The objective: to study the significance of the presence of hypothyroidism syndrome as a risk factor for the development of autonomic neuropathy (AN). Materials and methods. The study involved 77 women, of whom 31 had hypothyroidism syndrome, and 46 had euthyroid thyroid pathology. The age of patients with hypothyroidism syndrome ranged from 24 to 57 years and averaged 42.4±9.20 years, mean age patients of the euthyroid pathology group of the thyroid gland was equal to 40.0±12.22 years (age interval 20–63 years). Patients underwent an ultrasound examination of the thyroid gland, fasting glycemia levels, A1c, TSH, T4s, T3s, blood creatinine levels, and GFR were calculated using CKD-EPI on line equations. In addition, antithyroglobulin and antithyroperoxidase levels were determined. To diagnose autonomic neuropathy, a Valsalva test and a deep breathing test were used. Statistical analysis was performed using the standard computer program Microsoft Excel. Results. The state of autonomous innervation was normal in 71.0% of the examined hypothyroidism group and in 97.8% of the euthyroidism group. AN was absent both in hypothyroidism syndrome (0.0%) and in euthyroid thyroid pathology (0.0%). Moreover, the “borderline state” of autonomic innervation was observed in 29.0% of the examined groups of hypothyroidism syndrome and only 2.2% of the examined groups of thyroid euthyroid pathology (p=0.0019). The chance of finding parasympathetic disorders in the hypothyroidism group was 0.409, and in the group of euthyroid thyroid pathology was 0.022. Conclusions. Thus, in the presence of hypothyroidism, the chances of finding autonomic disorders were 18.409 times higher than those for euthyroid thyroid pathology, which requires appropriate correction when planning comprehensive treatment tactics. Целью научной работы является изучение структуры заболеваний щитовидной железы у больных сахарным диабетом 1 типа. В данной статье мы ретроспективно проанализировали 972 историй болезни больных детей с СД 1 типа, находившихся на стационарном лечении в ДГКБ №2 г. Алматы (Казахстан) в период с 2014 по 2019 гг. Были изучены и оценены показатели физического развития, объективные данные (кожные покровы, ЧСС, АД, пальпация ЩЖ), лабораторно - уровней гормонов ТТГ, свТ4, свТ3, а/т к ТПО, а/т к ТГ в сыворотки крови, инструментально - УЗИ ЩЖ. Всего за 2014-2019 гг. через отделение эндокринологии ДГКБ №2 прошли 972 детей с диагнозом СД 1 типа. Большинство детей (382 человек, 79,9%) имели стаж болезни СД до 5 лет. 88 детей (18,5%) со стажем от 5 до 10 лет, 8 человек (1,7%) страдали СД более 10 лет. СД1 в основном был диагностирован в возрасте 7-12 лет (245-51,3%), меньше всего выявили СД 1 типа у детей до 3 лет (21 - 4,4%). Из общего количества пациентов с СД1 (972) было обследовано на функцию ЩЖ 478 детей (49,2%). Среди них было выявлено 319 детей с дисфункцией ЩЖ, что составляет 66,7%. Так, за 2014 год из 92 детей - 7 (7,6%), обследованных на функцию щитовидной железы, в результате чего было выявлено 6 (85,7%) детей с дисфункцией щитовидной железы. С каждым годом росло количество детей, которых направляли на обследование ЩЖ, так в сравнении с 2014 годом, когда из 92 детей - 7 (7,6%) были обследованы на функцию щитовидной железы, в 2019 году были обследованы уже 222 (92,1%) детей из 241. Симптомы как гиперфункции, так и гипофункции ЩЖ, особенно их субклинические варианты протекают под маской других заболевании и не сразу обнаруживаются, исходя из этого следует сразу обследовать на функцию ЩЖ при поступлении и в дальнейшим их наблюдать в динамике. В ходе исследования дисфункция щитовидной железы диагностирована у 319 (67,7%) пациентов, что должно привлечь внимание не только эндокринологов, но и врачей общей практики, педиатров и настроить их на прицельный поиск этой патологии и своевременную коррекцию гипотиреоза или другой патологии ЩЖ при его наличии The purpose of this research is to study the structure of thyroid diseases in patients with type 1 diabetes. Type 1 diabetes was mainly diagnosed at the age of 7-12 years (245-51. 3%), the least detected type 1 diabetes in children under 3 years (21 - 4.4%). Out of the total number of patients with type 1 diabetes (972), 478 children (49.2%) were examined for thyroid function. Among them, 319 children with thyroid dysfunction were identified, which is 66.7%. So, in 2014, out of 92 children, 7 (7.6%) were examined for thyroid function, as a result of which 6 (85.7%) children had thyroid dysfunction. Every year, the number of children referred for thyroid examination increased, so compared to 2014, when out of 92 children - 7 (7.6%) were examined for thyroid function, in 2019, 222 (92.1%) children out of 241 were examined. Symptoms of both hyperfunction and hypofunction of the thyroid gland, especially their subclinical variants, occur under the guise of other diseases and are not immediately detected, so you should immediately investigate the function of the thyroid gland at admission and further observe them in dynamics. During the study, thyroid dysfunction was diagnosed in 319 (67.7%) patients, which should attract the attention of not only endocrinologists, but also General practitioners, pediatricians and set them up for a targeted search for this pathology and timely correction of hypothyroidism or other thyroid pathology if it is present.

