Abstract Background The purpose of this study was to analyze the relationship between body posture and sagittal dental overjet in children before and after early orthodontic treatment with removable functional orthodontic appliances. Methods Angle Class II patients (mean age 8.2 ± 1.2 years; 29 males and 25 females) with a distinctly enlarged overjet (> 9 mm) were retrospectively examined regarding body posture parameters before and after early orthodontic treatment. In addition, changes in overjet were investigated with the aid of plaster models. Forms of transverse dysgnathism (crossbite, lateral malocclusions) and open bite cases were excluded. Body posture parameters kyphosis, lordosis, surface rotation, pelvic tilt, pelvic torsion and trunk imbalance were analyzed by means of rasterstereographical photogrammetry to determine, if the orthodontic overjet correction is associated with specific changes in posture patterns. Results In nearly all patients an overjet correction and an improvement regarding all body posture and back parameters could be noted after early orthodontic treatment. Overjet reduction (− 3.9 mm ± 2.1 mm) and pelvic torsion (− 1.28° ± 0,44°) were significantly (p < 0.05) and moderately correlated (R = 0.338) with no significant associations found for the other posture and back parameters (p > 0.05). Conclusion Overjet reduction during early orthodontic treatment may be associated with a detectable effect on pelvic torsion.

(1) Background: This retrospective study aimed to examine the change of gingival recessions (GR) before and after orthodontic treatment (OT). (2) Methods: Patients who had at least one GR on the vestibular surface of maxillary/mandibular teeth mesial to first molars were included. GR was measured on good quality plaster models before and after OT. (3) Results: Significant improvement of mean GR at patient level was found between T0 and T1 (0.45 mm, 95% CI: 0.28, 0.62) (p < 0.001). In 51.4% of patients GRs became better, in 37.8% GRs did not change. and in 10.8% GRs became worse. At tooth level the mean GR improvement was 0.54 mm (CI: 0.42, 0.65; median 0.55 mm Q1; Q3: 0.12, 0.96) (p < 0.001). GR improved in 67 teeth (58.7%), did not change in 41 (36.0%), and worsened in 6 (5.3%). Full healing was observed in 15 teeth. A reduced GR improvement was found in cases with pre-treatment open bite (OR 3.89; CI: 1.35, 11.16; p = 0.008) and dental Class III patients (OR 2.6; CI: 1.11, 6.0; p = 0.026). (4) Conclusion: There was an improvement of GR after OT in more than half of the patients. Class III malocclusion and reduced pre-treatment overbite had a negative impact on GR improvement.

ABSTRACT Objective: The aim of this retrospective study was to evaluate the relationship between orthodontic treatment and gingival health. Materials and Methods: A total of 251 patients among whom 177 were girls and 74 were boys, recruited from the records pool of the Department of Orthodontics, Faculty of Dentistry, University of Gazi, were included in the study. Patients’ treatments have been completed by postgraduate students during the period between 2006 and 2012. Patients’ folders were analyzed according to their age, treatment time, and the type of orthodontic treatment. Intra-oral photographs were analyzed, and the presence or absence of visible plaque, visible inflammation, and gingival recession were recorded, and incisor inclinations analyzed on lateral cephalometric films, before and after orthodontic treatment. Results: No statistically significant difference was found in patients treated with functional appliances before and after treatment. In patients treated with fixed orthodontic appliances, visible plaque, visible inflammation, and gingival recession showed significant increases after treatment, gingival biotype did not show any significant difference. Positive correlation was found between lower incisor position and gingival recession in patients treated with fixed appliance and extraction. And also cuspids were the teeth with the highest prevalence of gingival recession. Conclusion: Considering the relationship between orthodontic treatment and gingival health, cooperation among patients, orthodontists, and periodontists is important.

