Purpose. The goal was to describe our experience in the surgical management and treatment of four patients with congenital upper eyelid colobomas.Methods. A descriptive, observational, retrospective study was performed including patients with congenital eyelid colobomas referred to Asociación para Evitar la Ceguera en México I.A.P. “Dr. Luis Sánchez Bulnes” between 2004 and 2014 and assessed by the Oculoplastics and Orbit Service.Results. The four cases required surgical treatment of the eyelid defects before one year of age and their evolution was monitored from the time of referral to the present day. One of the patients needed a second surgical procedure to repair the eyelid defect and correct the strabismus.Conclusions. Eyelid colobomas are a potential threat to vision at an early age, which requires close monitoring of the visual development of patients.

AbstractThe incidence of chronic keratoconjunctivitis, which potentially causes long-term loss of visual acuity due to corneal opacity, is considerably less common in children than in adults. It is therefore in danger of being overlooked. In children the appropriate treatment is therefore often introduced too late, or to an insufficient extent. In this article we would like to raise awareness about the diagnosis of chronic keratoconjunctivitis in children, and to present an effective treatment plan for severe stages of the disease. There are two forms of chronic keratoconjunctivitis that occur most frequently in children: hyperergic blepharokeratoconjunctivitis (hBKC) and vernal keratoconjunctivitis (VKC). With hBKC, the patient often has a history of recurring hordeolum and also presents with blepharitis; it is characterized by the marked presence of corneal neovascularization in the lower circumference of the cornea. VKC is typically characterized by changes under the upper eyelid, with marked changes to the superior limbus. If there is a risk of complications involving the cornea, or in the presence of such complications, a consistent long-term topical immunosuppressive and anti-inflammatory treatment is required. Both of these properties are combined in the active ingredient cyclosporine A. Other advantages of topical CSA treatment are its steroid-sparing effect and the long-term reduction of exacerbations. Parents need to be informed about the chronic nature of these two diseases and their tendency to recur; because of these characteristics, treatment, in most cases, should be envisaged for at least one year in order to effectively disrupt the complex immunologic processes. This safeguards the childʼs visual development and prevents amblyopia caused by scarring and astigmatism. We hope that the data presented will lower the barriers related to prescribing CSA for topical eye application in children.

AbstractObjective:To study the effect of Silastic®sheeting placed in the middle ear during tympanoplasty, including the effect on hearing.Design:Retrospective study.Background:Chronic inflammation of the middle ear is common. Surgical treatment sometimes results in middle-ear adhesions and hearing deterioration.Materials and methods:We selected 106 patients with chronic otitis media, middle-ear adhesions and intact ossicles, based on intra-operative findings. These patients underwent single-stage tympanoplasty either with or without insertion of Silastic sheeting. Audiometry was undertaken pre-operatively and one and 12 months post-operatively.Results:Patients who had undergone Silastic sheet insertion showed significantly better air conduction, bone conduction and air–bone gap averages one year post-operatively, compared with those who had not.

The aim of the study was to investigate the breed predisposition and the diagnostic and surgical management of horses referred for cryptorchidism. The breed, localization of retained testis, diagnosis, type of surgical treatment and complications were analyzed. Seventy horses were included in the study; the Western Riding horse breeds were the most affected (Quarter Horse 34/70, 48.5%; Appaloosa 9/70, 12.8%). In unilateral cryptorchids (65/70, 92.8%) the most common location for a retained testis was the left abdomen (28/65, 43%), while in bilateral cryptorchids (5/70, 7.1%), bilateral abdominal retention was the most frequent (3/5, 6%). Information about testis localization was achieved through transabdominal ultrasound (30/49 cases, 61.2%), through per rectum palpation (21/49 cases, 42.9%) and through inguinal palpation (14/49 cases, 28.9%). Cryptorchidectomy was achieved with standing laparoscopy (44/70 cases, 62.8%), or with open inguinal orchiectomy in general anesthesia (26/70 cases, 37.2%). Complications during laparoscopy were spleen puncture (1/44, 2.2%), a self-limiting bleeding from the spermatic cord (10/44 cases, 22.7%), hyperthermia (3/44 cases, 6.8%), and emphysema (15/44, 34%). During inguinal open cryptorchidectomy difficulties with identifying the inguinal testis during surgery (8/26 cases, 30.8%) and a moderate and self-limiting swelling of the inguinal region after surgery (17/26, 65.4%) were observed. For orchiectomy, a standing laparoscopy was confirmed as the preferred procedure for an abdominally retained testis with almost no complications.

