In modern dermatology and cosmetology, the task of developing a complex method of therapy for the symptomatic complex remains acute, despite a large number of studies on this problem. The effectiveness of the treatment of postoperative scars was evaluated by the combined use of laser ablation (ablative photothermolysis) on the SmartXide Dot (Deka) device and autologous platelet enriched plasma (auto-OTP). The result of treatment of a patient with clinical manifestations of atrophic and hypertrophic postacne scarring 4th grade (according to the scorecard scale of post acne D. Goodmantal, 2006) on the skin of the face and back is presented. The tactics of managing the patient during rehabilitation after combined use of laser ablation and auto-OTP with application of topical retinoids are described. As a result of the therapy, there were fewer clinical manifestations of atrophic scars on the face skin on 70-80% and mainly hypertrophic scars on the skin of the back on 50-60%.

The article presents a brief review of the literature about clinical manifestations, modern methods of diagnosticsand treatment of an extremely rare disease – generalized eruptive keratoacanthomas of Grzybowski. The disease is characterized by the appearance of hundreds or thousands of generalized itchy nodules on the skin of the face, trunk, limbs, genitals, oral mucosa and larynx within a short time. The disease develops at the age of over 40, has a recurrent course, spontaneous involution of individual elements with the formation of areas of depigmentation or atrophic scars is possible. The authors describe a case of their own clinical care of generalized eruptive keratoacanthomas of Grzybowski in a 50-year-old patient who consulted a dermatovenerologist at the clinic of the Ural Research Institute of Dermatovenerology and Immunopathology. The duration of the disease at the time of visit to a doctor was 2,5 years, its occurrence was preceded by repeated visits to countries with excessive insolation, which can be considered as an etiological factor in the development of the disease. The diagnosis was first suspected at a boarddermatovenerologists based on the clinical picture of the disease and subsequently verified by the data of a pathomorphological examination of the biopsy of the affected skin. The effectiveness of the use of aromatic retinoids in the treatment of generalized eruptive keratoacanthomas of Grzybowski is shown and the side effects that developed in the patient with an independent increase in the dose of acitretin to 70 mg/day are given. The article states the extremely rare occurrence of this disease, as evidenced by the above clinical case, diagnosed for the first time in more than 90 years of the institute's existence. The importance of consolidating the clinical experience of several leading dermatovenerologists for the diagnostics of generalized eruptive keratoacanthomas of Grzybowski is emphasized.

Background. The use of mobile applications in dermatology to support remote diagnosis is gaining acceptance, particularly in rural areas, where dermatology services are commonly managed by healthcare personnel with no specialty training. Moreover, ontologies—sets of concepts that represent knowledge in a given domain—are increasingly being used to support medical diagnosis. A specific case is ONTODerm: an ontology to aid dermatological diagnosis. However, there is little information on the combined use of mobile applications and ontologies as support solutions in dermatology. Objective. Assessing the reliability of ONTODerm as a tool to support remote dermatological diagnosis when used together with a mobile dermatological application in underprivileged areas. Methods. A mobile application that allows characterization of skin lesions was developed, and the information about the lesions was sent to ONTODerm. An exploratory study was conducted in a remote area without access to a dermatologist. A total of 64 dermatological queries were recorded in the application and consulted with ONTODerm. Later, an experienced dermatologist evaluated the characterization and diagnosis of each query to determine the accuracy of the system. Results. The results showed that the probability of obtaining a correct diagnosis was between 64.4% and 85.6% with a confidence interval of 95%. A higher accuracy rate was obtained when the skin lesion occurred on the face or when its border was categorized as poorly demarcated. Conclusions. This study demonstrates the implementation of a teledermatology strategy based on mobile applications and domain ontology-driven knowledge base to provide timely assistance to healthcare professionals. This approach was found to be pertinent in the Colombian rural context, particularly in forest regions, where dermatology specialists are not available. The results of this article do not represent a final validation of the proposed approach; they suggest how the ontology can be improved to effectively support medical staff in marginalized regions.

BACKGROUND: In the literature, there are no scientifically based studies on the use of combination therapy using radio wave lifting and autologous platelet-rich plasma for the correction of involutional changes in the skin of the lower third of the face. AIMS: To determine the effect of complex therapy of involutional changes in the skin of the lower third of the face with minimally invasive radio wave lifting using a cannula electrode and platelet-rich autoplasma using non-invasive diagnostic methods. MATERIALS AND METHODS: Examination and treatment of 55 healthy women of Slavic appearance with involutional changes in the skin of the lower third of the face were carried out. Group № 1 patients received radio wave lifting therapy (n=28) and group № 2 patients received combined therapy with radio wave lifting and autologous platelet-rich plasma (n=27). The study was conducted from January 2020 to December 2020. To assess the condition of the skin, laser Doppler flowmetry, ultrasound sonography, and elastometry were used before and after the study. Clinical manifestations were assessed by an anthropometric method. RESULTS: The most pronounced changes were observed in the group where the combined method of radio wave lifting therapy with autologous platelet-rich plasma was used. The change in microcirculation was of a multidirectional nature, however, the average values were in the range of 8.8668.89 pf units, which is the normalization of trophic processes in tissues (p 0.05). When analyzing the conduct of ultrasound sonography, statistically significant thickening of the epidermis and dermis was noted, however, in patients of the second group, the thickening was more pronounced (p 0.0005). The change in the echogenicity of the dermis was of a multidirectional nature; by the end of treatment, the mean values were in the range of 10.511.3 units, which, apparently, indicates a more homogeneous structure of the dermis (p 0.05). Anthropometric indices (measurement of ptosis of the lower cheeks) decreased in both groups, which is related to skin lifting as a result of treatment (p 0.0005). The amplitude of tissue upward mobility decreased in both groups (p 0.0005). The amplitude of downward movement of tissues after therapy was of a multidirectional nature in both groups, which is possibly associated with the density of the dermis and indicates the normalization of collagen and elastin fibers (p 0.05). Elastometry indices (F0, R4, R6, R8) decreased, R2 increased in both groups, however, the dynamics was more pronounced in the second group (p 0.0005). CONCLUSIONS: Minimally invasive radio wave lifting in combination using a channel electrode with autologous platelet-rich plasma is an effective and safe method of treating age-related changes in the skin of the lower third of the face.

Here, we propose a method of forecasting the development of the heavy forms of the tick-borne encephalitis using diagnostic tables. The method is based on evaluation of the patients with syndrome of endogenous intoxication using immunological and biochemical data along with evaluation of the clinical manifestations. We also propose an advanced form of the method that includes data based on the genotype of OAS1 — the gene, coding oligonucleotidesynthetase needed for immune response to RNA viruses. OAS1 genotype is an important risk factor since the differences in catalytic activity of OAS1 isoenzymes result in different susceptibility to the virus of the tick-borne encephalitis.
