CALM2 gene expression levels in the cumulus cells as a marker for chromosomal abnormalities in the embryo in in vitro fertilization programs

Abstract BackgroundThis observational clinical study evaluated the expression levels and predictive values of some apoptosis-related genes in granulosa cells (GCs) and follicular fluid (FF) of women undergoing in vitro fertilization (IVF).Methods GCs and FF were obtained at oocyte retrieval from 31 consecutive patients with heterogeneous infertility diagnosis (age: 34.3±5.8 years, body mass index: 24.02±3.12 kg/m2, duration of infertility: 4.2±2.1 years). mRNA expression of pro-apoptotic (BAX, CASP3, CASP8) and anti-apoptotic (BCL2, AMH, AMHR, FSHR, LHR, CYP19A1) factors was determined by quantitative RT-PCR using ROCHE LightCycler 480. Results No significant difference in GC or FF mRNA expression of pro- and anti-apoptotic factors could be demonstrated between IVF patients with (9 patients) or without (22 patients) clinical pregnancy. Each transcript investigated was detected in FF, but their levels were markedly reduced and independent of those in GCs. The number of retrieved oocytes was positively associated with GC AMHR (r=0.393, p=0.029), but the day of embryo transfer was negatively associated with GC LHR (r=-0.414, p=0.020) and GC FSHR transcripts (r=-0.535, p=0.002). When pregnancy positive group was analysed separately the impact of apoptosis- related gene expressions on some selected measures of IVF success could be observed. Strong positive relationship was found between gene expression levels of pro- and anti-apoptotic factors in GCs.ConclusionOur study provides only marginal evidences for the apoptosis dependence of IVF outcome and suggests that the apoptosis process induces adaptive increases of the anti-apoptotic gene expression to attenuate apoptosis and to protect cell survival.

Abstract Background: Preeminent interfering factor caused in vitro fertilization (IVF) failure has been broadly emphasized on poor ovarian response (POR) to controlled ovarian hyperstimulation (COH). The specific etiology and precise infertility treatment in POR patients, until now, has not been established and leads to detrimental effects on physical or psychological patient life qualities. To date, one biological stress phenomena, oxidative stress, is broadly implicated in varieties of female reproductive pathological conditions including poor oocyte quantity and quality. Hence, the present study is principally aimed to investigate oxidative stress scavenging system; enzymatic antioxidant-related gene expression level (SOD1, SOD2 and GPx4), in cumulus cells retrieved from poor ovarian responders (POR) and normal ovarian responders (NOR).Methods: A cross-sectional analytic study; fifty-six infertile women, aged between 25-49 years-old who underwent IVF procedure, were enrolled into this study with 28 participants in POR group (1-4 retrieved oocytes) and 28 participants in NOR group (5-15 oocytes). The cumulus cells from first punctured follicle were processed for mRNA gene expression levels of enzymatic antioxidant-related genes (Superoxide dismutase: SOD1, SOD2 and Glutathione peroxidase: GPx4) by quantitative real-time PCR (qPCR) and their association to other reproductive biological factors.Results: The relative mRNA expression of SOD1, SOD2 and GPx4 neither differed between poor and normal responders nor correlated with other relevant reproductive biological cohorts. However, the relative SOD2 mRNA expression level in patients who received human menopausal gonadotropin (hMG) was notably higher than the recombinant follicle stimulating hormone (rFSH) group (P = 0.017).Conclusions: Our major findings indicated that the enzymatic antioxidant-related gene mRNA expression in the cumulus cells do not associate with the ovarian response and other clinical outcomes (i.e. fertilization and pregnancy rates) after IVF procedure. However, the enzymatic antioxidant-related genes positively relate with type of gonadotropin administration.

The presence of cumulus cells (CCs) surrounding ovulated eggs is beneficial to in vitro fertilization and preimplantation development outcomes in several mammalian species. In the mouse, this contribution has a negligible effect on the fertilization rate; however, it is not yet clear whether it has positive effects on preimplantation development. Here, we compared the rates of in vitro fertilization and preimplantation development of ovulated B6C3F1 CC-enclosed vs. CC-free eggs, the latter obtained either after a 5 min treatment in M2 medium containing hyaluronidase or after 5–25 min in M2 medium supplemented with 34.2 mM EDTA (M2-EDTA). We found that, although the maintenance of CCs around ovulated eggs does not increment their developmental rate to blastocyst, the quality of the latter is significantly enhanced. Most importantly, for the first time, we describe a further quantitative and qualitative improvement, on preimplantation development, when CC-enclosed eggs are isolated from the oviducts in M2-EDTA and left in this medium for a total of 5 min prior to sperm insemination. Altogether, our results establish an important advancement in mouse IVF procedures that would be now interesting to test on other mammalian species.
