This article completes a two-part series. Previously the control of lower second molars following loss of first molars was discussed. It is now proposed to describe methods of uprighting the lower second molar when the first molar is still present. The impacted second molar is typically mesio-angularly inclined and may also be rotated. The severity of the impaction is related to the type of treatment recommended but the methods described are not original. This two-part series is not intended to be a comprehensive account of the many techniques available but selected methods are illustrated by case reports.

This article discusses Professor Nate Oman's excellent new book, "The Dignity of Commerce," which makes an impressive case for how markets can produce "desirable" outcomes for society. In addition to a comprehensive account of what he calls "virtues" of markets, such as their tendency to produce cooperation, trust, and wealth, the book is full of useful and persuasive supporting information and discussions.Oman is not only a fan of markets, but he asserts that markets are the "center" of contract theory, and provide its normative foundation. Elaborating, Oman concludes that "contract law exists primarily to support markets" and that "contracts are valuable because they facilitate commerce and extend the reach of markets. It is their beneficial consequences that justify the enforcement of contracts."The article focuses on two of the many important issues generated by Oman's thesis. First, has Oman done enough to convince that markets are what he calls the "centerpiece" of contract law? Second, does his effort to present what is essentially a unitary normative theory of contract handcuff his analysis of particular contract issues and doctrines? I will argue that markets are important and contract law should and does play an important role in supporting markets. However, we should not demote other visions of contract law, but see them all as important ingredients in understanding the subject. By largely espousing a unitary, integrative theory of contract law, Oman may have boxed himself into a corner that leads to a few debatable propositions, including with respect to consent to boilerplate and reliance on promises, which the article takes up in some detail.The article concludes that "The Dignity of Commerce" makes a solid case for the importance and virtues of markets and is rich in discussion and detail. As with any excellent work, it makes the reader ponder accepted wisdom and adds to the reader's perspective. Further, in making his case for markets, Oman does an excellent job of introducing, discussing and debunking many counterarguments. My effort in this article is only to reflect on whether the market argument really can capture the entire contract-law field.

Tooth transposition is a rare developmental disorder in which a normal tooth erupts in an inappropriate position. The change in position occurs between two adjacent teeth within the same quadrant. In this article, two cases of tooth transposition involving mandibular molars are described. Patient 1 presented with bilateral transposition between first and second mandibular molars. In addition, giroversion of the first molars was observed. Patient 2 presented with unilateral transposition between the mandibular left molars. Because this tooth disorder does not affect quality of life, both patients declined to seek dental treatment. Out of the 120 cases of tooth transposition found in the literature, 92,5% occurred in the maxilla and 15% were bilateral, 65,8% percent of these cases involved the canine and first premolar; 26.6% involved the canine and lateral incisor; 2,5% involved the canine and central incisor; and only 0.85% involved the first and second molar. The majority of tooth transposition cases occurred in women (66,6%). In literature, tooth transposition is a poorly-documented condition. Consequently, this report seeks to contribute to the existing information in the literature on this disorder.

AbstractThere have been infrequent case reports of bilateral root resorption in the lower second molar secondary from impacted third molar removal with bone regeneration without pain. Root resorption is commonly observed during third molar surgical impaction that affects mandibular second molars. The bone regeneration into the adjacent affected tooth is evident simply after the surgical extraction of the impacted tooth. However, there have been a few previous reports on the prognosis of the remaining resorbed tooth. Most dentists often choose to extract them when damaged if root resorption is observed. This report case involved bilateral root resorption of the lower second molar due to lower adjacent impacted third molar teeth and bone regeneration without pain sensation. After surgical extraction of bilateral lower impacted third molar teeth, the remaining teeth retained vital pulp and survived as functional teeth.

Background/Aims: Seminal vesicle (SV) stones are a rare, and thus readily misdiagnosed condition in practice. Understanding the etiology, diagnosis, and management are crucial to guide a urologist's care, and are provided in this literature review. Methods: The inclusion criteria for the literature search, using the search engines MEDLINE® and PubMed was conducted using a combined query of “seminal vesicle stone” and the following keywords: calculi, hematospermia, calcification, and transrectal ultrasound (TRUS). Results: The etiology of SV stones is currently unknown where majority of the patients present with having painful ejaculation and hematospermia. However, clinicians have reported potential etiologies by categorization as an inflammatory or non-in-flammatory. A majority of the previous cases had shown multiple stones being present in the SV duct system that are typically diagnosed through radiological examination such as TRUS, MRI, or plain radiographs. Amongst the many imaging approaches, TRUS remains the primary imaging diagnoses of SV calculi. Transurethral seminal vesiculoscopy has shown to be used in an abundant of the case reports to be an ideal surgical approach for managing small SV stones. In regard to larger stones, a transperitoneal laparoscopic protocol is proper. Conclusion: The current imaging techniques have increased the case reports and diagnosis of SV calculi; however, more research is warranted for understanding the pathogenesis of the formation of SV stones. An optimal management of the extraction of SV stones depends on a number of factors such as size and location.

Infraocclusion occurs at an early age and becomes worse with age, causing increased damage in young children. Extraction of affected teeth is the preferred treatment modality for prevention of possible complications. It is rare for a primary molar to temporarily exhibit secondary failure of eruption, followed by regeneration of full eruptive capacity. This report was written to describe two patients who experienced spontaneous eruption of an infraoccluded primary molar at approximately 7 years of age. While watchful waiting is not always a suitable treatment option, we propose that extraction be deferred until the first permanent molar erupts, unless significant problems occur.

This special conference issue of the Australian Journal of Environmental Education captures a snapshot of some of that thinking. Imipenem-cilastatin and piperacillin-tazobactam are two antibiotics with broad antimicrobial coverage. Besides the many well established adverse effects of these drugs, there have been few case reports of hypokalemia. Here we present an interesting case of resistant hypokalemia caused by these drugs leading to Torsades de Pointes which has never been reported in the past. Hypokalemia resolved with discontinuation of piperacillin.
