Background: Health-related quality of life (HRQL) is important for evaluating the impact of a disease in the longer term across the physical and psychological domains of human functioning. The aim of this study is to evaluate HRQL in COVID-19 survivors in Italy using the short form 36-items questionnaire (SF-36). Methods: This is an observational study involving adults discharged home following a coronavirus disease 2019 (COVID-19)-related hospital admission. Baseline demographic and clinical data including the Cumulative Illness Rating Scale (CIRS) and the Hospital Anxiety and Depression Scale (HADS) were collected. The validated Italian version of SF-36 was administered cross-sectionally. The SF-36 contains eight scales measuring limitations in physical and social functioning, the impact on roles and activities, fatigue, emotional well-being, pain and general health perception. Results: A total of 35 patients, with a mean age of 60 years, completed the SF-36. The results showed difficulties across the physical and psychological domains, particularly affecting the return to previous roles and activities. A higher burden of co-morbidities as well as a more severe muscle weakness was associated to a lower physical functioning. Younger age, rather than older, correlated to a perceived greater limitation in physical functioning and vitality. Conclusions: COVID-19 survivors particularly the ones of working age may need support for resuming their premorbid level of functioning and returning to work.

Οι ΔΔΠ είναι χρόνιες και περιοριστικού τύπου πνευμονοπάθειες με υψηλή νοσηρότητα, θνησιμότητα και κακή ποιότητα ζωής. Η συχνότητα και οι συνέπειες των διαταραχών του ύπνου καθώς και οι αλλαγές στη ποιότητα ζωής των ασθενών αυτών αποτελεί έναν τομέα έρευνας σε εξέλιξη, ωστόσο υπάρχουν σημαντικές αλληλοσυγκρουόμενες απόψεις μεταξύ παλαιότερων και πιο πρόσφατων μελετών.Σκοπός της παρούσας μελέτης ήταν η διερεύνηση της ύπαρξης των διαταραχών του ύπνου στην ΙΠΙ και τη σαρκοείδωση καθώς και η αξιολόγηση της ποιότητας ζωής των ασθενών αυτών. Επίσης πραγματοποιήθηκε συσχέτιση των παραμέτρων του ύπνου με τις παραμέτρους τις ποιότητας ζωής, με σκοπό να αξιολογηθεί κατά πόσο οι διαταραχές του ύπνου επηρεάζουν την ποιότητα ζωής και την καθημερινότητα των ασθενών αυτών. Συμπεριλήφθηκαν σαράντα ασθενείς, 19 με ΙΠΙ (11 άντρες και 8 γυναίκες) και 21 ασθενείς με σαρκοείδωση σταδίου II / III (7 άντρες και 14 γυναίκες). Έγινε σύγκριση με 15 υγιή άτομα χωρίς διαταραχές στον ύπνο, αντίστοιχης ηλικίας και ΒΜΙ με τους ασθενείς. Κατά τη διάρκεια της επίσκεψής τους καταγράφηκε πλήρες ιστορικό και έγινε κλινική εξέταση. Tα δεδομένα των ασθενών περιλάμβαναν στοιχεία από το ατομικό και ιατρικό ιστορικό του ασθενούς, φύλο, ηλικία, ΒΜΙ, κλινικά σημεία, συνοδά νοσήματα και φαρμακευτική αγωγή και τα αποτελέσματα της μελέτης ύπνου. Όλοι οι ασθενείς προσκόμισαν πρόσφατη σπιρομέτρηση με διάχυση η οποία εμφάνιζε περιοριστικού τύπου πνευμονοπάθεια και μείωση της διάχυσης, ιδιαίτερα στους ασθενείς με ΙΠΙ, και υποβλήθηκαν σε δοκιμασία βάδισης 6 λεπτών (6 min walk test 6MWT). Οι ασθενείς και οι μάρτυρες πραγματοποίησαν νυχτερινή πολυπνογραφία και συμπλήρωσαν τα ερωτηματολόγια Epworth, Berlin και Stop-Bang. Προκειμένου να αξιολογηθεί η ποιότητα ζωής, όλοι οι συμμετέχοντες συμπλήρωσαν το ερωτηματολόγιο Short-Form 36 (SF-36). Από τους ασθενείς με ΙΠΙ, το 68% διαγνώστηκε με ήπια αποφρακτική άπνοια ύπνου 5,2% με μέτρια έως σοβαρή, 5,2% με σοβαρό ΣΑΑΥ και 21% χωρίς αποφρακτική άπνοια ύπνου. Από τους ασθενείς με σαρκοείδωση, 52,4% διαγνώστηκαν με ήπιο ΣΑΑΥ και 4,8% με μέτριο. Το υπόλοιπο 42,8% δεν παρουσίασε ΣΑΑΥ. Η ποιότητα ζωής που σχετίζεται με την υγεία (Health Related Quality of Life HRQL) τόσο στους ασθενείς με ΙΠΙ όσο και στους ασθενείς με σαρκοείδωση παρουσίασε διαταραχές ειδικά στους τομείς της σωματικής υγείας και του βαθμού ανεξαρτησίας στην καθημερινότητά τους, σε σύγκριση με την ομάδα ελέγχου. Οι ασθενείς με ΙΠΙ παρουσίασαν θετική συσχέτιση του ΑΗΙ με το ερωτηματολόγιο Epworth, γεγονός που υποδεικνύει ότι η ημερήσια υπνηλία, που παρουσιάζουν οι ασθενείς αυτοί, οφείλεται στην κακή ποιότητα ύπνου. Στους ασθενείς με ΙΠΙ και σαρκοείδωση που ελέγχθηκαν στη παρούσα μελέτη, η αποφρακτική άπνοια στον ύπνο ήταν συχνή και ήπιας βαρύτητας. Η έγκαιρη και αξιόπιστη διάγνωση των διαταραχών του ύπνου και ιδιαίτερα του ΣΑΑΥ σε ασθενείς με ΙΠΙ και σαρκοείδωση και η βελτίωση της ποιότητας ζωής τους πρέπει να αποτελεί πρωταρχικό στόχο των θεραπόντων ιατρών. Το ερωτηματολόγιο SF-36, μόνο του ή σε συνδυασμό με άλλα ερωτηματολόγια ειδικά σχεδιασμένα για τις ασθένειες αυτές, αποτελεί ένα χρήσιμο και επικυρωμένο εργαλείο για την αξιολόγηση της ποιότητας ζωής των ασθενών αυτών.

