ABSTRACT: Spontaneous polydactyly has been described in several species, but only one report about it in Swiss mice. The aim of the current study was to report the spontaneous occurrence of pre-axial polydactyly in Swiss mice. Clinical examination showed one extra toe laterally to the first digit, in the plantar region, alopecia in the back, altered face growth anatomy and changed perineal region anatomy. Pre-axial polydactyly in the tibial side, fused metatarsals and Y-shaped free phalanges were evidenced in the radiographic images. Pre-axial polydactyly observed in the plantar region differed from that in reports on albino Swiss mice with post-axial polydactyly (Po/Po+) phenotype featured by one extra toe in the ulnar side of one, or both, front limbs, which is the dominant feature. The observed findings highlight the importance of both clinical examinations and close attention by professionals involved in rodents’ breeding on physical changes resulting from different causes, including the genetic ones, since they reveal mutations and, sometimes, new biomodels.

Введение. Рассматривается образ Петербурга в лирике Ивана Елагина, одного из заметных представителей второй волны эмиграции. Цель – проследить развитие образа в его эволюции, зафиксировать «общие места» петербургской поэтики, вместе с тем отметить и личный вклад автора в разработку темы в условиях эмиграции. Материал и методы. Методология исследования предполагает интерпретацию поэтических текстов с опорой на теоретико-литературоведческие понятия (акмеизм, литературная традиция, поэзия диаспоры и метрополии и пр.). Автор статьи также придерживался сравнительно-исторического метода при анализе текстов, сопоставляя стихотворения, имеющие приметы одного поэтического течения, но «разнесенных» во времени. Результаты и обсуждение. Образ Петербурга в литературе русского зарубежья не раз становился предметом научных изысканий, но лирика Ивана Елагина еще не привлекала пристального внимания литературоведов в плане трактовки темы Петербурга с акмеистических позиций. Был рассмотрен ряд стихотворений поэта как яркий пример следования заветам акмеизма в условиях эмиграции, хотя в лирике Елагина не менее важен и авангардный элемент. Но в образе города на Неве поэт сознательно ориентируется на поэтику акмеизма, о чем говорят многочисленные аллюзии и тематические переклички со старшими акмеистами – Н. Гумилевым, А. Ахматовой, О. Мандельштамом. Заключение. Лирическое воплощение образа Петербурга у Ивана Елагина продиктовано его акмеистической ориентацией и желанием вписать свое имя в ряд собственных поэтических учителей-акмеистов. Петербург Елагина актуализирует общие для «петербургского текста» установки, но, кроме того, демонстрирует эмигрантскую специфику в трактовке образа Северной Пальмиры. Доминирующей чертой образа становится его мортальность, устремленность в «потустороннее», отражение в нем экзистенциальной тоски и ностальгии – извечных «спутников» поэта-изгнанника.Introduction. The article considers the image of St. Petersburg in the lyrics of Ivan Elagin, one of the prominent representatives of the second wave of emigration. The objectives of the study are: to trace the development of the image in its evolution, to fix the “common places” of St. Petersburg poetics, at the same time to note the author’s personal contribution to the development of the theme in conditions of emigration. Material and methods. The research methodology involves the interpretation of poetic texts based on theoretical and literary concepts (acmeism, literary tradition, poetry of the diaspora and the metropolis, etc.). Results and discussion. The image of St. Petersburg in the literature of the Russian abroad has repeatedly become the subject of scientific research, but the lyrics of Ivan Elagin have not yet attracted the close attention of literary scholars in interpreting the theme of St. Petersburg from acmeistic positions. We considered a number of his poems as a vivid example of following the precepts of acmeism in conditions of emigration, although the avant-garde element is no less important in the lyrics of Elagin. But in the image of the city on the Neva, the poet deliberately focuses on the poetics of acmeism, as evidenced by numerous allusions and thematic exchanges with senior acmeists – N. Gumilev, A. Akhmatova, O. Mandelstam. Conclusion. The lyrical embodiment of the image of St. Petersburg by Ivan Elagin is dictated by his acmeistic orientation and the desire to write his name in a line of his own poetic acmeist teachers. St. Petersburg of Ivan Elagin actualizes the general settings for the “St. Petersburg text”, but, in addition, demonstrates the emigrant specificity in interpreting the image of Northern Palmyra. The dominant feature of the image is its mortality, striving for “beyond”, the reflection in it of existential longing and nostalgia – the eternal “companions” of the exiled poet.

