Influence of Handgrip Stabilization During Isokinetic Knee Strength Assessment in Older Women

The present study aimed to evaluate the influence of holding the chair handles during the assessment of older women’s knee flexion and extension isokinetic and isometric strength. Of 57 older women (≥60 years) selected from a convenience sample for this research, 23 were randomly chosen to perform the tests twice for reliability analyses. Each participant performed maximal isokinetic knee extension and flexion at 60°/s and 180°/s angular velocities and isometric tests under two conditions: (a) holding the lateral handle of the chair (HOLD) and (b) supporting the hands on the shoulders with the arms crossed (i.e., not holding the handle [NHOLD]). Participants performed significantly better in the HOLD compared with the NHOLD condition. Slightly lower intraclass correlation coefficients (ICC = 0.870–0.956) were noted for the HOLD than for the NHOLD condition (ICC = 0.900–0.981) so that the HOLD performance on Days 1 and 2 was different on some measures. Owing to the higher reliability coefficients in the NHOLD condition, results suggest that, when assessing knee strength in older women, the evaluee should place their hands over their shoulders with arms crossed in front of the chest (NHOLD) to maximize assessment reliability (reproducibility of performance).

Aims and Background Hypermobility is a common presentation in the community and is reported related to higher rates of injury and musculoskeletal pain, however the mechanism underpinning this relationship remains unclear. Poor proprioception in hypermobile joints has been proposed as a potential mechanism. This study aims to determine if there is a difference in proprioceptive acuity, as measured by joint position reproduction, in adults with generalised joint hypermobility. Design and methods A convenience sample of 26 university students and staff (mean age 29.23 years, range 18-47) were recruited, of which 12 participants displayed generalised joint hypermobility, and 14 did not. A laser light, mounted to the dominant thumb, was used to test joint position reproduction sense by pointing to targets using a unilateral active-active position reproduction protocol. Results/Findings Test reliability across a range of targets was poor to good (intraclass correlation coefficients ranged from 0.1163 to 0.7256), indicating significant variability between participants. No significant differences was found in absolute angle of error between generalised joint hypermobility and non-generalised joint hypermobility participants. For direction of error in relation to the proprioceptive targets, only 30° thumb extension above horizontal was found to be significantly different between the hypermobile and non-hypermobile groups, with hypermobile participants tending to underestimate distance to target. Age and sex were not correlated to thumb proprioception. Application and Conclusion The difference found in direction of error and tendency to underestimate angular distance may be protective against straying into possibly injurious end-ranges; however, larger studies are recommended to confirm this.

Context: Strength assessment is essential to prescribe exercise in sports and rehabilitation. Low-cost valid equipment may allow continuous monitoring of training. Objective: To examine the validity of a very low-cost hanging scale by comparing differences in the measures of peak force to a laboratory grade load cell during shoulder abduction, flexion, extension, and internal and external rotations. Design: Analytical study. Participants: Thirty-two healthy subjects (18 women, age 26 [10] y, height 172 [8] cm, mass 69 [13] kg, body mass index 23 [4] kg/m2). Main Outcome Measures: The dependent variable was the maximal peak force (in kilogram-force). The independent variable was the instrument (laboratory grade load cell and hanging scale). Results: No differences were observed while comparing the results. The intraclass correlation coefficients1,1 ranged from .96 to .99, showing excellent results. The Cronbach alpha test also returned >.99 for all comparisons. The SEM ranged from 0.02 to 0.04 kgf, with an averaged SD from 0.24 to 0.38 kgf. The correlation was classified as high for all tested movements (r > .99; P < .001), with excellent adjusted coefficients of determination (.96 < r2 < .99). Bland–Altman results showed high levels of agreement with bias ranging from 0.27 to 0.48. Conclusions: Hanging scale provides valid measures of isometric strength with similar output measures as laboratory grade load cell.

This investigation evaluated the use of OMNI–Resistance Exercise Scale ratings of perceived exertion as a method for selecting training intensities that are appropriate for improving muscular fitness. 20 women ( M = 21.3 yr., SD = 2.2) completed a one-repetition maximum for the chest press and knee extension. Participants returned for three sessions during which they were asked to select resistances which elicited the feelings of exertion associated with ratings of 3 (i.e., Easy to Somewhat easy), 6 (i.e., Somewhat hard) and 9 (i.e., Hard to Extremely hard). The average percentage of maximum lifted at a 3, 6, and 9 was 50, 69, and 88% for the chest press and 56, 74, and 90% for the knee extension, respectively. Effect sizes indicated large differences across the three ratings of perceived exertion within sessions and small differences between sessions. Intraclass correlation coefficients ranged from .69 to .95. The results indicated that OMNI–Resistance Exercise Scale ratings of 3, 6, and 9 can be used to accurately and reliably select intensities that are appropriate for improving muscular fitness.

