BACKGROUND Perianal fistulas account for a substantial discomfort and morbidity to the patient thus affecting productive man hours and quality of life. Accurate pre-operative assessment of course of the primary fistulous track and secondary extension or abscesses is required for successful surgical management of anal fistulas. The purpose of this study was to diagnose and classify pre-operative perianal fistulas. METHODS This is a cross-sectional study at Department of Radiodiagnosis in a tertiary level hospital of southern Rajasthan from November 2018 to November 2020. The study included a total of 50 patients referred to department of radiology for magnetic resonance imaging (MRI). Statistical analysis was done using chi square test and student t test. RESULTS Out of these patients, 56 % were having secondary tract on MRI, 12 % patients were having abscess and 4 % were having horseshoe abscess on MRI. The commonest type of ano-rectal fistula encountered in the study was Grade -II seen in 32 %. CONCLUSIONS MRI is a highly accurate, rapid and non-invasive tool in pre-operative evaluation of the perianal and anal fistulas. MRI evaluation and classification of perianal fistulae has a high degree of diagnostic accuracy. The use of MRI for the diagnosis and classification of perianal fistula can provide reliable information which has both pre-operative and prognostic value. St James University Hospital classification, which is an MR imaging-based grading system for perianal fistula is very useful for effective radiological-surgical communication thus contributing to improved patient care and reduced rate of recurrence. ABSTRACT Purpose Verify the position of lips and tongue at rest in newborns with and without ankyloglossia. Methods Cross-sectional study, carried out with 130 newborns in University Hospital. Data collection was performed by the researcher and speech-language pathologists from the Hospital. Information on gestational age, sex, weight, height and days of life was collected. The position of the lips and tongue at rest was evaluated through visual inspection with the newborns asleep. After the newborns were awakened, Neonatal Screening of the validated Protocol for the evaluation of the lingual frenulum for infants was performed to detect the alteration of the lingual frenulum. The data obtained were described and submitted to statistical analysis using the Chi-Square test to verify the association between the position of the lips with the tongue and to compare the position of the lips and tongue with and without ankyloglossia. The Mann-Whitney test was used to verify the behavior of the variables the differed between newborns with and without ankyloglossia. The significance level of 5% was adopted. Results When comparing the data, a significant difference was found between: weight and height with and without ankyloglossia; position of lips and tongue. An association between the position of the tongue and lips with and without ankyloglossia was also found. Conclusion Newborns without alteration of the lingual frenulum have a tendency to remain with their lips closed and their tongue elevated during rest and newborns with ankyloglossia have a tendency to keep their lips parted and their tongue low during rest.

Abstract Background The purpose of patient safety is to prevent harm occurring in the healthcare system. Patient safety is improved by the use of a reporting system in which healthcare workers can document and learn from incidents, and thus prevent potential medical errors. The present study aimed to determine patient safety challenges facing clinicians (physicians and nurses) in emergency medicine and to assess barriers to using e-OVR (electronic occurrence variance reporting). Methods This cross-sectional study involved physicians and nurses in the emergency department (ED) at King Khalid University Hospital (KKUH) in Riyadh, Saudi Arabia. Using convenience sampling, a self-administered questionnaire was distributed to 294 clinicians working in the ED. The questionnaire consisted of items pertaining to patient safety and e-OVR usability. Data were analyzed using frequencies, means, and percentages, and the chi-square test was used for comparison. Results A total of 197 participants completed the questionnaire (67% response rate) of which 48 were physicians (24%) and 149 nurses (76%). Only 39% of participants thought that there was enough staff to handle work in the ED. Roughly half (48%) of participants spoke up when something negatively affected patient safety, and 61% admitted that they sometimes missed important patient care information during shift changes. Two-thirds (66%) of the participants reported experiencing violence. Regarding e-OVR, 31% of participants found reporting to be time consuming. Most (85%) participants agreed that e-OVR training regarding knowledge and skills was sufficient. Physicians reported lower knowledge levels regarding how to access (46%) and how to use (44%) e-OVR compared to nurses (98 and 95%, respectively; p < 0.01). Less than a quarter of the staff did not receive timely feedback after reporting. Regarding overall satisfaction with e-OVR, only 25% of physicians were generally satisfied compared to nearly half (52%) of nurses. Conclusion Although patient safety is well emphasized in clinical practice, especially in the ED, many factors hinder patient safety. More awareness is needed to eliminate violence and to emphasize the needs of additional staff in the ED. Electronic reporting and documentation of incidents should be well supported by continuous staff training, help, and feedback.

