We explored the potential of asthma apps to support self-management and identified preferred features that enable users to live with asthma. We recruited patients from five UK practices and social media; observed their usage of our app, administered a questionnaire and interviewed a purposive sample of patients and professionals to explore preferred features. Thematic analysis of interview was synthesised with quantitative data. A total of 111 patients used our app for 3 months. We interviewed 15 patients and 16 professionals. Participants were interested in a broad range of self-management support strategies, including action plans, monitoring with feedback, allergy/weather warnings and tailor-made running coaching. Professionals wanted to integrate patients’ logs with practice records, though were concerned about data overload and risk of patient dependency. We propose a paradigm shift - from apps developed to provide features that are easy to implement technologically, to an approach in which apps are designed to deliver theoretically grounded preferred components.

IntroductionImproving the quality of self-management support (SMS) for treatment-related toxicities is a priority in cancer care. Successful implementation of SMS programmes depends on tailoring implementation strategies to organisational readiness factors and barriers/enablers, however, a systematic process for this is lacking. In this formative phase of our implementation-effectiveness trial, Self-Management and Activation to Reduce Treatment-Related Toxicities, we evaluated readiness based on constructs in the Consolidated Framework for Implementation Research (CFIR) and Normalisation Process Theory (NPT) and developed a process for mapping implementation strategies to local contexts.MethodsIn this convergent mixed-method study, surveys and interviews were used to assess readiness and barriers/enablers for SMS among stakeholders in 3 disease site groups at 3 regional cancer centres (RCCs) in Ontario, Canada. Median survey responses were classified as a barrier, enabler or neutral based on a priori cut-off values. Barriers/enablers at each centre were mapped to CFIR and then inputted into the CFIR-Expert Recommendations for Implementing Change Strategy Matching Tool V.1.0 (CFIR-ERIC) to identify centre-specific implementation strategies. Qualitative data were separately analysed and themes mapped to CFIR constructs to provide a deeper understanding of barriers/enablers.ResultsSMS in most of the RCCs was not systematically delivered, yet most stakeholders (n=78; respondent rate=50%) valued SMS. For centre 1, 7 barriers/12 enablers were identified, 14 barriers/9 enablers for centre 2 and 11 barriers/5 enablers for centre 3. Of the total 46 strategies identified, 30 (65%) were common across centres as core implementation strategies and 5 tailored implementation recommendations were identified for centres 1 and 3, and 4 for centre 2.ConclusionsThe CFIR and CFIR-ERIC were valuable tools for tailoring SMS implementation to readiness and barriers/enablers, whereas NPT helped to clarify the clinical work of implementation. Our approach to tailoring of implementation strategies may have relevance for other studies.

ObjectiveThere is a gap of knowledge among healthcare providers on characteristics of self-management among patients with chronic diseases and complex healthcare needs. Consequently, the objective of this paper was to identify characteristics of self-management among patients with chronic diseases and complex healthcare needs.DesignThematic analysis review of the literature.MethodsWe developed search strategies for the MEDLINE and CINAHL databases, covering the January 2000–October 2018 period. All articles in English or French addressing self-management among an adult clientele (18 years and older) with complex healthcare needs (multimorbidity, vulnerability, complexity and frequent use of health services) were included. Studies that addressed self-management of a single disease or that did not have any notion of complexity or vulnerability were excluded. A mixed thematic analysis, deductive and inductive, was performed by three evaluators as described by Mileset al.ResultsTwenty-one articles were included. Patients with complex healthcare needs present specific features related to self-management that can be exacerbated by deprived socioeconomic conditions. These patients must often prioritise care based on one dominant condition. They are at risk for depression, psychological distress and low self-efficacy, as well as for receiving contradictory information from healthcare providers. However, the knowledge and experiences acquired in the past in relation to their condition may help them improve their self-management skills.ConclusionsThis review identifies challenges to self-management for patients with complex healthcare needs, which are exacerbated in contexts of socioeconomic insecurity and proposes strategies to help healthcare providers better adapt their self-management support interventions to meet the specific needs of this vulnerable clientele.

Disclaimer labels on airbrushed media images have generated political attention and advocacy as a social policy approach to promoting positive body image. Experimental research suggests that labelling is ineffective and consumers’ viewpoints have been overlooked. A mixed-method study explored British consumers’ ( N = 1555, aged 11–78 years) opinions on body image and social policy approaches. Thematic analysis indicated scepticism about the effectiveness of labelling images. Quantitatively, adults, although not adolescents, reported that labelling was unlikely to improve body image. Appearance diversity in media and reorienting social norms from appearance to function and health were perceived as effective strategies. Social policy and research implications are discussed.

