The article presents the most modern positions of healthcare delivery for children with primary ciliary dyskinesia. Symptoms of this pathology in clinical practice vary that is conditioned by genetic heterogeneity of the disease. The most common disease manifestation in children is frequent inflammatory diseases of the upper and lower respiratory tract. They are recorded in most patients, especially in young children, and the diagnosis is often determined untimely due to a low awareness of specialists about this nosology. Differential diagnostic approach is described in detail, peculiarities of treatment and management of children with this nosology are specified. The material is based on clinical guidelines developed and approved by the professional association «Union of Pediatricians of Russia».

NB-BAL is an effective procedure for the diagnosis of pulmonary disease processes in ventilated infants and children. This procedure is, however, not without risks to both patients and staff. Numerous complications of NB-BAL exist, with hypoxia being themost common. As a result, care should be taken in performing NB-BAL on haemodynamically unstable patients; patients with coagulation defects; and patients with cardiac or brain abnormalities.This paper presents an overview of paediatric nonbronchoscopic bronchoalveolar lavage (NB-BAL) including: the rationale for NB-BAL; the complications associated with the procedure; indications and contraindications. It also recommends an evidence-basedclinical guideline for performing the procedure in the paediatric intensive care unit. By following the NB-BAL guidelines presented in this paper, one can ensure that an effective specimen is obtained from the lower respiratory tract, whilst minimising the risk to the patient.

This article presents the results of a comprehensive otorhinolaryngological and microbiological study of diseases of the nose and paranasal sinuses in patients with community-acquired pneumonia. These diseases are closely interconnected, since the upper and lower respiratory tract are a single whole, have structures and functions and mechanisms for the development of pathological conditions. The etiopathogenesis of bacterial inflammatory diseases of the upper and lower respiratory tract is based on the violation of the integrity of epithelial cells under the influence of respiratory viruses. Under the influence of the general toxic effect of the virus, phagocytosis is inhibited, immunological protection is disrupted, as a result, favorable conditions are created for microbial colonization in the nasopharynx and the further spread of infections to the paranasal sinuses and lower respiratory tract. As a result of a clinical and microbiological study, it was revealed that in community-acquired pneumonia, inflammatory diseases of the nose and paranasal sinuses such as acute and chronic rhinosinusitis, allergic and vasomotor rhinitis, curvature of the nasal septum were often encountered. To determine the nature of the microbial landsURTe of the upper and lower respiratory tract, a comparative microbiological analysis was performed from the nose and sputum. Analyzes showed that there was no significant difference in pathogenic microflora between the upper and lower respiratory tract. The main pathogen in both diseases was Streptococcus pneumoniae. Correspondence of the pathogenic microflora of the nasal cavity and lower respiratory tract when rhinosinusitis is detected in patients with community-acquired pneumonia confirms the pathogenetic relationship of inflammatory processes in these organs, as well as the need for parallel diagnosis, treatment, prevention.

This review is devoted to the 25th anniversary of the beginning of the widespread use of the prototype of “respiratory” fluoroquinolones levofloxacin in clinical practice. The article presents the evidence of clinical and microbiological efficacy, as well as good tolerability of levofloxacin obtained during numerous clinical studies, as well as evidence of long-term successful use of the antibiotic in real clinical practice, which determines its unique place in modern schemes of antibacterial therapy of adult patients with community-acquired lower respiratory tract infections.

Primary ciliary dyskinesia (PCD) is an autosomal recessive disorder associated with severely impaired mucociliary clearance caused by defects in ciliary structure and function. Although recurrent bacterial infection of the respiratory tract is one of the major clinical features of this disease, PCD airway microbiology is understudied. Despite the differences in pathophysiology, assumptions about respiratory tract infections in patients with PCD are often extrapolated from cystic fibrosis (CF) airway microbiology. This review aims to summarize the current understanding of bacterial infections in patients with PCD, including infections with Pseudomonas aeruginosa, Staphylococcus aureus, and Moraxella catarrhalis, as it relates to bacterial infections in patients with CF. Further, we will discuss current and potential future treatment strategies aimed at improving the care of patients with PCD suffering from recurring bacterial infections.