Abstract Health-related quality of life (HRQoL) is severely impaired in persons receiving dialysis. Malnutrition has been associated with some measures of poor HRQoL in cross-sectional analyses in dialysis populations, but no studies have assessed the impact of malnutrition and dietary intake on change in multiple measures of HRQoL over time. We investigated the most important determinants of poor HRQoL and the predictors of change in HRQoL over time using several measures of HRQoL. We enrolled 119 haemodialysis and 31 peritoneal dialysis patients in this prospective study. Nutritional assessments (Subjective Global Assessment [SGA], anthropometry and 24-hour dietary recalls) and HRQoL questionnaires (Short Form-36 [SF-36] mental [MCS] and physical component scores [PCS] and European QoL-5 Dimensions [EQ5D] health state [HSS] and visual analogue scores [VAS]) were performed at baseline, 6 and 12 months. Mean age was 64(14) years. Malnutrition was present in 37% of the population. At baseline, malnutrition assessed by SGA was the only factor independently (and negatively) associated with all four measures of HRQoL. No single factor was independently associated with decrease in all measures of HRQoL over 1 year. However, prevalence/development of malnutrition over one year was an independent predictor of 1-year decrease in EQ5D HSS and 1-year decrease in fat intake independently predicted the 1-year decline in SF-36 MCS and PCS, and EQ5D VAS. These findings strengthen the importance of monitoring for malnutrition and providing nutritional advice to all persons on dialysis. Future studies are needed to evaluate the impact of nutritional interventions on HRQoL and other long-term outcomes.

