Isolated fallopian tube torsion is an extremely rare occurrence in a young female. The lady concerned presented with acute abdominal pain and the ovaries were normal on the scan with dilated fallopian tubes. On laparoscopy, it was revealed that she was suffering from fallopian tube torsion and laparoscopic salpingectomy was performed. The patient recovered well postoperatively.

Isolated tubal torsion (ITT) is a rare cause of acute abdominal pain. Preoperative diagnosis is difficult because of a lack of specific signs. Surgery is recommended to preserve the integrity of the tube. Seven patients of median age of 13 years (range 9 to 15) came to our observation for worsening abdominal pain, nausea and vomiting. On admission, all girls had blood tests and ultrasound. Laparoscopy was performed for diagnosis in all cases. The girls had one-month and one-year ultrasound and clinic follow up. In all cases diagnosis was delayed, median 66 hours after the onset of symptoms and laparoscopy showed necrosis of the Fallopian tube. In five girls a laparoscopic salpingectomy was performed. In the other two, an open salpingectomy was necessary because of pelvic adhesions. Histology showed a hemorrhagic infarction of the Fallopian tubes. At follow up all patients were asymptomatic with normal ovaries, but one ovarian cyst. In the differential diagnosis of acute abdominal pain in children or female adolescents the possibility of ITT should be considered for a conservative treatment. Laparoscopy allows for definitive diagnosis and treatment.

Solid pseudopapillary neoplasm of pancreas (SPN) is a rare entity; therefore proper diagnosis, evaluation and formulation of treatment protocols is difficult. The commonest presenting symptom is abdominal pain. This article presents a case of solid pseudopapillary neoplasm of pancreas in a 35 years old lady. A high index of clinical suspicion is necessary to suspect and diagnose SPN. Abstract Background Isolated fallopian tube torsion associated with hydrosalpinx is a rare condition in the pediatric population. We present this unusual clinical case study in a sexually inactive girl. Case presentation a12-year-old Caucasian girl presented symptoms of acute abdominal pain. Pelvic ultrasound revealed a normal looking uterus and ovaries and next to left ovary a imaging compatible with hydrosalpinx. She was discharged 48 hours later after clinical monitoring with oral analgesia and normal blood workup. At 3 weeks, she was readmitted for acute abdominal pain. Leukocytosis with left shift and raised C-reactive protein were observed. Her clinical condition worsened, and complication of the preexisting hydrosalpinx was suspected. Exploratory laparoscopy confirmed torsion of the fallopian tube. Left salpingectomy was performed. Histopathologic study confirmed a fallopian tube with hemorrhagic infarct. Conclusion Torsion of the fallopian tube must be considered in the event of acute abdominal pain. Early diagnosis and trying conservative management with a view to preserving fertility in this group of patients are essential.

Congenital malformations of female genital tract are frequently seen in Gynaecological clinics, incidence being upto 5-6% in cases of infertility. Most of these anomalies are related to uterus and vagina, abnormalities related to ovaries and fallopian tubes are of rare occurrence and the exact incidence of these anomalies are not known, only a few incidental case reports are available in literature. Here, we present a rare case report of absent mid- tubal segment of fallopian tube in a patient of infertility.

Isolated torsion of fallopian tubes is a rare cause of acute abdomen. It is a diagnosis that is often reached after much delay. This is primarily because it has no pathognomonic clinical, radiological or laboratory signs. In such cases, it is advisable to have a low threshold for diagnostic laparoscopy. We managed a middle-aged lady with acute abdomen and after initial tests were inconclusive, we proceeded to diagnostic laparoscopy without much delay and found a twisted right hydrosalpinx. We performed a right salpingectomy as her family was complete. The fallopian tube and ovary usually act as a unit and an isolated torsion of the fallopian tube is a rare occurrence with few tell-tale sign. After cross sectional imaging, we can be fairly certain of an adnexal pathology. A delay in making a diagnosis can lead to catastrophic consequences in such conditions. An early laparoscopy can be both diagnostic and therapeutic in these cases.

Benign dermoid ovarian cysts uncommonly cause symptoms in young girls. It is estimated that they constitute 0.35% of all tumors occurring in children of both sexes less than 15 years of age. Symptoms produced by the cysts may mimic other intra-abdominal disease, and in three of the four cases reported in this series the girls were initially considered to have either acute appendicitis or infection of the urinary tract. All three had a pelvic mass palpable per rectum. The fourth girl was found to have bilateral calcified densities in the ovaries when examined roentgenographically for low back pain. In three of the four children, calcified or ossified material in the pelvis was seen in roentgenograms of the lower abdomen, and this finding was considered by the radiologist to be indicative of ovarian dermoid cyst. In two patients the cysts and associated Fallopian tubes had undergone torsion with infarction, and salpingo-oophorectomies were performed. In one girl, a simple oophorectomy was carried out, and, in the fourth child with bilateral dermoids, a bilateral ovarian cystectomy with preservation of tubes and as much ovarian tissue as possible was performed. All four girls have subsequently made satisfactory progress.

Intussusception is a common cause of intestinal obstruction and colicky abdominal pain in the children, particularly infants, the commonest being the ileocolic variety with colocolic variety being a very rare entity. We present a case of colocolic intussusception in a 13-year-old boy which is otherwise seen in adults, precipitated by colonic malignancies. The patient presented with acute abdominal pain and bleeding perrectum with obstipation for 7 days. Preoperative USG abdomen was normal, and abdominal X-rays showed multiple air fluid levels. Intraoperative findings included a polypoidal growth in the descending colon as the leading point with the formation of a colo-colic intussusception. Following reduction and segmental resection, histopathology reported mucinous adenocarcinoma of colon which is also a rarity in pediatric age group. This case has been compared with previous cases reported in the literature.
