When it comes to understanding experiences of illness, humanities and social sciences research have traditionally reserved a prominent role for narrative. Yet, depression has characteristics that withstand the form of traditional narratives, such as a lack of desire and an impotence to act. How can a ‘datafied’ approach to online forms of depression writing pose a valuable addition to existing narrative approaches in health humanities? In this article, we analyse lay people’s depression discourses online. Our approach, ‘digital hermeneutics’, is inspired by Gadamer’s dialogical hermeneutics. It consists of a ‘scaled reading’ on five different scales: platform hermeneutics; contextual reading with term frequency—inverse document frequency (TF–IDF); distant reading with natural language processing topic modelling; hyper-reading with concordance views and close reading. Our corpus consisted of three data sets, from the blogs and message boards of, respectively, time-to-change.org.uk, a UK-based social organisation and movement that aims to counter mental health discrimination and alleviate social isolation by spreading awareness; Sane.org.uk, a leading UK mental health charity that seeks to help people in facing the challenges of mental illness and to improve quality of life; and the subreddit ‘r/depression’ on web discussion platform reddit. We found that the manner in which people express experiences of illness online is very much dependent on the specific affordances of platforms. We found degrees of ‘narrativity’ to be correlated to authorship and identity markers: the less ‘anonymous’ the writing, generally speaking, the more conventionally ‘narrative’ it was. Pseudonimity was related to more intimate and singular forms, with less pressure to conform to socially accepted and positive narratives of the ‘restitution’ type. We also found that interactive affordances of the platforms were used to a limited extent, nuancing assumptions about the polyvocality of online depression writing. We conclude by making a claim for increased cooperation between digital and medical humanities that might lead to a field of ‘Digital Medical Humanities’.

Word embeddings have been successfully applied in many natural language processing tasks due to its their effectiveness. However, the state-of-the-art algorithms for learning word representations from large amounts of text documents ignore emotional information, which is a significant research problem that must be addressed. To solve the above problem, we propose an emotional word embedding (EWE) model for sentiment analysis in this paper. This method first applies pre-trained word vectors to represent document features using two different linear weighting methods. Then, the resulting document vectors are input to a classification model and used to train a text sentiment classifier, which is based on a neural network. In this way, the emotional polarity of the text is propagated into the word vectors. The experimental results on three kinds of real-world data sets demonstrate that the proposed EWE model achieves superior performances on text sentiment prediction, text similarity calculation, and word emotional expression tasks compared to other state-of-the-art models.

BACKGROUND There is increasing need to explore the value of soft-intelligence, leveraged using the latest artificial intelligence (AI) and natural language processing (NLP) techniques, as a source of analysed evidence to support public health research activity and decision-making. OBJECTIVE The aim of this study was to further explore the value of soft-intelligence analysed using AI through a case study, which examined a large collection of UK tweets relating to mental health during the COVID-19 pandemic. METHODS A search strategy comprising a list of terms related to mental health, COVID-19, and lockdown restrictions was developed to prospectively collate relevant tweets via Twitter’s advanced search application programming interface over a 24-week period. We deployed a specialist NLP platform to explore tweet frequency and sentiment across the UK and identify key topics of discussion. A series of keyword filters were used to clean the initial data retrieved and also set up to track specific mental health problems. Qualitative document analysis was carried out to further explore and expand upon the results generated by the NLP platform. All collated tweets were anonymised RESULTS We identified and analysed 286,902 tweets posted from UK user accounts from 23 July 2020 to 6 January 2021. The average sentiment score was 50%, suggesting overall neutral sentiment across all tweets over the study period. Major fluctuations in volume and sentiment appeared to coincide with key changes to any local and/or national social-distancing measures. Tweets around mental health were polarising, discussed with both positive and negative sentiment. Key topics of consistent discussion over the study period included the impact of the pandemic on people’s mental health (both positively and negatively), fear and anxiety over lockdowns, and anger and mistrust toward the government. CONCLUSIONS Through the primary use of an AI-based NLP platform, we were able to rapidly mine and analyse emerging health-related insights from UK tweets into how the pandemic may be impacting people’s mental health and well-being. This type of real-time analysed evidence could act as a useful intelligence source that agencies, local leaders, and health care decision makers can potentially draw from, particularly during a health crisis.