The aim of this retrospective study was to review pleural complications and results of surgical management of patient with hydatid disease. Between 2000 and 2010, 34 patients among 260 patients with hydatid disease, were diagnosed with pleural complications. Findings are presented in relative frequencies tables. The most common pleural complication was empyema in 9 patients. The most common procedure was cystotomy, evacuation and decortication in 25 patients. In endemic area, pleural complications of hydatid cyst should be considered for differential diagnosis. And because of higher morbidity and mortality, surgical treatment should be carried out before complications.DOI: http://dx.doi.org/10.3126/ijls.v8i4.10893

Purpose To evaluate the long-term results of classical “à la carte” surgical treatment of objective patellar instability as proposed by Dejour in 1987. Methods A multicentric retrospective study was conducted evaluating patients that underwent surgical procedure with a 10-year minimum follow-up (mean 12.7; range, 10–15). Surgical procedures were medial transfer of the tibial tubercle transfer according to Elmslie and Trillat et al in 38 cases, plasty of the vastus medialis obliquus according to Insall in 15 cases, open lateral retinacular release in 13 cases, capsuloplasty in 3 cases, and trochleoplasty in 1 case. Different combinations of surgical procedures were adopted according to the pathological features. Subjective outcome was assessed with the visual analog scale (VAS), Kujala score, subjective International Knee Documentation Committee (IKDC) score, Tegner score, and Crosby and Insall scale. Radiographic exams were used to assess the patellar tilt by the Laurin's angle and patellofemoral osteoarthritis (OA) according to the Iwano radiological OA scale. Results Forty patients were evaluated. Subjectively, 60% of patients achieved a result judged good, 34% sufficient, and 6% poor. There were only two cases of recurrence of instability. Mean score results were Kujala score 73.4 ± 9.9 (range, 55–95), VAS 4.5 ± 1.2 (range, 1–6), IKDC 64.8 ± 7.9 (range, 51–88), and Tegner score 4. Only nine patients returned to sports activities). Ten patients developed a grade I patellofemoral OA, 8 patients a grade II, and 22 patients a grade III. Average patellar tilt was 10° ±  3.9°. Conclusion This retrospective study showed that the traditional surgical procedure was successful for the treatment of patellar instability, but it did not prevent symptomatic patellofemoral OA. Introduction : About one fourth of pregnant women present with bleeding in the rst trimester .The four major sources of nontraumatic bleeding in early pregnancy are ectopic pregnancy, miscarriage (threatened, inevitable, incomplete, complete), implantation of the pregnancy, cervical, vaginal, or uterine pathology (eg, polyps, inammation/infection, trophoblastic disease). Although 50% of cases presenting with vaginal bleeding continue to have a normal healthy pregnancy, but the maternal anxiety about risk of miscarriage should be assessed and counselled. The present study is an overview of etiologies and evaluation of bleeding upto 12 weeks of gestational age. Aim:To evaluate the incidence and etiology of rst trimester bleeding. Objective: To correlate the association between rst trimester bleeding and miscarriage. Methodology:A retrospective study among pregnant women with rst trimester bleeding was conducted for a period of 1 year at Chettinad hospital and Research Institute.Detailed History taking and pelvic examination was done for 139 patients.Specic blood investigation along with Transvaginal USG probe 3-5 MHz was performed and appropriate treatment was given. Results: Out of 900 pregnant women attending the out patient (OP) over a period of one year, 139 patients presented with rst trimester bleeding , incidence being 15.44%. The present study suggest that 41.007% women had miscarriage following rst trimester bleeding . It is depicted that 20.14% of women had history of previous abortions and 12.23% had history of bleeding in previous pregnancy. The major cause of bleeding in the rst trimester in our study was threatened abortion (32.37%). Conclusion :We conclude that the present study helps in giving appropriate treatment to women presenting with rst trimester bleeding . Ultrasonography plays a key role in the diagnosis of cause of bleed . Early care and close monitoring will inevitably improve pregnancy outcome .

