AbstractCartilage regeneration with cell-free matrices has developed from matrix-associated autologous cartilage cell transplantation (MACT) over ten years ago. Adjustments to the legal framework and higher hurdles for cell therapy have led to the procedures being established as an independent alternative to MACT. These procedures, which can be classified as matrix-induced autologous cartilage regeneration (MACR), all rely on the chemotactic stimulus of a cross-linked matrix, which mostly consists of collagens. Given the example of a commercially available type I collagen hydrogel, the state of clinical experience with MACR shall be summarized and an outlook on the development of the method shall be provided. It has been demonstrated in the clinical case series summarized here over the past few years that the use of the matrix is not only safe but also yields good clinical-functional and MR-tomographic results for both small (~ 10 mm) and large (> 10 mm) focal cartilage lesions. Depending on the size of the defect, MACR with a collagen type I matrix plays an important role as an alternative treatment method, in direct competition with both: microfracture and MACT.

(1) Background: Gingival cancer has a significant incidence and is often diagnosed at advanced stages. The aim of this paper is to highlight its clinical aspects on the basis of a case series analysis in order to promote awareness and improve the diagnosis process. (2) Methods: Oral cancers diagnosed and treated at three Italian University Hospitals over ten years were retrospectively investigated. Cancer location on the gingiva, edentulous ridge, and retromolar pad was addressed. Data regarding clinical features, stage at the diagnosis, and time from presenting symptoms to first medical consultation were retrieved. (3) Results: Thirty-three cancers located on the gingiva, edentulous ridge, and retromolar pad were retrieved from 276 total oral cancer cases (11.9%). A median of 50 days (range 2–300) passed for the patient to seek for a medical evaluation. At the time of diagnosis, 63.3% were advanced stage cancers, mainly located at the mandible (91%), especially in the retromolar pad (48.5%) and the edentulous alveolar ridge (24.2%). Lesions were red (45.5%), red and white (45.4%), or white (9.1%), appearing as an ulcer (69.7%), exophytic mass (12.1%) or flat lesion (12.1%). Sixty-six percent of cancers were completely asymptomatic, regardless their clinical appearance. A statistically significant association between the time from the presentation of symptoms to the first medical consultation and the cancers stage was found. (4) Conclusions: The clinical appearance of gingival cancer is very polymorphous; its understanding may be significant to improve patient education and early medical consultation.

Background:Graves’ Orbitopathy (OG) is the main extrathyroid manifestation of Graves’ disease (GD). Up to 25% of patients have moderate-severe inflammatory activity with a risk of major sequelae. Intravenous (i.v.) glucocorticoids (GC) pulses are the standard therapy. In refractory cases, several classic and some biological immunosuppressantsare used, obtaining variable responses. Interleukin 6 (IL6) is involved in the pathogenesis of OG, which has justified the off-label treatment with Tocilizumab (TCZ)1, whose use by route i.v. has shown favorable results in published small case series and a single multicenter trial2.Objectives:To analyse the ocular effectiveness of anti-IL6 therapy used subcutaneously (s.c.) in patients with moderate-severe active refractory OG in usual clinical practice.Methods:Retrospective descriptive observational study of a series of cases of moderate-severe OG patients treated with anti-IL6 s.c. The patient medical records of those who had received at least 1 cycle of anti-IL6 treatment were reviewed (December 2013-December 2019). The primary effectiveness outcome was the change of the Clinical Activity Score (CAS). Favorable response was considered: reduction of CAS≥2 points together with obtaining inactivity (CAS <3). Demographic data, personal medical history, clinical aspects of GD, previous therapies and data on the use and safety of anti-IL6 were collected. The SPSS11 package was used for statistical analysis, using non-parametric tests for quantitative variables. The study was approved by the local Ethical Committee.Results:12 of the 15 patients (80%) were women with a mean of 50.27 years (21-72). 60% (n=9) had smoking history, 40% (n=6) active. 