BACKGROUND:Determine the effect of personal anxiety, sleep disorders on the 15-year risk of developing (RR) diabetes mellitus (DM) type 2 among men (M) 45-69 years in Russia / Siberia (Novosibirsk). METHODS:In 20032005 As part of the IV screening of the international epidemiological study HAPIEE, a representative sample of m 4569 years was examined (n=781 M, mean age-56.480.20 years, response 61.00%). The level of personal anxiety (PA) was assessed using the Spielberger scale (HPA high, MPA medium, LPA low). With the help of the scale Knowledge and attitude to ones health, the level of sleep disorders (SD) was assessed. The period of observation of the cohort was 15 years. To check the statistical significance of the differences between the groups, the Pearson 2test was used. For risk assessment, Cox-regression single-factor and multivariate regression proportional hazards model was used (Cox-regression). RESULTS:Among persons with the first occurrence of DM type 2, HPA at the screening was 59.50%, and 63.30% had SD. The combination of HPA and SD was significant among M with the first-onset diabetes mellitus. During the 15-year period, among M with HLA, the risk of developing type 2 diabetes was 1.60 times higher than without. Among people with SD, therisk of developing type 2 diabetes was 2.40 times higher than without. In Cox the proportional multifactor model, each variable had its own independent influence. HPA increased the RR DM of the 2nd type by 1.90 times, and SD by 2.80 times. CONCLUSION:It was established that a high level of PA and SD are independent predictors of type 2 diabetes, moreover, with SD, the risk of developing type 2 diabetes is higher than with a high level of PA.

AbstractOver the last few decades, Mizoram has shown an increase in cases of type 2 diabetes mellitus; however, no in-depth scientific records are available to understand the occurrence of the disease. In this study, 500 patients and 500 healthy controls were recruited to understand the possible influence of their dietary and lifestyle habits in relation with type 2 diabetes mellitus. A multivariate analysis using Cox regression was carried out to find the influence of dietary and lifestyle factors, and an unpaired t test was performed to find the difference in the levels of biochemical tests. Out of 500 diabetic patients, 261 (52.3%) were males and 239 (47.7%) were females, and among the control group, 238 (47.7%) were males and 262 (52.3%) were females. Fermented pork fat, Sa-um (odds ratio (OR) 18.98), was observed to be a potential risk factor along with tuibur (OR 0.1243) for both males and females. Creatinine level was found to be differentially regulated between the male and female diabetic patients. This is the first report of fermented pork fat and tobacco (in a water form) to be the risk factors for diabetes. The unique traditional foods like Sa-um and local lifestyle habits like tuibur of the Mizo population may trigger the risk for the prevalence of the disease, and this may serve as a model to study other populations with similar traditional practices.

Abstract Aim Our aim was to compare the risk of developing inflammatory bowel disease [IBD] between ever users and never users of metformin. Methods Patients with newly diagnosed type 2 diabetes mellitus from 1999 to 2005 were enrolled from Taiwan’s National Health Insurance. A total of 340 211 ever users and 24 478 never users who were free from IBD on January 1, 2006 were followed up until December 31, 2011. Hazard ratios were estimated by Cox regression incorporating the inverse probability of treatment weighting using a propensity score. Results New-onset IBD was diagnosed in 6466 ever users and 750 never users. The respective incidence rates were 412.0 and 741.3 per 100 000 person-years and the hazard ratio for ever vs never users was 0.55 [95% confidence interval: 0.51–0.60]. A dose–response pattern was observed while comparing the tertiles of cumulative duration of metformin therapy to never users. The respective hazard ratios for the first [<26.0 months], second [26.0–58.3 months] and third [>58.3 months] tertiles were 1.00 [0.93–1.09], 0.57 [0.52–0.62] and 0.24 [0.22–0.26]. While patients treated with oral antidiabetic drugs [OADs] without metformin were treated as a reference group, the hazard ratios for patients treated with OADs with metformin, with insulin without metformin [with/without other OADs] and with insulin and metformin [with/without other OADs] were 0.52 [0.42–0.66], 0.95 [0.76–1.20] and 0.50 [0.40–0.62], respectively. Conclusion A reduced risk of IBD is consistently observed in patients with type 2 diabetes mellitus who have been treated with metformin.

