Abstract Background This qualitative pilot study aimed to establish views of traditional and biomedical practitioners towards collaboration between the two sectors on the treatment of people with mental illness in Zanzibar, Tanzania. Methods Six traditional healers (known as “waganga” in Swahili) and six nurses working in government secondary mental health services were invited to participate in a series of focus group discussions (FGDs). Two sets of FGDs took place approximately seven weeks apart. In each set, FGDs were conducted with traditional healers only, nurses only, and finally nurses and traditional healers together. FGDs were conducted in Swahili, audio-recorded and then translated to English by an independent translator and coded thematically using NVivo software. Results All participants expressed that they were in favour of collaboration between traditional and biomedical practitioners on mental healthcare. Opinions varied regarding what form this collaboration should take. For many nurses and healers, there was acknowledgement of the role of the other group in providing treatment for people with mental illness, with support for the idea of bi-directional referrals between the two sectors. For some nurses, the value of collaboration would be purely in the education of traditional healers in the recognition of mental illness, with subsequent referral to biomedical services. For some traditional healers, the idea of collaboration seemed to appeal in part because of a perceived opportunity to learn additional skills from biomedical practitioners. Both categories of participant expressed a belief that patients possessed by a jinn (a spirit) or those that had been bewitched needed treatment by traditional healers. On the other hand, those with what participants considered to be “mental illness” needed treatment at the hospital clinic. However, some nurses felt that that traditional healers might be able to provide helpful treatment for mental illness, as well as those suspected to be affected by jinn or witchcraft. There was agreement on the need to establish clear referral pathways between the two service providers. The creation of an office for traditional healers at the hospital was an area where there was disagreement among participants. Conclusions We conclude that there is a positive view of collaboration among traditional healers and nurses who participated, and a willingness to work towards actual collaboration. The results suggest that views vary as to what form this collaboration should take, with opinions differing between nurses, as well as between traditional healers. Additional work is needed in order to further explore the nature of potential collaboration and extend the research to the wider population of traditional and biomedical practitioners in Zanzibar, to include primary health care workers.

BackgroundContrary to popular myth, majority of mentally ill women are mothers with increasing number of them seeking help. Little is known about their own experiences in this regard and the extent to which their needs are met.ObjectivesTo assess the barriers and facilitators in seeking help from mental health care providers in matters of pregnancy and parenting.MethodsThe study used qualitative design with social constructivist paradigm. A purposive sample of 30 mothers with severe mental illness was obtained. Data was collected through one-to-one in-depth semi-structured interviews. After verbatim transcription, inductive thematic analysis was used to explore transcripts.ResultsMost women considered motherhood “central” to their lives and almost all of them experienced the burden of the “dual role”. Main barriers in seeking help were stigma, treatment side effects, wrong information and time constraints. Whereas self-advocacy, early engagement, education of women and involvement of the family with service providers were the facilitating factors. The prime expectations of the mothers as identified were early and direct communication, patient audience and basic guidance in regards to child health and parenting issues.ConclusionWomen who are mothers and also users of mental health services face special challenges in managing the contradictory aspects of their dual identity. Hearing their voices are essential for service provision and ensuring adequate mental health needs. Background: Traditional healing methods are considered central to mental health care in low-income countries such as Ghana, because they are perceived to be more easily accessible, more affordable and generally ascribe similar causal beliefs to those of the patients. However, not much is known about the work of traditional healers largely because their methods are shrouded in mysticism and secrecy. There is a need to understand the ideology and beliefs of traditional healers surrounding mental disorders, including knowledge about their practices in mental health care. Aims: In this article, we discuss the causal beliefs and treatment methods of traditional medicine-men from Accra, Ghana. We also describe their diagnostic and treatment methods for mental disorders. Methods: Eight medicine-men, indigenous to the Greater Accra Region, were interviewed through individual semi-structured interviews. The data were analysed through thematic analysis. Results: The medicine-men’s beliefs about mental illness were dominated by supernatural ideas. Mental illness was also seen as a form of punishment or resulting from envy, and there was a strong reliance on spiritual direction from the gods for diagnosis and treatment. Conclusion: These themes are discussed with emphasis on their potential implications for patients, as well as for collaborative efforts.

A survey of attitudes held by a large sample of university students as well as by smaller samples of psychologists, general medical practitioners, members of the public, psychiatric hospital staff and patients in the central eastern Cape toward mental illness and mental health-care service providers was conducted during the early part of 1994. It was found that marked differences existed between the different samples and that the extent of a person's knowledge about mental illness, as well as the degree of contact with mental-health professionals and their services, were important influences on the attitudes of respondents. In particular, third-year psychology students tended to be more negatively disposed to psychiatric treatment than those students in their first year of study. Conversely, the attitudes of final-year students toward the discipline of psychology were more positive than those held by students in their first year of studying psychology. Within both the student and the patient samples, only a small minority indicated that they would first seek help from general medical practitioners if they were ever to contemplate taking their own lives or if they were seriously mentally ill. Psychiatric patients and service providers indicated their confidence in psychiatric treatment and the psychiatrist was considered to be the most appropriate professional to deal with mental illness. Members of the general public were found to be more optimistic than psychologists about the efficacy of psychological and psychiatric treatment, but less so than general medical practitioners. Although mental health-care professionals were viewed in a favourable light, most respondents indicated that they would nevertheless prefer to approach a friend in times of psychological distress.

