The most common extracranial solid malignant tumor of childhood – neuroblastoma (NB) is characterized by the presence of two metastatic forms (stages 4 and 4S). Stage 4S is described to have metastatic spread to the liver, skin, and minimal bone marrow invasion. Certain rare localisations of NB metastases (pleura, testicles, and others) may not worsen the prognosis in children of the first year of life in the absence of severe bone marrow invasion and metastases to the skeletal bones. The article describes a clinical case of retroperitoneal NB with atypical metastasis to the pleura in a child of the first months of life with a favorable outcome with a minimum amount of chemotherapy.

Introduction. Catatrauma is one of the main causes of childhood injuries and deaths in Russia. The severity of the condition of catatrauma is caused by combined injuries, high disability and lethality depend on them. Most often, children fall from the balconies of houses, windows, trees, and playgrounds. The purpose of the study was to show, on a clinical example, the case of a fall of a child from the height of the 16th storey of a dwelling house and the survival with a favorable outcome. Material and methods. The clinical example demonstrates the provision of medical care to a 5 years old child with catatrauma in a metropolis. After falling from a critical height (48 m) at the prehospital stage, he was assisted by ambulance brigades, and transportation was carried out by a helicopter. At the hospital stage, the victim was treated at the Research Institute of Emergency Surgery and Traumatology, where all departments of the clinic were involved. Results. This clinical case is unique in that the child, having fallen from the 16th sorey, from a height of more than 48 m, having received multiple combined injuries with an ISS score of 34 points, survived. An outcome was favorable due to the fact that the child was dressed in winter clothes with a jacket hooded over his head and fell into the snow, as well as coordinated actions of the services and units providing medical care at the prehospital and hospital stages in the metropolis. Conclusion. A favorable outcome after the child had fallen from a critical height, was affected by facts that both the child was dressed in winter clothes with a hood and a hat, put on the head, falling into the snow, and the coordinated activity of the structures and units participating in the prehospital and hospital stages of medical care.

Accordingtonewclassificationofgeneralizedformofinfections (Sepsis 3) it’sadvisabletoclassifiedpatients with biliary obstruction to three groups: mechanical jaundice, acute cholangitis and biliary sepsis with defying appropriate diagnostic criteria for each of them.Theaimofourworkistoimprovetreatmentresultsforpatientswithhyperbilirubinemia, biliary hypertension and system inflammation response syndrome (SIRS) by dividing them on groups and determining diagnostic criteria for each of them. Intheperiodfrom 2014 to 2018 wehavetreated 208 patientswithbiliaryobstruction. Accordingtodevelopeddiagnosticcriteriaallpatients was divided on three groups: mechanical jaundice, acute cholangitis and biliary sepsis.Afterthetreatmentthenextvalueswereanalyzed: timefromadmissiontohospitaltooperation, thetimeofoperation, thefrequencyofpostoperativecomplications, mortality, the duration of hospital stay, economic efficiency.Toassesstheeffectivesofdevelopedcriteriacomparativeanalysiswithretrospectivegroup, whichcontained 182 patients with hyperbilirubinemia, biliary hypertension and SIRS treated in a period from 2010 to 2014, was performed. Accordingtoobtainedresultswecanconcludethatdividingpatientswithhyperbilirubinemia, biliary hypertension and SIRS on groups and developeddiagnosticcriteria, routing and manage tactics let us to improve treatment results in this category of patients, about what we can judge by such figures as timefromadmissiontohospitaltooperation, thefrequencyofpostoperativecomplications, mortality, the duration of hospital stay, economic efficiency.

This article contains a clinical case of a favorable outcome of treatment of chronic keratoconus by the collagen cross-linking with modified parameters. Possible mechanisms of action of collagen cross-linking on biomechanical and biochemical properties of the cornea are presented as well as survey data and postoperative dynamic monitoring of the patient before and after collagen cross-linking. Reasoning about new ways to perfect standard approaches with the aim of improving the results of the application of this treatment are also given.

Toxocarosis can be manifested by pathology of various organs and systems of the body, occurring under the mask of many diseases. This dictates necessitates a more thorough familiarization with this pathology of doctors of various specialties, in particular, dermatologists, ophthalmologists, therapists, neuropathologists. The article describes a clinical case of successful diagnosis and treatment of the most severe, in terms of consequences, the forms of this disease – cerebral toxocarosis. Timely diagnosis of pathology includes cerebral MRI and immunological studies of blood serum for the presence of antitoxocarosis antibodies. Albendazole – a broad-spectrum antihelminthic drug has proven to be highly effective in the therapy of this pathology. Proper diagnosis and, as a result, adequate therapy are the guarantors of a favorable outcome in this form of the disease. Keywords: Toxocarosis, brain, diagnostics, treatment, albendazole

AbstractAcute pathology of the biliary tract including cholangitis and cholecystitis can lead to biliary sepsis if early decompression is not performed. This article provides an overview of the presenting signs and symptoms and role of interventional radiology in the management of patients with acute cholangitis or acute cholecystitis. It is especially important to understand the role of IR in the context of other treatment options including medical management, endoscopy, and surgery.
