Excessive occlusal surface wear can result in occlusal disharmony, functional and esthetic impairment. As a therapeutic approach, conventional single crowns have been proposed, but this kind of treatment is complex, highly invasive and expensive. This case report describes the clinical outcomes of an alternative minimally invasive treatment based on direct adhesive-pin retained restorations. A 64-year-old woman with severely worn dentition, eating problems related to missing teeth and generalized tooth hypersensitivity was referred for treatment. Proper treatment planning based on the diagnostic wax-up simulation was used to guide the reconstruction of maxillary anterior teeth with direct composite resin over self-threading dentin pins. As the mandibular remaining teeth were extremely worn, a tooth-supported overdenture was installed. A stabilization splint was also used to protect the restorations. This treatment was a less expensive alternative to full-mouth rehabilitation with positive esthetic and functional outcomes after 1.5 years of follow-up.

ABSTRACTThis clinical report describes an 8-year follow-up evaluation using different thicknesses of porcelain laminate veneers of diastema and malformed anterior teeth. Minimally invasive treatment with no preparation or minimal reduction options could be considered and chosen based on the characteristics of each case. Laminate veneers with or without reduction can offer an excellent result regarding esthetics and function. Despite its failure without reduction, this treatment is indicated because it can be applied regardless of the structure of the teeth. At first, no tooth preparation with rotatory instruments was performed, and porcelain laminates of minimum thickness were made for the four maxillary anterior teeth. After 6 years, the adhesive interfaces were stained, and the patient was unsatisfied with the esthetics of her smile. Therefore, at that time, the professional decided to prepare the anterior teeth for porcelain laminate veneers. Diastema closure and/or correction of malformed anterior teeth using porcelain laminate veneers is a viable option for the clinician because it restores esthetic harmony. The patient was very pleased with the new laminate veneers.

Pain of the tempro-mandibular joint (TMJ) has a direct bearing to missing teeth and excessive physical activity. Consumption of qat requires chewing on the leaves to extract their juice for long hours. A 65-year-old male Yemeni patient, a Qat chewer, reported to the university dental hospital at King Khalid University complaining of pain in left temporomandibular joint with missing mandibular anterior teeth. A multidisciplinary approach for the overall treatment of the patient was decided. Initial treatment was the relief of patient’s pain with the help of a night guard. This was followed by a fabrication of anterior FPD. The case was under maintenance and follow-up protocol for a period of 8 months with no complaint of pain discomfort.

SUMMARY Pronounced white color alterations due to structural anomalies of the enamel are often insufficiently masked by bleaching techniques or resin infiltration procedures alone. This frequently leads to the choice of more invasive prosthetic restorations in order to correct tooth color and form. This article describes a minimally invasive treatment option for esthetic and functional rehabilitation in the case of a 13-year-old female patient with suspected severe fluorosis and misalignment of the anterior teeth. The restorations were performed using underlying resin infiltration to homogenize the tooth shade. In a second step, direct composite veneers were applied on top to attain a natural tooth color and adjust tooth alignment and form. By joining the two minimally and noninvasive techniques, this treatment option combines the directive for preservation of hard tooth structure while treating adolescents with the benefits of easy adaptation and repair when the occlusion is still in adjustment.

SUMMARY Dental biocorrosion can produce a devastating impact on oral health. The restorative phase of the treatment should not cause additional damage of the remaining sound tooth structure. Ultrathin occlusal veneers are a conservative alternative to traditional onlays and complete crowns for the treatment of severe biocorrosive lesions. This strategy is explained in the present case report through a full-mouth rehabilitation of a patient with moderate biocorrosion. Maxillary anterior teeth were restored using the bilaminar technique (lingual direct composite veneers with labial ceramic veneers) and posterior teeth using ultrathin CAD-CAM ceramic occlusal veneers. The technical aspects required for the implementation of this new restorative design are presented with a special emphasis on the control of tooth preparation based on diagnostic wax-up, provisionalization, and the use of CAD-CAM technology.

