A Community Partnered Approach to Promoting COVID-19 Vaccine Equity

The coronavirus disease 2019 (COVID-19) pandemic has disproportionately affected communities of color. To dismantle these disparities, it is critical to promote COVID-19 vaccine equity, both through increasing vaccine access and addressing vaccine mistrust. This article describes a community–academic collaboration (the Community Vaccine Collaborative [CVC]), whose mission is to ensure COVID-19 vaccine equity among marginalized communities. Based in Pittsburgh, Pennsylvania, our group has focused on inclusion of marginalized groups into vaccine clinical trials, addressing vaccine mistrust, and building systems to ensuring equitable access to the COVID-19 vaccine. We review formation of the CVC, activities to-date, and recommendations for other communities interested in developing similar collaboratives.

Democratic decentralization is the process of devolving the state’s roles and resources from the centre to the lower-level elected officials to encourage greater direct participation of people in governance. The 73rd Constitutional Amendment Act (1993), which gave the constitutional mandate to the Panchayati Raj Institutions in India, is considered as an opportunity for historically marginalized groups like women, Dalits (Scheduled Caste) and others to actively engage in the grassroot governance as members of local government institutions. Further, gram panchayats have been mandated for formulating their own annual development plan for economic development and social justice in their area. Although in these 25 years, since the enactment of the Act, various measures have been introduced through Panchayati Raj Institutions for increasing people’s participation and effective implementation of developmental programmes, but there is a little or no evidences—either the theoretical or the empirical, which can give a hint on its performance on social and political inclusion of marginalized communities. Against this backdrop, this empirical research paper attempts to explore the linkages between the democratic decentralization and socio-political inclusion of marginalized, by focussing on the Musahar community in Bihar state of India, using primary data collected from the field. The researchers argue that decentralization has ameliorated the social inclusion to some extent but shows very dismal rate of political inclusion of the Musahar Community.

Drawing from my analysis of sex worker and homeless protests as well as my experience doing ethnographic research with people experiencing homelessness and people in the sex trade, I put forth recommendations for ethical, policy-relevant research with groups of people who experience routine, normalized violence, and who are frequently silenced and misrepresented by academics and policy makers. This article analyzes protests against what activists identify as oppressive knowledge production by “outsiders” who are not sex workers or homeless. Protest events against research “about us without us” occurred between 2012 and 2015, and targeted academic researchers and policymakers. I draw lessons from marginalized groups’ protests against knowledge production by outsider “experts” to present three problems with traditional poverty research: pathologization, paternalism, and extractive exotification. I use my observations of protests and service provision to develop guidelines for solidarity research, a knowledge production practice that prioritizes the needs and perspectives of marginalized communities.

Environmental injustice occurs when marginalized groups face disproportionate environmental impacts from a range of threats. Environmental racism is a particular form of environmental injustice and frequently includes the implementation of policies, regulations, or institutional practices that target communities of color for undesirable waste sites, zoning, and industry. One example of how the United States federal and state governments are currently practicing environmental racism is in the form of building and maintaining toxic prisons and immigrant detention prisons, where people of color and undocumented persons are the majority of inmates and detainees who suffer disproportionate health risk and harms. This article discusses the historical and contemporary conditions that have shaped the present political landscape of racial and immigration conflicts and considers those dynamics in the context of the literature on environmental justice. Case studies are then presented to highlight specific locations and instances that exemplify environmental injustice and racism in the carceral sector. The article concludes with an analysis of the current political drivers and motivations contributing to these risks and injustices, and ends with a discussion of the scale and depth of analysis required to alleviate these impacts in the future, which might contribute to greater sustainability among the communities affected.

