A customized treatment plan is important to reach results that will satisfy the patient providing esthetics, function, and long-term stability. This type of oral rehabilitation requires professionals from different dental specialties where communication is a major key point. Digital Smile Design allows the practitioners to plan and discuss the patient’s condition to establish the proper treatment plan, which must be driven by the desired zenith position. The ideal gingival position will guide the professionals and determine the need to perform surgical procedures or orthodontic movement before placing the final restorations. In this article, the zenith-driven concept is discussed and a challenging case is presented with 4-year follow-up where tooth extraction, immediate implant placement, bone regeneration, and a connective tissue graft were performed.

Introduction. Gingival fenestration (GF) is a distinct clinical entity of uncertain etiology that is seldom documented in the literature. It has been associated mainly with submucosal mechanical irritants such as calculus that subsequently create an opening in the oral soft tissue, usually at facial anterior sites. Surgical correction may be indicated to address functional and/or esthetic concerns. Case Presentation. The patient, a 74-year-old male, presented to the clinic with a chief complaint of “something is poking through my gum.” Clinical exam revealed a gingival fenestration on the facial of tooth #11, associated with what appeared to be a pronounced noncarious cervical lesion (NCCL). Surgical treatment consisted of a connective tissue graft and odontoplasty of the sharp protruding edge of the root surface. Healing was uneventful with excellent closure of the fenestration and no evidence of recurrence after 18 months of follow-up. Conclusion. GF is a perforation of the mucosa typically associated with underlying sharp mechanical etiology. This report describes a fenestration that developed from a probable abfractive lesion, which later was successfully closed and exhibits long-term stability.

The decision-making process of complex clinical cases should involve multiple specialists to obtain a predictable result on a long-term basis. In view of the above, the present report is aimed at describing the multidisciplinary management of a partially edentulous female patient presenting with a severely compromised residual dentition. To improve function and aesthetics, the treatment combined multiple extractions, temporary rehabilitation with a complete removable denture, guided bone regeneration and implant insertion, soft tissue management, tooth alignment, and restorative dentistry. Thus, several dental branches were embraced during the treatment phases, including oral surgery and implantology, periodontology, orthodontics, and prosthodontics. The involvement of different specialists ensured the achievement of a good result from biological, functional, and aesthetic aspects. The patient was satisfied with the final outcome. In conclusion, to meet the patient’s expectations particularly in complex clinical situations, the interdisciplinary approach becomes essential from the early phases in order to identify the ideal treatment plan with the correct time sequence.

Purpose: The present study aimed to evaluate the long-term stability of esthetic outcomes of soft tissue around maxillary anterior single-tooth implants after 10-to-12 years of loading. Methods: Patients who had been treated for single-tooth implants in the anterior maxilla between February 2000 and July 2002 were invited to participate in the study. All implants had been placed according to delayed implant placement and conventional loading protocols without any connective tissue graft or papilla preservation flaps. Pink Esthetic Score (PES) was rated using standardized clinical photographs to assess the esthetic outcomes of the implant treatment at the time of crown placement and at time of follow-up examination which was at least 10 years after the crown placement. Results: A total of 19 patients were included. The mean score of PES was 11.63 (SD 1.61; range 7-14) at baseline. After 10-to-12 years of function, a mean PES score of 11.05 (SD 2.09; range 6–14) was recorded. No significant differences were found in the esthetic outcomes, categorized based on clinically relevant levels, between the baseline and follow-up session (p>0.05). Conclusion: Within limitation of the present study, it can be concluded that the esthetic outcomes of soft tissue around the maxillary anterior single-tooth implants placed using conventional implant placement technique remained stable in the long-term.

Immediate implant placement is the placement of dental implant in the extraction socket immediately after extraction with no healing of bone or soft tissues. The timing of implant placement has increased the debate, while patients have become persistent in their desire to be treated in an efficient and expedient manner. When seeking a solution, they wish for safe and effective treatment that can be performed in the shortest possible time and with as little pain and discomfort as possible. Dental professionals with their expertise are addressing patients request as much as possible. Immediate implant placement and loading are valuable techniques for the clinician to call upon, but their skill and experience, as well as case selection, are all crucial elements to ensure successful long-term results [1]. The placement of implants immediately in fresh sockets with immediate loading is a well known treatment modality, but the practitioner has to follow a very strict protocol. This case report describes the detailed procedures of immediate implant placement with immediate loading for 2 – implants supporting 4 – units’ provisional restorations with 5 years follow-up