Background The COVID-19 pandemic has disrupted lives and livelihoods, and people already experiencing mental ill health may have been especially vulnerable. Aims Quantify mental health inequalities in disruptions to healthcare, economic activity and housing. Method We examined data from 59 482 participants in 12 UK longitudinal studies with data collected before and during the COVID-19 pandemic. Within each study, we estimated the association between psychological distress assessed pre-pandemic and disruptions since the start of the pandemic to healthcare (medication access, procedures or appointments), economic activity (employment, income or working hours) and housing (change of address or household composition). Estimates were pooled across studies. Results Across the analysed data-sets, 28% to 77% of participants experienced at least one disruption, with 2.3–33.2% experiencing disruptions in two or more domains. We found 1 s.d. higher pre-pandemic psychological distress was associated with (a) increased odds of any healthcare disruptions (odds ratio (OR) 1.30, 95% CI 1.20–1.40), with fully adjusted odds ratios ranging from 1.24 (95% CI 1.09–1.41) for disruption to procedures to 1.33 (95% CI 1.20–1.49) for disruptions to prescriptions or medication access; (b) loss of employment (odds ratio 1.13, 95% CI 1.06–1.21) and income (OR 1.12, 95% CI 1.06 –1.19), and reductions in working hours/furlough (odds ratio 1.05, 95% CI 1.00–1.09) and (c) increased likelihood of experiencing a disruption in at least two domains (OR 1.25, 95% CI 1.18–1.32) or in one domain (OR 1.11, 95% CI 1.07–1.16), relative to no disruption. There were no associations with housing disruptions (OR 1.00, 95% CI 0.97–1.03). Conclusions People experiencing psychological distress pre-pandemic were more likely to experience healthcare and economic disruptions, and clusters of disruptions across multiple domains during the pandemic. Failing to address these disruptions risks further widening mental health inequalities.

BACKGROUND The COVID-19 pandemic has imposed additional stress on population health that may result in a higher incidence of insomnia. In this study, we hypothesized that using natural language processing (NLP) to explore social media would help to identify the mental health condition of the population experiencing insomnia after the outbreak of COVID-19. OBJECTIVE In this study, we hypothesized that using natural language processing (NLP) to explore social media would help to identify the mental health condition of the population experiencing insomnia after the outbreak of COVID-19. METHODS We designed a pre-post retrospective study using public social media content from Twitter. We categorized tweets based on time into two intervals: prepandemic (01/01/2019 to 01/01/2020) and pandemic (01/01/2020 to 01/01/2021). We used NLP to analyze polarity (positive/negative) and intensity of emotions and also users’ tweets psychological states in terms of sadness, anxiety and anger by counting the words related to these categories in each tweet. Additionally, we performed temporal analysis to examine the effect of time on the users’ insomnia experience. RESULTS We extracted 268,803 tweets containing the word insomnia (prepandemic, 123,293 and pandemic, 145,510). The odds of negative tweets (OR, 1.31; 95% CI, 1.29-1.33), anger (OR, 1.19; 95% CI, 1.16-1.21), and anxiety (OR, 1.24; 95% CI: 1.21-1.26) were higher during the pandemic compared to prepandemic. The likelihood of negative tweets after midnight was higher than for other daily intevals, comprising approximately 60% of all negative insomnia-related tweets in 2020 and 2021 collectively. CONCLUSIONS Twitter users shared more negative tweets about insomnia during the pandemic than during the year before. Also, more anger and anxiety-related content were disseminated during the pandemic on the social media platform. Future studies using an NLP framework could assess tweets about other psychological distress, habit changes, weight gain due to inactivity, and the effect of viral infection on sleep.

The quality, completeness and coverage of ethnicity data in mental health services has long been regarded as unsatisfactory. The Department of Health's new 5-year action plan for delivering race equality in mental healthcare seeks to improve this key building block by setting out actions to improve both the quality of information and its analysis and dissemination. However, those that are tangible and specific are few: annual surveys of service users, national censuses of mental health in-patients and tables of National Confidential Inquiry suicide cases and in-patient deaths by ethnicity. The opportunity to seek improvements in the quality and coverage of key routine data-sets such as ethnic monitoring in primary care and the Hospital Episode Statistics database has not been seized. Moreover, the plan does not mention proposed changes in civil registration (births and deaths) and the coroner service and their potential benefit. The continuing gaps in the information base justify a stronger emphasis on the processes necessary to bring about change rather than on what ethnic monitoring should provide.