Diagnostics, tactics of medical and surgical treatment of various diseases of the thyroid gland is a topical issue in patients of older age groups. The peculiarity of this category of patients is high comorbidity, so the volume of surgery should be determined not only by the type and severity of the course of thyroid pathology, but also by the clinical characteristics of concomitant diseases. To date, the issue of choosing the volume of surgery in elderly and senile patients remains debatable.

IntroductionThe aim of the study was to assess of clinical aspects of thyroid follicular adenoma in children and attempt to elaborate management algorithm useful in a clinical practice.Material and methodsClinical database of all children operated due to thyroid disease in years 1993-2018 at the university pediatric surgical centre was reviewed. The children in whom postoperative histology report showed follicular adenoma were selected and their medical files were analysed in detail. Clinical course of the disease, medical and family history, results of imaging studies, details of surgical management and final outcome were assessed in each case.ResultsAmong 183 children operated on for thyroid pathology in the study period there were 56 patients with follicular adenoma ( 30,6%). Their age ranged from 7 to 18 years. Seven patients were found hypothyroid and further seven showed hyperthyroid status initially. Ultrasound scan showed a solitary nodule in 37 children (66,1%). Multiple nodules in one lobe were detected in 5 patients, while further 14 children showed bilateral thyroid pathology. The result of preoperative cytologic examination was suspicious in 12 children (21,4%). All children were subjected to an operative management. Forty- two children (75%) underwent at least unilateral total lobectomy. No patient showed a recurrent thyroid disease.ConclusionsFollicular adenoma in children presents a wide clinical and morphological spectrum. High incidence of associated diseases of other systems and congenital anomalies of the thyroid gland may indicate on complex etiology of follicular adenoma with participation of yet unknown endogenic factors in children.

As Residências Multiprofissionais em Saúde (RMS) encontram-se distribuídas por todo o território nacional e estão estruturadas em abordagem multiprofissional com uma visão mais humana e abrangente, cujo foco não se restringe apenas aos procedimentos curativos. O objetivo do presente estudo foi avaliar o perfil dos cirurgiões-dentistas regularmente matriculados em RMS da região Sul do país, bem como sua atuação e satisfação com os programas. Para isso, foi realizada uma pesquisa quantitativa, exploratória e descritiva, utilizando-se um questionário on-line, cujo link foi enviado a cirurgiões-dentistas matriculados em RMS (Atenção Básica, Saúde da Família e Comunidade e Saúde Coletiva), localizadas na região Sul do Brasil. Os dados coletados foram tabulados no Microsoft Excel® e tratados por estatística descritiva (frequência simples e relativa). Constatou-se que a maioria dos residentes era do gênero feminino (75%) com graduação em IES privada (70%), sem experiência profissional prévia (70%) e com experiência acadêmica prévia em saúde pública (85%). Para os dados referentes à RMS, a maioria relatou como motivo pela escolha da residência o interesse em atuar na atenção básica. Oitenta e cinco por cento dos dentistas relataram atuar em Unidades Básicas de Saúde. A maioria dos residentes mostrou-se satisfeito com a RMS (75%) e a falta de capacitação dos docentes foi apontada como a principal fragilidade dos programas. O estudo possibilitou traçar um panorama do perfil, atuação e satisfação dos cirurgiões-dentistas inseridos em RMS da região Sul do Brasil. Os dados podem auxiliar na melhoria das práticas e organização das RMS com cirurgiões-dentistas, conduzindo melhor a atuação de residentes e docentes frente às principais fragilidades apontadas.

Atualmente, para os turistas, os smartphones são imprescindíveis para a realização de uma viagem, quer para procurar informação, quer para comunicar com os amigos e os familiares, neste sentido, o presente artigo pretende investigar a importância dos smartphones para a experiência associada à viagem turística. Foi elaborado e aplicado um questionário on-line, com o objetivo de averiguar a opinião dos utilizadores de smartphones sobre a importância deste tipo de tecnologia e de que forma é utilizada. Após recolha dos dados, procedeu-se à sua limpeza e verificação para detetar as respostas válidas; em seguida, foram aplicadas as ferramentas de Business Intelligence integradas no Microsoft Excel. Como resultados, obteve-se que os turistas consideram relevantes a utilização destes dispositivos, que os utilizam preferencialmente para consultar informações sobre o destino turístico e pontos de interesse, bem como a rede social que mais utilizam é o TripAdvisor.

