Mechanisms of Change in Mindfulness-Based Stress Reduction: Self-Compassion and Mindfulness as Mediators of Intervention Outcomes

Research has demonstrated support for the efficacy of mindfulness-based stress reduction (MBSR) in alleviating psychological distress and symptoms. Less is known, however, about the mechanisms through which MBSR achieves its outcomes. This study examined mindfulness and self-compassion as potential mediators of MBSR’s effects on several processes and behaviors related to emotion regulation, using data from a randomized trial of MBSR versus waitlist (WL), in which MBSR participants demonstrated significantly greater improvements in worry, fear of emotion, difficulties in emotion regulation, suppression of anger, and aggressive anger expression. Mediation analysis using bootstrap resampling indicated that increases in self-compassion mediated MBSR’s effects on worry, controlling for change in mindfulness. Increases in mindfulness mediated the intervention’s effects on difficulties in emotion regulation, controlling for change in self-compassion. Both variables mediated MBSR’s effects on fear of emotion. These findings highlight the importance of mindfulness and self-compassion as key processes of change that underlie MBSR’s outcomes.

The COVID-19 pandemic imposed extreme living conditions of social distancing, which triggered negative mental health problems and created challenges in seeking mental health support. Mindfulness-based interventions (MBIs) have been found to enhance wellbeing and mental health by reducing stress and anxiety and improving emotion regulation. Preliminary evidence suggests that online, synchronous MBIs may produce beneficial effects similar to face-to-face programs. However, the effectiveness of such online-MBIs to support mental health in highly stressful times, such as a global pandemic, requires further study. To this end, we investigated the effect of an online 8-week Mindfulness-Based Stress Reduction (MBSR) program on aspects of mental health during the first wave of the COVID-19 pandemic. Participants (N=92) who expressed interest in discounted online-MBSR programs were recruited for the study. The division into experimental and control groups was based on actual enrollment to the courses. Those who enrolled in a program were assigned to the experimental condition and those who decided not to enroll served as controls. Participants were assessed pre-intervention, post-intervention, and 1-month post-intervention for levels of mindfulness, perceived stress, anxiety, emotion regulation, and intolerance of uncertainty. Differences between the groups were tested using the general linear mixed effects model (GLMM) and Individual Growth Curve Models (IGCM) in intent to treat analysis. The findings indicated that, relative to the control group, MBSR improved mindfulness abilities (p <0.001), decreased anxiety (p <0.001), and stress (p <0.001) and increased emotion regulation (p <0.001). These effects were found to persist 1 month after the end of the program, despite the increased governmental public-health restrictions due to COVID-19 at that time. The ability to tolerate uncertainty, a central characteristic of the pandemic, was not found to be affected by the program. A mediation analysis revealed that the effect of the intervention on mental health improvement was partially mediated by the improvement in emotion regulation. Overall, the findings provide positive evidence for the feasibility of an online-MBSR program to support the mental health of individuals from the general population through the mediation of emotion regulation in challenging times, such as a global pandemic.

Introduction: Considering the high prevalence of anger problems in adolescence and the importance of emotion regulation in this age period, this study aims to assess the effectiveness of emotional regulation training in anger management and emotion regulation difficulties in adolescents by means of body postures, breathing pattern, and facial expressions.Method: A quasi-experimental research design along with pretest-posttest and control group was employed for the conduct of this study. For this purpose, the number of 50 adolescents was selected via purposive sampling through screening based on inclusion and exclusion criteria. These participants were randomly assigned to experimental and control groups and the experimental group then received 8 sessions of emotion regulation training. In this period, the control group received the treatment unrelated to emotion regulation training. Both groups were assessed in terms of Difficulties in Emotion Regulation Scale (DERS) and State-Trait Anger Expression Inventory-2 (STAXI-2) before and after the treatment as well as in the three-month follow-up. The collected data were analyzed by descriptive and inferential statistics, including covariance analysis.Results: Data analysis showed that emotion regulation training has a significant effect on anger management and components emotion regulation difficulties in adolescents.Conclusion: As per the results of this study, it is recommended that emotion regulation training be extensively conducted in the form of educational and training programs for adolescents, especially in schools.

This study aims to develop a clinical trial to test the efficacy of a mindfulness-based stress reduction (MBSR) and self-compassion (SC) program on self-reported values of anxiety, depression, and stress in parents of children with autism spectrum disorder (ASD) in primary school, in order to assess their integration into the framework of community intervention programs in Spain. Methods: A brief 8-week training program using mindfulness-based intervention (MBSR) and self-compassion (SC) has been applied to twelve Valencian ASD parents, ten of whom completed the program. Participants were assigned to two groups; both groups received the same treatment but at two different measurement moments. Depression, anxiety, stress, satisfaction with life and mindful attention awareness measurements were performed, in all participants, in three testing stages. Results: Analysis of variance results suggested that MBSR and SC training reduces stress and anxiety and increases mindful attention awareness. No significant changes were found in life satisfaction measurements. Conclusions: The small number of participants prevents us from generalizing the results found. More MBSR and SC clinical trials are needed in parents of ASD with results on anxiety, depression and stress in order to demonstrate the relevance of the inclusion of these programs in community-based early intervention services.

Background: The undesirable conditions resulting from addiction can be mitigated with timely diagnosis and effective interventions. Distress tolerance can be promoted in adolescents with a drug-dependent parent. Objectives: The present study aimed to investigate the effectiveness of mindfulness-based stress reduction (MBSR) therapy and emotion regulation training (ERT) on the distress tolerance of adolescent girls with a drug-dependent parent in Ahvaz, Iran. Methods: This clinical trial was performed on the experimental and control groups as the pretest-posttest design with follow-up. The statistical population included all the adolescent girl students with a drug-dependent parent in Ahvaz. The sample consisted of 45 adolescents with a drug-dependent parent selected by cluster sampling. The participants were randomly divided into two experimental groups of MBSR (eight 60-min sessions) and ERT (eight 45-min sessions) and a control group (n = 15 per group). All three groups were followed up after 45 days. The research instrument was the Distress Tolerance Scale (DTS), and data were analyzed using the analysis of covariance. Results: MBSR and ERT enhanced distress tolerance in adolescent girls with a drug-dependent parent (P < 0.001). The effects of the two interventions were not significantly different, and this result persisted in the follow-up stage. Conclusions: MBSR and ERT increased distress tolerance in adolescent girls with a drug-dependent parent. Therefore, these two interventions can be administered to enhance the mental health of adolescents.