26.7% (n=4) were diabetic, all without retinopathy. 100% of patients received imidazole antithyroid treatment. 46.7% (n=7) required β-blockers and 20% (n=3) diuretics. 66.7% thyroidectomy (n=10) and 20% (n=3) decompressive eye surgery and/or blepharoplasty were performed. Thyroid and ocular radiotherapy were used in 2 patients. 3 patients received botox. 80% (n=12) of them had previously received GC. 93.3% (n=14) were naïve to biological therapy, only 1 patient previously used Rituximab. All except one patient who was treated with SRL received TCZ as IL6 therapy. A significant favorable response was obtained in 100% of the patients (p=0.008), decreasing CAS average from 4.9 (2-7) to 1.7 (0-2) at the end of the therapy [Figure 1]. The severity of the OG changed from being moderate in 72.7% of the patients to mild in 66.7% of the total. The median time to inactivity was 8 months (2-15). 73.3% (n=11) of the patients finished the treatment reaching inactive OG, the rest (although inactive) maintained therapy. After 6 months, 100% of those who completed the treatment remained inactive with average CAS of 1.3 (0-2). Smoking did not influence the response, nor any other variable collected. Adverse events appeared in 26.7% (n=4) of the cases, all of them mild and without widrawal.Conclusion:Treatment with anti-IL6 s.c. steadily decreases the clinical activity measured by CAS in patients with moderate-severe refractory OG, despite poor prognosis factors (such as smoking), with a good safety profile.References:[1]Taylor PN, Zhang L, Lee RWJ, Muller I, Ezra DG, Dayan CM, et al. PurposeTo report the long-term outcomes of Boston keratoprosthesis type I (B-KPro type I) implantation in the management of severe ocular surface disorders.MethodsRetrospective case series. Patients who underwent B-KPro type I implantation at the People’s Liberation Army General Hospital were enrolled between March 2011 and September 2019. Data regarding visual acuity (VA), B-KPro type I retention and postoperative complications were recorded and analysed.ResultsA total of 103 eyes of 100 patients who underwent B-KPro type I implantation were included. The main indications were chemical burn (59.2%), ocular trauma (25.2%), herpetic keratitis (11.7%) and autoimmune diseases (3.9%). The percentage of eyes with postoperative VA of 10/200 or better was 82.7% at 6 months, 82.8% at 12 months, 77.9% at 2 years, 72.4% at 3 years, 71.1% at 4 years, 69.4% at 5 years, 58.9% at 6 years, 56.8% at 7 years and 42.9% at 8 years. Preoperatively, 8.7% eyes were diagnosed with new-onset glaucoma. Retroprosthetic membrane formation occurred in 19.4% eye. Corneal melting occurred in 18.4% eyes. Sterile vitritis was diagnosed in 4.9% eyes and infectious endophthalmitis in 2.9% eyes. Retinal detachment occurred in 0.9% eyes.ConclusionsIn a Chinese patient group, B-KPro type I is a viable option for treating severe ocular surface disorders in eyes where conventional keratoplasty would have a poor prognosis, especially in patients with chemical and thermal burns. Improved visual outcomes and high retention rate can be achieved and maintained in most cases.

The most frequent ocular adnexal tumors and simulating lesions are lymphoproliferative disorders (LPDs), including malignant lymphomas and orbital inflammation with lymphoid hyperplasia or infiltration. IgG4-related orbital inflammation (IgG4-ROI) often involves lacrimal glands and other orbital tissues and is an important differential diagnosis. The present study evaluated clinical aspects of IgG4-ROI in a case series of orbital LPD. Sixty-two consecutive cases of orbital LPD, pathologically diagnosed from November, 2004, through March, 2011, were investigated. Histological types were 22 cases with MALT lymphoma, 11 cases with diffuse large B-cell lymphoma (DLBCL), 3 cases with other malignant lymphomas, 16 cases with IgG4-ROI, and 10 cases with non-IgG4-ROI. Ages of the IgG4-ROI group (56±10?yrs) were significantly lower than the MALT lymphoma (71±12?yrs) and DLBCL (75±14?yrs) groups. Orbital lesions other than lacrimal glands were present in six cases including extraocular muscle swelling, mass lesions surrounding the optic nerve, and supraorbital and infraorbital nerves enlargements. Although none of the malignant lymphomas were related to IgG4, previous evidence suggested that malignant lymphomas can arise from IgG4-ROI. Based on this study (26%) and another report (33%), it is likely that nearly a quarter of orbital LPD are IgG4-ROI.

