Background: The method of dermotension is successfully used in surgical practice to close extensive defects or as a result of treating fractures using the Ilizarov apparatus. However, to obtain the desired result, surgeons often neglect the condition of the skin flap itself. In this regard, the purpose of our study was to study the dynamics of changes in fibrous structures in the dermis of the skin during dermotension. Methods and Results: The material for the 14-day study was a skin flap of Wistar rats obtained after distraction with the Ilizarov apparatus. Analyzing the morphological picture of the state of the dermis after the study, we found a decrease in the thickness of both the epidermis and the dermis by 2.3 and 3.3 times, respectively. A decrease in the density of collagen structures of both types I and III was also noted. Conclusion: The results obtained indicate the restructuring, first of all, of the fibrous component of the dermis, which consists in reparative-restorative processes, which must be taken into account when choosing the rate and duration of dermotension.

The ADP:O ratios and State 3 (ADP-stimulated) and State 4 (controlled) rates of succinate, beta-hydroxybutyrate, isocitrate, glutamate, pyruvate + malate, alpha-ketoglutarate, and ascorbate + N,N,N',N'-tetramethylphenylenediamine (TMPD) oxidation were examined in liver mitochondria from germ-free and conventional rats of both Lobund Wistar (100-day-old) and Fisher (120-day-old) strains. The State 3 respiration rates of isolated mitochondria from germ-free and conventional rats were comparable except for the rate of succinate oxidation in the Wistar rats, which was significantly lower (approx. 10%). The State 4 respiration rates were generally lower in mitochondria isolated from germ-free Fisher rats (approx. 8%) and significantly lower (approx. 18%) in germ-free Wistar rats. The ADP:O ratios were similar in germ-free and conventional rats. Serum thyroxine concentrations indicated delayed maturation of thyroid function in young germ-free rats, but adult animals had concentrations similar to those found in conventional rats. The results indicate that, although absence of a microflora results in a 20-30% reduction in metabolic rate, the germ-free state has little influence on the functional respiration or oxidative phosphorylation of mitochondria isolated from the liver of the adult rat.

PURPOSE: To assess the viability of McFarlane skin flaps in rats with administration of sildenafil. METHODS: Twenty Wistar rats were distributed into two groups: Control (dorsal skin flap, subdermal application of saline solution at 0.9%) and Study (dorsal skin flap, subdermal application of sildenafil). Seven days after the surgery, flaps were photographed and graphically rendered. Then, they were analyzed with AutoCAD software. Three biopsies (proximal, medial and distal) of each flap were collected for histological analysis. RESULTS: Macroscopic analysis showed that animals of the study group had greater necrotic areas (p=0.003) in the dorsal skin flaps. Additionally, histological analysis of the distal third of these flaps showed a tendency to less granulated tissue formation in animals treated with sildenafil. CONCLUSION: Sildenafil subdermally was associated with lower viability of the random skin flap in rats.

In this article data on the analysis of the effect of transgenic Kalanchoe pinnate extract with cecropin P1 on the state of uninfected wounds in rats are presented. The study used Wistar rats weighing 220 ± 20 g. A wound of 20 × 20 mm was modeled under anesthesia in sterile conditions. The wound was treated during 14 days with the 3% hydrogen peroxide solution in all experimental groups. Additional treatment with the 10% cefazolin solution was used in group No. 2; with Kalanchoe juice it was used in group No. 3, and with transgenic Kalanchoe extract cecropin P1 it was used in group No. 4. The effectiveness of the treatment of the wound process was evaluated by the external state of the wound and planimetry indicators. It was found that all analyzed parameters of the experimental groups were better than in the control group of animals. However, the effect was most pronounced in the group treated by transgenic Kalanchoe extract with cecropin P1, The results of the study allow us to recommend further preclinical study of the substance for the possible treatment of trophic ulcers in patients with diabetes mellitus.

PURPOSE: To evaluate the effects of capsaicin on the viability of ischemic random-pattern skin flaps in rats. METHODS:Forty EPM1-Wistar rats were randomized into two groups of 20 animals each, the capsaicin group and the control group. A random-pattern skin flap measuring 10 x 4cm was raised and a plastic barrier was placed between the flap and the donor site. After the surgical procedure, the control group was treated with an inert vehicle in the form of a cream applied uniformly to a rayon bandage which, in turn, was applied to the surface of the skin flap. The capsaicin group was treated in the same way, but in this case capsaicin was added to the cream. This procedure was repeated for two consecutive days. RESULTS: There was a significantly smaller amount of flap necrosis in the capsaicin group (35.07%) than in the control group (44.75%) (p=0.035). CONCLUSION:Topical administration of capsaicin improved the viability of ischemic random-pattern skin flaps in rats.