The broadest application of transmission electron microscopy (EM) in diagnostic medicine is the identification of tumors that cannot be classified by routine light microscopy. EM is useful in the evaluation of approximately 10% of human neoplasms, but the extent of its contribution varies considerably. It may provide a specific diagnosis that can not be reached by other means, but in contrast, the information obtained from ultrastructural study of some 10% of tumors does not significantly add to that available from light microscopy. Most cases fall somewhere between these two extremes: EM may correct a light microscopic diagnosis, or serve to narrow a differential diagnosis by excluding some of the possibilities considered by light microscopy. It is particularly important to correlate the EM findings with data from light microscopy, clinical examination, and other diagnostic procedures.

Acute thiamine deficiency was produced in mice by the administration of oxythiamine, a thiamine analogue, superimposed upon a thiamine deficient diet. Adult male Swiss mice (30 gm. B.W.) were fed with a thiamine deficient diet ad libitumand were injected with oxythiamine (170 mg/Kg B.W.) subcutaneously on days 4 and 10. On day 11, severe lassitude and anorexia developed, followed by death within 48 hours. The animals treated daily with subcutaneous injections of thiamine (300 μg/Kg B.W.) from day 11 through 15 were kept alive. Similarly, feeding with a diet containing thiamine (600 μg/Kg B.W./day) from day 9 through 17 reversed the condition. During this time period, no fatal illness occurred in the controls which were pair-fed with a thiamine deficient diet.The oxythiamine-treated mice showed a significant enlargement of the liver, which weighed approximately 1.5 times as much as that of the pair-fed controls. By light and electron microscopy, the hepatocytes were markedly swollen due to severe fatty change and swelling of the mitochondria.

Abstract Since 1999 when Medicare caps first became effective, providers have had to pay close attention to the claims process. This article summarizes the Medicare Exceptions Process that, for 2007, underwent a number of changes. The Centers for Medicare and Medicaid Services (CMS) Physician Fee Schedule Final Rule of November 27, 2007 made three important changes. These changes addressed certification for patient plan of care, personnel qualifications for therapists, and a review of Part B policies and their application to Part A settings that are projected to go into effect in July of 2008. Particular attention was given to explanations of the manual submission process and the change in definitions of “complexities” and of a “therapist.”

Dementia is a condition caused by and associated with separate physical changes in the brain. The signs and symptoms of dementia are very similar across the diverse types, and it is difficult to diagnose the category by behavioral symptoms alone. Diagnostic criteria have relied on a constellation of signs and symptoms, but it is critical to understand the neuroanatomical differences among the dementias for a more precise diagnosis and subsequent management. With this regard, this review aims to explore the neuroanatomical aspects of dementia to better understand the nature of distinctive subtypes, signs, and symptoms. This is a review of English language literature published from 1996 to the present day of peer-reviewed academic and medical journal articles that report on older people with dementia. This review examines typical neuroanatomical aspects of dementia and reinforces the importance of a thorough understanding of the neuroanatomical characteristics of the different types of dementia and the differential diagnosis of them.

Zusammenfassung. Hintergrund: Aufgrund der demografischen und epidemiologischen Entwicklung wird die Vermittlung adäquater pflegerischer Kompetenzen in der pflegerischen Ausbildung zunehmend diskutiert. Kompetenzen sind in den Examina angemessen zu überprüfen. Hierfür haben sich international OSCEs bewährt. Ziel: Ziel der vorliegenden Analyse war die Ermittlung von Kompetenzen, die in pflegerischen Erstausbildungen mit OSCEs überprüft werden. Methodik: In internationalen Datenbanken wurden einschlägige Publikationen recherchiert. Analyse-Einschlusskriterium war die Nennung mindestens einer überprüften Kompetenz. Die Kompetenzen wurden – in Anlehnung an den «Fachqualifikationsrahmen Pflege für die hochschulische Bildung» – nach Wissen, Fertigkeiten und Haltung kategorisiert. Ergebnisse: 36 Artikel erfüllten das Einschlusskriterium. Relevante Studien stammen mehrheitlich aus Großbritannien (UK), Kanada und Australien. Es wurden insgesamt n = 166 Kompetenzen in allen Kategorien identifiziert, die anhand verschiedener Methoden gemessen wurden. Am häufigsten wurden Fertigkeiten überprüft. Dabei wurde der sichere Umgang mit Medikamenten am häufigsten thematisiert. Weitere wichtige Themen waren die Kommunikationskompetenz in Bezug auf PatientInnen und die Fähigkeit zur Selbsteinschätzung. Diskussion: Es werden sehr unterschiedliche Kompetenzen mit differenten Formaten per OSCE gemessen. OSCE ermöglichen eine Überprüfung sowohl auf individueller als auch institutioneller Ebene, also sowohl für die Lehre als auch das Gelernte. Weiterer Forschungsbedarf wird festgestellt.