However, there is a lack of information about changes to pre-existing GR during OT. Methods The aim of this retrospective study was to examine the change of GR before and after OT. Intraoral photographs of 993 patients treated with fixed appliances between 2005 and 2017 were evaluated. Patients who had at least one GR on the vestibular surface of maxillary/mandibular teeth mesial to first molars were included. GR was measured on good quality plaster models before and after OT. The change of GR (T0-T1) was categorised into the three groups: worsened, did not change and improved (this group consisted of GR, which became better but did not disappear, and fully healed, which disappeared after OT)). The change by ≥0.5 mm was recorded. Statistical analysis was performed using SPSS Statistics (Version 22.0. Armonk, NY). Results Fifty-one (5.1%) patient had ≥1 GR before OT. Of them, 37 (72.5%) patients were included in the final analysis according to the inclusion criteria. GR before OT was found on 114 buccal/labial surfaces. The mean GR improvement was 0.51 (95% CI: 0.40, 0.63) mm (p<0.001). GR improved in 71 teeth (62.3%), did not change in 37 (32.4%), and worsened in 6 (5.3%). Of the 71 GR, which improved, full healing was observed in 15 (21.1%) teeth. GR had a greater chance of improvement in cases with a thick/normal gingival biotype compared with the thin biotype (OR 2.4; 95% CI: 1.07; 5.28) (p=0.03). There was a lower chance for GR improvement in cases with pre-treatment open bite (OR 3.29; 95% CI: 1.10; 9.81) (p=0.03) and Class III patients (OR 2.79; 95% CI: 1.14; 6.83) (p=0.03). Conclusions Based on the results of this retrospective study, we conclude that orthodontic treatment may change and even influence the healing of GR.

Introduction: During orthodontic tooth movement, the early response of periodontal tissues to mechanical stress is an acute inflammatory reaction. Mechanical stress from orthodontic appliances is believed to induce cells in the periodontal ligament (PDL) to form biologically active substances, such as enzymes and cytokines, responsible for connective tissue remodeling (Nishijima Y et al 2006). Leptin, a polypeptide hormone has been classified as a cytokine (Zhang et al 1994). Earlier findings concluded that leptin at high local concentrations protects the host from inflammation and infection as well as maintaining bone levels. It has been also suggested that leptin plays a significant role in bone formation by its direct effect on osteoblasts (Alparslan et al 2010). This pilot study aimed to study leptin in saliva and its association with tooth movement during initial orthodontic alignment. Objectives: To determine if there are any differences in saliva leptin level before and after orthodontic alignment. Material and methods: Ten orthodontic patients (7 girls and 3 boys; mean age, 16.76 ± 1.1 years) with crowding (up to 5mm) that required orthodontic fixed appliances, on a non-extraction basis as part of the treatment plan, were recruited in this longitudinal study. Orthodontic study models were constructed at baseline and at 6- weeks after orthodontic treatment commenced. Full fixed orthodontic appliances with initial 0.014” Nickel Titanium archwire placed. The amount of crowding was measured, before and after initial alignment with an electronic digital caliper (Max-Cal, Japan Micrometer Ltd, Tokyo, Japan) with an accuracy of up to 0.01mm. Unstimulated morning saliva sample were collected at all visits, after at least an 8-hour period of fasting and no-toothbrushing. After centrifugation (4000x g;10min), the samples were stored at -25C and tested using Leptin Abnova LEP Human ELISA kit (KA3080) which was subsequently analyzed. Subjects’ periodontal health status was also monitored throughout the study. Ethical approval (ID IREC 262) was received on 7th April 2014 from International Islamic University Malaysia Research Ethics Committee (IREC). Results: Leptin concentration in saliva was significantly decreased in a time-dependant manner (t(9)=8.60, p<0.001), from before orthodontic treatment (7016.45± 425.15 pg/mL) and 6 weeks after bond-up (4901.92±  238.64 pg/mL). Conclusion: Leptin concentration in saliva is decreased during orthodontic tooth movement in initial alignment stage.

Background: Previous studies on root resorption were reviewed by panoramic radiographs. Cone-beam computed tomography (CBCT) showed that 41.5% of teeth experienced resorption when panoramically examinated, while 68% of teeth experienced resorption when the examination method used was CBCT. Root resorption occurs in the maxillary central incisor (as much as 74%) and in the maxillary lateral incisor (as much as 82%). The maxillary canines have the most resorption, followed by the lateral maxillary incisors. Purpose: The aim of this study was to determine the differences of apical resorption in anterior maxillary teeth before and after orthodontic treatment in skeletal Class I/II cases of extraction. Methods: Samples from this study were the results of panoramic photographs of 50 patients treated by fixed orthodontic appliances at the Dental and Oral Hospital Airlangga University. These were selected according to the sample criteria. The evaluation method consists of measuring root and crown lengths with a digital application (RadiAnt DICOM Viewer). Subsequently, the measurements were evaluated using CBCT images. Results: The data were statistically analysed using normality tests with Shapiro–Wilk and Kolmogorov–Smirnov tests. Based on the results of paired sample tests, it was found that every treatment group had significant differences in the average length of the crowns and roots, with a result of p=0.000 (p<0.05). Conclusion: The use of CBCT is considered quite effective and accurate in evaluating root resorption compared to panoramic photographs.

