Background: Psychosocial stress, have been implicated as risk indicators for periodontal disease. Minimal evidence in literature exists to assess the effect of stress reduction therapy (SRT) in the outcome of non-surgical periodontal therapy (NSPT).Objective: Hence, we aimed to explore the possibility of employing SRT in stressed patients as an adjunctive intervention in the management NSPT.Materials and Methods: Sixty male patients divided into Group 1 comprised of 20 unstressed chronic periodontitis patients receiving non-surgical periodontal therapy alone, Group 2a (20 stressed chronic periodontitis patients) received intervention focused on SPT with NSPT and Group 2b (20 stressed chronic periodontitis patients) received only NSPT without SPT. Their stress level was evaluated using a standard questionnaire method and salivary cortisol levels, at baseline and 3 months along with periodontal parameters.Results: There was significant reduction in stress parameters for Group 2a patients. The reduction of total periodontitis affected sites was highest in Group I patients (55.4%), followed by Group 2a patients (53.2%) and Group 2b patients (38%).Conclusion: SRT may result in comparable treatment outcome to unstressed chronic periodontitis patients. This study opens new avenues to investigate the effects of stress management as an adjunctive to conventional NSPT, which should be explored further.

A study was undertaken at Ngwelezana hospital to determine the requirements for a cost effective physiotherapy service for patients with stabbed chests. Forty male patients between the ages of 16 and 60 who had sustained unilateral penetrating stab wounds to the chest which required intercostal drainage were randomised into one of two groups on admission to Ngwelezana hospital.The patients in group I received physiotherapy immediately after insertion of the intercostal drain, while the patients in group 2 received physiotherapy 12 to 24 hours after insertion of the drain. Mean duration of intercostal drainage in group I was 2.35 days while that of group 2 was 7.55 days. This represented a significantly shorter drainage time for the patients who had been treated immediately after insertion of the drain. Patients were discharged from hospital on the day that the intercostal drain was removed. No complications were experienced by the patients in group 1 while four patients in group 2 developed an empyema.The cost of the patients in group 2 exceeded the costs of those in group 1 by R78 728.00. It is thus imperative that patients admitted with stabbed chests should have physiotherapy immediately after insertion of the intercostal drain.

Visfatin is a pleiotropic mediator which acts as growth factor, cytokine, enzyme involved in energy including nicotinamide adenine dinucleotide metabolism and has been recently demonstrated to exert several pro-inflammatory functions. The purpose of this study is to evaluate the Visfatin concentration in gingival crevicular fluid (GCF) and serum in patients with chronic periodontitis, and to evaluate the effect of non-surgical periodontal therapy on the GCF and serum visfatin concentration. 30 subjects (age range: 25 to 52 years) were selected and divided into two groups based on the gingival index, probing depth, periodontal attachment level, and radiologic parameters (bone loss): group 1 (15 subjects with healthy periodontium), group 2 (15 subjects with chronic periodontitis), while, Group 2 patients after 8 weeks of the treatment (scaling and root planning, SRP) constituted group 3. GCF samples (by microcapillary pipettes) and serum samples (by venipuncture) were collected to estimate the levels of Visfatin using enzyme linked immunosorbent assay kit. The mean Visfatin concentration in GCF and serum was observed to be the highest in group 2 and lowest in group 1. While concentration in group 3 was similar to group 1. The concentration of Visfatin in GCF and serum decreased after SRP. The Visfatin concentration in GCF and serum found to be highest in chronic periodontitis group and decreases after treatment. Hence Visfatin values can be considered as an “inflammatory marker” can be explored in future as a potential therapeutic target in the treatment of periodontal disease.

Background. This study evaluated the effect of periodontal therapy on mucous membrane thickening in maxillary sinus in chronic periodontitis patients using radiovisiography (RVG) and cone-beam computed tomography (CBCT). Methods. The study population included 30 patients diagnosed with chronic periodontitis, exhibiting bilateral mucosal thickening of maxillary sinus. The selected sites were randomly assigned to group I (control group - not receiving periodontal therapy) and group II (test group - receiving periodontal therapy). The clinical parameters and mucosal thickening of the maxillary sinus were evaluated at baseline and after 9 months. Results. There was a significant decrease in the PPD, CAL as well as mucosal thickening in group II while, group I showed an increase in these parameters. In group II at the end of 9 months the mean mucosal thickening reduction as assessed by CBCT was 0.76±0.18, 0.73±0.24, 0.88±0.42 and 1.13±0.43 mm at the most anterior point (AP), the most posterior point (PP), the mid-point (MP), point of maximum thickness (MT) as well as in the length of the thickened mucosal lining, respectively. Conclusion. The results of our study indicated a reduction in the mucosal thickening of the maxillary sinus after surgical periodontal therapy. The trial was registered with the Clinical Trial Registry of India (Trial REF/ 2016/02/010805).

Recently discovered interleukin 29 (IL-29) has antiviral properties and its production is induced by herpes viruses. This study was aimed at analyzing the effect of non-surgical periodontal treatment on IL-29 levels in gingival crevicular fluid (GCF) of chronic and aggressive periodontitis patients. A total of 60 participants were divided into healthy group (group 1;n= 20), chronic periodontitis group (group 2;n= 20), and aggressive periodontitis group (group 3;n= 20). GCF samples collected from each subject at baseline and 6–8 weeks after scaling and root planing were quantified for IL-29 levels using ELISA. The mean IL-29 concentration in GCF was found to be highest in group 3 (92.37 pg/μl). The mean IL-29 level in group 1 and group 2 was 36.88 pg/μl and 69.35 pg/μl respectively. After scaling and root planing, the mean concentration of IL-29 in GCF was increased to 85.99 pg/μl in group 2 and to 114.64 pg/μl in group 3. Results of the present study indicate that antiviral IL-29 level was highest in GCF of aggressive periodontitis patients and least in subjects with healthy periodontium, while that of chronic periodontitis lying in between. After non-surgical periodontal therapy, IL-29 levels increased both in chronic and aggressive periodontitis patients and deserve further investigation as a potential therapeutic agent in treating periodontitis.

Background: The Triggering Receptor Expressed On Myeloid Cells-1(TREM-1) is a cell-surface receptor of the immunoglobulin superfamily and found to be involved in the amplification of the inflammatory response to various microbial infections, including periodontal diseases.Objectives: The present study was designed to examine gingivalcrevicular fluid(GCF) levels of soluble TREM-1 (sTREM-1) levels in periodontal health and disease as well as evaluate the effect of scaling and/or root planing on the same.Methods: Based on gingival index, probing pocket depth, clinical attachment level, and radiologic parameters (bone loss), 45 subjects were initially divided into three groups- Group, I (Periodontally healthy), Group II (Gingivitis) and Group III (Chronic Periodontitis).From each of the subjects, GCF sample was collected at baseline and scaling and/or root planing was instituted in group II and group III patients. GCF samples were subsequently collected at eight-week interval. Levels of sTREM-1 in collected GCF samples were estimated using enzyme-linked immunosorbent assay.Results: The lowest GCF levels of sTREM-1 were found in periodontal health (69.50±1.8pg/ml) followed by gingivitis (257.17±79pg/ml) and chronic periodontitis (3658.14±55pg/ml) in increasing order, suggesting that levels of sTREM-1 in crevicular fluid increased with the severity of periodontal disease. sTREM-1 levels decreased significantly from baseline to the end of 8 weeks following non-surgical periodontal therapy.Conclusion: Increased GCF levels of sTREM-1 from periodontal health to disease strengthen its association with periodontal status.