Objective: to compare the performance of the rapid triage conducted by nurses at the emergency entrance and of the Manchester Triage System (MTS) in identifying the priority level of care for patients with spontaneous demand and predicting variables related to hospitalization. Method: a cross-sectional study carried out in an Emergency Department (ED) of a university hospital in São Paulo. The priority levels established in the rapid triage performed by nurses were high priority (patients of spontaneous demand directed to the emergency room) or low priority (those referred to the institution’s usual flow). Diagnostic accuracy measures were calculated to assess the performance of the indexes. Results: of the 173 patients (52.0% female, with mean age of 60.4 ± 21.2 years old) evaluated, it was observed that rapid triage was more inclusive for high priority and had better sensitivity and worse specificity than the MTS. The probability of non-severe patients being admitted to the emergency observation unit was lower due to the rapid triage. For the prediction of the other variables, the systems presented unsatisfactory results. Conclusion: the nurses overestimated the classification of patients as high priority, and rapid triage performed better than MTS in predicting admission to the emergency observation unit.

ABSTRACT Objective: To estimate the prevalence and factors associated with anxiety among multiprofessional health residents during the COVID-19 pandemic. Methods: Cross-sectional study, conducted in July 2020 with multiprofessional health residents (n = 67) from a university hospital. We used the Beck Anxiety Inventory to assess anxiety. Analyzing data through the chi-square test, likelihood ratio, and multiple analysis using Poisson regression with robust variance. Results: The proportion of moderate/severe anxiety was 31.3%, which showed significant association with working in sectors involving COVID-19 and directly with suspected/confirmed cases of COVID-19. During the multiple analysis, we found prevalence of anxiety in participants who needed psychological support after entering their residence and those who used psychotropic meds. Conclusion: The results seem to indicate that residents had their mental health impaired during the pandemic, but the maintenance of the variables in the model also suggests that they sought help to control anxiety.

Faktor Risiko Kejadian Penyakit Kulit Pada Pekerja Pengangkut Sampah Di Kota Jambi 1Entianopa, 2Ranissa Dwi Imansari, 3Irwandi Rachman       123Program Studi Kesehatan Masyarakat STIKES Harapan Ibu, Jambi   Abstrak Latar Belakang: Kulit merupakan organ terbesar pada tubuh manusia yang membungkus otot-otot dan organ-organ dalam serta merupakan jalinan jaringan pembuluh darah, saraf, dan kelenjar yang tidak berujung, semuanya memiliki potensi untuk terserang penyakit yang salah satunya adalah penyakit kulit. Penyakit kulit merupakan salah satu gangguan kesehatan yang sering dialami oleh pekerja pengangkut sampah. Berdasarkan komposisi sampah yang diangkut serta waktu paparan kerja. Tujuan penelitian ini untuk mengetahui hubungan antara masa kerja, pemakaian Alat Pelindung Diri (APD), dan personal hygiene dengan kejadian penyakit kulit pada pekerja pengangkut sampah di Kota Jambi. Metode: Penelitian ini merupakan penelitian kuantitatif dengan desain penelitian cross sectional study. Sampel penelitian yaitu sebanyak 62 pekerja pengangkut sampah yang berada di Kantor Pekerjaan Umum dan Penata Ruang, yang mana seluruh populasi dijadikan sampel. Data dikumpulkan berdasarkan pemeriksaan kesehatan oleh dokter dan dengan kuesioner, kemudian dianalisa menggunakan uji statistik chi-square. Hasil: Hasil menunjukan bahwa pekerja yang mengalami penyakit kulit sebanyak 35 pekerja (56,5%). Berdasarkan hasil analisis chi-square didapatkan bahwa ada hubungan yang bermakna antara masa kerja dengan kejadian penyakit kulit pada pekerja pengangkut sampah dimana nilai (p-value= 0,006), Pemakaian Alat Pelindung Diri (APD) nilai (p-value= 0,008), personal hygiene nilai (p-value= 0,008). Kesimpulan: Untuk meminimalisir risiko terjadinya penyakit kulit pada pekerja pengangkut sampah disarankan perlunya disusun standar operasional prosedur yang aman, penyediaan sarana sanitasi agar dapat mengurangi resiko terkena penyakit kulit. Pentingnya pemakaian APD dan perilaku hidup bersih dan sehat selama bekerja, serta diharapkan pekerja menggunakan APD pada saat bekerja dan lebih memperhatikan personal hygiene.   Kata kunci      : Masa Kerja, APD, Personal Hygiene

