Metastases to the breast from extramammary primaries are uncommon and account for 0.5–6% of all breast malignancies (Georgiannos et al., 2001, and Vizcaíno et al., 2001). Malignant melanoma, lymphoma, and lung and gastric carcinomas are the most frequently encountered nonmammary metastases to the breast in adults (Georgiannos et al., 2001, and Chaignaud et al., 1994). Primary colorectal adenocarcinoma (CRC) metastatic to the breast is extremely rare, with the medical literature having only 19 recorded cases. Typically CRC metastatic to the breast is indicative of widely disseminated disease and a poor prognosis. Here we present a case of poorly differentiated colon cancer metastatic to the breast and review the current literature on this rare event.

Melanomas arising in the nasal cavity or paranasal sinus are rare and have a poor prognosis and is more aggressive than cutaneous counter part. The incidence rate reaches its peak when the patients are in the 5th to 8th decades of life, and the disease is slightly more common in males than females; age and sex do not affect the prognosis. We present a case with histologically deceiving features which was proven Malignant Melanoma by application of Immunohistochemistry. On sinoscopic examination, a mass noted of which incisional biopsy was obtained. Histologically, poorly differentiated Sinonasal carcinoma and Malignant Vascular Neoplasm was differential diagnosis. On immunohistochemistry, strong expression of SOX-10, Melan-A, HMB-45 was observed and p63, CK-7, CK20, Synaptophysin, CD56, CD138, CD45, S100, Desmin, Myogenin were all non-immunoreactive. Thus Malignant Melanoma was final diagnosis.Awareness of melanoma mimickers is very important for clinicians in general, and

We report a case of primary melanoma of the larynx in a patient who died of disseminated disease 13 months later. It is a rare malignant tumor in the head and neck, accounting for 0.4-10% of all melanomas of this site. Histologically, the presence of in situ changes within the laryngeal mucosa strongly supports our diagnosis of primary melanoma of the larynx.

A pesar de su baja frecuencia -que no suele superar el dos por ciento en varias series de melanoma- es muy importante reconocerlos precozmente pues un diagnóstico tardío está asociado a un muy mal pronóstico. Abstract: Several infrequent subtypes of malignant melanoma have been described in the medical literature. Although their low frequency -less than two percent in various series of melanoma- it is very important to early recognize them because late diagnosis is associated with a very poor prognosis.

Introduction. Gestational trophoblastic neoplasm (GTN), choriocarcinoma in coexistence with primary cervical adenocarcinoma, is a rare event not easy to diagnose. Choriocarcinoma is a malignant form of GTN but curable if metastases do not appear early and spread fast. Case report. We presented choriocarcinoma in coexistence with primary cervical adenocarcinoma in a 48-year-old patient who had radical hysterectomy because of confirmed cervical carcinoma (Dg: Carcinoma porto vaginalis uteri FIGO st I B1). Histological findings confirmed cervical choriocarcinoma with extensive vascular invasion and apoptosis but GTN choriocarcinoma was finally confirmed after immunohystochemical examinations. Preoperative serum human gonadotropine (beta hCG) level stayed unknown. This patient did not have any pregnancy-like symptoms before the operation. The first beta hCG monitoring was done two months after the operation and found negative. According to the final diagnosis the decision of Consilium for Malignant Diseases was that this patient needed serum hCG monitoring as well as treatment with chemotherapy for high-risk GTN and consequent irradiation for adenocarcinoma. Conclusion. The early and proper diagnosis of nonmetastatic choriocarcinoma of nongestational origine in coexistence with cervical carcinoma is curable and can have good prognosis.

Libman-Sacks endocarditis is non-infectious thrombotic involvement of cardiac valves seen in systemic lupus erythematosus.1,2 Mitral valve followed by aortic valve are involved in systemic lupus erythematosus. However, involvement of tricuspid valve is rare.3 A case of a 36-year-old woman with systemic lupus erythematosus with isolated tricuspid valve endocarditis has been reported here. Within the background of the relevant medical literature, this is a rare event. In systemic lupus erythematosus (SLE) patients, mostly the aortic and mitral valves are affected.4 Echo shows evidence of vegetation in tip and septa of tricuspid valve which is suggestive of Libman-Sacks endocarditis. In our case we have discussed Libman-Sacks endocarditis involving tricuspid valve.

Myelofibrosis (MF), or fibrosis of the bone marrow, is an uncommon condition in children. Fewer than 100 cases have been described in the medical literature. Most cases in children arise secondary to other disease processes. We present a case of Myelofibrosis in a 12-year-old girl. The purpose of reporting this case is that this child had a diagnostic dilemma and was sent home with poor prognosis but with proper diagnosis and treatment she improved and may have a complete cure, as the myelofibrosis was secondary to tuberculosis. AbstractNeuroendocrine tumors (NETs) of the ureter are rare, with less than 40 cases described in the literature. A majority of tumors described are poorly differentiated tumors with a poor prognosis. We present the case of a moderately differentiated atypical carcinoid NET of the ureter with a good postoperative outcome. A literature review was also performed to identify similar cases to compare their management and postoperative outcomes.

We report an extremely rare case report of primary malignant melanoma of female urethra. A 65 years old diabetic elderly postmenopausal female presented with a history of intermittent blood spots on undergarments for few days. Genital examination revealed a single, tan colored, soft chestnut size and polypoidal non ulcerated mass lesion protruding through the urethral meatus. Mass biopsy revealed poorly differentiated epithelial malignancy and immuno-histological analysis revealed positive with HMB 45 and protein S-100 suggestive of melanoma. Metastatic work up for the malignancy was negative. Complete urethrectomy with Mitrofanoff procedure with inguinal lymph node dissection was performed. Pathological diagnosis was malignant melanoma of urethra. We discuss the clinicopathological features and treatment option possible in this scenario.

<p>Metastases to the nose and paranasal sinuses usually respond poorly to treatment and have a poor prognosis. We present a 31 year old lady with rare metastatic paranasal sinuses lesion which is adenocarcinoma that metastasis from breast carcinoma. All of clinical presentation, radiological findings, histopathological and immunohistochemically markers examination reveal metastatic adenocarcinoma. This study presents rare metastasis in head and neck cancer especially from breast to paranasal sinuses.</p>