Birthed in the Bronx in response to socioeconomic frustration and political invigoration, hip hop equipped youth of color in the U.S. with a dynamic platform to both communication resentment towards structures of systemic oppression and act as an outlet for fun, innovative, and uninhibited creative expression. However, as this cultural revolution transformed from an underground movement to a mainstream commodity, rap music that reflected stereotypes of Black hyper-masculinity and heteronormativity were prioritized, and rappers who embodied these roles and the ideologies paired with them rose to the top of the charts. As a result of the music industry’s prioritization of hyper-masculine, heteronormative rap, a genre with the power to project the voices of marginalized communities of color in America at the same time became an active participant in the marginalizing of another—the LGBT+ community. Yet as rap music is notorious for its attacks on the LGBT+ community, I argue that this homophobia is not contained or exacerbated within hip hop, but rather is a by-product of a homophobic society that, through hyper-masculine and heteronormative norms, nurtures these attitudes within the male-driven genre. Through a textual analysis of rap lyrics, my paper highlights where the homophobic attitudes within hip hop stem from by locating broader societal structures that foster these ideals outside the genre. Taking this step back and looking at the larger issues within American society that nurture this homophobic environment within hip hop allows us to understand and address the phenomenon within the genre more clearly.

It is widely recognized that subspecialized multidisciplinary care improves neuro-oncology outcomes. Optimizing patient outcomes relies on the expertise of the treating physicians, neuroradiology and neuropathology, and supportive services familiar with common neurologic syndromes that occur after brain tumor diagnosis and treatment. Despite an increasing number of providers, patient access to specialized multidisciplinary care and clinical trials remains limited. Barriers to equitable health care exist across the United States, with marginalized communities being impacted disproportionately. Such disparity causes increased morbidity and mortality for patients from backgrounds with various elements of diversity. Limited attention to this inequity has resulted in an incomplete understanding of the spectrum of experiences that patients with neuro-oncologic diseases encounter. Clinical trials represent the highest standard and quality of care in medicine, but inclusion of under-represented and underserved groups consistently lags behind counterpart participants from majority racial and ethnic groups. Through provider education as it pertains to issues from bias and health literacy to increasing clinical trial enrollment and offering opportunities through telemedicine, opportunities for improving access to high-quality neuro-oncologic care are explored.

This study investigates social network site affordances and their implications for perceptions of marginalized communities. I employ Facebook as a case study and speak with young adult users to comprehend how socially marginalized groups are perceived through Facebook’s affordances. In particular, I consider: How familiar are users with Facebook’s tools and functionalities? How are issues of gender and race represented through the site’s interface? How do users conceive of gender and race? The findings suggest that gender is perceived as a more important identifier than race and that Facebook is post-racial, because of the user interface choices made. In addition, my participants view Facebook as an official social space that should include “authentic” identities; although Facebook has shaped authentic to mean accurate. I conclude that while the construction of affordances is a negotiation between user, interface, and designer, the designers have the most power because they have created the spaces in ways that will most benefit Facebook. In addition, users who are more situated in the socio-cultural majority have no desire to enact agency within Facebook’s structure because they are accustomed to forms and official documents that are well suited to fit their identification needs.

It is critical to ensure that COVID-19 studies provide clear and timely answers to the scientific questions that will guide us to scalable solutions for all global regions. Significant challenges in operationalizing trials include public policies for managing the pandemic, public health and clinical capacity, travel and migration, and availability of tests and infrastructure. These factors lead to spikes and troughs in patient count by location, disrupting the ability to predict when or if a trial will reach recruitment goals. The focus must also be on understanding how to provide equitable access to these interventions ensuring that interventions reach those who need them the most, be it patients in low resource settings or vulnerable groups.  We introduce a website to be used by The Bill & Melinda Gates Foundation, Wellcome Trust, and other funders of the COVID Therapeutics Accelerator that accept proposals for future clinical research. The portal enables evaluations of clinical study applications that focus on study qualities most likely to lead to informative outcomes and completed studies.

This paper explores access to, and engagement of, marginalized communities in the public education system. It begins by interrogating the meaning of access and engagement in education from a social justice position, and then turns to challenges that practitioners experience as they attempt to help marginalized groups gain access to education. Possibilities are suggested for overcoming and addressing such challenges. The article then reconceptualizes student engagement in a way that challenges educators to rethink how they currently understand student engagement, followed by a discussion about how to connect with school communities as a way to encourage student and community access and engagement in education. Lastly, strategies for building community relationships are presented.