Background: Long-term morbidity and mortality outcomes of the arterial switch operation (ASO) in patients with transposition of the great arteries and Taussig-Bing anomaly are excellent. With an increasing number of patients reaching adolescence and adulthood, more attention is directed toward quality of life. Our study aimed to determine the health-related quality of life (hrQoL) outcomes in patients after the ASO and identify factors influencing their hrQoL. Methods: In this cross-sectional study, hrQoL of patients after ASO was assessed with the German version of the Short Form-36 (SF-36) and the potential association of specified clinical factors was analyzed. Patients of at least 14 years of age who underwent ASO in our institution from 1983 were considered eligible. Results: Of the 355 questionnaires sent to eligible patients, 261 (73%) were available for analysis. Compared to the reference population, patients who had undergone ASO had a significantly higher score in all subscales of the SF-36 except for vitality ( P < .01). Patients with an implanted pacemaker ( P = .002), patients who required at least one reoperation ( P < .001), and patients currently taking cardiac medication ( P < .004) or oral anticoagulation ( P = .036) had lower physical component scores compared to patients without these factors. Conclusions: Patients’ self-assessed and self-reported hrQoL after ASO (using German version of the Short Form 36) is very good. In this population, hrQoL is influenced by reoperation, the need for a pacemaker, and current cardiac medication or anticoagulant use. The development of strategies designed to mitigate or minimize the requirements for, and/or impact of these factors may lead to better hrQoL in this patient population.

Aim. To evaluate how health-related quality of life (HRQOL) and traditional Chinese medicine (TCM) constitutions of Yin-Xu, Yang-Xu, and Stasis are related in type 2 diabetes patients. Method. Seven hundred and five subjects were recruited in 2010 for this study from a Diabetes Shared Care Network in Taiwan. Generic and disease-specific HRQOL were assessed by the short form 36 (SF-36) and the diabetes impact measurement scale (DIMS). Constitutions of Yin-Xu, Yang-Xu, and Stasis were then assessed by the body constitution questionnaire (BCQ), a questionnaire consisting of 44 items that evaluate the physiological state based on subjective symptoms and signs. Results. Estimated effects of the Ying-Xu and Stasis on all scales of the SF-36 were significantly negative, while estimated effects of the Yang-Xu on all scales (except for SF, RE, MH, and MCS) were significantly negative. For DIMS, the estimated effects of the Ying-Xu and Stasis on all scales were significantly negative except for Stasis on well-being, while Yang-Xu has a significantly negative effect only on symptoms. Conclusions. This study demonstrates that TCM constitutions of Yin-Xu, Yang-Xu, and Stasis are closely related to a reduction in HRQOL. These findings support the need for further research into the impact of intervention for TCM constitutions on HRQOL in patients with type 2 diabetes.

<b><i>Introduction:</i></b> While eccentric (ECC) training appears to be more efficient than concentric (CON) training at improving body composition in adolescent with obesity, its impact on health-related quality of life (HRQOL) has never been studied. <b><i>Objective:</i></b> The aim of this study is to compare the effects of 2 cycling training modalities, i.e., ECC vs. CON, in adolescents with obesity on HRQOL and health perception (HP). <b><i>Methods:</i></b> A total of 24 adolescents with obesity, aged 12–16 years, were randomized to either a 12-week ECC or a CON cycling training program performed at the same oxygen consumption (VO<sub>2</sub>). Anthropometric measurements, body composition, maximal incremental tests, HRQOL (Vécu et Santé Percue de l’Adolescent [VSP-A], Medical Outcome Study Short Form [SF-36]), and HP were assessed at before and after training. <b><i>Results and Conclusion:</i></b> Both CON and ECC cycling trainings promoted significant improvements in BMI, VO<sub>2peak</sub>, total fat mass, and fat-free mass, with better improvements in body composition parameters in the ECC group (<i>p</i> &#x3c; 0.05). The VSP-A total score increased after CON (<i>p</i> &#x3c; 0.01) and ECC (<i>p</i> &#x3c; 0.001) training, with better enhancement for the ECC group (<i>p</i> &#x3c; 0.05). The SF-36 physical score increased after both CON (<i>p</i> &#x3c; 0.01) and ECC (<i>p</i> &#x3c; 0.001) trainings. The global HP score increased only after ECC training (<i>p</i> &#x3c; 0.001). Except for the energy-vitality item, no significant correlation was found between changes in HRQOL and its subdomains and anthropometric, body composition, and functional parameters. Both ECC and CON cycling trainings are associated with positive changes in HRQOL and HP. However, ECC seems to induce greater improvements in HRQL and HP than CON cycling training, which is probably not due to the anthropometric, body composition, and functional changes.
