This case study reports the successful treatment of an immature upper premolar with periapical pathosis and sinus tract using revascularization technique. Clinical and radiographic examination demonstrated the recovery of vitality, continued root development, and periapical healing at the 7-month follow-up. In addition, severe calcification of the canal was noted at the 36-month follow-up. At the 66-month follow-up, cone-beam computed tomography (CBCT) revealed complete periapical healing, apical closure, increase in root length and thickness of dentin, and severe calcification of the root canal. Even though the nature of tissue within the root canal is unknown, revascularization appears to give good clinical and radiographic success. This case report highlights that severe calcification of the canal is one of the long-term outcomes of revascularized root canals.

Abstract BackgroundEstimates of future survival can be a key evidence source when deciding if a medical treatment should be funded. Current practice is to use standard parametric models for generating extrapolations. Several emerging, more flexible, survival models are available which can provide improved within-sample fit. This study aimed to assess if these emerging practice models also provided improved extrapolations.MethodsBoth a simulation study and a case-study were used to assess the goodness of fit of five classes of survival model. These were: current practice models, Royston Parmar models (RPMs), Fractional polynomials (FPs), Generalised additive models (GAMs), and Dynamic survival models (DSMs). The simulation study used a mixture-Weibull model as the data-generating mechanism with varying lengths of follow-up and sample sizes. The case-study was long-term follow-up of a prostate cancer trial. For both studies, models were fit to an early data-cut of the data, and extrapolations compared to the known long-term follow-up.ResultsThe emerging practice models provided better within-sample fit than current practice models. For data-rich simulation scenarios (large sample sizes or long follow-up), the GAMs and DSMs provided improved extrapolations compared with current practice. Extrapolations from FPs were always very poor whilst those from RPMs were similar to current practice. With short follow-up all the models struggled to provide useful extrapolations. In the case-study all the models provided very similar estimates, but extrapolations were all poor as no model was able to capture a turning-point during the extrapolated period. ConclusionsGood within-sample fit does not guarantee good extrapolation performance. Both GAMs and DSMs may be considered as candidate extrapolation models in addition to current practice. Further research into when these flexible models are most useful, and the role of external evidence to improve extrapolations is required.

This study aims to report the clinical case of a 16 years-old male patient, who attended a private office reporting that at an accident suffered at the age of 10 years-old, which caused extrusive dislocation in both teeth 11 and 21. At that time, the teeth were repositioned, without an adequate follow-up. Upon physical and radiographic examination, the following features were observed: presence of recurrent sinus tract on the vestibular surface, area of ​​external cervical resorption, and a periapical lesion on tooth 21. Endodontic treatments (necropulpectomy) were performed on teeth 11 and 21, with the placement of a root canal dressing of calcium hydroxide and subsequent root canal filling. In addition, soft tissue flap folding was performed to treat the resorption area and to seal it with glass ionomer cement. A 7 months follow-up radiograph shows stabilization in the process of the tooth resorption and remission of the periapical lesion. In conclusion, the endodontic treatment with intracanal medication, and the sealing of the resorption area were successful to preserve the traumatized tooth. In addition, it is noteworthy that following up with the patient after the trauma episode is essential to monitor the pulp vitality of the tooth involved.

Traumatic dental injuries are particularly common in school-age children and often occur in the anterior region. Process management of cases is possible with alternative treatments according to the root development levels. This case series is aimed to present the treatment and 2-year follow-up of permanent anterior teeth with traumatic dental injuries. Case 1: An 8-year-old patient, who had a bicycle accident 20 days earlier, was diagnosed with extrusion of #31. Due to late admission to the clinic, no repositioning procedure was applied to the tooth. Regenerative endodontic treatment was performed. During the radiological follow-up, the apex was closed in the 12th month; however, it was observed that obliteration started in the root canal at the 24th month. The case is still being followed up at regular intervals. Case 2: A 13-year-old patient, who had a traffic accident 3 days prior, was diagnosed with subluxation in #11, and a root fracture was detected in the apical third of #21. In #21, root canal treatment was applied to the coronal part of the fragments. After the diagnosis of pulp necrosis in #11 in the 2nd month of the follow-up period, root canal filling was applied. During the follow-up period, no pathology was detected and no granulation tissue was formed between the fragments in #21. In traumatic dental injuries, long-term follow-up, well-timed endodontic treatments, and material selection play an important role in success. With regenerative endodontic treatment, successful results can be obtained even in treatments applied in late-admitted patients.

SummaryA 13-year-old cat was diagnosed with a synovial cyst originating from the glenohumeral joint. The cat presented with severe front limb lameness and a non-painful fluctuating swelling on the medial aspect of the humerus. Radiographic examination showed severe bilateral shoulder osteoarthritis with osteophyte formation. The mass was surgically resected and histopathology confirmed the diagnosis of a synovial cyst. Three weeks postoperatively the cyst recurred. Surgical resection was repeated followed by omentalisation of the defect. Throughout the following 3 months the cyst did not recur. Unfortunately, for reasons unrelated to the presented disease, the cat was euthanized shortly afterwards, therefore making a long-term follow-up impossible.
