehabilitation, and perceptions of the benefits of sport psychology services. Design: Quantitative study. Setting: A Web-based survey was administered to a national sample of DI ATs. Patients or Other Participants: A total of 659 (341 men, 318 women) ATs completed the survey. Main Outcome Measure(s): Athletic trainers' experience with SPCs, willingness to encourage athletes to seek sport psychology services, and perceptions of the benefits of those services in injury-rehabilitation settings were self-reported using a rating scale that ranged from 1 (never or not at all) to 5 (definitely or extremely). Results: Logistic regression revealed that the availability of SPCs, previous encouragement to athletes to seek sport psychology services, and previous positive interactions with SPCs predicted the ATs' willingness to encourage athletes to use these services (P &lt; .0001). The services ATs rated the highest for injury rehabilitation were managing anxiety and emotion, improving coping techniques, and building confidence (ie, confidence in returning to sport and building confidence). Chi-square analyses indicated that female ATs' ratings of perceived benefits were higher (P ≤ .001) than those of male ATs, and the ratings of ATs who were likely to encourage the use of SPCs were higher (P ≤ .001) than those who were unlikely to encourage SPC service use. Conclusions: Athletic trainers in our study who had previous positive SPC experiences were most likely to use SPCs and their services during the injury-rehabilitation process. Possible implications are offered for how ATs interested in sport psychology services might call on SPCs to complement their work with injured athletes.

Motivational characteristics are influential in shaping adolescents' desire to persist in sport or to discontinue their sport participation. Self-determination theory (Deci & Ryan, 2000) was utilized as the theoretical framework for this study. This theory examines whether sustained participatory involvement, defined as continued participation in the sport through the next year, was influenced by individuals' self-determined motivation and by the fulfillment of the three basic psychological needs of autonomy, competence, and relatedness. Four hundred ninety two soccer players between the ages of 13 and 17 years comprised the sample. Results indicated sport dropout was explained by higher levels of amotivation, external regulation, and introjected regulation and by lower satisfaction of relatedness and autonomy needs. The findings of this study contribute to the knowledge base on sport dropout as they supported many of self-determination theory.