Abstract Context: Insufficient lower extremity strength may be a risk factor for lower extremity injuries such as noncontact anterior cruciate ligament tears. Therefore, clinicians need reliable instruments to assess strength deficiencies. Objective: To assess the intrarater, interrater, intrasession, and intersession reliability of a portable fixed dynamometer in measuring the strength of the hip and knee musculature. Design: Crossover study. Setting: Sports medicine research laboratory. Patients or Other Participants: Three raters (A, B, C) participated in this 2-phase study. Raters A and B tested 11 healthy college graduate students (2 men, 9 women) in phase 1. Raters A and C tested 26 healthy college undergraduate students (7 men, 19 women) in phase 2. Main Outcome Measure(s): The dependent variables for the study were hip adductor, hip abductor, hip flexor, hip extensor, hip internal rotator, hip external rotator, knee flexor, and knee extensor peak force. Results: The phase 1 intrasession intraclass correlation coefficients for sessions 1, 2, and 3 ranged from 0.88 to 0.99 (SEM  =  0.08–3.02 N), 0.85 to 0.99 (SEM  =  0.26–3.88 N), and 0.92 to 0.96 (SEM  =  0.52–2.76 N), respectively. Intraclass correlation coefficients ranged from 0.57 to 0.95 (SEM  =  1.72–13.15 N) for phase 1 intersession values, 0.70 to 0.94 (SEM  =  1.42–9.20 N) for phase 2 intrarater reliability values, and 0.69 to 0.88 (SEM  =  1.20–8.50 N) for phase 2 interrater values. Conclusions: The portable fixed dynamometer showed good to high intrasession and intersession reliability values for hip and knee strength. Intrarater and interrater reliability were fair to high, except for hip internal rotation, which showed poor reliability.

Twelve university females were studied to determine the reliability of four different methods of calculating concentric and eccentric peak torque (PT) and angle-specific torques (ASTs) for knee extension. Each subject was tested on the Kin-Corn isokinetic dynamometer on two separate occasions, performing five concentric and eccentric contractions at 60° • s−1. PT and AST at 30°, 60°, and 75° were calculated by averaging the first three contractions, averaging all five contractions, taking the single best value of the first three contractions, and taking the single best value of all five contractions. Intraclass correlation coefficients derived from these calculations showed high correlation among the four methods. Additionally, z tests performed on correlation coefficients transformed to Fisher's Z revealed no differences between pairs of correlation coefficients. These data appear to show there is no difference among the four methods of calculating PT and AST.

This study examined the intratester and intertester reliability of an active-knee-extension test (AKET) for determining hamstring muscle length (flexibility). Three testers performed repeated AKET measurements on 22 subjects. Intraclass correlation coefficients (ICC were used to calculate intratester and intertester reliability. Also, standard error of measurements (SEM) were calculated. The ICC and SEM were .96 and 1.82°, respectively, for Tester 1, .99 and 1.75° for Tester 2, and .99 and 1.80° for Tester 3. Intratester 95% confidence intervals ranged from 60.54 to 69.82°. Intertester ICC and SEM for two testers were .93 and 4.81°, respectively. A 95% intertester confidence interval ranged from 56.35 to 75.21 °; this reveals that intertester AKET values contained more error and suggests that only intratester AKET values should be used when comparing hamstring flexibility values. The AKET may provide a more accurate method for determining hamstring flexibility and quantifying changes that occur as a result of injury and stretching procedures.

This study examines whether observers reliably categorize selected speech production behaviors in hearing-impaired children. A group of experienced speech-language pathologists was trained to score the elicited imitations of 5 profoundly and 5 severely hearing-impaired subjects using the Phonetic Level Evaluation (Ling, 1976). Interrater reliability was calculated using intraclass correlation coefficients. Overall, the magnitude of the coefficients was found to be considerably below what would be accepted in published behavioral research. Failure to obtain acceptably high levels of reliability suggests that the Phonetic Level Evaluation may not yet be an accurate and objective speech assessment measure for hearing-impaired children.

