“Blind football” was initiated in the last century. What has significantly contributed to its development are newly established organizations, the primary goal of which was to manage various forms of activating people with visual impairment. Popularisation of “blind football” is a long-term, complicated process, in which a plethora of factors are involved. The aim of this article is to reflect on “blind football”, especially on the problems with spreading it. Most of all, the article is an attempt to define the mechanism of the functioning of “blind football” in the sphere of meanings of contemporary culture.

Abstract BACKGROUND: Craniopharyngiomas (CRPs) often cause visual deterioration (VD) due to the close vicinity of the optic apparatus. OBJECTIVE: To evaluate longitudinal visual outcomes after surgery of CRP and determine the prognostic factors thereof. METHODS: One hundred forty-six adult patients who underwent surgery for newly diagnosed CRP were retrospectively reviewed. There were 87 male patients (60%), and the median age was 41 years (range, 18–75). The mean follow-up duration was 88.7 months (range, 24–307). A visual impairment score was used to assess the short-term (<1 month) and long-term (>2 years) visual outcomes. RESULTS: Gross total removal was performed in 53 patients (36%), and tumor recurrence occurred in 40 patients (27%). The average preoperative, short- and long-term visual impairment scores were 44.4, 38.5, and 38.1, respectively, on a 0- to 100-point scale (with 100 indicating the worst vision). Short- and long-term VD occurred in 28 (19%) and 39 patients (27%), respectively. Subtotal removal (STR) alone (P = .010; OR = 4.8), short-term VD (P < .001; OR = 39.7), and tumor recurrence (P < .001; OR = 28.2) were significant risk factors for long-term VD in the multivariate analysis. Patients undergoing STR alone had higher tumor recurrence rates in comparison with those who underwent gross total removal or STR with adjuvant therapy (P < .001). CONCLUSION: Short-term VD secondary to the surgical insult and the recurrence of the tumor were strong predictors of long-term visual outcomes after surgical treatment for CRP. STR alone may be an ineffective strategy for achieving tumor control and optimal visual outcomes in patients with CRP.

The paper examines the events of the Gaidar Forum – 2021 dedicated to the features of digital transformation. By analyzing the speakers’ presentations, the author notes the need to minimize the negative consequences of the coronavirus pandemic for the world economy, to unleash the creative potential and sustainable development of countries. At the same time, the question of creating an image of the future, a semantic paradigm, a strategy for the development of an integral system remains relevant. This requires strengthening business partnerships, well-coordinated interaction of government agencies (departmental and interdepartmental), and improvement of the digital economy during its transformation. The digitalization of economy enhances economic growth, creates opportunities for structural transformations, and necessary conditions for the “post-covid” development of the healthcare system. Digitalization should bring about a proactive, standardized online public service delivery. Digital transformation brings many significant structural changes affecting many areas of our lives. Since digitalization is a long-term and complicated process, particular attention should be paid to maintaining the stability of the labor market, the efficiency of the education system, and the security of big data. Digital transformation should improve the quality of public services, the overall number of digital service users, and the growth of citizens’ well-being

Purpose To determine the long-term visual outcomes and risk factors for visual loss in children with primary congenital glaucoma (PCG) who underwent angle surgery over a 21-year period. Methods This was a retrospective study of patients who underwent trabeculotomy, trabeculectomy, or combined trabeculotomy-trabeculectomy for PCG. Visual acuity (VA), surgeries, and underlying cause of visual impairment were recorded and compared. Main outcome measures were final best-corrected good VA (20/20 to 20/50), moderate VA (<20/50 to 20/200), or poor VA (<20/200); age at surgery, sex, laterality, type of initial glaucoma surgery, and number of surgeries were recorded. Results Fifty-three eyes were eligible to be included in the study. The mean logMAR VA was 0.61 ± 0.57. A good VA was attained in 51%, moderate VA in 30%, and poor VA in 19%. The main cause of visual impairment with VA <20/50 was deprivation amblyopia (64%). There was no association between VA level and status of success (controlled with or without medications) (p = 0.202). The mean spherical equivalent of refraction was -4.47 ± 5.66; myopia was the predominant refractive error (74%) and astigmatism >2 D (40%). A statistically significant high myopic shift was more frequent in the visually impaired group. None of the studied factors was statistically associated with moderate to poor visual outcome (all p>0.05). Conclusions A favorable VA outcome was achieved. Topical antiglaucoma medication has an adjuvant role in maintaining the success rate of surgery without risking the visual outcome. Corneal opacification and anisometropia were the cause of amblyopia.