BACKGROUND A new illness can come to public attention through social media before it is medically defined, formally documented, or systematically studied. One example is a condition known as breast implant illness (BII), which has been extensively discussed on social media, although it is vaguely defined in the medical literature. OBJECTIVE The objective of this study is to construct a data analysis pipeline to understand emerging illnesses using social media data and to apply the pipeline to understand the key attributes of BII. METHODS We constructed a pipeline of social media data analysis using natural language processing and topic modeling. Mentions related to signs, symptoms, diseases, disorders, and medical procedures were extracted from social media data using the clinical Text Analysis and Knowledge Extraction System. We mapped the mentions to standard medical concepts and then summarized these mapped concepts as topics using latent Dirichlet allocation. Finally, we applied this pipeline to understand BII from several BII-dedicated social media sites. RESULTS Our pipeline identified topics related to toxicity, cancer, and mental health issues that were highly associated with BII. Our pipeline also showed that cancers, autoimmune disorders, and mental health problems were emerging concerns associated with breast implants, based on social media discussions. Furthermore, the pipeline identified mentions such as rupture, infection, pain, and fatigue as common self-reported issues among the public, as well as concerns about toxicity from silicone implants. CONCLUSIONS Our study could inspire future studies on the suggested symptoms and factors of BII. Our study provides the first analysis and derived knowledge of BII from social media using natural language processing techniques and demonstrates the potential of using social media information to better understand similar emerging illnesses. The LOD data sets consist of RDF Triples based on the Ontology, a specification of existing facts, and by linking them to previously disclosed knowledge based on linked data principles. These structured LOD clouds form a large global data network, which provides a more accurate foundation for users to deliver the desired information. However, it is difficult to identify that, if the presence of the same object is identified differently across several LOD data sets, they are inherently identical. This is because objects with different URIs in the LOD datasets must be different and they must be closely examined for similarities in order to judge them as identical. The aim of this study is that the prosed model, RILE, evaluates similarity by comparing object values of existing specified predicates. After performing experiments with our model, we could check the improvement of the confidence level of the connection by extracting the link value.

Michael Ondaatje’s Warlight (2018), his latest novel to-date, contains nostalgic elements of strangeness and cartography. In this paper, I short-circuit such themes with health under medical humanities, which heeds a Nietzschean counsel of close reading in literature. To do so, I explore the case of Rachel’s illness, namely her epileptic seizures, as an instance that drives her impetus for active forgetting and eventual convalescence. A close hermeneutical reading of the novel can reveal that both of Nietzsche’s ideas on active forgetting and convalescence provide traction in terms of what this paper constructs as Rachel’s pathography or narration of illness. Shifting the focus from the main narrator, Nathaniel, I argue that it is not the novel’s reliance on memory but the subplot events of Nathaniel’s sister and her epilepsy that form a substantial case of medical or health humanities.

Natural Language Inference (NLI) plays an important role in many natural language processing tasks such as question answering. However, existing NLI modules that are trained on existing NLI datasets have several drawbacks. For example, they do not capture the notion of entity and role well and often end up making mistakes such as “Peter signed a deal” can be inferred from “John signed a deal”. As part of this work, we have developed two datasets that help mitigate such issues and make the systems better at understanding the notion of “entities” and “roles”. After training the existing models on the new dataset we observe that the existing models do not perform well on one of the new benchmark. We then propose a modification to the “word-to-word” attention function which has been uniformly reused across several popular NLI architectures. The resulting models perform as well as their unmodified counterparts on the existing benchmarks and perform significantly well on the new benchmarks that emphasize “roles” and “entities”.

Human gaze reveals a wealth of information about internal cognitive state. Thus, gaze-related research has significantly increased in computer vision, natural language processing, decision learning, and robotics in recent years. We provide a high-level overview of the research efforts in these fields, including collecting human gaze data sets, modeling gaze behaviors, and utilizing gaze information in various applications, with the goal of enhancing communication between these research areas. We discuss future challenges and potential applications that work towards a common goal of human-centered artificial intelligence.
