Clinical and anamnesis analysis as well as analysis of the course and outcome of the present pregnancy was performed on 595 patients with complicated obstetrical and gynecological history (COGH) habitual pregnancy loss, infertility, genital chronic processes. Control groupconsisted of 5O healthy women. Immunofluorescent analysis was used to reved antiphospholipid antibodies on blood plasma. Antiphospholipid antibodies were found in 42% of COGH women. No antiphospholipid antibodies were seen in the control group. In all patients of the studied groupthe course of pregnancy was accompanied with signs of threatened abortion, gestosis. 29% (p O, O1) patients had preterm labor and spontaneous abortion occurred in 18% (p O, O1) of cases. Perinatal mortality was 9, 8%4, 16.

model of chronic gastritis and duodenitis was conducted in 48 immature Wistar rats both gender by intragastric injection of medical bile. Control group consisted of 10 animals. After approximation model of the gastritis and duodenitis rats were divided into five subgroups: the 1st subgroup (before treatment, 9 rats), subgroup 2 (10 animals) received the antioxidant, subgroup 3 (11 animals) – received NO donator, subgroup 4 (10 animals) - received an antioxidant and NO donator, subgroup 5 (8 animals) – received the antioxidant, NO donator and calcium carbonate. In rats with gastritis and duodenitis was observed redistribution of stable metabolites of nitric oxide: decreasing level in the gingival homogenates and increasing in the blood plasma. Level of malondialdehyde and aldehyddehydrogenase in the gingival tissue increased, and in the blood plasma decreased, on a background of catalase activation; content of ketone phenilhydrazone remained in the level of control group. Decreasing markers of oxidation lipids and proteins in the blood plasma, on a background of increasing levels of catalase and nitric oxide metabolites (in 14 times), indicates about a key role of NO in the antioxidant protection of organism in a case of disease.Application of antioxidant could not impact on the indicators of oxidation lipids and proteins. It had been shown decreasing content of the nitric oxide metabolites in the gingival tissues and blood plasma. Drug correction with donator of nitric oxide have to increase markers of oxidation lipids and proteins in the gingival tissues, on a background of high catalase activity and low levels of nitric oxide metabolites. In the blood plasma content of nitric oxide metabolites was higher, which indicated about an active inflammatory process in the stomach and duodenum mucosa. Indicators of the protein molecules fragmentation and malondialdehyde were not differ from those before treatment. Level of catalase was in two times higher, than in the control group, but level of superoxide dismutase was decreased. Complex of antioxidant and donator of nitric oxide helped to stabilise the indicators of oxidation lipids and proteins, although level of nitric oxide metabolites in the gingiva was significantly decreased. Application complex from the antioxidant, donator of nitric oxide and calcium carbonate at the treatment of chronic gastritis and duodenitis restored an oxidative-antioxidative status in the blood plasma and gingival tissues, increasing the production of nitric oxide to a level in the control group.

Background: It is certain from clinical experience of many that one or more hemorrhages in early pregnancy can still end up in good fetal outcome. So, our study deals with comparison of cytohormonal study in pregnancy and threatened abortion. The study was conducted with the aim of utilizing colpocytogram as a tool in assessing and treating cases of threatened abortion and comparing them with normal pregnant women.Methods: Patients attending antenatal care unit on outdoor basis and labelled as normal pregnancy cases were considered as control group. The patients of threatened abortion were studied when they were admitted in Gynecology department for indoor treatment. Patients were studied taking into consideration their age, parity, number of abortions, complaints (P/V bleeding, pain in abdomen), gestational age, per abdomen and per vaginal findings and also USG findings and vaginal smear pattern.Results: Maximum number of patients was present in the age group of 21-25 years in both the groups. While only 13.33% had normal smear pattern in threatened abortion group. 86.67% patients in threatened abortion group showed abnormal smear pattern. There is statistically significant difference was found (p<0.05). There is statistically significant was found (P<0.01) and indicates good effects of the drug on the vaginal epithelium.Conclusions: The cytohormonal study acts as a simple, reliable, good, noninvasive method for evaluation of hormonal pattern in normal pregnancy and threatened abortion.

