Psychological and Social Factors Contributing to Post-traumatic Growth After Spinal Cord Injury

Introduction: Spinal cord injury (SCI) results in significant changes in a person's life; it has a tremendous psychological impact not only at the individual level but also on their caregiver. There is still little known about the positive psychological changes following SCI. The aim of this study was to determine the psychological and social factors affecting post-traumatic psychological growth after SCI. Methodology: This multi-center cross-sectional study was conducted on 66 patients with traumatic SCI between November 2020 and May 2021 using PWB-PTCQ, HADS, and social support scale. Data were analyzed using the SPSS program. Result: This study included 66 patients from multilabel territorial centers. The Mean ± SD of the patients’ age was 32.1 ± 14.5 years, and 34 (51.5%) of them were aged <32 years. Male patients were more dominant compared to their female counterparts; 48 (72.7%) versus 18 (27.3%), respectively. The most common type of injury was ASIA-A 24 (36.4%), and the most common nature of stress was financial 33 (53.2%). Conclusion: Anxiety and depression were less common among our patients with SCI, whereas there were high life stressors, and financial stress was the dominant type of stress among patients. The most affected dimension of PTG was a sense of mastery, whereas relationships and personal growth were the least affected among patients with SCI. Achieving PTG was significantly affected by depression, education level, and life stressors. There was a regression relation between PTG with depression and anxiety, thus PTG can predict depression and anxiety in patients with SCI.

Abstract Flexion-distraction injuries frequently cause traumatic cervical spinal cord injury (SCI). Post-traumatic instability can cause aggravation of the secondary SCI during patient's care. However, there is little information on how the pattern of disco-ligamentous injury affects the SCI severity and mechanism. This study objective was to analyze how different flexion-distraction disco-ligamentous injuries affect the SCI mechanisms during post-traumatic flexion and extension. A cervical spine finite element model including the spinal cord was used and different combinations of partial or complete intervertebral disc (IVD) rupture and disruption of various posterior ligaments were modeled at C4-C5, C5-C6 or C6-C7. In flexion, complete IVD rupture combined with posterior ligamentous complex rupture was the most severe injury leading to the most extreme von Mises stress (47 to 66 kPa), principal strains p1 (0.32 to 0.41 in white matter) and p3 (-0.78 to -0.96 in white matter) in the spinal cord and to the most important spinal cord compression (35 to 48 %). The main post-trauma SCI mechanism was identified as compression of the anterior white matter at the injured level combined with distraction of the posterior spinal cord during flexion. There was also a concentration of the maximum stresses in the gray matter after injury. Finally, in extension, the injuries tested had little impact on the spinal cord. The capsular ligament was the most important structure in protecting the spinal cord. Its status should be carefully examined during patient's management.

This study investigated (i) the prevalence of the metabolic syndrome (MetS) in people with a long-standing spinal cord injury (SCI); (ii) whether personal or lesion characteristics are determinants of the MetS; and (iii) the association with physical activity or peak aerobic capacity on the MetS. In a cross-sectional study, persons with SCI (N = 223; time since injury of ≥10 years) were tested. The individual components of the MetS were assessed together with the physical activity measured by the Physical Activity Scale for Individuals with Physical Disabilities (PASIPD), while peak aerobic capacity was tested during a graded wheelchair exercise test on a treadmill. Thirty-nine percent of the participants had MetS. In a multivariate logistic regression analyses and after performing a backward regression analysis, only age and education were significant determinants of the MetS. A 10-year increase in age leads to a 1.5 times more chance to have the MetS. Furthermore, people with a low education will multiply the relative risk of MetS compared with people with high education by almost 2. With and without correcting for confounders, no significant relationship was found between PASIPD or peak aerobic capacity and the MetS. It can be concluded that the prevalence of the MetS is high (39%) in people with a long-standing SCI but is comparable to the general Dutch population. Older people and those with a lower education level are most at risk for the MetS. Physical activity and peak aerobic fitness were not related to the MetS in this group with a long-standing SCI.

Background: Spinal cord injuries (SCI) are a variety of chronic diseases that various causes such as trauma may contribute to its onset. One of the problems in these patients is the problem of physical activity and, consequently, daily activities. Objectives: This study aimed to assess daily living of patients with SCI. Methods: In this descriptive cross-sectional study in 2019, 120 patients with SCI in Kermanshah were included in the study using purposive sampling. The instruments used in this study fell into two parts. One part included the demographic characteristics of the SCI patients, and the other part was a questionnaire of the rate of the Impact on Participation and Autonomy questionnaire (IPA-P). Data were analyzed by SPSS software version 16 using descriptive tests such as mean and standard deviation. Results: The result showed there was a significant relationship between demographic characteristics such as education (P < 0.007), time of spinal cord injury (P < 0.01), and income (P = 0.000). Also, the results showed there was a relationship between Autonomy and Participation, and the age of patients and their autonomy and participation decreased with age (P = 0.000, R = 0.72). Most of the patients had severe problems with daily activities. Also, most patients had very poor scores in relation to daily living activities. Conclusions: Considering the low rate of participation and autonomy in patients with SCI, it is suggested to conduct studies aimed at improving their self-care and social participation.

Spinal cord injury (SCI) is a traumatic injury to the spinal cord which is not a consequence of the disease. Mesenchymal stem cells (MSCs) have gradually become one of the most used stem cells in research and clinic trial. Based on the previous reports employed the cells ranged from 4 • 105 to 1 • 106, the present study was performed to figure out the best number of MSCs for transplantation of the chronic SCI. Magnetic nanoparticles were used for proving the precise transplantation strategy. Using magnetic resonance imaging (MRI), diffusion tensor imaging (DTI), diffusion tensor tractography (DTT), and behavior testing evaluations, we focused the effect of varying numbers of MSCs on reducing lesion cavity and post–traumatic syrinx formation, suppressing glial scar formation, enhancing neuronal fibers remodeling, promoting axonal regeneration and sprouting, improving vascularization, ameliorating the neuronal factors expressional level, and function improvement. Magnetic nanoparticles were precisely transplanted into the post–traumatic syrinx (PTS). MSCs can restore function after chronic SCI through stimulating the regeneration and sprouting of the axons, reducing the formation of PTS. The effect of MSCs on PTS management and functional improvement post chronic SCI was cell number–dependent, and within the range of 4 • 105 to 1 • 106, 1 • 106 cells were proved to be the best dose.