RiassuntoScopo - Analizzare l'attività del Servizio Psichiatrico di Diagnosi e Cura di Pavia nel periodo compreso tra il 1978 (anno dell'apertura del servizio) ed il 1990. Disegno - Analisi retrospettiva dei dati contenuti in un archivio computerizzato, nel quale, a partire dal 1978, sono state raccolte informazioni relative alle caratteristiche socio-demografiche e nosologiche dei pazienti ricoverati, nonché alia modalità ed all'andamento dei ricoveri. Setting - Il Servizio Psichiatrico di Diagnosi e Cura di Pavia, il cui bacino di utenza è costituito dall'ULSS 77, comprendente la città di Pavia e 61 comuni limitrofi. Principali indici utilizzati - Per ciascun anno considerato sono stati ricavati: indicatori d'uso (primi ricoveri, ricoveri complessivi, giornate di degenza, pazienti ricoverati); presenza media giornaliera e media annuale dei massimi mensili di pazienti ricoverati; indicatori d'intensita d'uso o di flusso (degenza media, indice di occupazione media dei posti-letto, indice di turnover, indice di rotazione dei posti-letto). La produttivita del servizio è stata indagata utilizzando il monogramma di Barber. Risultati - Tra il 1978 ed il 1983 si è verificato un progressivo incremento nel numero dei pazienti ricoverati, dei ricoveri complessivi e delle giornate di degenza; è seguito un periodo stazionario protrattosi fino al 1987; infine, durante gli ultimi tre anni il numero dei ricoveri complessivi e quello dei pazienti ricoverati sono leggermente diminuiti, mentre le giornate di degenza sono rimaste sostanzialmente stabili. Il numero annuale dei pazienti entrati in contatto per la prima volta con il servizio ha segnato due picchi negli anni 1979 e 1983 (N = 250), diminuendo poi progressivamente fino ai valori di 139 nel 1989 e di 152 nel 1990. Le frazione percentuali dei primi ricoveri rispetto ai ricoveri complessivi e rispetto ai pazienti ricoverati si sono ridotte nel corso del periodo considerato: nel 1990 esse erano pari a 30% e 50% rispettivamente. Presenza media giornaliera e media annuale dei massimi mensili di pazienti ricoverati sono aumentate tra il 1979 ed il 1983, passando da 12 a 21 pazienti/die, e da 14 a 25 pazienti/die, rispettivamente; negli ultimi otto anni i due parametri si sono stabilizzati su valori di 20-21 e 24-25 pazienti/die rispettivamente. II nomogramma di Barber, costruito sulla base dei valori assunti dagli indicatori d'intensita d'uso nel corso di ciascun anno considerato, ha mostrato che tutti gli anni in esame si sono distribuiti entro la zona di produttivita accettabile, ad eccezione del 1980 e del 1981. Design - Retrospective analysis of data stored in a computerized database, in which, since 1978, information has been collected on sociodemographic and nosological characteristics of patients admitted to the service, as well as on clinical aspects of all admissions. Setting – A General Hospital Psychiatric Inpatient Service in Pavia, whose catchmentarea (ULSS 77) includes the town of Pavia and 61 close municipalities. Main outcome measures -For each yearover the period considered we reported on: the indices of service utilization (first admissions, total admissions, days of stay, number of admitted patients); the mean daily number of inpatients, and the mean of the largest daily numbers of inpatients by month; the indices of intensity of service utilization (mean lenght of stay, index of bed occupation, index of turnover, index of bed rotation). The service productivity was determined according to the Barber's nomogram. Results – Between 1978 and 1983 patients admitted to the service, total admissions, and days of stay gradually increased; a steady period then followed until 1987; over the last three years, total admissions and the number of admitted patients slightly decreased, while daysof stay remained substantially stable. The annual number of patients admitted for the first time to the service marked two peaks in 1979 and 1983 (N = 250), and then gradually decreased to 139 in 1989 and 152 in 1990. The percent ratio of first admissions by total admissions and that of first admissions by the number of admitted patients showed a reduction over the period considered: in 1990 they were 30% and 50% respectively. The mean daily number of inpatients and the mean of the largest daily numbers of inpatients by month increased between 1979 and 1983, from 12 to 21 and from 14 to 25 patients/day respectively; over the last eight years the values remained rather constant, around 20-21 and 24-25 patients/day respectively. The Barber's nomogram, obtained on the basis of the indices of intensity of service utilization over the period considered, showed that each year fell within the area of acceptable productivity, with the exception of 1980 and 1981. Conclusions – After an initial phase of gradual expansion, the service then reached asteadystate, providing an acceptable productivity over most of the period considered.