This research is organized to investigate if the thermal gradient (Tgrad) (difference between temperature maximum and minimum in the region of interest over thyroid gland on neck thermogramm) could be a marker of normal/ abnormal thyroid function and, if so, find out the cut-off value and check it screening validity.Materials and methods. Thermography examination was performed using a TVS-300 med (S-Petersburg, Russia) thermal imaging camera with a resolution of 384 × 288 pixels and sensitivity was <0.04 °C according to European Association of Thermology standards. Anterior neck thermografic images were performed and collected in cloud database, where, by mean of program tools, region of interest were marked and Tgrad calculated. Totally 1025 pts with different thyroid pathology (group 1: female 944, age 15–90 (55,4±15,8), male 81, age 14–86 (53,6±18,8)), and 395 healthy persons (group 2: male 324 age 17–26 (21,8±4,4), female 71 age 16–70т (34,6±12,7)) were investigated. To check the quality of the model another 314 persons (18 male and 296 female) were studied in screening mode.Results. Calculated Tgrad value was 1,65±0,53 °C, Ме 1,53(1,24; 1,90) and 0,98±0,23 °C, Me 1,01(0,82; 1,15) for group 1 and 2 accordingly. By mean of binary logistic regression analysis we found good correlation between Tgrad value and presence/absence of thyroid pathology: significance level р<0,0001, χ² = 838,84, df = 1, β 0 = 9,984, β 1 = –9,033. Area under curve in ROC analysis was 0,922 (95 % CI 0,908; 0,935), р<0,001; optimal cut off value for Tgrad was 1,21 °C for maximal sensitivity (79,8 %) and specificity (83,3 %). In screening evaluation, according to optimal cut-off value 1,2 °C for Tgrad , 82,2 % of cases were classified correctly: true-positive and true-negative results were achieved in 258 from 314 pts.Conclusion. Thermography test, based on Tgrad value calculation, could help to distinguish persons with normal and abnormal function of thyroid gland.

Objetiva-se identificar as evidências científicas sobre o isolamento social em pacientes com feridas neoplásicasexsudativas. Revisão integrativa realizada nas bases de dados da Biblioteca Virtual de Saúde (BVS) e PubMed: LiteraturaLatino Americana e do Caribe em Ciências da Saúde (LILACS), Medical Literature Analysis and Retrieval Sistem on-line(MEDLINE) e Cochrane Database of Systematic Reviews (COCHRANE) com a questão norteadora: quais são as evidênciascientíficas sobre o isolamento social em pacientes com feridas neoplásicas exsudativas? Utilizou-se estratégia PICOcom recorte temporal de 2002 a 2014, nos idiomas português, inglês e espanhol. As informações foram tabuladas emprograma Microsoft Excel® e processadas a partir da média aritmética descritiva simples, frequência absoluta e relativa.Identificaram-se 288 artigos em sua totalidade. Após aplicação dos critérios de elegibilidade, foram selecionadas 23evidências científicas avaliadas a partir da categoria: os principais aspectos psicossociais prejudicados pelo exsudatoem pacientes com feridas neoplásicas. Concluí-se que o exsudato é um dos sintomas que interfere negativamente nosaspectos psicossociais do paciente com feridas neoplásicas, causando alteração da imagem corporal, perda dos limitesfísicos, ansiedade e depressão, fatores que favorecem diretamente o isolamento social.Palavras-chave: Cuidados Paliativos; Enfermagem Oncológica; Ferimentos e Lesões; Exsudatos e Transudatos;Isolamento Social. ABSTRACTThe aim is to identify the scientific evidence on social isolation in patients with exudative neoplastic wounds.An integrative literature review conducted in the Virtual Health Library databases (VHS) and PubMed: LiteratureLatin American and Caribbean Health Sciences (LILACS), Medical Literature Analysis and Retrieval System online(MEDLINE) and Cochrane Database of Systematic Reviews (COCHRANE), with the guiding question: what are thescientific evidences about the social isolation in patients with exudative neoplastic wounds? We used a PICOstrategy with a temporal cut from 2002 to 2014, in Portuguese, English and Spanish languages. The informationwas tabulated in Microsoft Excel® program and processed from simple descriptive arithmetic mean, absoluteand relative frequency. It was identified 288 articles in their entirety. After applying the eligibility criteria, wasselected 23 scientific evidence evaluated from the category: the main psychosocial aspects impaired by exudatein patients with neoplastic wounds. It concludes that exudate is one of the symptoms that negatively interferesthe psychosocial aspects of the patient with neoplastic wounds, causing alterations in body image, loss of physicallimits, anxiety and depression, factors that directly favor social isolation.Keywords: Palliative Care; Oncology Nursing; Wounds and Injuries; Exudates and Transudates; Social Isolation.
