Implant therapy is now well established, and there is an increasing need for shorter rehabilitation time. Original prerequisites of osseointegration have been reassessed to satisfy continuously increasing patient's expectation of reduced treatment time, improved esthetics, and increased comfort. Shorter healing time may be appropriate in some circumstances, and examples of early loading have been reported in animal and human studies. However, to date there are insufficient data to determine a universally acceptable opinion on early loading of implants for single-tooth replacement. This case report involves early loading, combined with construction of a restoration, inserted directly after 6 weeks of implant surgery and followup of one year.

Abstract This clinical report describes the traumatic avulsion of the left maxillary central and lateral incisors with minimal tissue trauma, followed by placement of provisional restorations of two root-form implants in a 14-year old female patient. Ten days following the avulsion, implant therapy was performed with Summers osteotomes and flapless technique using a root-form 3i osseotite NT. Angulated abutments and acrylic provisional crowns were placed. No occlusal contact was detected. An impression was made 30 days after implant insertion and ceramic restorations were placed 3 months later. No significant soft tissue contraction was observed during the provisional period. The patient exhibited no clinical or radiologic complications for 8 months after loading. In carefully selected under-aged patients implant therapy and immediate loading in the anterior maxilla for traumatic tooth avulsion may prove to be valuable to avoid soft tissue contraction, alveolar bone resorption, and positive esthetic/psychological response. INTRODUCTION: Immediate dental implants have greatly reduced the treatment time and the number of surgical interventions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. CASE REPORT: In the present case report a 24 year old male patient reported to our institution with fractured upper front teeth. After careful examination and treatment planning immediate implant treatment was initiated. The teeth were extracted atraumatically. We placed three implants into the extraction sockets. The prosthetic rehabilitation was done with metal ceramic crowns. CONCLUSION: It was found that the immediate implant therapy has several advantages such as reduced treatment length, preservation of soft and hard tissues surrounding the implant and reduced number of operations. Immediate implant treatment therefore has a great future in the treatment of aesthetic zones.

Background: Different approaches were proposed in the literature for the treatment of malpositioned ankylosed teeth. The present case report describes a modification of Orthodontic Bone Stretching Technique (OBS) for the repositioning of ankylosed teeth, consisting of dentoalveolar segmental osteotomies performed with piezoelectric instruments followed by orthodontic and orthopedic traction. Case Report: A 22-year-old female in good general health was referred by her orthodontist due to an infraoccluded and ankylosed maxillary upper left canine. Attempts of conventional and corticotomy-assisted orthodontic alignment of the tooth were previously performed with no success. After elevating a full-thickness flap, three osteotomies were performed by using piezoelectric inserts (Piezotome Cube, Acteon, Merignac, France). Mesial and distal cuts were full-thickness osteotomies, parallel to the long axis of the tooth, through the buccal and palatal cortical plates. The apical osseous incision was a horizontal corticotomy, involving only the buccal plate, and connecting the vertical osteotomies two millimetres over the apex of the tooth. Heavy orthodontic forces were immediately applied by using both dental and skeletal anchorage. The initial movement of the dentoalveolar segment was observed three weeks after surgery and case finishing has been completed in two months. At a one-year follow-up, the repositioned canine showed good periodontal conditions, no discoloration and positive pulp response to the electric test. Conclusion: Modified OBS technique was effective in repositioning an infraoccluded and ankylosed maxillary canine, providing satisfactory function and esthetics with short treatment time.

Purpose: Ocular features of Alport syndrome include anterior lenticonus, posterior polymorphous corneal dystrophy, and fleck-and-dot retinopathy in most cases. Keratoconus in such patients has been rarely mentioned in previous studies. To our knowledge, this is the first report of corneal cross-linking for halting the progression of keratoconus in a patient with Alport syndrome. Case report: A 22-year-old male was referred for his initial corneal topography, after he was already prescribed with rigid gas-permeable contact lenses. Alport syndrome was diagnosed in his infancy and gene COL4A5 mutation was confirmed. Ophthalmological evaluation confirmed keratoconus. One-year follow-up showed a progression on his right eye and standard corneal cross-linking was performed. Stabilization of the disease marked by normalization in visual function and corneal tomography values was noticed 1 year after the procedure. Conclusions: When diagnosing ocular clinical findings of Alport syndrome, keratoconus should be considered. Standard corneal cross-linking protocol can halt its progression.

ABSTRACT:Objective:To describe a case of Pasteurella multocida meningitis associated with acute disseminated encephalomyelitis (ADEM).Case report:A 33-year-old woman employed in a dog pound presented herself to hospital with fever and meningismus and was found to have culture positive Pasteurella multocida meningitis. Despite appropriate antibiotic treatment her clinical course was characterized by a persistent fever and worsening encephalopathy, which prompted further neurological investigation. Spinal fluid exam and serial MRI scans as well as her one-year clinical course were found to be compatible with ADEM.Conclusion:Persistent fever and worsening encephalopathy in meningitis may indicate a para-infectious immune process such as ADEM, and may serve as indications for further neurological investigation.

A case of total ischemia of the triquetrum after a crushing injury to the right wrist by a dumbbell is reported. He was treated conservatively with splinting and analgesia. There was complete clinical and radiological recovery after a follow-up of one year.

Extracranial meningiomas may be subdivided into primary and secondary types based on absence or presence of intracranial attachments respectively. Primary sinonasal tract meningiomas are rare with unknown etiopathology and non-specific clinical presentation. Of these PEMs only 11.5% are in the nasal cavity and nasal septum. To our knowledge less than 50 cases of PEM of nasal cavity have been reported in the literature. We report a case of a 22 years old male who presented with epistaxis and increasing nasal stuffiness for the last one year.
