Background: Saliva based diagnostic can play an important role in the translational research related to cancer diagnostics and treatment. It is easily available, noninvasive, low storage cost, has less contamination chances with simple collection procedure. Cancers related to tobacco use, including oral cancer account for about 30% of all cancers in males and females. Five years' survival rate remains the same even after decades of advancement of detection, prevention, and treatment of OSCC (oral squamous cell carcinoma) mainly due to late diagnosis of oral potentially malignant disorders (OPMDs). Aim: Combinatorial characterization of saliva, endorsing multidimensional spectroscopic signatures using suitably designed biochamber. Methods: Eighteen saliva samples (6 normal, 6 OSF [oral submucous fibrosis, a type of OPMD] and 6 confirmed OSCC) were collected from GNIDSR (Guru Nanak Institute of Dental Science and Research) Kolkata. Ethical approval was obtained for the study and all the participants were explained the objectives of the study and a written informed consent was obtained from them. Participant's demographic detail and clinical characteristics were also recorded. The participants were asked not to consume food 1 hour before sample collection and were suggested to rinse their mouth 30 minutes prior to saliva expectoration to minimize the contamination of food in saliva. Empty, sterile, graded tubes were used for this purpose. The subjects were asked not to clear nose or throat during the process of saliva expectoration to avoid forced phlegm from other part of the respiratory tract. The saliva samples were then immediately transferred to −20 degrees and later in −80 degrees for long storage. The electrical impedance (EI) of saliva was measured in custom made biochambers with copper electrodes. The EI was measured for the frequency sweep from 20 Hz to 2 MHz using an impedance analyzer. Apart from EI measurement, the corresponding samples were subjected to FTIR (Fourier-transform IR spectroscopy) analysis. SPSS and OMNIC software were used for the data analysis of EI and FTIR respectively. The multivariate analysis of the FTIR data indicates the significant differences ( P < 0.005) among the different study groups such as normal, OSF and OSCC. The eigen values (normal 0.917, OSF 0.962, OSCC 0.975) from component matrix analysis also indicate the same. Conclusion: The spectroscopic characterization (EI and FTIR) of saliva was effective in evaluating normal and OPMD condition. This noninvasive paradigm can serve as a complimentary technique to the existing gold standard methods for the early detection of oral cancer.

Minimally invasive cancer detection using bio-fluids has been actively pursued due to practical limitations, though there are better suited noninvasive and online in vivo methods. Saliva is one such clinically informative bio-fluid that offers the advantages of easy and multiple sample collection. Despite its potential in cancer diagnostics, saliva analysis is challenging due to its heterogeneous composition. Recently, there has been an upsurge in saliva exploration using optical techniques. Forms of saliva such as precipitate and supernatant have been monitored, but this sampling method needs to be standardized due to the obvious loss of analytes in processing. In that context, present work details the comparison of four different saliva sampling methodologies, i.e., air-dried, lyophilized, pellet, and supernatant using Raman spectroscopy collected from 10 healthy samples. Composition-driven spectral features of all forms were compared and classified using principal component analysis and linear discriminant analysis. Analysis was carried out on all four groups in the first step. In the second step, groups of pellet and supernatant , and air-dried and lyophilized were analyzed. Findings suggest that pellet and supernatant exhibit discrete spectroscopic features and demonstrate high classification efficiency, which is indicative of their distinctive biochemical composition. On the other hand, air-dried and lyophilized forms showed overlapping spectral features and low classification, suggesting these forms retain majority spectroscopic features of whole saliva and are less prone to sampling losses. Thus, this study indicates air-dried and lyophilized forms may be more appropriate for saliva sampling using Raman spectroscopy providing the comprehensive information required for cancer diagnosis. Furthermore, the method was also tested for the classification of oral cancer and healthy subjects ( n = 27) which yielded 90% stratification. The findings of the study indicate the utility of minimally invasive salivary Raman-based diagnostics in oral cancers.

Oral cancer is one of the most common head and neck malignancies and has an overall 5-year survival rate that remains below 50%. Oral cancer is generally preceded by oral potentially malignant disorders (OPMDs) but determining the risk of OPMD progressing to cancer remains a difficult task. Several diagnostic technologies have been developed to facilitate the detection of OPMD and oral cancer, and some of these have been translated into regulatory-approved in vitro diagnostic systems or medical devices. Furthermore, the rapid development of novel biomarkers, electronic systems, and artificial intelligence may help to develop a new era where OPMD and oral cancer are detected at an early stage. To date, a visual oral examination remains the routine first-line method of identifying oral lesions; however, this method has certain limitations and as a result, patients are either diagnosed when their cancer reaches a severe stage or a high-risk patient with OPMD is misdiagnosed and left untreated. The purpose of this article is to review the currently available diagnostic methods for oral cancer as well as possible future applications of novel promising technologies to oral cancer diagnosis. This will potentially increase diagnostic options and improve our ability to effectively diagnose and treat oral cancerous-related lesions.

