The endothelium keratoplasty of various modifications (DSEKTDMEK) is the first operation of choice under endothelium pathology of cornea of different genesis. The endothelium transplantation is most often implemented against the background of pseudophakia or at the same time instant with extraction of cataract and implantation of artificial crystalline lens. The clinical cases of damage of transparency of artificial crystalline lens after a routine extraction of cataract are known for a long time while dimness after endothelium transplantation is a new occurrence and related publications are singular. The study was carried out to present cases of damage of transparency of artificial crystalline lens after endothelium transplantation. During 2006-2015 more than 500 patients underwent endothelium transplantations in modifications of DLEK, DS(A)EK, FS-DSEK, invFS-DSEK and DMEK. The most of them (n=291) continue to be under regular or periodic observation. During average period of observation (78±16.8 months) four cases of dimness of artificial crystalline lens after endothelium keratoplasty were established. The biomicroscopy, visiometry and photoregistration were the main analysis techniques of establishing and studying of dynamics of dimness of crystalline lens. It is extremely difficult to establish the nature of dimness of intraocular lens after endothelium keratoplasty because explantation of intraocular lens is required. The surgical inflammatory reaction and presence of air in front camera are common for all cases of dimness of intraocular lens. The damage of transparency of intraocular lens is a complication of remote period after endothelium transplantation. The application of original technique of pneumocorneopexia promotes significant reduction of recurrent injections of air and possibly decreasing of probability of damage of transparency of intraocular lens.

AbstractWe describe a simply modified intrascleral fixation using round flange (SMURF) technique and report the clinical outcomes of the surgery. Forty-one eyes of 41 consecutive patients, with intraocular lens (IOL) dislocation, crystalline lens subluxation, and zonular weakness, who underwent surgery using the SMURF technique were included. The modified technique included the use of a conventional 27-gauge needle, a non-bent needle, oblique sclerotomy, direct threading of the leading haptic, and simple placement of the following haptic. IOLs were successfully placed and showed good centring. There were no cases of wound leakage or hypotony during the early postoperative period. Postoperative complications included vitreous haemorrhage in one eye (2.4%), intraocular pressure elevation in one eye (2.4%), and iris capture in six eyes (14.6%). There were no cases of postoperative retinal detachment, cystoid macular oedema, endophthalmitis, or IOL dislocation during the follow-up period. We proposed a few modifications in the intrascleral flanged technique for IOL fixation. The modified technique is a simple, easy, and minimally invasive procedure for successful IOL intrascleral fixation.

The Navy Debriefing Service is a central implementing agency whose main task is a series of supply chain management, which has several problems related to the uneven competence of Disbekal personnel, limited human resources and lack of creation, innovation, improvisation, as a result of the large number of personnel who have been around for a long time. served in Disbekal. So the purpose of this research is to analyze the influence of competence and the influence of organizational culture partially on the work productivity of Disbekal. As well as analyzing the influence of competence and organizational culture simultaneously on the work productivity of Disbekal. Descriptive verification research method with data analysis techniques using Path Analysis. The number of samples is 88 personnel, with data collection through stratified random sampling. The results showed that there was a significant influence between competence, and organizational culture on the work productivity of the TNI AL debriefing service partially and simultaneously. Where the direct influence of the competence variable is 30.2%, and the organizational culture variable is 18.5%. The total effect is 68.1% overall, while 39.1% is the influence of other factors that were not researched.

The dimensions of the dikes in the Ijsselmeer are mainly determined by wave-attack. The dimensions of the waves as a result of the design gale are calculated with the diagram of the Hydraulics Laboratory at Delft (ref« 1). This diagram is based on data of Sverdrup for deep water and principally on laboratory studies for shallow water. For a long time there has been a need of wave recordings on the lake in order to verify the calculated wave heights. A problem is the impossibility of maintaining a permanent recording station on the lake due to ice-drift in wintertime. Otherwise the Ijsselmeer lends itself admirably to wave-research, because there are vast regions with only small variations in waterdepth. Another advantage is that frequently more or less stationary conditions will occur under the influence of winds of constant force and direction. When Dr. Dorrestein of the Royal Dutch Meteorological Institute introduced his new floating waverecorder, it was possible to take observations in every place of the lake. Soon it appeared that this recorder has many advantages. The equipment consists of an accelerometer mounted on a little raft of one meter each way, that follows the movement of the water surface. The signal of the accelerometer is transmitted by an electric cable to the ship, where it is double integrated and then recorded (ref. 3). During the last winter several observations have been carried out with an instrument of this type* As a result of initial troubles with the electronic equipment the number of observations during gale-conditions has been limited. The usual duration of each recording is about 15 minutes. The average period of the waves lies between three and a half and five seconds, so each diagram consists of 180 to 250 waves. Wave height is measured as the difference in height between a trough and the next crest. The average period is determined by dividing the total recording time by half the number of zerocrossings.

The finite-time synchronization control is studied in this paper for a class of nonlinear uncertain complex dynamic networks. The uncertainties in the network are unknown but bounded and satisfy some matching conditions. The coupling relationship between network nodes is described by a nonlinear function satisfying the Lipchitz condition. By introducing a simple Lyapunov function, two main results regarding finite-time synchronization of a class of complex dynamic networks with parameter uncertainties are derived. By employing some analysis techniques like matrix inequalities, suitable controllers can be designed based on the obtained synchronization criteria. Moreover, with the obtained control input, the time instant required for the system to achieve finite-time synchronization can be estimated if a set of LMIs are feasible or an assumption on the eigenvalues of some matrices can be satisfied. Finally, the effectiveness of the proposed results is verified by numerical simulation.

Background. Refractive errors are widespread in the human population; nowadays, numerous surgical options allow for efficient and safe correction them. One of the main elements to ensure success in this kind of intervention will depend on the careful patient and surgical approach selection. Excimer laser corneal surgery is considered by most for low to moderate ametropias. Another option, which has been suggested may be safer, is to cut a small corneal lenticule with femtosecond laser, and then extracting it through a small incision. Nevertheless, in some specific cases, such as patients with high refractive error or those with some corneal abnormality, laser corneal ablations are considered unsafe from either a biomechanical or refractive standpoint. In this kind of particular cases, Phakic Intraocular Lens (P-IOL) implantation constitutes attractive, highly predictable and safe option. Objective. The authors want to show the case of a young high-myopic woman, already pseudophakic in one eye, where the P-IOL implantation in the fellow eye yielded excellent short-term visual results, and high patient’s satisfaction, is presented. Materials. The authors present the case of a 32-years-old, highly myopic female patient underwent a Refractive Lens Exchange (RLE) with a monofocal Intraocular Lens (IOL) implantation in her left eye elsewhere, and developed severe visual issues, especially regarding near-work. Symptoms resolved through the implantation of a posterior chamber P-IOL in the contralateral eye. Results. The postoperative course was unremarkable, inflammation was mild, and visual recovery was quick. There was no need to perform any procedure on her left eye or to use any reading glasses, as unilateral effective near vision through her right eye was enough for all her daily tasks. Conclusions. RLE in young pre-presbyopic highly myopic patients may not be an advisable alternative in most cases, because of the high risks of retinal complications observed. In addition, eliminating accommodation will cause significant limitations, and multifocal IOLs currently available are far from the quality of vision that a young human crystalline lens yields. On the other hand, implantation of a P-IOL is a good option if eye conditions are optimal, as it preserves natural accommodation. In this case an EyeCryl Phakic Toric® IOL showed excellent short-term refractive predictability and safety.