The aims of this study were to (I) determine the differences and relationship between the overhead press and split jerk performance in athletes involved in weightlifting training, and (II) explore the magnitude of these differences in one-repetition maximum (1RM) performances between sexes. Sixty-one men (age: 30.4 ± 6.7 years; height: 1.8 ± 0.5 m; body mass 82.5 ± 8.5 kg; weightlifting training experience: 3.7 ± 3.5 yrs) and 21 women (age: 29.5 ± 5.2 yrs; height: 1.7 ± 0.5 m; body mass: 62.6 ± 5.7 kg; weightlifting training experience: 3.0 ± 1.5 yrs) participated. The 1RM performance of the overhead press and split jerk were assessed for all participants, with the overhead press assessed on two occasions to determine between-session reliability. The intraclass correlation coefficients (ICC) and 95% confidence intervals showed a high reliability for the overhead press ICC = 0.98 (0.97 – 0.99). A very strong correlation and significant differences were found between the overhead press and split jerk 1RM performances for all participants (r = 0.90 [0.93 – 0.85], 60.2 ± 18.3 kg, 95.7 ± 29.3 kg, p ≤ 0.001). Men demonstrated stronger correlations between the overhead press and split jerk 1RM performances (r = 0.83 [0.73-0.90], p ≤ 0.001) compared with women (r = 0.56 [0.17-0.80], p = 0.008). These results provide evidence that 1RM performance of the overhead press and split jerk performance are highly related, highlighting the importance of upper-limb strength in the split jerk maximum performance.

AbstractThe lack of a Swedish patient-reported outcome instrument for eosinophilic esophagitis (EoE) has limited the assessment of the disease. The aims of the study were to translate and validate the Eosinophilic Esophagitis Activity Index (EEsAI) to Swedish and to assess the symptom severity of patients with EoE compared to a nondysphagia control group. The EEsAI was translated and adapted to a Swedish cultural context (S-EEsAI) based on international guidelines. The S-EEsAI was validated using adult Swedish patients with EoE (n = 97) and an age- and sex-matched nondysphagia control group (n = 97). All participants completed the S-EEsAI, the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Oesophageal Module 18 (EORTC QLQ-OES18), and supplementary questions regarding feasibility and demographics. Reliability and validity of the S-EEsAI were evaluated by Cronbach’s alpha and Spearman correlation coefficients between the domains of the S-EEsAI and the EORTC QLQ-OES18. A test–retest analysis of 29 patients was evaluated through intraclass correlation coefficients. The S-EEsAI had sufficient reliability with Cronbach’s alpha values of 0.83 and 0.85 for the “visual dysphagia question” and the “avoidance, modification and slow eating score” domains, respectively. The test–retest reliability was sufficient, with good to excellent intraclass correlation coefficients (0.60–0.89). The S-EEsAI domains showed moderate correlation to 6/10 EORTC QLQ-OES18 domains, indicating adequate validity. The patient S-EEsAI results differed significantly from those of the nondysphagia controls (p < 0.001). The S-EEsAI appears to be a valid and reliable instrument for monitoring adult patients with EoE in Sweden.

Abstract Aim To develop a method for diagnosing left ventricular (LV) hypertrophy from cardiac perfusion 15O-water positron emission tomography (PET). Methods We retrospectively pooled data from 139 subjects in four research cohorts. LV remodeling patterns ranged from normal to severe eccentric and concentric hypertrophy. 15O-water PET scans (n = 197) were performed with three different PET devices. A low-end scanner (66 scans) was used for method development, and remaining scans with newer devices for a blinded evaluation. Dynamic data were converted into parametric images of perfusable tissue fraction for semi-automatic delineation of the LV wall and calculation of LV mass (LVM) and septal wall thickness (WT). LVM and WT from PET were compared to cardiac magnetic resonance (CMR, n = 47) and WT to 2D-echocardiography (2DE, n = 36). PET accuracy was tested using linear regression, Bland–Altman plots, and ROC curves. Observer reproducibility were evaluated using intraclass correlation coefficients. Results High correlations were found in the blinded analyses (r ≥ 0.87, P < 0.0001 for all). AUC for detecting increased LVM and WT (> 12 mm and > 15 mm) was ≥ 0.95 (P < 0.0001 for all). Reproducibility was excellent (ICC ≥ 0.93, P < 0.0001). Conclusion 15O-water PET might detect LV hypertrophy with high accuracy and precision.