Background: Lymphoma is one of the most ten common cancers in the world as well as in Vietnam which has been ever increasing. It was divided into 2 main groups Hodgkin and non – Hodgkin lymphoma in which non-Hodgkin lymphoma appeared more frequency, worse prognosis and different therapy. Objectives: - To describe some common characteristics in patients with non – Hodgkin lymphoma; - To determine the proportion between Hodgkin and non- Hodgkin lymphoma, histopathological classification of classical Hodgkin by modified Rye 1966 and non-Hodgkin lymphoma by Working Formulation (WF) of US national oncology institute 1982. Materials and Method: This cross-sectional study was conducted on 65 patients with Hodgkin and non- Hodgkin lymphoma diagnosed definitely by histopathology at Hue Central Hospital and Hue University Hospital. Results:. The ratio of male/female for the non-Hodgkin lymphoma was 1.14/1, the most frequent range of age was 51-60 accounting for 35%, not common under 40 years. Non - Hodgkin lymphoma appeared at lymph node was the most common (51.7%), at the extranodal site was rather high 48.3%. The non - Hodgkin lymphoma proportion was predominant 92.3% comparing to the Hodgkin lymphoma only 7.7%; The most WF type was WF7 (53.3%), following the WF6 18,3% and WF5 11,7%; The intermediate malignancy grade of non- Hodgkin lymphoma was the highest proportion accouting for 85%, then the low and the high one 8.3% and 6.7% respectively. Conclusion: The histopathological classification and the malignant grade of lymphoma for Hodgkin and non - Hodgkin lymphoma played a practical role for the prognosis and the treatment orientation, also a fundamental one for the modern classification of non - Hodgkin lymphoma nowadays. Tujuan dilakukan penelitian untuk mengetahui hubungan antara umur dan obesitas pada kejadian Diabetes Gestasional di Puskesmas Dahlia Makassar 2017. Penelitian ini menggunakan metode penelitianan analitik dengan melakukan pendekatan Cross Sectional Study untuk mengetahui hubungan antara umur dan obesitas pada kejadian Diabetes Gestasional di Puskesmas Dahlia Makassar 2017 dengan jumlah populasi 374 orang dengan dan jumlah sampel 374 orang dengan menggunakan teknik Total  Sampling.Dari hasil uji statistic dengan menggunakan uji Chi-Square (Pearson chi-square) diperoleh untuk variabel umur ibu nilai p = 0,162 > a = 0,05 artinya tidak ada hubungan antara umur ibu dengan kejadian Diabetes Gestasional pada ibu hamil.   Untuk variabel obesitas nilai p = 0,003 < a = 0,05 artinya ada hubungan antara obesitas dengan kejadian Diabetes Gestasional pada ibu hamil.Kesimpulan dari dua variabel yaitu umur dan obesitas, hanya variabel obesitas yang berhubungan dengan kejadian Diabetes Gestasional pada ibu hamil di Puskesmas Dahlia Makassar 2017. Saran diharapkan penelitian ini dapat dijadikan sebagai salah satu bahan masukan atau sumber data untuk penelitian selanjutnya.

