Endoscopic surgery in gynecology: state and development prospects

In the clinical lecture the main principles of arranging endovideosurgical carein gynecology are substantiated on the basis of the material of 17,5 thousandslaparoscopies and 10 thousands hysteroscopies. The detailed analysis of possibilities of endoscopic methods in operative gynecology is given. The peculiarities of laparoscopys application are depicted in cases of tube-peritoneal infertility, uterine and ovarian tumors, pyoinflammatory diseases of uterine adnexa, ectopic pregnancy, maldevelopment of female genital organs, stress incontinence in women. Perspectives of endovideosurgical technologies in gynecological practice are also shown.

Tumors of the ovaries in general and dermoids in particular do not particularly complicate a normal pregnancy, causing the last more or less inconvenience.All ovarian tumors, with the exception of dermoid tumors, differing in relatively large sizes, generally speaking and with the exception of very bulky tumors, lessen the development of pregnancy and especially the course of labor, since these tumors have a tendency to grow upward into the abdominal cavity, thus being removed from small pelvis.

Background: Primary synchronous cancer of the female genital tract is a relatively uncommon. Simultaneously detected endometrial and ovarian malignancies constitute the commonest occurrence. A set of pathological criteria has been described to differentiate synchronous primaries from metastatic tumors. The purpose of this study was to characterize patients with synchronous primary endometrial and ovarian tumors. Materials and Methods: This was a retrospective study done in Department of Pathology, Tribhuwan University Teaching Hospital from September 2006 to August 2011. The datas were retrieved from computer database. Results: There were totally of 10 cases of simultaneously detected endometrial and ovarian cancers. Out of 10 cases, 7 cases were synchronous primary endometrial and ovarian cancers while three were metastatic. Median age at presentation was 47.4 years. Six (85.8%) of these patients presenting with dual primary tumors were premenopausal. Grade 1 histology was seen in 57% of endometrial and 42% of ovarian tumors. Atypical endometrial hyperplasia was found in 42.8% of cases while none of the cases showed endometriosis. Conclusion: Though limited by relatively small number of cases, younger and premenopausal women were predisposed to developing synchronous primary tumors of the endometrium and ovary. Female genital schistosomiasis is a significant risk factor for ectopic pregnancy and infertility in schistosomiasis-endemic areas. A case of one previous ectopic pregnancy and subsequent obstruction of the contralateral tube in a secondary subfertility patient with chronic genital schistosomiasis is presented, emphasizing the need for a detailed history and parasitic evaluation of patients presenting with ectopic pregnancy or subfertility in areas where the disease is endemic.

This article consist of the main approaches of the foreign sports industry to corporate social responsibility that are currently used. The work reveals the content of the concept of corporate social respon-sibility, its role in modern society and development prospects. Much attention is paid to the description of corporate social responsibility in the sports industry. The relevance of the study is due to the growing interest of society both in social responsibility and in sports. American and European views on CSR were chosen as a comparison. A detailed analysis of the material leads the author to the conclusion that corporate social responsibility today is an integral part of foreign leagues, associations and clubs.

Wide range of indications for laparoscopy in case of different gynecological diseases is shown, relative contraindications are determined. The authors analyze the effectiveness of different endovideosurgical invasions applied for treatment of womens reproductive health dysfunction.

The article presents periods of development of intraluminal endoscopy in general. Especially, the input of national specialists is considered since their works yield not to works of foreign specialists. The intraluminal endoscopy, prior to becoming independent clinical direction as effective technique of diagnostic and treatment of various diseases, - passed long and thorny path from the first invention of P. Bozzini in 1807 to our time of digital endoscopy. The modern arsenal of endoscopic methods includes biopsy, radiopaque endoscopic through-papillary interventions (endoscopic retrograde pancreocholangiography, endoscopic papillotomy), endoscopic electroexcisio of neoplasms of gastrointestinal tract, techniques of hemostasia, fistuloscopy, chromoendoscopy, endoscopic ultrasonography, narrow band endoscopy, balloon-assisted and capsular enteroscopy, etc. The intraluminal endoscopic surgery under certain diseases became alternative to common surgical operations though its active development as a new direction was apprehended negatively by many doctors and heads of clinics.