BACKGROUND In many rural communities, the cause of mental illness is attributed to black magic, spirit possession of past sin and the coastal region of Karnataka is not exempted from it. The natives of this region ascribe the cause of mental illness to the spirit or demigod, and they seek the help of traditional healers such as spirit dancers for the recovery. This help-seeking behaviour of the people results in delay in seeking psychiatry care and affects the recovery of the person with mental health problems. Therefore, this study explores the opinion of clients undergone traditional healing for mental health problems and the results of the study could contribute to planning an appropriate health promotion activity to promote community mental health. METHODS The present study was explorative, undertaken in the Udupi district of Karnataka state, which explores the views of the respondents about the cause of mental health problem and the outcome of traditional healing for their problems. Altogether 200 clients visiting traditional healers for mental health care were interviewed based on the snowball sampling technique and the interview schedule was used as a tool to gather the data. RESULTS Of the 200 respondents interviewed, 27.5 percent were adults (31 to 40 years), while 43.1 percent were unemployed. Black magic was found to be the major cause for mental health problems among 25.5 percent of the respondents; whereas, 26 percent of the respondents felt recovered completely after undergoing traditional healing for mental health problems. CONCLUSIONS The recognition of mental health problems is very much essential for people with mental health problems to seek professional help. This could help mental health professionals to diagnose illness at the very beginning and provide better mental health care. However, the explanatory model of the patients needs to be taken into consideration while providing modern medical care. IT IS ONLY BY EXAMINING the impact of public policies on the lives of people that we can begin to assess the success or failure of those policies. With this as a starting point, Not for service presents an extensive (just over 1000 pages) and balanced picture of the impact of policies on the lives of people with mental illness and their families and friends. The authors take care to state that their research is not a rigorous examination of the extent to which the National Standards for Mental Health Services have been implemented, but do observe that the ?volume and consistency of the information demonstrate the gaps and difficulties governments have had in meeting these standards? (p 14). The report is lengthy, but is well laid out and ?easy? to read, although the content will leave any reader feeling uneasy about the current state of our mental health system. There is also a shorter (96 page) summary report available.

社區精神復康服務在西方國家發展悠久,不同的相關文獻與實証爲本研究 ( evidence-based research) 顯示,該服務形式對生活於社區的精神病康復者有顯著成效。自2005年開始,在政府和非政府機構推動下,社區精神健康照顧服務在香港不同的地方推行。然而,有關本港社區精神健康照顧服務的研究卻相當貧乏。是次研究運用單組前測後測設計 (one group pre-test and post-test design) 方式,評估一個在本港推行的社區精神健康照顧服務,對離院後重返社區生活的服務使用者的成效。是次研究邀請了120名精神病康復者參與研究,共有87名研究對象完成所有測試。當中測試範疇包括精神病徵狀、生活質素、自我效能、社區生活技巧。研究發現在開始接受服務後的6至12個月,社區精神健康照顧服務減少精神病徵狀 ( p <.001)、提高生活質素 ( p <.05)、增強自我效能 ( p <.001)和提升社區生活技巧 ( p <.001) 能夠爲康復者帶來非常正面的效果。 Community mental health care services have been developing in Western countries for decades and have been proved to be effective in facilitating the rehabilitation of people with mental illness into the community in many evidence-based research studies. In Hong Kong, with the support of the government and non-government organisations, community mental health care services have been set up in different districts since 2005. However, research on the services is limited. By using the one-group pre-test and post-test design method, this study examines the effects of one Community Mental Health Care Services on the rehabilitation of people with mental illness discharged from hospitals. A total number of 120 service users participated in the study and 87 subjects completed all the assessments in different areas, including psychiatric symptoms, quality of life, self-efficacy and community living skills, before the commencement of intervention, 6 months after the intervention and at 12 months immediately after the intervention. Results showed that the subjects had lessened psychiatric symptoms ( p <.001), better quality of life ( p <.05), enhanced self-efficacy ( p <.001) and increased community living skills ( p <.001), after the services. The study revealed significant positive effects of the community mental heath care services on people with mental illness in the community.

The aim of this study was to explore and describe the perceptions of professional nurses concerning barriers to care for people with mental illness in the Eastern Cape Province, South Africa. The study was based on a qualitative, explorative and descriptive design.  A total of nine professional nurses working in primary, secondary and tertiary health care facilities were purposively selected. Unstructured interviews were conducted to collect data. Participants’ responses were captured on an audio recorder and later transcribed verbatim.Participants’ responses were then analysed thematically. Two main themes and their related sub-themes were identified. The first theme concerns the perceptions of professional nurses regarding the societal barriers that may hinder people with mental illness from accessing and utilising mental health services. These barriers include socioeconomic hardships, lack of knowledge and insight, lack of family support, embedded cultural beliefs and practices and stigma. The second theme highlights the barriers that professional nurses perceive within the health care system that influence access and utilisation of mental health services. These barriers include inadequate support from stakeholders and leaders in the mental health sector and lack of financial, human and infrastructure resources. Professional nurses made recommendations to improve mental health care. Those of high priority included enhanced mental health literacy among members of the public and a need for mental health stakeholders and leaders to increase their support of the mental health sector in an effort to improve access to mental health care.
