The Assessment and Minimally Invasive Management of Existing Restorations

This article provides an overview of current knowledge and understanding of existing criteria for the assessment of dental restorations and encourages dental practitioners to shift, if not already doing so, to considering minimally invasive interventions for manging deteriorating restorations. The repair of restorations in such a way extends longevity of the restoration without sacrificing intact, healthy tooth tissue, and is in the best interest of patients in terms of biological and economic costs. The replacement of a restoration should be only considered as a last resort, when there are no other viable alternatives. CPD/Clinical Relevance: Standardised assessment of dental restorations, using established criteria for clinical judgement and decision-making, is particularly important when managing deteriorating restorations in clinical practice. Minimally invasive management of such restorations, in terms of restoration repair strategies, should be viewed as a safe, viable and effective alternative to other more invasive treatments. The reader should understand the clinical evaluation of dental restorations based on reported standardised parameters and appreciate the benefits of minimally invasive management of deteriorating, yet serviceable, dental restorations in clinical practice.

Noncompaction cardiomyopathy (NCCM) has gained increasing attention over the past twenty years, but in daily clinical practice NCCM is still rarely considered. So far, there are no generally accepted diagnostic criteria and some groups even refuse to acknowledge it as a distinct cardiomyopathy, and grade it as a variant of dilated cardiomyopathy or a morphological trait of different conditions. A wide range of morphological variants have been observed even in healthy persons, suggesting that pathologic remodeling and physiologic adaptation have to be differentiated in cases where this spongy myocardial pattern is encountered. Recent studies have uncovered numerous new pathogenetic and pathophysiologic aspects of this elusive cardiomyopathy, but a current summary and evaluation of clinical patient management are still lacking, especially to avoid mis- and overdiagnosis. Addressing this issue, this article provides an up to date overview of the current knowledge in classification, pathogenesis, pathophysiology, epidemiology, clinical manifestations and diagnostic evaluation, including genetic testing, treatment and prognosis of NCCM.

Pancreatic ductal adenocarcinoma (PDAC) exhibits the poorest prognosis of all solid tumors, with a 5-year survival of less than 10%. To improve the prognosis, it is necessary to advance in the development of tools that help us in the early diagnosis, treatment selection, disease monitoring, evaluation of the response and prognosis. Liquid biopsy (LB), in its different modalities, represents a particularly interesting tool for these purposes, since it is a minimally invasive and risk-free procedure that can detect both the presence of genetic material from the tumor and circulating tumor cells (CTCs) in the blood and therefore distantly reflect the global status of the disease. In this work we review the current status of the main LB modalities (ctDNA, exosomes, CTCs and cfRNAs) for detecting and monitoring PDAC.

The aim of this review is to provide an update on the current status of evolving minimally invasive therapies for stress urinary incontinence. Bioinjectables have been available for some time and their current status is reviewed. The adjustable continence device has been used as a salvage procedure for females for a number of years in clinical trials, yet many are unfamiliar with it. Lastly, radiofrequency via a transurethral route has also been utilized in small numbers and will be updated. These later two emerging technologies need further exposure to better define their role in our clinical practice.

Running English for Specific Academic Purposes (ESAP) courses at tertiary institutions is becoming more challenging due to a number of factors and it leaves impact on lecturers’ emotions. Emotional dimension is among the aspects that significantly influences the way lecturers view assessments and their choice of assessment practices. This paper specifically looks at empathy as one emotional aspects present within lecturers as they undertake tasks of developing assessment materials and mitigate the challenges that they encounter. Six senior ESAP lecturers teaching engineering students at two engineering related universities reflected on this scenario via interviews and self-reflections. Their empathy in planning and developing assessment materials were elicited and scrutinised via qualitative approaches. All the lecturers exuded empathy based on professional grounds and directed towards essential learner factors and the intended ESAP course outcomes attainment. Empathy was found to be consistently demonstrated towards language learner factors and their language learning context that comprise aspects like background of students, language proficiency level, familiarity with language materials, current knowledge and skills required for test-taking. Such empathy for the best interest of the learners have induced careful and selective practices among the lecturers when preparing assessment materials. The presence of empathy in assessment practices appeared to be second nature to the lecturers and it helps to complement assessment best practices and upholds quality delivery of the ESAP courses. Empathy at a positive level has enabled the lecturers to embrace the spirit of assessment for learning where more time and efforts were devoted towards planning suitable assessments that promote understanding and mastery of the language, before going for the assessment of learning or summative assessments.

Clinical practice guidelines (CPGs), developed to inform clinicians, patients, and policy makers about what constitutes optimal clinical care, are one way of increasing implementation of evidence into clinical practice. Many factors must be considered by multidisciplinary guideline panels, including strength of available evidence, limitations of current knowledge, risks/benefits of interventions, patient values, and limited resources. Grading of Recommendations Assessment, Development and Evaluation (GRADE) is a framework for summarizing evidence that has been endorsed by many national and international organizations for developing CPGs. But is GRADE the right choice for CPGs developed by the American Academy of Otolaryngology–Head and Neck Surgery Foundation (AAO-HNSF)? In this commentary, we will introduce GRADE, discuss its strengths and limitations, and address the question of what potential benefits GRADE might offer beyond existing methodology used by the AAO-HNSF in developing CPGs.