The prevalence of an anterior open bite ranges in the literature from 1.5% to 11%, with great racial variance. Stable non-surgical treatment of an anterior open bite is notoriously unpredictable, with a high risk of relapse and an uncertainty of true skeletal change. Temporary anchorage devices (TADs) are increasingly used to enhance and simplify orthodontic biomechanics, enabling clinicians to push the boundaries of orthodontic treatment. In anterior open bite cases, TADs may be used predictably for molar intrusion and improvement of the overbite. We describe a 16-year-old male with a Class I incisal relationship on a skeletal I base and increased vertical proportions, complicated by a 4-mm anterior open bite secondary to a previous digit sucking habit. Treatment involved fixed orthodontic appliances on an extraction basis, and molar intrusion using TADs. TADs provide a safe and effective alternative to reducing an anterior bite in a patient whose growth is complete. CPD/Clinical Relevance: Understanding the options for the treatment of an anterior open bite and the limits of orthodontic camouflage will help clinicians provide their patients with the necessary information with which to make informed decisions.

Aim. To improve the graphic method of analysis of the upper dental arch and its segments, to increase the accuracy of upper dental arch abnormalities detection and to minimize orthodontist’s man-hours. Methods. The plaster models of 32 maxillae with dentoalveolar abnormalities were studied before and after treatment. The graphs of individual normal dental arches and its sizes were created by using the geometric and graphic method by Hawley-Herber-Herbst. The regularities were found based on the gained data; graphs of individual normal dental arches (stereotyped patterns) were created using the Adobe Photoshop CS3 software. Dentoalveolar abnormalities were visually examined by directed applying the stereotyped patterns and pictured before and after treatment. 64 models were examined overall. Results. A relation was found: incisors and canines dental crowns half-sum relates to the length and width of Hawley-Herber-Herbst individual normal dental arch graph as 1:2,3:1,3. Considering the relation found, the set of stereotyped patterns was created. The pattern was chosen for the studied model according to the patient incisors and canines dental crowns width. The visual examination allows to recognize and to record (take a picture) teeth malposition, abnormalities of the upper dental arch and its segments in transverse and sagittal planes for correction control. The photos of the models at different stages of orthodontic treatment with stereotyped patterns applied can be added to the patients’ medical chart, indicating the present occlusal rehabilitation success. Conclusion. The offered way of detection of upper dental arch and its segments abnormalities does not require significant time to be performed, is informative and can be implied to clarify the dentoalveolar abnormalities treatment plan.

ABSTRACT Objective:  To compare personality traits, attitude toward orthodontic treatment, and pain perception and experience before and after orthodontic treatment. Materials and Methods:  One hundred subjects (50 male and 50 female) were included in this study. The mean (SD) age was 17.5 (2.05) years at T1 and 19.15 (2.32) years at T2. The instruments for data collection were questionnaires that included assessment of patients' personality traits, attitudes toward orthodontic treatment, and pain perception/experience. Subjects completed the questionnaires at two different times: before orthodontic treatment (T1) and after fixed orthodontic treatment (T2). Subjects were treated by fixed orthodontic appliances for an average (SD) period of 18.64 (0.35) months. Paired sample t-test and chi-square test were used to detect any differences. Results:  Significant changes in personality traits were detected after orthodontic treatment irrespective of gender. Neuroticism, openness, agreeableness, and conscientiousness scores were improved (P &lt; .001). A positive attitude toward orthodontic treatment was reported at T1 (4.31 [±1.26]) and improved at T2 (3.98 [±1.16]) irrespective of gender (P &lt; .05). The average (SD) expected pain score (T1) was 4.73 (1.88) and the average (SD) experienced pain score (T2) was 4.63 (1.58). Significant difference in the expected and experienced pain scores was not detected (P  =  .11). Conclusions:  Personality traits and attitude toward orthodontic treatment improved after orthodontic treatment. Reported actual pain experience during orthodontic treatment was similar to that expected before treatment.