Migraine headache (with and without aura) is common in the general population and is known to be influenced by genetic factors with heritability estimates between 34-57±. Antiphospholipid syndrome (APS) is a hypercoagulable state characterized by clinical features including venous and arterial thromboses, pregnancy loss and migraine, and by association with antiphospholipid antibodies (aPL). Numerous small studies have investigated whether aPL are associated with migraine in the general population—-with contradictory results. In this study, the question was addressed by studying the prevalence of aPL in members of monozygotic (MZ) twin pairs differing in their migraine status. Such twins provide a unique natural experiment, matched as they are for age, sex and genetic factors, and allow the role of environmental factors, such as aPL, to be determined. Despite 95± power to detect a difference of 0.59 IgG units per litre in anticardiolipin antibody IgG titres, no difference in prevalence of aPL could be detected in migraine-discordant MZ twins.

Aim. To study the features of gestation in women with a combination of antiphospholipid and TORCH syndromes in relation to preconception care.Materials and Methods. We analyzed 137 medical records of women with a past medical history of pregnancy loss and antiphospholipid syndrome (APS), focusing on the presence or absence of plasmapheresis in the preconception period, and further ranking the patients into 2 subgroups (with and without TORCH syndrome). As a control group, we included 28 pregnant women without both syndromes.Results. Gestation in women with combined APS and TORCH syndromes was accompanied by a 10-fold higher risk of threatened abortion in the first trimester and 3-fold higher risk of placental insufficiency as compared to those without both syndromes. Notably, the combination of the syndromes doubled the risk of placental insufficiency in comparison with APS alone. The lack of plasmapheresis in patients with APS and TORCH syndrome was associated with > 2-fold higher risk of threatened abortion. Further, in patients with APS and TORCH syndrome, lack of plasmapheresis increased the likelihood of developing fetal hypoxia by a factor of 2 and 3 in comparison with those diagnosed with APS alone or control patients.Conclusions. TORCH syndrome is a major risk factor of adverse outcome in pregnant women with APS. Inclusion of plasmapheresis into the preconception care in women with APS and TORCH syndrome significantly reduced the development of pregnancy complications.

Background: Threatened abortion is the most common complication in the first half of gestation. Spontaneous abortion occurs in less than 30% of the women who experience threatened abortion. In order to prevent pregnancy loss several supportive therapies including hormonal therapy like human chorionic gonadotropin (hCG) or 17-alpha-hydroxyprogesterone (progesterone) have been advocated. The exogenous administration of hCG is aimed at stimulating and therefore optimizing progesterone production. Aim of this study was to compare the efficacy of supportive therapy with hCG and progesterone in women with threatened abortion. Methods: This prospective study was carried out in the department of obstetrics and gynecology of the Combined Military hospital (CMH), Savar, Dhaka, Bangladesh from July 2016 to June 2017. One hundred pregnant patients admitted with the history of per vaginal bleeding before 20 weeks of gestation without having any other co-morbidity were included in this study. Patients were randomized to two treatment groups. The participants in group A (52, 52%) received injection hCG weekly while those in group B (48, 48%) received injection progesterone from recruitment up until 20 weeks of gestation. Further USG were performed one week and four weeks after recruitment to the study and again at 20 weeks and subsequently when indicated. The final outcome of pregnancy were recorded and analyzed. Results: Among 100 patients majority belonged to the 26-30 year age group. Mean age of the patients was 27.2±10.5 years. There was not much significant difference between the groups in terms of parity. More than 75% of patients in both the groups presented before 16 weeks of gestation with threatened abortion. In both the groups more than 75% of the patients had previous history of pregnancy loss. In terms of pregnancy outcome more patients in hCG group had live pregnancy than progesterone group (88.5% vs 66.7%) (p=0.012). Out of 46 live birth in hCG group, 4 (7.7%) were preterm labor between 31-35 weeks of pregnancy and one baby died in neonatal ICU, one died at 31 weeks of gestation which was delivered by vaginally. On the other hand out of 32 live birth in progesterone group, there was 3 (6.3%) preterm labor. Growth retardation was less in hCG group compared to progesterone group (9.6% vs 14.6%). However cesarean section rate was high in both the groups. Conclusion: Treatment with injection hCG has better pregnancy outcome than that of injection17-alphahydroxyprogesterone in early pregnancy with threatened abortion of unexplained cause. Pregnancy loss (PL) is one of the common complications that women can experience during pregnancy, with an occurrence rate of 1 to 5%. The potential causes of pregnancy loss are unclear, with no effective treatment modalities being available. It has been previously reported that the level of miR-125b was significantly increased in placentas of PL patients. However, the role of miR-125b in the development of PL still remains unknown. In the current study, an miR-125b placenta-specific over-expression model was constructed by lentiviral transfecting zona-free mouse embryos followed by embryo transfer. On gestation day 15, it was observed that the placenta was significantly smaller in the miR-125b placenta-specific overexpression group than the control group. Additionally, the abortion rate of the miR-125b placenta-specific overexpression group was markedly higher than in the control group. The blood vessel diameter was larger in the miR-125b-overexpressing specific placenta. In addition, miR-125b-overexpressing HTR8 and JEG3 cell lines were also generated to analyze the migration and invasion ability of trophoblasts. The results showed that miR-125b overexpression significantly suppressed the migration and invasion ability of HTR8 and JEG3 cells. Overall, our results demonstrated that miR-125b can affect embryo implantation through modulating placenta angiogenesis and trophoblast cell invasion capacity that can lead to PL.

