Objectives. This study is aimed at investigating the root and root canal morphology by cone beam computed tomography (CBCT) and palatal furcation groove of the buccal root by microcomputed tomography (micro-CT) of maxillary first premolars in a Chinese subpopulation. Methods. This study assessed CBCT images of 440 patients aged 14-80 years. Based on Vertucci’s classification, the number of roots and the canal configuration were determined. Forty-eight maxillary first premolars with furcation grooves were analyzed by micro-CT in patients aged 18-25 years. Results. Based on the CBCT assay, 70.22% and 29.32% of maxillary first premolars were 1 root and 2 roots, respectively. The configuration indicated statistical difference ( P < 0.05 ) between male and female patients. The most common canal type was type IV and was found in 44.32% of cases, followed by type I in 27.84%, and then type II in 20.57%. Root bifurcations had 40.13% prevalence which was distributed more in the middle third than in the cervical and the apical third. For the micro-CT study, 95.83% of the furcation groove configuration was found in the bifurcated maxillary first premolars. The length varied from 1.02 to 7.63 mm. The mean depth of this groove was 0.57 mm in the root coronal, 0.47 mm in the root middle, and 0.22 mm in the root apical level. Palatal dentin width was smaller than 1 mm. Conclusion. The anatomy of the root and root canal system and the irregular wall width of maxillary first premolars with furcation grooves may help dentists to understand the anatomical morphology and improve the outcomes of endodontic treatment.

The present study was conducted to determine the number of roots and morphology of the root canal system of permanent mandibular first molars (M1) in a Sri Lankan population. Sample of 529 M1 teeth was used. The number of roots was examined and the lengths of the mesial and distal roots were measured to the nearest 0.01 mm. Vacuum injection protocol was used to inject China ink into the root canal system, making it transparent. Root canal morphology was recorded using Vertucci’s classification. Presence of furcation canals, position of lateral canals, intercanal communications, level of bifurcation, and convergence of the root canal system were recorded. M1 showed three roots in 4.1% of the sample. Commonest root canal morphology of the mesial root was type IV and the distal root was type I. The level of bifurcation of the root canals was commonly observed in the cervical one-third of the root while convergence was observed in the apical one-third in both roots. Prevalence of three rooted mandibular first molars is less than 5%. Mesial root showed the most variable canal morphology. Prevalence of furcation canals was 1.5% while that of middle mesial canals was 0.2%.

Introduction: Complexity in root canal morphology leads to chances of failure in endodontic treatment due to missed canal. Availability of Cone-beam Computed Tomography (CBCT) has contributed to assess second mesiobuccal canals in maxillary first molars.Objective: To study the presence of second mesiobuccal (MB2) canals, classify morphology of canals according to Vertucci’s classification and to measure the distance between MB1 and MB2 canals.Materials & Method: 72 CBCT scans including 115 permanent maxillary first molars were evaluated by endodontist and oral radiologist separately using Dicom software to study the mesiobuccal root canal morphology. Oral radiologist and endodontist individually evaluated the scans through coronal, axial and oblique views to study canal configurations. Vertucci’s Classification was used to classify the canal configurations. Statistical tests were done to assess the gender variation and arch side difference.Result: Among 115 images evaluated; 30.4% samples had second mesiobuccal canal. The most common canal configuration was Type I (60.9 %) in mesiobuccal root. The mean distance between MB1 and MB2 canals was 2.39 mm in female and 2.56 mm in male.Conclusion: The occurrence of second canal in mesiobuccal root of maxillary permanent molar is considerable in Nepali patients. CBCT can be an effective tool to detect MB2 canal and other complexities of root canal system to achieve clinical success

Objective. The aim of the study was to investigate the root canal morphology of maxillary first and second premolars in a Saudi population using Cone-Beam Computed Tomography (CBCT).Methods. This retrospective cross-sectional study assessed CBCT images of 707 Saudi patients. The number of roots and canal configuration were identified based on Vertucci’s classification. Fisher’s exact Chi-square tests were performed to analyze the association between sex and number of roots and sex and root canal configuration.Results. Most teeth had two roots in maxillary first premolars (75.1%) and one root in maxillary second premolars (85.2%). Type IV was the most prevalent canal configuration in maxillary first premolars (69.1%), while Type I was the most in maxillary second premolars (49.4%). All types of canal configurations were observed in maxillary premolars except Type VII for the maxillary second premolar. Chi-square tests showed no significant association between gender and number of roots and sex and root canal configuration in both maxillary first and second premolars although higher number of roots was seen in men (P > 0.05).Conclusion. Most maxillary first premolars had two roots with Type IV being the most predominant canal configuration, while a single root with Type I canal configuration was the most frequently observed morphology in maxillary second premolars. In maxillary first premolars, 21.3% had one canal apically, 75.4% had two canals apically, and 3.3% had three canals apically. In maxillary second premolars, 80.2% had one canal apically, 18.9% had two canals apically, and 0.9% had three canals apically.

