“Can We Break Through the Identity Dilemma?”: The Multiple Governance, Identities Conflict, and Self-Managements for Chinese PWUD.

Background: Exploring why the national drug policy failed is an important issue. Based on it, this study considers both the drug governance system and the voice of people who use drugs (PWUD) to analysis drug policy effects and the lives of PWUD in the Chinese context. Methods: This study takes PWUD living in urban communities as the research participants. Using qualitative research data, exploring how individual using drug experiences are identified and classified by structural forces and how individuals manage their daily life. Results: The study finds that law enforcement agencies (Narcotics Control Office (NCO), Police Station , and Community Assistance Group (CAG)) have different attitudes toward PWUD and generate different managements, forming multiple governance models, and then shaping the multiple identity dilemma. PWUD perceive the dilemma, and they adopt the strategies of spatial isolation (hidden flow), disconnection from experience (temporal isolation), and instrumental cooperation to continuously re-adjust and achieve identity recovery from the bottom-up. Conclusion/importance: Through evaluating the effectiveness of self-management strategies, this research integrates the Chinese drug rehabilitation situation, expands the specific performance of law enforcement system, and presents the limitations and positive value of self-management strategies.

‘Long Covid’ (post-covid syndrome): Mechanism of occurrence, diagnosis and rehabilitation

The article is devoted to the study of an urgent problem of modern humanity – the fight against a pandemic caused by a new coronavirus infection, namely: the study of the mechanism of development of ‘long Covid’ (post-covid syndrome), a new clinical and laboratory method of its diagnosis, issues of drug and non-drug rehabilitation of patients who have suffered COVID‑19. The paper describes the etiopathogenesis of post-covid syndrome (PS), the distinctive features of which are the defeat of the cardiovascular, respiratory, nervous, digestive, immune systems of the body, ENT organs and musculoskeletal system. For the diagnosis of post-covid syndrome, a monochrome nanoparticle analyzer (MAN) has been tested for the first time in the Russian Federation, which allows determining pathophysiological shifts in the homeostasis system. It was found that the MAN method has a sufficiently high diagnostic sensitivity (78%), and shifts in the homeostasis system in post-covid syndrome are statistically significant (p < 0.001). A fundamentally important point of this study is that an algorithm has been developed for noninvasive diagnosis of PS by saliva (oropharyngeal flushes), based on the detection of a high contribution to the scattering of laser radiation in the mid-frequency range of the spectrum on nanoparticles ranging in size from 119 to 121 nm. The article also pays special attention to medicinal and non-medicinal methods of treatment of patients with ‘long Covid’ experiencing the consequences of a new coronavirus infection. It has been established that the best effect is provided by complex treatment combining methods of the traditional European school (drug therapy with the use of cardiovascular drugs, nootropic agents, chondroprotectors, vitamins and mineral complexes, hormones, hepatoprotectors, biostimulants, sedatives and anti-inflammatory drugs and other drugs) and non-drug methods of restorative treatment practiced by Oriental medicine (acupuncture, hirudotherapy, massage, osteopathy, etc.).

Contribution of Lifestyle Related Shaming in Drug Relapse Management in Indonesia

In the statistical calculation of the National Narcotics Board of the Republic of Indonesia or so called BNN, there were 70% of drug offenders with the status of drug relapse in 2019 reached (BNN: 2019). Such condition illustrates that the social rehabilitation mechanism for drug offenders in Indonesia still has the opportunity to present shameless people. Therefore, this study is intended to observe the significance of Lifestyle Related Shaming as an effort to reduce the number of drug relapses in Indonesia. Lifestyle Related Shaming is supported by 3 (three) macro theories, namely reintegrative shaming theory proposed by John Braithwaite, desistance theory proposed by Farral and routine activity theory proposed by Hiddlenlang The literature review method was applied here through the utilization and effort to develop an existing theory (grounded theory) by presenting certain concepts (variables) in accordance with the affordability of study data. In addition to qualitative approach, this research also performed a quantitative approach through survey method among respondents who experienced drugs relapse in Indonesia as well as in-depth interviews with drug relapse residents with various backgrounds and educational levels at the Center for Drug Rehabilitation of the Republic of Indonesia at Lido Bogor. It is expected that this study may provide a broad understanding to the government to prepare the best program for solving the drug relapse phenomenon, especially drug abuse in Indonesia. For the science, this study is expected to be a basis for the implementation of shaming in Indonesia. Furthermore, this study is also expected to be able to provide input in the process of drug relapse management in Indonesia, especially in an effort to prevent residents from falling into a shameless condition.

NERVOUS FACTORS OF ARTERIAL HYPERTENSION PROSPECTS FOR PREVENTION AND REHABILITATION

The article provides a theoretical analysis of the causes of arterial hypertension, describes a three-month observation of the initial stage of the development of hypertension and experiments on non-drug normalization of blood pressure on a group of volunteers. It has been shown that arterial hypertension is associated with spastic conditions of the intervertebral muscles in the lower thoracic spine, which lead to compression of the sympathetic nerves that control the transport of water through the kidneys and disturb the balance of fluid circulation through the circulatory system. The possibility of prevention of arterial hypertension and non-drug rehabilitation of patients with this disease has been shown.

