Malignant melanoma of the nose and paranasal sinuses can be a devastating disease, typically presenting at an advanced stage, with a 5-year survival rate ranging between 20 and 30%. It is an uncommon process, often misdiagnosed both clinically and pathologically. We present the case of an 80-year-old man who had a 2-month history of progressively worsening left-sided epistaxis and nasal obstruction. Radiographic evidence indicated the presence of soft tissue in the left maxillary sinus and nasal cavity resembling massive nasal polyposis and chronic fungal sinusitis. Magnetic resonance imaging was not performed because the patient had a pacemaker. After endoscopic debridement of the soft-tissue mass, frozen-section analysis detected no evidence of tumor. The final pathologic diagnosis was malignant melanoma. Otolaryngologists should be familiar with the difficulties inherent in the diagnosis and management of sinonasal melanomas.

Inflammatory pseudotumor (IPT) is a rarely occurring lesion with no identifiable local or systemic cause. First described in 1905 by Birch-Hirschfield,1 it remains somewhat of an enigmatic disease entity despite multiple otolaryngologic, radiologic, and pathologic reports. The term “pseudotumor” was used because these lesions mimic invasive malignant tumors, both clinically and radiologically. IPT most commonly involves the lung and orbit, but has also been reported to occur at sites that make biopsy or excision difficult or potentially disfiguring.2 Its diagnosis and prompt recognition may help avoid radical surgery for this benign lesion.     CASE               A 27-year-old male was seen at our outpatient department due to a progressively enlarging left infraorbital mass. Two years prior, the patient noted a swelling over his left infraorbital area. The swelling was somewhat painful and rapidly grew in size so that it measured almost 2.5x2.5cm after a week. Still tender, it became firm and violaceous in color. He sought medical attention at a local hospital after one more week of persistent swelling and increasing cheek pain, but denied excessive lacrimation, blurring of vision, orbital pain, eye discharge or numbness.               Incision and drainage of the left infraorbital mass drained purulent material with resolution of the swelling and associated symptoms, but a pea-sized mass was still palpable over the post operative site.  Over the months that followed, the mass gradually increased in size, with occasional serosanguinous discharge from the incision site. There was no pain, numbness or blurring of vision.  He self-medicated with Cefalexin, taken irregularly for 8 months without any improvement, before finally consulting again.               An orbital CT scan requested by the referring Ophthalmology service showed an expansile, mildly enhancing soft tissue mass with few peripheral foci of calcifications measuring 8.2 x 4.4 x 6.4 cm (Figures 1 A, B) completely occupying the left maxillary sinus and extending up to the infero-lateral aspect of the left orbital cavity. There was erosion of the lateral portion of the left orbital floor and disruption of the frontal process of the left zygomatic bone with obliteration and effacement of the left pterygopalatine fossa.   Our physical examination revealed a firm, fixed, nontender 4x4cm left inferior orbital mass with serosanguinous discharge, and a bulging lateral nasal wall. Epiphora from the left eye suggested nasolacrimal duct obstruction, but vision and extraocular movements were intact.               Caldwell-Luc biopsy surprisingly yielded only necrotic and inflammatory tissues despite generous samples from multiple sections of the maxillary portion, and inflammatory polyps from the intranasal component.  At surgery after a few weeks, the mass still occupied the entire left maxillary sinus despite the previous biopsy which had removed a significant amount of tumor. Furthermore, the mass now extended beyond the maxillary sinus into the left upper gingivobuccal area thru the previous maxillary window. The entire clinically aggressive maxillary sinus mass was removed under endoscopic guidance, but the final histopathology report was still similar to the previous findings of necrotic and inflammatory tissues.               A month after surgery, the patient was seen at the Emergency room for left infraorbital swelling and discharge. Contrast-enhanced MRI of the nasopharynx showed a large expansile left maxillary sinus lesion bulging into the nasal cavity, extending into adjacent lateral orbital soft tissue and extending into the buccal space through an apparently disrupted left inferolateral maxillary wall. Intravenous antibiotics and a high-dose steroid trial resulted in complete disappearance of the left infraorbital mass and discharge within a week, and the patient was discharged on a tapering steroid dose.     DISCUSSION               Inflammatory pseudotumor is a quasi-neoplastic lesion that has been reported to occur in nearly every site in the body, most commonly involving the lung and the orbit, and rarely the maxillary sinus1. Its diagnosis is usually by exemption since clinical and histopathologic findings are sometimes vague and inconsistent. The exact etiology of these lesions is not clear. It has been postulated that they might be the result of a post-inflammatory repair process, a metabolic disturbance, or an antigen-antibody interaction with an agent that was no longer identifiable in aspiration or biopsy material.3 The clinical findings in a patient with an inflammatory pseudotumor are variable, depending on the growth rate of the lesion and the specific structures that have been affected. Inflammatory pseudotumors have been reported to cause chronic cough (as a result of endobronchial growth), dry cough, fever, pleuritic pain, right upper quadrant or epigastric pain,  and several constitutional symptoms, such as malaise, weight loss, fatigue, and syncope. Inflammatory pseudotumors have been found incidentally during imaging examinations for other reasons.3 Extraorbital inflammatory pseudotumor of head and neck can occur in the nasal cavity, nasopharynx, maxillary sinus, larynx and trachea. Perineural spread along maxillary, mandibular and hypoglossal nerves had been described. Sinonasal inflammatory psuedotumors do not affect a particular age group and cause no systemic symptoms.  However, they have a more aggressive appearance than those of the orbit, with bony changes such as erosion, remodelling and sclerosis usually seen on radiographic studies.4 On CAT scans, a moderately enhancing soft tissue mass is usually seen, accompanied by bony changes common among malignant processes.5 On cut sections, inflammatory cells dominate as well as necrotic tissues. In some patients, laboratory findings are normal; in others, there might be an elevated erythrocyte sedimentation rate and C-reactive protein level and sometimes a high white blood cell count3. However, none of the published reports on inflammatory pseudotumor have mentioned any presence of positive tumor markers. Complete surgical resection if possible is the treatment of choice for sinonasal inflammatory pseudotumors, followed by corticosteroids in cases of incomplete excision.  Response to steroids is often unpredictable, but these drugs are the primary treatment method for orbital inflammatory pseudotumor. The only cases in which radiation therapy is indicated are those patients for whom surgery or corticosteroid therapy is unsuccessful or contraindicated.6

