BackgroundThe family physician is key to facilitating access to psychiatric treatment for young people with first-episode psychosis, and this involvement can reduce aversive events in pathways to care. Those who seek help from primary care tend to have longer intervals to psychiatric care, and some people receive ongoing psychiatric treatment from the family physician.AimsOur objective is to understand the role of the family physician in help-seeking, recognition and ongoing management of first-episode psychosis.MethodWe will use a mixed-methods approach, incorporating health administrative data, electronic medical records (EMRs) and qualitative methodologies to study the role of the family physician at three points on the pathway to care. First, help-seeking: we will use health administrative data to examine access to a family physician and patterns of primary care use preceding the first diagnosis of psychosis; second, recognition: we will identify first-onset cases of psychosis in health administrative data, and look back at linked EMRs from primary care to define a risk profile for undetected cases; and third, management: we will examine service provision to identified patients through EMR data, including patterns of contacts, prescriptions and referrals to specialised care. We will then conduct qualitative interviews and focus groups with key stakeholders to better understand the trends observed in the quantitative data.DiscussionThese findings will provide an in-depth description of first-episode psychosis in primary care, informing strategies to build linkages between family physicians and psychiatric services to improve transitions of care during the crucial early stages of psychosis.Declaration of interestNone.

Background: The Family Nurse Partnership (FNP) is a licensed intensive home visiting intervention programme delivered to teenage mothers which was originally introduced in England in 2006 by the Department of Health and is now provided through local commissioning of public health services and supported by a national unit led by a consortium of partners. The Building Blocks (BB) trial aimed to explore the effectiveness and cost-effectiveness of this programme. This paper reports the results of an economic evaluation of the Building Blocks randomised controlled trial (RCT) based on a cost-consequence approach. Methods: A large sample of 1618 families was followed-up at various intervals during pregnancy and for two years after birth. A cost-consequence approach was taken to appraise the full range of costs arising from the intervention including both health and social measures of cost alongside the consequences of the trial, specifically, the primary outcomes. Results: A large number of potential factors were identified that are likely to attract additional costs beyond the implementation costs of the intervention including both health and non-health outcomes. Conclusion: Given the extensive costs and only small beneficial consequences observed within the two year follow-up period, the cost-consequence model suggests that the FNP intervention is unlikely to be worth the substantial costs and policy makers may wish to consider other options for investment. IntroductionThe leveraging of multi-sector, whole-population, linked administrative data is advantageous for conducting research on complex real-world problems. However, such large and complex data repositories can sometimes appear impersonal and overwhelming. Establishing organizational empathy (OE) in thecontext of a multi-sector partnership between academic, government and community representatives can help us understand the data better for social policy research. Evidence stemming from this research can then inform policy decisions, ultimately increasing the potency of linked data analysis and creating more meaningful student experiences. Our objective is to examine the role of OE in the student research experience. Objectives and ApproachSPECTRUM (Social Policy Evaluation Collaborative Team Research at Universities in Manitoba) is a multi-disciplinary partnership working to provide evidence-based solutions to ‘wicked’ social issues by using linked data from multiple sectors. SPECTRUM provides fellowships to students to become partners in the collaboration. Students have participated in quarterly workshops, building relationships with community leaders, government decision-makers and academic researchers. Students are from various faculties, bringing their unique frameworks and research interests to the collective. Through OE, students observeand participate in SPECTRUM, relating its goals and outcomes to society and their own research. ResultsStudent inclusion in SPECTRUM enhances the partnership by providing a greater range of perspectives and facilitates the development of OE among SPECTRUM members. Students are using linked administrative data, while actively engaging in dialogue with stakeholders, thereby enriching their knowledge and understanding of research. Conclusion / ImplicationsData linkage involves more than just use of the repository; it requires establishing common ground since the data have different meaning to each partner. OE developed through SPECTRUM provides invaluable insight into and context for the data. Knowledge transfer among members of the partnership will enrich SPECTRUM’s research outcomes while building capacity among Students.