Abstract BackgroundThe management of Bacille Calmette-Guérin (BCG) vaccine-related regional lymphadenitis in infants is still controversial. We present the results of surgical treatment for this condition.AimTo report our experience in the surgical management in infants with BCG vaccine-related regional lymphadenitis.ResultsFrom January 2013 to June 2020, 65 patients with BCG vaccine-related regional lymphadenitis were retrospectively reviewed. The patients included 55 boys and ten girls. Median age was 3.35 months (5.83±8.17, IQR 3.35-5.1). The infants were all full-term with normal birth weight and received vaccination in the first 24 h of life. All patients underwent surgical excision of the abscess and involved regional lymph nodes, and tolerated anesthesia and the surgical procedure well. Postoperative seroma occurred in 20 patients (20/65). The effusion was aspirated with a fine needle. Sixty-four patients recovered uneventfully. No anti-tuberculosis drugs were given postoperatively. Postoperative abscess recurred in one case and re-operation was required one year later. Postoperative follow-up of three to 12 months showed that all patients had a good outcome.ConclusionSurgical management of infantile BCG vaccine-related lymphadenitis is safe and effective with a low recurrence rate and few complications. Surgical intervention shortened the treatment period and avoided the use of anti-tuberculosis drugs. Surgeons should follow the natural course of this disease and be cautious in choosing surgical treatment.

Pelvic injuries account for 16 to 25% of all skeletal trauma in small pets. Small dogs as companion dogs are now widespread. Unusual exterior and behavioral features have made them popular and at the same time vulnerable in the modern urban environment. The author defined the nature of injuries and adapted technique of transosseous osteosynthesis for the treatment of toy-breed dogs with pelvic trauma. The study of the X-rays in 226 dogs with pelvic injuries demonstrated that toy-breed dogs accounted for 16% of the total. Multiple pelvic trauma was revealed in 95% of them. Surgical treatment was performed in 24 dogs, conservative one - in 13, that amounted to 65% and 35%, respectively. Surgical treatment consisted in open juxtaposing of fragments and focal transosseous and/or internal osteosynthesis with subsequent external fixation using devices of various designs. The external structures were of three types: half-ring support or U-shaped staple, pair of parallel curved plates connected by two threaded rods with each other, and pair of curved plates located bilaterally on both sides of the pelvis and connected by two threaded rods with the help of threaded ends. The terms of fixation for pelvic injuries in toy-breed dogs at the age under one year were 42 days on the average, and in the dogs at the age above one year - 60 days. Surgical treatment in toy-breed dogs with pelvic injuries was performed more often in comparison with conservative one. Pelvic trauma was accompanied by pronounced pain shock. The osteosynthesis techniques used in toy-breed dogs are little traumatic and not limiting functions thereby contributing to recovery of all the structures of pelvis and pelvic limbs.

Authors report on the use of Integra dermal regeneration template after excision of an extended, recurrent skin tumor in the temporal region. The area covered with Integra was 180 cm2. Skin grafting to cover Integra was performed on the 28th day. Both Integra and the skin transplant were taken 100%. Integra dermal regeneration template can provide good functional and aesthetic result in the surgical management of extended skin tumors over the skull. Orv. Hetil., 2013, 154, 225–227.