Background: Thyroid diseases occur more frequently in people exposed to ionizing radiation, but the relationship between occupational exposure to ionizing radiation and thyroid pathologies still remains unclear. Objective: To evaluate the prevalence of thyroid diseases in healthcare workers exposed to low-level ionizing radiation compared with a control group working at the University Hospital of Bari, Southern Italy, and living in the same geographical area, characterized by mild iodine deficiency. Methods: We ran a cross-sectional study to investigate whether healthcare workers exposed to ionizing radiation had a higher prevalence of thyroid diseases. Four hundred and forty-four exposed healthcare workers (241 more exposed, or “A Category”, and 203 less exposed, or “B Category”) and 614 nonexposed healthcare workers were enrolled during a routine examination at the Occupational Health Unit. They were asked to fill in an anamnestic questionnaire and undergo a physical examination, serum determination of fT3, fT4 and TSH, anti-TPO ab and anti-TG ab and ultrasound neck scan. Thyroid nodules were submitted to fine needle aspiration biopsy when indicated. Results: The prevalence of thyroid diseases was statistically higher in the exposed workers compared to controls (40% vs 29%, adPR 1.65; IC95% 1.34-2.07). In particular, the thyroid nodularity prevalence in the exposed group was approximately twice as high as that in the controls (29% vs 13%; adPR 2.83; IC95% 2.12-3.8). No statistically significant association was found between exposure to ionizing radiation and other thyroid diseases. Conclusion: In our study, mild ionizing radiation-exposed healthcare workers had a statistically higher prevalence of thyroid diseases than the control group. The results are likely due to a closer and more meticulous health surveillance programme carried out in the ionising radiation-exposed workers, allowing them to identify thyroid alterations earlier than non-exposed health staff.

BACKGROUND Occupational conditions are deadly health hazards especially where dust exposure is inevitable causing chronic disabilities, impaired respiratory function and ultimately leading to death if no intensive measures are put in place. Unhealthy practices and negative attitudes rise in the number of cases of pneumoconiosis due to poor health education and awareness strategies. Pneumoconiosis is not only a health problem but also a social and economic burden on the livelihood of people living in mining areas around the globe. OBJECTIVE to assess knowledge, attitude and practices of miners and post-occupational miners towards pneumoconiosis in Wusakile Township, Kitwe, Zambia. METHODS A cross-sectional study was employed to conduct a research in Wusakile Township and a questionnaire was customized in order to syphon data relevant to the study as well to be brief. The study was conducted among 73 participants who were randomly selected among miners and post-occupational miners and all satisfied the inclusion criteria. Both quantitative and qualitative methods were used to collect data. The data was entered and analysed using IBM SPSS software version 23. RESULTS Among 73 participants interviewed, 33.99% of participants had poor knowledge on the complications of pneumoconiosis. However, despite this poor knowledge, all participants had an idea about pneumoconiosis particularly silicosis. 13.70% of the respondents had bad practices towards pneumoconiosis while 86.30% had some good practices towards pneumoconiosis. Of the total participants, 19.18% of the participants had a negative attitude towards pneumoconiosis. Correlation between the level of education and practices of participants using Pearson Chi-Square, a p value of 0.021 (significant) was found ruling out the null hypothesis. CONCLUSIONS Information about pneumoconiosis and awareness programs towards pneumoconiosis are not widely disseminated among miners and post-occupational miners. There is still a significant number of participants who need to be educated more about pneumoconiosis and its complications so that attitude and practices are improved and also promote full community participation by involving competent health professionals to help in implementing preventive measures.