Objective. To determine the effects ofTinospora crispaon serum glucose and insulin levels in healthy subjects and patients with type 2 diabetes mellitus.Method. Serum from 10 healthy subjects and 10 diabetic participants, who had fasted overnight, were obtained every 30–60 minutes during the 3 hours of continued fasting and during the 3 hours after ingestion of 75 g of glucose with or without ingestion of 125 or 250 g ofTinospora crispadry powder capsule. Glucose and Insulin levels were analyzed and the areas under the curve for mean serum glucose and insulin levels were calculated.Result. The areas under the curve of mean serum glucose and insulin levels in both healthy and diabetic participants were not significantly different between with or withoutTinospora crispadry powder capsule. In diabetic participants the area under the curve of glucose was slightly lesser when 250 mg ofTinospora crispawas ingested, but not reaching statistical significance (478 and 444 mg min/ml, resp.,P=0.57).Conclusion. The results suggest thatTinospora crispaingestion cannot affect serum glucose and insulin levels in healthy subjects or patients with type 2 diabetes mellitus.

Background : Obesity and type 2 diabetes mellitus are associated to inflammatory state, which can be set off by the adipose tissue, once it is a metabolically active organ that can cause a chronic mild inflammatory state. Aim : To evaluate the correlation between preoperative C-reactive protein and postoperative complications risk in obese patients (grades II and III) after Roux-en-Y gastric bypass, with and without type 2 diabetes mellitus. Methods : Between 2008 and 2013 were analysed 209 patients (107 with diabetes), presenting body mass index >40 kg/m2or >35 kg/m2with comorbidities. During the postoperative period, two groups were evaluated: with and without complications. Preoperative ultra-sensitive C-reactive protein was measured by immunonephelometry method. Results : Complications occurred in seven patients (pulmonary thromboembolism, fistula, two cases of suture leak, pancreatitis, evisceration and upper digestive hemorrhage). No statistical significance was found regarding lipid profile and C-reactive protein between patients with and without type 2 diabetes mellitus. When compared to each other, both groups (with and without complications) presented with statistical significance regarding C-reactive protein level (7,2 mg/dl vs 3,7 mg/dl, p=0,016) and had similar weight loss percentage after 3, 6 and 12 months follow-up. Conclusions : Preoperative C-reactive protein serum level was higher in the group which presented complications after Roux-en-Y gastric bypass when compared to the group without postoperative complications.

Objectives.To disclose the link between the composition of urolithiasis, especially that of uric acid calculi, and obesity, prediabetes, type 2 diabetes mellitus, and hypertension.Materials and Methods.Patients who had urinary calculi and underwent surgical treatment were registered in the study. The composition of urinary calculi was analyzed and correlated with clinical features and biomedical profiles of the patients before surgical intervention.Results.A total of 666 patients with urolithiasis who underwent surgical management were registered and analyzed. In those who had uric acid calculi, there was a significant association with prediabetic (OR: 20.11, 95% CI: 7.40–54.63,P<0.001) and diabetic states (OR: 11.55, 95% CI: 4.41–29.97,P<0.001). It also seemed that uric acid calculi were related to obesity but there was no statistical significance (OR: 2.45, 95% CI: 0.91–6.62,P=0.078). There was no association of uric acid calculi with hypertension (OR: 1.08, 95% CI: 0.54–2.17,P=0.822) and concurrent urinary tract infection (OR: 0.93, 95% CI: 0.44–1.96,P=0.841).Conclusion.There was a remarkable association of uric acid calculi with prediabetic and diabetic states. The uric acid calculi were also seemingly associated with obesity in patients with urolithiasis undergoing surgical management.