The paper presents a case of severe gunshot wound of the shin in a 16-year-old adolescent as a result of gunshot from a small distance. Comminuted fracture of the tibia, damage of the anterior tibial artery, defect of the soft tissue in the injured area were detected. When admitted to the hospital, the patient underwent surgical treatment of the wound and fixation of fragments in carboxylic rod apparatus of external transosseous fixation. Osteomyelitis was developed. Ten days later, reosteosynthesis was performed. The first stage of bilocal osteosynthesis of the right shin was implemented, necrotized soft tissues and bone fragments and detritus were removed, initial purulent process was arrested. There was started distraction of the distal fragment of the tibia obtained as a result of its corticotomy in the lower third into the bone defect measuring 5 cm, which occurred when implementing surgical treatment of the wound. Twenty-eight days after, open alignment of tibial fragments in the upper third and reinstallation of Ilizarov apparatus was performed. The site of fragment junction was closed with unfree skin flap on the pedicle measuring 4 6 cm, the transplant bed was closed with full-layer skin graft using Parins technique. The further consolidation of fragments was reached simultaneously with maturation of the bone regenerate. The total period of fixation in Ilizarov apparatus lasted for 8 months. The bone and soft tissue structural integrity was restored, full function of the lower limb and adjacent joints was reached. Ilizarov apparatus is indispensable in multistage treatment of gunshot fractures and provides dynamic correction in changeable conditions; it permits to preserve the limb and its functionality.

Preeclampsia remains a formidable disease in the second half of pregnancy. Therefore, the search for new drugs for the treatment and prevention of preeclampsia is an urgent task of modern pharmacology. Materials and methods. The experiment was performed on female Wistar rats. The studied drugs were administered from 10 to 21 days of pregnancy. Functional tests were performed on the 21st day of pregnancy. Results. The use of a peptide that imitates erythropoietin α-helix B in experimental preeclampsia leads to a pronounced correction of pathological changes: blood pressure decreased, microcirculation in the placenta increased, and the morphological picture in the placenta improved. Resume. The results indicate the promise of the use of a peptide imitating α-helix B erythropoietin in experimental preeclampsia.

AbstractVascular complication is one of the causes of skin flap healing failure. Sildenafil and tadalafil, a type-5 phosphodiesterase inhibitor, can improve flap viability, however, the action mechanisms involved in this process are still unclear. To assess the effects of orally administered sildenafil and tadalafil on the healing kinetics and skin flap viability, sixty-two Wistar rats were divided into three groups: control (n = 22), sildenafil (n = 20), and tadalafil (n = 20). The solutions were administered orally (dose: 10 mg/kg) immediately after the surgical procedure and then every 24 h. At postoperative days 7 and 14, the skin flap samples were collected, submitted to histological processing and evaluated under optical microscopy. In experimental groups (sildenafil and tadalafil), we found an increased vascularization (p < 0.05) on the 7th and 14th day associated with the ulcer size decrease on the 14th day, although it was not significant. There was a higher influx of neutrophils and a decrease of mononuclear population on the 7th day (p < 0.05). On the 14th day, these differences were observed only in the tadalafil group (p < 0.05). This study suggested positive results with the use of sildenafil and tadalafil as adjuvant drugs in skin flap viability.

ABSTRACT Background: Hepatectomies promote considerable amount of blood loss and the need to administrate blood products, which are directly linked to higher morbimortality rates. The blood-conserving hepatectomy (BCH) is a modification of the selective vascular occlusion technique. It could be a surgical maneuver in order to avoid or to reduce the blood products utilization in the perioperative period. Aim: To evaluate in rats the BCH effects on the hematocrit (HT) variation, hemoglobin serum concentration (HB), and on liver regeneration. Methods: Twelve Wistar rats were divided into two groups: control (n=6) and intervention (n=6). The ones in the control group had their livers partially removed according to the Higgins and Anderson technique, while the rats in the treatment group were submitted to BCH technique. HT and HB levels were measured at day D0, D1 and D7. The rate between the liver and rat weights was calculated in D0 and D7. Liver regeneration was quantitatively and qualitatively evaluated. Results: The HT and HB levels were lower in the control group as of D1 onwards, reaching an 18% gap at D7 (p=0.01 and p=0.008, respectively); BCH resulted in the preservation of HT and HB levels to the intervention group rats. BCH did not alter liver regeneration in rats. Conclusion: The BCH led to beneficial effects over the postoperative HT and serum HB levels with no setbacks to liver regeneration. These data are the necessary proof of evidence for translational research into the surgical practice.

The present study focused on evaluation of induction, maintenance and emergence of inhaled anesthesia and comparison of these anesthetics times for isoflurane and sevoflurane. The biological material consisted of 12 Wistar rats divided into 2 groups, each containing a number of 6 rats. One group was anesthetized with isoflurane and the other with sevoflurane. We monitorized the rats during all stages of anesthesia, observing how the induction established, the state of the rats throughout the anestesia and the moment of the first recovery signs, the moment the rats became active as well as the moment of full recovery from anesthesia. Both isoflurane and sevoflurane rapidly induced the anesthesia, without excitation signs and the emergence was significantly faster in sevoflurane group in comparison to the isoflurane group, being accompanied by horipilation in both anesthetics.