Allergic fungal sinusitis is a noninvasive disease first recognized approximately one decade ago. It accounts for approximately 6% to 8% of all chronic sinusitis requiring surgical intervention and has become a subject of increasing interest to otolaryngologists and related specialists. Although certain signs and symptoms, as well as radiographic, intraoperative, and pathologic findings, may cause the physician to suspect allergic fungal sinusitis, no standards have been defined for establishing the diagnosis. It is extremely important to recognize allergic fungal sinusitis and differentiate it from chronic bacterial sinusitis and other forms of fungal sinusitis because the treatments and prognoses for these disorders vary significantly. To delineate a set of diagnostic criteria, we prospectively evaluated our most recent 15 patients with allergic fungal sinusitis. An allergy evaluation confirmed atopy through a strong history of inhalant mold allergies, an elevated total immunoglobulin E level, or a positive result of a skin test or radioallergosorbent test to fungal antigens in 100% of patients. All 15 patients had nasal polyposis, and 8 of 15 had asthma. There was a unilateral predominance in 13 of 15 cases. A characteristic computerized tomography finding of serpiginous areas of high attenuation in affected sinuses was seen in all patients, and 12 of 15 patients had some degree of radiographic bone erosion. Pathologic examination uniformly revealed eosinophilic mucus without fungal invasion into soft tissue; Charcot-Leyden crystals and peripheral eosinophilic were each observed in 6 of 15 patients. Every patient had fungus identified on fungal smear, although only 11 of 15 fungal cultures were positive. Therefore, for the diagnosis of allergic fungal sinusitis to be established, the following criteria should be met: (1) type I hypersensitivity confirmed by history, skin tests, or serology; (2) nasal polyposis; (3) characteristic computed tomography signs; (4) eosinophilic mucus without fungal invasion into sinus tissue; and (5) positive fungal stain of sinus contents removed during surgery. Radiographic bone erosion does not necessarily imply invasive disease, and a positive fungal culture, although desirable, is not necessary to confirm the diagnosis. Unilateral predominance of disease, a history of asthma, Charcot-Leyden crystals, and peripheral eosinophilla corroborate the diagnosis but are not always present. Perhaps because of the novelty of the disease, much misunderstanding surrounds allergic fungal sinusitis. Misdiagnosis is common, recurrence rates are high, and proper treatment remains elusive. Before proceeding with other advances, a common understanding of the diagnosis of allergic fungal sinusitis is mandatory.

Clear cell sarcoma is a rare malignant neoplasm usually presenting as soft tissue mass in foot of young adult. Ultrastructural studies reveal neuroectodermal origin with melanocytic differentiation. Thus it is considered as malignant melanoma of soft parts. Though the prognosis is apparently better as compared to the conventional melanoma, local recurrences and metastases are frequently seen. Large tumor size and extent of necrosis are factors for poor prognosis. A case of clear cell sarcoma with classical presentation is reported.