Background: Medical humanities is taught in medical schools in the  western countries, partly to address the lack of compassion within healthcare. It seeks to develop understanding of human experiences relating to disease, disability and death, through humanities, arts and social sciences. In 2018, Patan Academy of Health Sciences Nepal introduced an eight-week medical humanities course for new medical students. This study aims to evaluate the course from the student participants’ view, exploring their perceptions and experiences. Methods: A mixed method study was used to assess the perceptions of  65 students who completed a semi-structured survey, comprising eight items, with five point- Likert scale and three open response questions. Quantitative data was analysed with results expressed as mean, standard deviation and percentage. Qualitative data was coded and analysed thematically. Results: The students’ perception of the course was strongly positive where 98.5% agreed or strongly agreed it was enjoyable and interesting (items 1,2) and should be continued (item 9). 97% agreed or strongly agreed it made them think differently (item 5) and 96.9% that it was relevant to future careers (item 6). 96.9% agreed or strongly agreed the course helped them understand doctor’s caring roles (item 10) and 92.3% believed it will make them better doctors (item 11). Three themes emerged from open response questions related to perceptions- enjoyable and interesting, positive personal impact and valuable and important. Conclusions: The students had positive perceptions of the medical humanities course, recognizing its’ impact, importance and its value in medical education. This study observed challenges faced by Ward Officers (WOs) in practicing accountability and participation in the wards in Arusha city. The study adopted descriptive research design and used mixed method research approach. The population involved 175 ward officers in 25 wards in Arusha city from which a sample of 70 ward officers was drawn, but only 68 (97%) of them participated. Purposive sampling and simple random technique were used to obtain respondents. Data was collected through questionnaires and interview guide. Quantitative data were analysed using descriptive statistics, while qualitative data were subjected to thematic analysis. Findings revealed that challenges faced by WOs in practicing accountability and participation in their wards includes: little turn up of citizens in political election, lack of interests to participate in decision making, misuse of resources and environmental pollution, corruption and insufficient funds. The study recommended that local government authorities should educate its people on the importance of participating on different development activities from planning to implementation, and WOs should ensure ward information are accessible by the public.

The report presents both quantitative and qualitative date indicating factors affecting use of female condom among youths in Migori County, Kenya. Quantitative data was analyzed using SPSS version 23 and qualitative data was analyzed using N- Vivo version 11. Tables were used to present the results. The purpose of this study was to investigate whether a sense of community could be enhanced via learning activities delivered through a Facebook group within a self-paced distance-learning course. A closed Facebook group was created for a voluntary English as a foreign language course. Four types of learning activities were delivered via the Facebook group including both syn-chronous and asynchronous structured activities, and synchronous and asyn-chronous unstructured activities. The study was designed as convergent mixed-method study. Quantitative data were gathered from a total of 179 students by using the Classroom Community Scale; quantitative data were concurrently gathered from 17 students via semi-structured interviews. The results revealed that the sense of community among learners was found to be high, however, the type of learning activities had no effect on building a sense of community. The study was intended to guide teachers, instructors, administrators and practitioners interested in designing and delivering self-paced open and distance learning courses.

Background. Cancer survivors must manage a range of adverse symptoms and side effects postdiagnosis. These effects often co-occur with preexisting comorbid conditions. Recognizing the complex chronicity of the disease, self-management support interventions have been developed to promote cancer survivors’ knowledge, skills, and confidence to self-manage their health. Though shown to be beneficial, the processes underlying self-management have yet to be explicated. Aim. To explore how a community-based self-management support intervention fosters cancer survivors’ knowledge, confidence, and skills to self-manage their health. Method. A qualitative case study adopting multiple viewpoints was utilized. Seventeen cancer survivors who participated in a self-management support intervention (referred to as a cancer coaching program) were interviewed and six program staff took part in a focus group. Transcripts from the interviews and focus group were analyzed using a hybrid inductive–deductive approach guided by principles of qualitative description. Results. Cancer survivors and program staff offered complementary perspectives that enhanced our understanding of how the self-management support intervention fosters cancer survivors’ knowledge, confidence, and skills to self-manage their health. Four themes captured strategies necessary to promote self-management: (1) looking beyond the disease: the importance of holistic person-centered care, (2) cocreating plans: the key to effective and meaningful self-management support, (3) fostering activation via tailored, targeted, and expert-sourced information and resources, and (4) having dependable and impartial emotional support. Within each theme, pertinent self-management support strategies were described by cancer survivors and program staff. Discussion and Conclusion. Findings offer an understanding of how one community-based self-management support intervention promotes self-management and highlight valuable self-management support strategies that could be incorporated into existing and future interventions.

Abstract Burn disaster is defined as a massive influx of patients that exceeds a burn center’s capacity and capability. This study investigates the capacity and capability of burn centers to respond to burn disasters in the Belgian ground. Quantitative survey and qualitative semistructured interview questionnaires were administered directly to key informants of burn centers. The data collected from both methods were compared to get a more in-depth overview of the issue. Quantitative data were converted into a narrative to enrich the qualitative data and included in the thematic analysis. Finally, data from both methods were analyzed and organized into five themes. The Belgian Association of Burn Injury (BABI) has a specific prehospital plan for burn disaster management. Once the BABI Plan is activated, all burn centers respond as one entity. Burn Team (B-Team) is a professional team that is formed in case of urgent need and it is deployed to a scene or to nonburn specialized hospitals to help in disaster relief. The challenges for burn disasters response occur particularly in the area of triage, transfer, communication, funding, and training. We conclude that there is a variation in the capacity and capability of burn centers. Overall, the system of burn disaster management is advanced and it is comparable to other high-income countries. Nevertheless, further improvement in the areas of preparation, triage, communication, and finally training would make disaster response more resilient in the future. Therefore, there is still space for further improvement of the management of burn disasters in Belgium.