To explore patients’ existential challenges when living with chronic pain or multiple sclerosis, 23 semistructured patient interviews were conducted together with an interpretative, phenomenological analysis inspired by Søren Kierkegaard. Patients experienced their illness as a “stroke of fate” resulting in despair and a profound struggle to find out, who they were, and how they could relate to themselves in their new life-situation. Becoming oneself was experienced as a long-term existential process realized by synthesizing the existential poles of necessities and possibilities through “the other,” such as relationships, nature, spirituality, or God. The different ways to respond to the existential challenges were identified as subjective choices of aesthetic, ethic, and religious life-spheres. The Kierkegaardian relational self and the life-spheres as diverse possibilities are valuable for understanding patients’ existential challenges and the complicated process of finding a new way to relate to oneself when life is fundamentally transformed by illness. To resolve the existential struggles, it seems essential to take a subjective stand in life and have a relationship to something bigger than oneself. The findings can inform clinical practice and inspire health care professionals to identify patients who would welcome a dialogue about illness-related existential struggles for the benefit of their health.

Transformation of the oceanic crust into the continental one in orogenic belts is an important problem in petrological studies. In the paleocontinental sector of the Urals, a key object for tracing the stages of metamorphism and investigating the origin of anatectic granites is the Murzinka-Adui metamorphic complex. We have analyzed trace elements in zircons and established their genesis, sources, crystallization conditions, and stages of metamorphic events and granite generation in this complex. Zircons compositions were determined by the LA-ICP-MS method. Temperatures were calculated from Ti contents in the zircons. We distinguish three geochemical types of zircons, which differ in the ratios of light and heavy REE, U, Th, Ti, Y and show different values of Ce- and Eu-anomalies and Zr/Hf ratios, which are indicative of different crystallization conditions, as follows. Type I: minimal total LREE content; clear negative Eu- and Ce- anomalies; features of magmatic genesis; crystallization temperatures from 629 to 782 °C. Type II: higher contents of Ti, La, and LREE; low Ce-anomaly; assumed crystallization from highly fluidized melts or solutions. Type III: low positive Eu-anomaly; high REE content; low Th/U-ratio; zircons are assumed to originate from a specific fluidized melt with a high Eu-concentration. Ancient relict zircons (2300–330 Ma) in gneisses and granites show features of magma genesis and belong to types I and II. Such grains were possibly inherited from granitoid sources with different SiO2 contents and different degrees of metamorphism. Based on the geological and petrogeochemical features and zircon geochemistry of the Murzinka-Adui complex, there are grounds to conclude that the material composing this complex was generated from the sialic crust. The main stages of metamorphism and/or granite generation, which are traceable from the changes in types and compositions of the zircons, are dated at 1639, 380–370, 330, and 276–246 Ma. Thus, transformation of the oceanic crust into the continental one was a long-term and complicated process, and, as a result, the thickness of the sialic crust is increased in the study area.

Abstract Background To explore the association of clinical characteristics and retinal microstructural features on optical coherence tomography in predicting 1-year visual response following intravitreal bevacizumab injections in eyes with visual impairment from center-involved diabetic macular edema (CI-DME). Methods Medical records of patients with visual impairment from CI-DME, who initiated intravitreal bevacizumab injections between Jan 2012 and Dec 2016 and were followed for a minimum of 12 months were retrospectively reviewed. Results The study included 226 eyes with a mean (SD) baseline visual acuity (VA) of 51.8 (19.1) letters. At week 12, following the three initial treatments, a mean (SD) VA improved to 61.7 (17.8) letters. Visual gain ≥ 10 letters was observed in 109 eyes (48.2%), while a limited early visual gain < 5 letters was noted in 80 eyes (35.4%). At one year, 110 eyes (48.7%) achieved a good VA gain ≥ 10 letters. In addition, eyes with poor baseline VA had a higher proportion of eyes that obtained limited early VA gained at week 12 (< 5 letters) and maintained in this visual response category at moth 12 compared to eyes with better baseline VA (74.1% versus 59.1%). In the multivariable logistic regression, the following factors reduced the probability of 1-year visual gain ≥ 10 letters: elderly (p = 0.040), better baseline vision (p = 0.001), and limited early visual gain < 5 letters at week 12 (p < 0.001). In multivariable linear regression, male (p = 0.010) and eyes with the presence of hyperreflective foci on baseline OCT (p = 0.010) were likely to have higher VA improvement. However, eyes with better baseline VA (p = 0.002), limited early VA gain at week 12 (p < 0.001), and a presence of EZ disruption at week 12 (p = 0.002) were likely to have less VA improvement. Conclusions Although bevacizumab is considered as effective management for CI-DME, variability in treatment responses is expected. This study revealed that baseline characteristics and visual responses at week 12 might help predict the long-term treatment response. Eyes with characteristics at risk of limited long-term visual outcome may require attention in optimizing their individual treatment strategies.
