ABSTRACT Aim This clinical report presents a rare case of maxillary central incisor with two separate roots. Background Unusual morphology of the roots and root canals may exist in any tooth. Recognition of the dental anatomy and its variations is necessary for successful endodontic therapy. It is well known that maxillary incisors are usually single-rooted teeth. Case report The root canals were instrumented with conventional hand files and Gates Glidden and obturated by using the lateral technique. Recall radiograph after 1 year shows the healing process of the preoperative apical periodontitis. Conclusion and clinical significance Clinicians should be aware of unexpected root canal morphology when performing root canal therapy. ABSTRACTThis case report describes the repositioning of a laterally luxated primary central incisor with occlusal interference, using a composite inclined plane. The patient was a 4-year-old girl who applied to our clinic three days after the injury. Because of the time delay between injury and presentation, it was not possible to reposition the tooth with pressure. Following a root-canal treatment, an inclined plane was prepared on the lower primary incisors, using composite resin. The tooth was repositioned in two weeks, and the inclined plane was then removed. After 1 year of follow-up, the treatment was found to be successful, both clinically and radiographically. The use of a composite inclined plane, accompanied by careful follow-up, is an effective alternative to extraction for laterally luxated primary incisors with occlusal interference. This case report describes delayed replantation of an avulsed maxillary central incisor in a 17-year-old male patient following an injury on fall one day earlier. Avulsed maxillary right permanent central incisor was replanted back into the socket after extra-oral root canal treatment. One year follow up showed validity of treatment, with no evidence of resorption in the replanted tooth. Successful regenerative endodontic procedure was performed in nonvital immature permanent central incisor (Stage-4 root development) using human amniotic membrane (HAM) as a novel scaffold. The treatment was performed according to the American Association of Endodontics guidelines with minimal canal instrumentation, 1% Sodium hypochlorite as irrigant and calcium hydroxide as intracanal medicament. During the second appointment, HAM was placed as a scaffold and Biodentine™ was layered over the HAM with glass ionomer cement and resin composite as coronal seal. Preoperative and post-operative cone beam computed tomography (at three years) was taken to assess the treatment outcome. The resolution of disease process and increase in canal width, as well as positive response to pulp sensitivity tests, were observed by the end of three years. There was approximately 78–86% reduction in the volume of periapical lesion size. This case report confirms that HAM can be used as a scaffold material for successful regenerative endodontic procedure (REP).

Objective: This study evaluated through stereomicroscopy the effectiveness of hand and rotary instrumentation techniques, either isolated or combined, in the preparation of oval-shaped root canals. Materials and method: Thirty single-rooted human mandibular incisors were selected. After endodontic access and coronal preflaring, teeth were mounted in a modified Bramante muffle and then sectioned transversely at 3 and 6 mm from the root apex. Images of each section were made under a stereomicroscope at 30× magnification. Teeth were reassembled in the muffle and divided into three groups (n=10) according to the root canal preparation technique: GI - rotary preparation with Mtwo™ basic sequence (10/.04; 15/.05; 20/.06; 25/.06), followed by Mtwo™ complementary instruments (30/.05; 35/.04; 40/.04); GII - rotary preparation with Mtwo™ basic sequence, complemented by hand instruments (#30, #35 and #40); GIII - hand instrumentation using the conventional technique (#10 to #40). All instruments were used in brushing motion. The muffles were separated again so that new images of each section could be obtained. The following parameters were evaluated in pre- and postoperative images: root canal area; perimeter; mesiodistal (MD) and buccolingual (BL) diameters; and mesial (M) and distal (D) wall thickness. Next, the values measured in post- and preoperative images were subtracted forcomparison of experimental groups. Data were analyzed using the Kruskal-Wallis test (α=0.05). Results: No significant difference between groups was observed for all parameters. Conclusion: There was no difference among hand, rotary, or combined instrumentation; all techniques were able to increase root canal area, perimeter, and diameter in MD and BL directions. Moreover, the amount of dentin wearon proximal root canal walls was similar for all tested groups.

Introduction: The purpose of this study was to compare root and root canal width measurements between digital intraoral radiography (IOR) and micro-computed tomography (μCT). The accuracy of IOR measurements of canine mandibular molars was scrutinized to assess feasibility of developing a model to estimate animal age based on dentinal thickness. Materials and Methods: Thirty-nine canine mandibular first molars were imaged using μCT and IOR. For each tooth, the root and root canal width of the mesial and distal roots were measured by a single observer at 3 marked sites on μCT and IOR. Two different software programs were used to measure the radiographs. The radiograph measurements were compared to each other and to the μCT measurements. The μCT images were considered the anatomic reference standard for structural representation. Results: The data collected demonstrated IOR bias and variability throughout all measurement sites, with some sites being more affected than others. Neither IOR system produced unbiased measurements that closely reflected the μCT measurements consistently. The overall lack of agreement between measurements demonstrated the difficulties in developing a standardized protocol for measuring root and root canal width for the first molar teeth in dogs. Conclusion: Developing a protocol to accurately measure and compare μCT and IOR measurements is challenging. Designing a measurement system that would allow for universal application to age dogs would require continued research utilizing a standardized approach to overcome the limitations identified in this article.