BACKGROUND Medicinal plants and their extracts have been found to possess potent antiinflammatory, antioxidant and anti-cancer properties that slow down the cellular proliferation and malignant transformation rate of orally potential malignant disorders such as oral leukoplakia, oral submucous fibrosis and oral lichen planus. They are a potential source of antioxidants such as vitamin A, E, and C that neutralize the free radicals produced in various oral mucosal lesions thereby preventing the mucosal damage. They have been found to modulate cellular signalling pathways and, this activation influences the cellular defence mechanisms that protect the normal cells against reactive oxygen species or reactive metabolites and induces apoptotic death of cancerous cells, thus chemoprevention ensues to prevent, reverse or delay carcinogenesis. They have been studied to enhance tumour cell killing and inhibition of metastasis by increasing number of immunocompetent cells such as macrophages, cytotoxic T lymphocytes, natural killer cells and by recruitment of antibodies at the tumour site. Now a days, natural herbs or phytochemicals are gaining attention due to side effects and increased antimicrobial resistance that often occur with prolonged use of chemotherapeutic agents. Therapeutic effect of phytochemicals in oral PMDs is very promising as they prevent the progression of dysplasia from mild to severe. Literature has revealed significant reduction in burning sensation, difficulty in swallowing and speech, and pain associated with the oral mucosal lesion. However, more research should be conducted in future to establish their use as an adjunctive therapy in oral potentially malignant disorders. By this review, an attempt has been made to highlight therapeutic potential of diverse herbs such as aloe vera, tulsi, neem, curcumin, lycopene, green and black tea, peppermint etc. in the reduction of clinical symptoms of burning sensation and pain associated with the oral mucosal lesions, and discusses their indispensable role in prevention of oral cancer. Oral Cancer (OC) is a serious and growing problem which constitutes a huge burden on people in more and less economically developed countries alike. The scenario is clearly depicted from the increase in the expected number of new cases in the US diagnosed with OC from 49,670 people in 2016, to 49,750 cases in 2017. The situation is even more alarming in India, with 75,000 to 80,000 new cases being reported every year, thus making it the OC capital of the world. Leukoplakia, erythroplakia, oral lichen planus, oral submucous fibrosis, discoid lupus erythmatosus, hereditary disorders such as dyskeratosis congenital and epidermolisys bullosa are highlighted by WHO expert working group as the predisposing factors increasing the risk of OC. Consumption of tobacco and alcohol, genetic factors, and human papilloma virus are assigned as the factors contributing to the aetiology of OC. On the other hand, pathogenesis of OC involves not only apoptosis but also pain, inflammation and oxidative stress. Inspite of current treatment options (surgery, radiotherapy, and chemotherapy), OC is often associated with recurrence and formation of secondary primary tumours resulting in poor overall survival rates (∼50%). The intervention of nano technology-based drug delivery systems as therapeutics for cancers is often viewed as a cutting edge for technologists. Though ample literature on the usefulness of nano-coutured cancer therapeutics, rarely any product is in pipeline. Yet, despite all the hype about nanotechnology, there are few ongoing trials. This review discusses the current and future trends of nano-based drug delivery for the treatment of OC.

Perineural invasion is an underrecognized route of metastatic spread along the nerve bundles within the nerve sheath into the surrounding tissues. It hinders the ability to establish local control as tumour cells can traverse along nerve tracts well beyond the extent of any local invasion rendering them inoperable and unresectable. Perineural invasion is a marker of poor prognosis. Oral submucous fibrosis with oral cancer constitutes a clinicopathologically distinct disease. Our case highlights an enigmatic presentation of oral submucous fibrosis and its coexistence with oral cancer presenting with unusual neurological disturbance of the inferior alveolar nerve and facial nerve and diffuse widening of the mandibular canal. The objective of this case report is to enumerate the significance of perineural invasion in determining the course of the disease and necessitate the need for future studies that can shed light on molecular mediators and pathogenesis of perineural spread.

ABSTRACTA metagenomic library was constructed from microorganisms associated with the brown algaAscophyllum nodosum. Functional screening of this library revealed 13 novel putative esterase loci and two glycoside hydrolase loci. Sequence and gene cluster analysis showed the wide diversity of the identified enzymes and gave an idea of the microbial populations present during the sample collection period. Lastly, an endo-β-1,4-glucanase having less than 50% identity to sequences of known cellulases was purified and partially characterized, showing activity at low temperature and after prolonged incubation in concentrated salt solutions.