Background. The pathogenesis of nonalcoholic fatty liver disease (NAFLD), which develops in obesity and type 2 diabetes mellitus (T2DM), is associated with the effects of inflammatory factors on the liver parenchyma and liver mitochondrial dysfunction.Aim. To determine the role of sTNFSF14 in the regulation of liver mitochondrial biogenesis in obese patients with and without T2DM.Materials and methods. The study included 263 obese patients with and without T2DM and 42 apparently healthy donors. Quantitative determination of cytokines in the blood plasma was performed by fluorescence flow cytometry. The level of relative gene expression in the liver biopsy samples was investigated by real-time PCR. Semi-quantitative determination of proteins in the liver biopsy samples was studied by western blotting.Results. The study showed that the levels of sTNFSF14, interleukin (IL)-10, gp130 / sIL-6Rb, and sIL-6Ra in the blood plasma of the obese patients without T2DM significantly exceeded the similar values in the control patients and obese patients with T2DM. In the liver biopsy samples of the obese patients with T2DM and a body mass index (BMI) > 40 kg / m2, the expression level of the dynamin-1-like protein (DRP1 / DNM1L) gene was lower than in the control group, and the expression level of the mitofusin 2 (MFN2) gene tended to be higher. Compared with the control group, an increase in the expression level of the NADH-ubiquinone oxidoreductase chain 4 (MT-ND4) gene was recorded in the liver of all the obese patients. The patients with obesity showed a decrease in the amount of mitochondrial DNA (mtDNA) compared with the control group.Conclusion. Thus, sTNFSF14, interacting with IL-10 and gp130 / sIL-6Rb in the circulation, positively effects the liver in the obese patients without T2DM. A low level of sTNFSF14 in the blood plasma of the obese patients with T2DM results in decreased mitochondrial division and increased cellular respiration.

Aim: To compare the difference in the cervical length measured digitally or by transabdominal and transvaginal sonogram examination for prediction of preterm labour Method: This was a prospective randomized, controlled study in the Department of Obstetrics and Gynaecology at TMA Pai Hospital, Udupi from March 2001 to May 2002 where 200 women coming for routine antenatal care were recruited for the study. A total of 168 women were evaluated, out of which 145 were control group and 23 cases of threatened preterm labor formed study group. Results: The mean length of cervix measured by TVS at 20- 24wks of pregnancy was 3.87 cm. There after there was a slight decrease in the length mainly towards term and or 36 wks of pregnancy i.e. 3.27 cm. Cervical length measured on an average 0.8 cm more than TVS(4.9CM versus 3.52 cm) Similarly digital examination of cervix yielded shorter length, compared to TVS measurement (2.86 cm versus 3.5 cm). Conclusion: This study has shown TVS is a simple tool for prediction of preterm labour as decrease in cervical length was observed in women with threatened preterm labor (2.34 cm) and this decrease was statistically significant compared to that of the control (3.7 cm).