ABSTRACT Excessive occlusal wear can result in pulpal injury, occlusal disharmony, impaired function and esthetic deformity. Loss of anterior guidance can result from severe wear of anterior teeth, which protects the posterior teeth during excursive movement. The collapse of posterior teeth also results in the loss of normal occlusal plane and the reduction of the vertical dimension. This clinical report describes the use of Hobo Twin- stage procedure for rehabilitation of a patient with severe tooth wear, resulting in reduced VDO.

Abstract Objective To compare the accuracy of complete-arch scans and quadrant scans obtained using a direct chairside intraoral scanner. Material and methods Intraoral scans were obtained from 20 adults without missing teeth except for the third molar. Maxillary and mandibular complete-arch scans were carried out, and 4 quadrant scans for each arch were performed to obtain right posterior, right anterior, left anterior, and left posterior quadrant scans. Complete-arch scans and quadrant scans were compared with corresponding model scans using best-fit surface-based registration. Shell/shell deviations were computed for complete-arch scans and quadrant scans and compared between the complete-arch scans and each quadrant scans. In addition, shell/shell deviations were calculated also for each individual tooth in complete-arch scans to evaluate factors which influence the accuracy of intraoral scans. Results Complete-arch scans showed relatively greater errors (0.09 ~ 0.10 mm) when compared to quadrant scans (0.05 ~ 0.06 mm). The errors were greater in the maxillary scans than in the mandibular scans. The evaluation of errors for each tooth showed that the errors were greater in posterior teeth than in anterior teeth. Comparing the right and left errors, the right side posterior teeth showed a more substantial variance than the left side in the mandibular scans. Conclusion The scanning accuracy has a difference between complete-arch scanning and quadrant scanning, particularly in the posterior teeth. Careful consideration is needed to avoid scanning inaccuracy for maxillary or mandibular complete-arch, particularly in the posterior area because a complete-arch scan might have potential error than a quadrant scan.

The decision of whether to operate on elderly patients with brain tumors is complex, and influenced by pathology-related and patient-specific factors. This retrospective cohort study, based on a prospectively collected surgical database, aims at identifying possible factors predicting clinical worsening after elective neuro-oncological surgery in elderly patients. Therefore, all patients ≥65 years old who underwent BT resection at a tertiary referral center between 01/2018 and 12/2019 were included. Age, smoking, previous radiotherapy, hypertension, preoperative functional status, complications occurrence, surgical complexity and the presence of comorbidities were prospectively collected and analyzed at discharge and the 3-month follow-up. The series included 143 patients (mean 71 years, range 65–86). Sixty-five patients (46%) had at least one neurosurgical complication, whereas 48/65 (74%) complications did not require invasive treatment. Forty-two patients (29.4%) worsened at discharge; these patients had a greater number of complications compared to patients with unchanged/improved performance status. A persistent worsening at three months of follow-up was noted in 20.3% of patients; again, this subgroup presented more complications than patients who remained equal or improved. Therefore, postoperative complications and surgical complexity seem to influence significantly the early outcome in elderly patients undergoing brain tumor surgery. In contrast, postoperative complications alone are the only factor with an impact on the 3-month follow-up.

Abstract This case report describes an esthetic treatment to improve the shape and alignment of the anterior teeth, reestablishing smile harmony, using feldspathic porcelain veneers. Results of clinical follow up after 36 months are also presented. The advantages, disadvantages and limitations of the technique are detailed with reference to the relevant literature. This suggests that the success of treatment depends on adequate conditions of bonding between the veneers and the tooth complex, which involves parameters such as the strength and durability of the bond interface. Therefore, the clinical success of feldspathic porcelain veneers depends on the accurate selection of cases and correct execution of clinical and laboratory procedures. The rehabilitation involved from first right premolar to the left with feldspathic porcelain veneers made on refractory dies. After the 3-year follow up, excellent clinical results and patient satisfaction were achieved.

Borderline Ovarian tumors are tumors of uncertain malignant potential. They have favour able prognosis. They occur in younger women and present at an early stage. They are difficult to diagnose preoperatively as macroscopic picture is a combination of benign and invasive ovarian tumors. Over the years surgical treatment has changed from radical to conservative approach without overlooking oncologic safety. Follows up is essential. Prolonged follow up (>10 yrs) is required because of later recurrences. Special attention is to be paid to the conserved ovary in follow up.