Knowledge of dental anatomy through the assessment of the anatomic variations of each tooth’s root canal system is essential to undertake endodontic therapy. The aim of this systematic review was to analyze the different studies on the internal morphology of permanent mandibular incisors where Cone-Beam Computed Tomography (CBCT) X-ray imaging is used. Pubmed, CENTRAL, Wiley Library and Web of Science electronic databases were searched for scientific studies included until March 2020. The terms used in the search were: “permanent mandibular incisors”, “root canal morphology” and “cone-beam computed tomography”. The search was limited to studies whose aim was the analysis of the morphology of the root canal system evaluating the parameters of methodology, population, sample, number and configuration. A total of 19 studies met the inclusion criteria. There was a noticeable lack of unanimity in the setting adjustments of each of the CBCT devices used. The presence of two root canals varied from 0.4% to 45%. The most frequent configurations were Vertucci’s Types I, III, II, V, IV, VII and VI. Type VIII configuration was non-existent. CBCT revealed the existence of anatomical symmetry patterns, and there was no unanimity of criteria regarding the presence of a second root canal. Results concerning the presence of a second root canal in the mandibular incisors differ widely, with a possible influence of the geographic area where the study was conducted. The prevalence of a second canal is higher in mandibular lateral incisors than in mandibular central incisors. There was no direct relationship between voxel size (0.125–0.3 mm) and increased prevalence of a second canal.

AbstractThis study aimed to analyze the root anatomy and root canal system morphology of mandibular first premolars in a Chilean population. 186 teeth were scanned using micro-computed tomography and reconstructed three-dimensionally. The root canal system morphology was classified using both Vertucci’s and Ahmed’s criteria. The radicular grooves were categorized using the ASUDAS system, and the presence of Tomes’ anomalous root was associated with Ahmed’s score. A single root canal was identified in 65.05% of teeth, being configuration type I according to Vertucci’s criteria and code 1MP1 according to Ahmed’s criteria. Radicular grooves were observed in 39.25% of teeth. The ASUDAS scores for radicular grooves were 60.75%, 13.98%, 12.36%, 10.22%, 2.15%, and 0.54%, from grade 0 to grade 5, respectively. The presence of Tomes’ anomalous root was identified only in teeth with multiple root canals, and it was more frequently associated with code 1MP1–2 of Ahmed’s criteria. The root canal system morphology of mandibular first premolars showed a wide range of anatomical variations in the Chilean population. Teeth with multiple root canals had a higher incidence of radicular grooves, which were closely related to more complex internal anatomy. Only teeth with multiple root canals presented Tomes’ anomalous root.

We aimed to analyze the morpho-geometric changes of the root canal system created by two rotary systems (TF Adaptive and BioRace) using micro-CT technology. Two concepts of rotary file system kinematics, continuous rotation and adaptive kinematics, were used in root canal preparation. Twenty mandibular molars (n = 20) were selected with the following criteria: the teeth have mesial roots with a single and continuous isthmus connecting the mesiobuccal and mesiolingual canals (Vertucci’s Type I configuration) and distal roots with independent canals. Teeth were scanned at a resolution of 14 μm. Canals were divided equally into two groups and then enlarged sequentially using the BioRace system and TF Adaptive system according to manufacturer protocol. Co-registered images, before and after preparation, were evaluated for morphometric measurements of canal surface area, volume, structure model index, thickness, straightening, and un-instrumented surface area. Before and after preparation, data were statistically analyzed using a paired sample t-test. After preparation, data were analyzed using an unpaired sample test. The preparation by both systems significantly changed canal surface area, volume, structure model index, and thickness in both systems. There were no significant differences between instrument types with respect to these parameters (p > 0.05). TF Adaptive was associated with less straightening (8% compared with 17% for BioRace in the mesial canal, p > 0.05). Both instrumentation systems produced canal preparations with adequate geometrical changes. BioRace straightened the mesial canals more than TF Adaptive.

AIM: The purpose of this study was to describe the external and internal anatomy of the mandibular premolars. MATERIAL AND METHODS: 100 extracted human permanent mandibular first and second premolars teeth were collected. The length of the tooth from the cusp tip to root apex was measured with the help of Vernier calliper. In case of curved roots, ligature wire was adapted to the root and then straightened and measured. Each tooth was examined for the number of roots, curvature of root, presence of root canal invagination. India ink was injected into the pulp chamber through the access opening with a no.27 gauge needle mounted on a disposable syringe. The ink was then drawn through the root canal system by applying negative pressure to the apical end of tooth with the use of central suction system. RESULTS: The average (mean) length of mandibular first premolar was 22.25 mm. Average (mean) length of mandibular second premolar was 21.90 mm. Mandibular first premolar had a ribbon shaped root canal orifice in 35% of teeth. Mandibular second premolar had a ribbon shaped of root canal orifice in 44% teeth. Mesial invagination of the root was found in 18% of first and 8% second mandibular premolar teeth. 78% had a Type I canal pattern with Type II, Type IV, Type V, Type VI and Type VII canals being identified in 1%, 2%, 13%, 2% and 1% of the teeth respectively. CONCLUSIONS: Complex root canal anatomy frequently found with mandibular premolars among which more common with mandibular first premolars in Indian population.

The complex root canal system of maxillary second premolars shows variations among different populations. The aim of this study was to evaluate the root canal morphology of second maxillary premolars in the romanian population. The number of roots, root canals and the root canal configuration were investigated using radiographs in both buccal-oral and mesio-distal incidence. They were categorized using Weine’s classifi cation. Among the 480 teeth, 90.83% had one root and 66.67% one root canal. The frequency of secondary maxillary premolars with two root canals (33.12%) shouldn’t be underestimated. Knowing the variations of the internal root canal morphology of maxillary second premolars in a certain population can lead the clinician to a better approach of the endodontic treatment.
