Objective. To improve the estimation of healthcare expenditures by introducing a novel method that is well-suited to situations where data exhibit strong skewness and zero-inflation. Data Sources. Simulations, and two sources of real-world data: the 2016-2017 Medical Expenditure Panel Survey (MEPS) and the Back Pain Outcomes using Longitudinal Data (BOLD) datasets. Study Design. The super learner is an ensemble machine learning approach that can combine several algorithms to improve estimation. We propose a two-stage super learner that is well suited for use with healthcare expenditure data by separately estimating the probability of any healthcare expenditure and the mean amount of healthcare expenditure conditional on having healthcare expenditures. These estimates can be combined to yield a single estimate of expenditures for each observation. The method can flexibly incorporate a range of individual estimation approaches for each stage of estimation, including both regression-based approaches and machine learning algorithms such as random forests. We compare the performance of the two-stage super learner with a one-stage super learner, and with multiple individual algorithms for estimation of healthcare cost under a broad range of data settings in simulated and real data. The predictive performance was compared using Mean Squared Error and R2. Data collection/Extraction methods. MEPS data include only adults and exclude observations with missingness, BOLD data include observations without missingness. Principal Findings. Our results indicate that the two-stage super learner has a better performance compared with a one-stage super learner and individual algorithms, for healthcare cost estimation under a wide variety of settings in simulations and empirical analyses. The improvement of the two-stage super learner over the one-stage super learner was particularly evident in settings when zero-inflation is high. Conclusions. The two-stage super learner provides researchers an effective approach for healthcare cost analyses in environments where they cannot know the best single algorithm a priori. Keywords. Background: The main aim of hypospadias repair is to preserve the urethral plate whenever possible. This study was undertaken to compare the outcome of repair of severe hypospadias with moderate chordee by single-stage Snodgrass with two-stage repair with dermal graft corporoplasty. Methods: This prospective cross-section study was done in the Department of Urology, Chalmida Anand Rao Institute of Medical Sciences, Bommakal, Karimnagar. The selection of the patients the type of surgery was based on the type of case. A total of n=31 patients were operated for severe hypospadias out of which n=14 single surgeries and n=17 two-staged surgeries were performed. Results: N=25 patients were found with a moderate degree of chordee. No complications were found in n=11(78.57%) in single-stage repairs and two-stage repairs n=15(88.23%) were without complications the p values were found to be 0.012 which is significant. HOSE scores Erection correction was better in two staged surgeries as compared to single-stage repairs with p values <0.001 which is considered significant. Conclusion: We can conclude that the repair of severe hypospadias with moderate chordee by two-staged repair showed better outcomes as compared to single-stage repair. The incidence of postoperative complications is also lesser with two-staged repairs. However, the choice of the procedure must be based on the type of hypospadias, choice of patients and economical constraints.

We are studying the crystallization and annealing behavior of high performance polymers, like poly(p-pheny1ene sulfide) PPS, and poly-(etheretherketone), PEEK. Our purpose is to determine whether PPS, which is similar in many ways to PEEK, undergoes reorganization during annealing. In an effort to address the issue of reorganization, we are studying solution grown single crystals of PPS as model materials.Observation of solution grown PPS crystals has been reported. Even from dilute solution, embrionic spherulites and aggregates were formed. We observe that these morphologies result when solutions containing uncrystallized polymer are cooled. To obtain samples of uniform single crystals, we have used two-stage self seeding and solution replacement techniques.

Die mit dem Aufkommen der elektronischen Medien einhergehende Informationsflut hat die Erwartungen an den Dokumentationsdienst (DOKDI) der Schweizerischen Akademie der Medizinischen Wissenschaften verändert: Insbesondere Evidence Based Medicine (EBM) verlangt nicht nur die Beschaffung von Information, sondern auch eine Selektion hinsichtlich Qualität und Relevanz: Die sich aus der klinischen Situation ergebende Frage fordert eine Antwort, die inhaltlich richtig ist und in der konkreten Situation auch weiterhilft. Dem Ideal, sich durch kritische Lektüre der Originalarbeiten ein Bild über die vorhandene Evidenz für die Richtigkeit eines bestimmten Prozederes zu verschaffen, stehen in der Praxis meist Zeitmangel und methodische Schwierigkeiten im Weg; man wird sich deshalb oft auf die durch andere erarbeitete Evidenz abstützen müssen und z.B. die Cochrane Library konsultieren. Der DOKDI engagiert sich sowohl bei der Erarbeitung von systematischen Übersichtsarbeiten als auch bei der Dissemination der gefundenen Evidenz, indem er seine Erfahrung in der Dokumentation mit elektronischen Medien und die entsprechende Infrastruktur zur Verfügung stellt. Als Ergänzung zu diesen Aktivitäten hat die Akademie einen Grant zur Ausbildung von EBM-Tutoren gesprochen. In einem einwöchigen Kurs in Oxford werden Kliniker zu EBM-Tutoren ausgebildet: Dies wird zukünftig ermöglichen, vermehrt EBM-Workshops in der Schweiz durchzuführen.