Abstract.Donnellan R, Moodley M. Vulval myxoid liposarcoma.A 26 year old woman presented with a 4-year history of a gradually enlarging fluctuant mass on the left labium majus. Histologic examination following excision revealed myxoid liposarcoma. Following the diagnosis, further surgery was performed to ensure complete resection. Routine excision of draining lymph nodes is not advocated. Although rare, myxoid liposarcoma should be considered in the differential diagnosis of a vulval soft tissue mass.

Incidental soft tissue lumps in the scalp are a common presenting complaint in clinical practice. However, they may signify more sinister underlying pathologies. Our report examines a 63-year-old man presenting with impaired co-ordination in his left hand following a 3-month history of a painless left retroauricular scalp lump. MRI revealed a large left occipital soft tissue mass eroding through the underlying skull with infiltration into the underlying cerebellum and temporal lobe. Open biopsy confirmed a diagnosis of high-grade intracranial neuroendocrine tumour (NET). At approximately 5 months following successful tumour resection and adjuvant chemotherapy, he developed tumour recurrence and was subsequently palliated, and died at 1 year post diagnosis. Herein, we review other cases of primary intracranial NET, clinical findings, histopathological features and prognosis.

We present a case of a 59-year-old male veteran with a chronic history of right foot soft-tissue mass that was causing pain in his shoes, but not functional or neurologic symptoms. Excision of the mass and pathologic evaluation resulted in multidisciplinary involvement and evaluation. In this example, the mass was found to be either an unusual schwannoma that happens to be psammoma body–rich or an unusual psammomatous melanocytic schwannoma that deviates from conventional examples, as a diagnostic consensus was unable to be reached. Schwannomas are an uncommon finding in the foot, and even more uncommon are those that contain psammoma bodies. Although rare, these tumors can be concerning for more systemic, life-altering diseases such as Carney complex, for which our patient refused genetic testing.

Sarcoidosis of the parathyroid gland is a rare occurrence. Parathyroid sarcoidosis is usually associated with parathyroid adenomas, and, therefore, hypercalcaemia is a common presentation of this entity. We present a case of parathyroid sarcoidosis and review the world literature regarding this rare condition. A woman with a history of diffuse large B cell lymphoma underwent a surveillance positron emission tomography scan that showed increased fluorodeoxyglucose uptake in multiple thoracic and abdominal lymph nodes and in a left upper extremity soft tissue mass. Biopsy of the soft tissue mass showed non-caseating granulomas consistent with sarcoidosis. Blood work showed a serum calcium of 11.1 mg/dL with an intact serum parathyroid hormone of 92 pg/dL. Primary hyperparathyroidism was suspected. A neck ultrasound and sestamibi parathyroid scintigraphy demonstrated a parathyroid nodule. She underwent surgical resection, and the histopathology revealed a parathyroid adenoma and non-caseating granulomata consistent with a diagnosis of sarcoidosis.

Case summary A 7-year old male neutered domestic shorthair cat was presented with a 2 month history of a slow-growing mass on the right zygomatic area. A CT scan revealed a soft tissue mass in the right zygomatic region with no alterations of the underlying bone and features of local invasiveness. Cytology was suggestive of a mesenchymal tumour and histopathology from an incisional biopsy was consistent with a soft tissue sarcoma (STS). The cat was treated with neoadjuvant intravenous doxorubicin chemotherapy at a dose of 25 mg/m2, every two weeks. The patient experienced a partial response and underwent surgical excision of the tumour. Doxorubicin was continued as an adjuvant treatment for three further chemotherapy sessions, at a dose of 25 mg/m2 every 21 days. Local tumour recurrence was detected on clinical examination and cytologically confirmed 259 days following surgery. Relevance and novel information Treatment with neoadjuvant doxorubicin can be considered in cases of inoperable STSs in order to cytoreduce the tumour and improve the chances of achieving complete surgical margins. The role of adjuvant chemotherapy in this setting remains unclear.

A total of 329 patients with osteoblastoma were retrospectively reviewed from the archives of the Armed Forces Institute of Pathology, of which 41 (12.5%) presented with tumors in the foot and ankle. This was the third most common site of disease after the spine and femur. Overall, the mean age was 22.5 years, which was the same for the foot and ankle subset of patients; however, there was a significant male predominance in foot and ankle patients compared with the whole group. The majority of patients were skeletally mature (85.4%). Clinically, most patients presented with pain (97.2%), although one-third of the total related a history of antecedent trauma. The interval between the onset of symptoms and biopsy was 84 days (range, 0–572 days). Radiographically, the majority of lesions were in the hindfoot (N = 18; 44%) of which 16 of 18 tumors (89%) were in the talus. Of these, one-half were subperiosteal and dorsally based and were associated with osseous tumor matrix and a soft tissue mass. Two osteoblastomas, both in the metatarsals, transitioned into sarcomas; the rest were histologically benign. For diagnostic purposes, it was essential to obtain clinical, radiographic, and histologic correlation.