ObjectiveTo evaluate the association between snoring frequency and type 2 diabetes mellitus (T2DM) according to age and gender in Chinese population.DesignA cohort study was performed in Suzhou site of the China Kadoorie Biobank. Residents who didn’t suffer from T2DM at baseline survey (2004–2008) and in half a year after baseline were enrolled in this study and followed cause-specific morbidity until 31 December 2013. All participants were requested to complete a detailed questionnaire and undergo anthropometric measurements. Cox regression models were used to estimate HRs and 95% CIs for the snoring and T2DM association.SettingWuzhong district, Suzhou, China.ParticipantsA total of 49 453 participants (men: 41.8%; mean age: 51.14±10.28 years) were enrolled in this study.Outcome measuresT2DM cases were defined as International Classification of Diseases 10th Revision code of E11 and were identified through disease registries and health insurance databases.ResultsDuring a media of 7.18 years follow-up, 1120 T2DM cases were identified. Higher T2DM incidence was observed in participants with frequent and occasional snoring compared with those without (4.80 and 2.87 vs 2.39 per 1000 person-years). The multivariable-adjusted model found snoring was independently associated with T2DM (HR 1.28, 95% CI 1.20 to 1.38), both in men (HR 1.25, 95% CI 1.10 to 1.41) and women (HR 1.28, 95% CI 1.17 to 1.39). Moreover, a significant multiplicative interaction effect between snoring and age was detected on T2DM risk (p=0.015).ConclusionsSnoring was independently associated with an increased risk of T2DM in Chinese population, both in men and women. Meanwhile, there was an interaction effect between snoring and age on T2DM risk.

ABSTRAK Retinopati diabetik adalah komplikasi mikrovaskular diabetes mellitus tipe 2 (DM Tipe 2) yang menjadi penyebab utama kebutaan pada orang dewasa. Penderita DM Tipe 2 berisiko 25 kali lebih mudah mengalami kebutaan dibanding nondiabetes. Minimnya pengetahuan pasien mengenai gejala, tidak adekuatnya pelayan kesehatan, dan masalah ekonomi merupakan faktor yang menyebabkan sulitnya penanganan retinopati diabetik. Kombinasi curcumin pada kunyit dan epigallocatechin-3-gallate (EGCC) pada daun teh hijau memiliki keunggulan sebagai terapi pencegahan diabetik retinopati. Curcumin adalah komponen utama kunyit, senyawa polifenol golongan flavonoid ini memiliki beragam khasiat sebagai antiinflamasi, antioxidant, antiangiogenik, dan antiproliferatif. EGCG merupakan komponen paling aktif yang ditemukan pada teh hijau dan memiliki efek antioxidant paling kuat. Senyawa aktif yang ada pada kedua tumbuhan tersebut memilki efek yang spesifik dalam mencegah terjadinya diabetik retinopati. Beberapa mekanisme berlangsung dalam tingkat sel sehingga efek yang diperoleh lebih efisien. Senyawa aktif curcumin dan EGCG merupakan senyawa alami yang berasal dari tanaman sehingga tidak akan menimbulkan efek samping pada dosis normal. Penggunaan metode mikroenkapsulasi dalam mengemas ekstak kunyit dan daun teh hijau akan membantu efektivitas kerja masing –masing zat aktif yang terkandung di dalam kedua tumbuhan tersebut. Zat aktif tersebut nantinya akan dikemas dalam bentuk butiran kecil yang penggunaannya dapat dikonsumsi langsung oleh penderita diabetes melitus tipe 2 sebagai terapi pencegahan diabetik retinopati. Kata Kunci: Curcumin, DM Tipe 2, EGCC, Mikroenkapsulasi, Retinopati Diabetik   ABSTRACT Diabetic retinopathy is a microvascular complication of type 2 diabetes mellitus (DM type 2) which is a major cause of blindness in adults. Patients with Type 2 diabetes are 25 times more likely to replace blindness than non-diabetics. The lack of patient knowledge about the symptom, inadequate health services, and economic problems are factors that cause difficulties in handling diabetic retinopathy. The combination of curcumin in turmeric and epigallocatechin-3-gallate (EGCC) on green tea leaves has an advantage as a preventative therapy for diabetic retinopathy. Curcumin is the main component of turmeric, the polyphenol compound of the flavonoid group has a variety of properties as anti-inflammatory, antioxidant, antiangiogenic, and antiproliferative. EGCG is the most active component found in green tea leaves and has the strongest antioxidant effect. The active compounds in both plants have specific effects on the use of diabetic retinopathy. Some of the ongoing results at the cellular level produced are obtained more efficiently. The active compound curcumin and EGCG are natural compositions needed from plants so that they will not cause side effects at normal doses. The use of microencapsulation method in packaging turmeric extract and green leaves will help each of them - support the active substances contained in both plants. This active substance will be packaged in small granules that can be used directly by people with type 2 diabetes mellitus as a preventative therapy for diabetic retinopathy.