How to Enhance the Motivation for Drug Detoxification: Consciousness Guidance and Behaviour Restriction of Family Intergenerational Ethics

Assisting substance users to recover from the behaviour of drug addiction and maintain long-term rehabilitation is a long and complicated process, in which the motivation to undergo drug rehabilitation plays a decisive role. So far, the cultural connotation of family and its mechanism of promoting behavioural change of substance users have not been fully explored. Through in-depth interviews with 15 drug rehabilitants, among which there were 7 women and 8 men, it is found that the motivation for drug rehabilitation is stimulated under the guidance and restriction of family ethics based on obligation and responsibility, which is mainly reflected in the longitudinal intergenerational responsibility. On the one hand, negative consequences such as intergenerational liability deficit and reputation damage lead substance users to reflect on ethical values. On the other hand, disciplines such as intergenerational responsibility and obligation and mutual assistance can correct the actual behaviour of substance users in ethical practice. In contrast to Western countries, which focus on external environmental factors such as family function, family relationships and family support, the motivation for drug rehabilitation in China places more emphasis on their identity and role as family members and corresponding responsibilities, which provides inspiration for developing social work services for substance users from family cultural norms.

PATHOPHYSIOLOGICAL COMPONENTS OF ARTERIAL HYPERTENSION. PROSPECTS FOR PREVENTION AND REHABILITATION

We show that arterial hypertension is associated with spastic conditions of the intervertebral muscles in the lower thoracic spine, which leads to compression of sympathetic nerves that control the transport of water through the kidneys, which in turn upsets the balance of fluid circulation through the circulatory system.Theoretical analysis and experimental data made it possible to formulate a hypothesis about the dominant role of disorders in the activity of the sympathetic part of the nervous system in the development of primary and persistent arterial hypertension. The possibility of prevention of arterial hypertension and non-drug rehabilitation of patients with this disease has been shown.

Imaginary Penalty and Identity Conflict: The Dilemma and Strategies of Multiple identities in Substance Abusers in Chinese Drug Policy

Although the theory of "Imaginary Penalty" points out the relationship between ideology and policy-making, it ignores the systemic nature of drug control and the voice of drug addicts. Methods: This study takes substance abusers who live in urban communities as the research participants. Using qualitative research data, this study explores how individual drug abuse experiences are identified and classified by structural forces and how individuals manage their daily life. Results: The study finds that law enforcement agencies (Narcotics Control Office (NCO), Police Station , and Community Assistance Group (CAG)) have different attitudes toward Substance abusers and generate different managements, forming multiple governance models, and then shaping the multiple and incomplete Substance abusers’ identity dilemma. Drug addicts perceive the dilemma, and they adopt the strategies of spatial isolation (hidden flow), disconnection from experience (temporal isolation), and instrumental cooperation to continuously re-adjust and achieve identity recovery from the bottom up with the "possible citizens" narrative logic. Conclusion/importance: Through evaluating the effectiveness of self-management strategies, this research integrates the Chinese drug rehabilitation situation, expands the specific performance of "Imaginary Penalty" in the law enforcement system, and presents the limitations and positive value of self-management strategies.

Prefrontal fNIRS-based clinical data analysis of brain functions in individuals abusing different types of drugs

Background The activation degree of the orbitofrontal cortex (OFC) functional area in drug abusers is directly related to the craving for drugs and the tolerance to punishment. Currently, among the clinical research on drug rehabilitation, there has been little analysis of the OFC activation in individuals abusing different types of drugs, including heroin, methamphetamine, and mixed drugs. Therefore, it becomes urgently necessary to clinically investigate the abuse of different drugs, so as to explore the effects of different types of drugs on the human brain. Methods Based on prefrontal high-density functional near-infrared spectroscopy (fNIRS), this research designs an experiment that includes resting and drug addiction induction. Hemoglobin concentrations of 30 drug users (10 on methamphetamine, 10 on heroin, and 10 on mixed drugs) were collected using fNIRS and analyzed by combining algorithm and statistics. Results Linear discriminant analysis (LDA), Support vector machine (SVM) and Machine-learning algorithm was implemented to classify different drug abusers. Oxygenated hemoglobin (HbO2) activations in the OFC of different drug abusers were statistically analyzed, and the differences were confirmed. Innovative findings: in both the Right-OFC and Left-OFC areas, methamphetamine abusers had the highest degree of OFC activation, followed by those abusing mixed drugs, and heroin abusers had the lowest. The same result was obtained when OFC activation was investigated without distinguishing the left and right hemispheres. Conclusions The findings confirmed the significant differences among different drug abusers and the patterns of OFC activations, providing a theoretical basis for personalized clinical treatment of drug rehabilitation in the future.