Background: Dental problems often lead to missing teeth. After tooth extraction, the alveolar bone will undergo a healing phase, and this will cause some vertical and horizontal resorption. Immediate implant placement can shorten treatment time and preserve the rest of the alveolar bone. Purpose: The purpose of this study is to present a case of oral function, equilibrium and aesthetic rehabilitation using immediate implant placement. Case: A male patient aged 31 came to Dental Hospital Universitas Airlangga with a fractured anterior tooth. The tooth had fractured two weeks before he came to the hospital, and he wanted to improve his appearance. Case management: The mandibular incisor was fractured, and its residual root remained. The treatment plan was to undertake an immediate implant placement. The type of implant chosen was a bone level tapered implant SC roxolid® SLA Ø 2.9 mm and 10 mm long. Surgery was performed in two stages. The first stage was to extract the residual root, position the implant and apply the bone graft. The second stage was to position the healing abutment. A crown impression was made using the closed tray technique. The crown was cemented to the abutment. Conclusion: Immediate implant placement is an aesthetic means of rehabilitating a missing tooth, such as an anterior mandibular tooth.

The aim of the present study was to develop a method to study the healing process after gingival grafting and to observe the histologic results after use of the modified edentulous ridge expansion technique. A 47-year-old nonsmoking woman with a noncontributory past medical history affected by edentulism associated with a horizontal alveolar ridge defect was referred to the authors for surgical correction of the deficit to improve implant support and the final esthetics of an implant-borne prosthesis. At the 4-month follow-up visit, a biopsy was performed by a punch technique in the same sites of healing abutment connection. The tissue was elevated from the attached gingival. Clinically, the grafted tissues seemed to be attached to the bone surfaces. The histologic findings revealed dense grafted tissues, providing long-term stability to the area. No ligament or bone, characteristic for periodontal regeneration, were observed. The presence of thick attached keratinized tissue around implants may constitute a protective factor against marginal inflammation or trauma.

The aim of this article is to present a clinical case report where a female patient, 62 years old, presented with element 14 poorly positioned and mobility. The initial planning was immediate implantation with immediate provisionalization. The case described was carried out with a minimally invasive extraction, implant placement of the 3.75 x 11.5 mm (Helix, GrandMorse, Acqua, Neodent®, Brazil), immediate loading with a universal healing abutment (3.3 x 2.5 mm, Neodent®, Brazil) and a provisional cylinder of the universal healing abutment click (3.3 x 6.0 mm, Neodent®, Brasil) to approach of capture from a denture teeth and thus, the cementation of provisional implant crown. For modification of the gingival area, a subepithelial connective tissue graft was performed through the tunneling technique to improve peri-implant tissue conditioning. A 6-month postoperative follow-up was performed, showing satisfactory clinical and radiographic conditions during the most important phase of healing, and finally, a final follow-up of 12 months with the definitive cemented prosthesis. According to the literature and clinical experience, rehabilitation with immediate implant placement is one of the most challenging procedures when associated with immediate loading, where many criteria must be evaluated before the surgical phase, it means, and excellent initial planning.

Introduction. The concept of orthodontic retention is moving toward the idea that teeth will move unless retained indefinitely. However, permanent retention implies permanent supervision, and that is where reality clashes with stability. The cornerstone of Essix permanent retention is the complete delegation of responsibility to the patient. Essix retainers have nothing to adjust, but maximum collaboration of the patient is essential to achieve long-term stability. The purpose of this paper is to show the effectiveness of the thermoplastic appliance as a retainer, using a clinical case with a 10-year follow-up. Case Report. A 33-year-old male patient presented with a class II malocclusion division 2 and normal skeletal pattern. According to the patient’s desire, a treatment plan was proposed to obtain the aesthetic result of the smile, maintaining the molar and canine class II relationship. The orthodontic therapy was performed by using the Invisalign System. In this case, it was possible to appreciate a posterior occlusal stability after 10 years. Conclusion. Currently, among orthodontists, the use of removable plastic devices is gaining popularity thanks to their capability to encapsulate and retain both posterior and anterior teeth. In this article, the technical features of thermoformed retainers will be described and one clinical case with a 10-year follow-up will be presented to emphasize the effectiveness of these retainers.
