High prevalence of various infectious diseases including but not limited to HCV, HBV and HIV alongside TB in Pakistani population indicates there must be similarly high prevalence of these, if not higher, amongst (intravenous drug users) IDUs serve as a reservoir for infectious diseases, blood borne as well as others due to injection abuse, poor hygiene and bad quality of life. This continues to be a serious threat in spreading out of these diseases, decreased health quality of the society and increased death rate. This survey study was conducted to check the prevalence of HCV, HBV, HIV and TB in several drug rehabilitation centers in Punjab region of Pakistan. 78 centers were either visited or contacted via phone and 21 out of them cooperated to share their data. An analysis of respondent centers returned useable data revealed that only 7.2% patient were found positive for Hepatitis B. While, 33.23% patients were positive for HCV, 8.74% patients were positive for HIV and 0.87% were positive for TB. What is worrisome is the absence of proper periodic screenings for these as well as other potential infections. Also we found out that upon diagnosis of these diseases mostly rehabilitation centers refuse admittance, which aggravates the problem further as well as leaves those IDUs and their families helpless, who are looking for it. Majority of the respondent centers, doctors, psychologists believed that the patients, partner of the patients and their immediate family members must be offered for infection screenings regularly.

The aftercare program of a drug rehabilitation center is a continuing treatment receive by ex-drug abusers immediately after being discharged from a residential rehabilitation center. This program caters services that help in the development of skills of ex-drug abusers as they move out from the facility of the rehabilitation center for them earn for living. The rehabilitation centers offer opportunities for employment as they allow other agencies to offer job and training and seminar services for ex-drug abusers. In the implementation of the job opportunities as an aftercare program, the selection of ex-drug abuser who is ready to work, ready to be trained, and ready for a further referral is a crucial decision. To this end, an architectural model that can match ex-drug users profile and relevant jobs, training, and referrals were needed. The developed architecture will serve as their basis for a decision support system development

Although there have been repeated calls for the establishment of safe injection facilities (SIFs) in Vancouver since the early 1990s, questions remain concerning the feasibility of SIFs due to the high prevalence of injection cocaine and the concomitant problems cocaine use presents. Therefore, we determined the prevalence of willingness to attend SIFs among cocaine injectors in Vancouver and explored the factors associated with this willingness, using data from the Vancouver Injection Drug Users Study. After considering the results derived from this analysis, additional qualitative methods were employed to explore further willingness to use SIFs, barriers and facilitators of SIF utilization, and methods of maximizing use among selected subpopulations of cocaine injectors. The results suggest that a high proportion of cocaine injectors, including some of those most at risk, would attend an SIF if one were available. However, in order to better accommodate cocaine injectors, several modifications could be made to conventional SIF service design and delivery. The vast majority of these modifications relate to ensuring effective responses to cocaine toxicity. Given the acceptability of SIFs among cocaine injectors, it appears that an SIF pilot could result in significant and immediate benefits in terms of public health and community safety.
