Abstract IntroductionTo the best of our knowledge the combination of multiple synchronous primary malignant tumors which involves breast carcinoma, thyroid carcinoma and cervical carcinoma has not been previously reported.Case reportHerein, we report a 47-year-old female who was diagnosed with multiple synchronous primary malignancies: cervical squamous-cell carcinoma, invasive ductal carcinoma of left breast, papillary carcinoma of the left thyroid gland and had a gene mutation of both CHEK2 and PIK3CA. We adopted a comprehensive treatment approach .ConclusionWhen a cancer patient presents with the clinical manifestation of another tumor, the possibility of synchronous occurrence of multiple primary malignant tumors, as well as tumor metastasis should be considered. In addition, the follow-up time of patients with malignant tumors should be extended, because timely detection and treatment of more primary malignancies may extend the survival time of patients and improve their quality of life.

A 49-year-old woman presents with an extensive violaceous rash, rapidly progressive proximal muscle weakness, and dysphagia to solids, consistent with a diagnosis of dermatomyositis. Two weeks later, she palpates a mass in her left breast and is diagnosed with her2-positive metastatic invasive ductal carcinoma of the breast.There is a well-established association between dermatomyositis and malignancy. However, the specific association between breast cancer and dermatomyositis has not been well characterized. No guideline for oncologists managing these patients has been established. Recently, 3 cases of breast cancer and dermatomyositis were diagnosed at our institution. A review of the literature was pursued to characterize the association between breast cancer and dermatomyositis.A review of 178 papers identified 22 cases of breast cancer with dermatomyositis. Most patients (71%) presented with stage iii or iv breast cancer. The median time between the diagnosis of breast cancer and the onset of dermatomyositis symptoms was 1 month. Three quarters of the patients were steroid-responsive and able to taper. Half the women with follow-up data experienced a documented cancer relapse associated with a new flare of cutaneous symptoms.The presence of dermatomyositis appears to be associated with more-advanced breast cancer stage and is most commonly associated with invasive ductal carcinoma. In our review, treatment of cancer alone is insufficient to adequately control the cutaneous and myopathic manifestations of dermatomyositis, which can significantly affect quality of life. A multidisciplinary approach, including close collaboration with rheumatologists and dermatologists, is therefore important in the diagnosis and management of oncology patients with dermatomyositis.

Aim: to systematize data on rehabilitation methods and management tactics for women with external form of genital endometriosis.Materials and Methods. We searched for publications in international scientific databases: scientific electronic library eLibrary, Scholar, ScienceDirect, Cochrane Library, PubMed/MEDLINE for the last 5 years. The data on the modern approach to the therapy and rehabilitation of women with external genital endometriosis are presented. Search terms in Russian and English were used: “endometriosis”, “rehabilitation”, “gynecology”, “quality of life”.Results. Endometriosis is considered an independent risk factor for the development of malignant tumors not only of the reproductive system, but also of the large intestine, bladder, mammary glands. The review presents a current view of the treatment and rehabilitation of women with external genital endometriosis. Endometriosis is a polyetiological disease that can manifest itself as dysmenorrhea, dyspareunia, chronic pelvic pain, as well as dysuria and dyshesia when involving adjacent organs in the pathological process. Relapses, infertility, emotional and psychosexual disorders, problems with socialization are often detected in these patients.Conclusion. It is necessary not only to conduct comprehensive treatment, but also to develop personalized rehabilitation programs that can improve the quality of life, as well as create comfortable conditions for social adaptation in society.

This paper presents the positive characteristics of preoperative autohemochemotherapy associated with the drug "Melaxen” as a synthetic analogue of the epiphysial hormone melatonin, produced by Unipharm-USA in the complex treatment of patients with ovarian cancer on the stage III-IV. The authors emphasize that the "Melaxen" has a powerful antioxidant, anticarcinogenic and adaptogenic action, reduces stress response and regulates neuroendocrine function and biological rhythms. The authors believe that neoadjuvant autohemotherapy by the "Melaxen" with a powerful immunomodulatory and lipoic effects, improves general non-specific resistance of the organism, has a positive effect on the immediate and remote results of complex treatment of patients. In comparison with polychemotherapy, the autohemotherapy in combination with the “Melaxen" has the advantages. This therapy is significant in several basic methods of treatment of malignant tumors of the localization. It was found that with autohemochemotherapy by the "Melaxen" improves efficiency polychemotherapy, reduces its toxicity, consequently, clinically warranted and expedient in this connection, can be used as subsidiary means complex treatment of ovarian cancer recurrence in the spread process. The drug allows to improve the quality of life, to increase its duration in critically ill patients.

Objective: Patients with non-Hodgkin lymphoma (NHL) occasionally present with multiple primary malignant tumors (MPMTs). This study aimed to determine the clinical characteristics, survival, and risk factors of these patients. Methods: The median follow-up of 92 patients was 13.5 months (range 0.3–72). Overall, 21 patients had synchronous MPMTs and 71 had metachronous MPMTs. We classified patients in the latter group into metachronous first group (n=27) and metachronous second group (n=44). Results: Diffuse large B-cell lymphoma was the most frequent histologic lymphoma type. The digestive system was the commonest site affected by the solid cancer. The 1- and 2-year survival rates were 86.5% and 70.5%, respectively. The overall survival (OS) rates were 67.9% and 36.2% at 2 and 3 years, respectively, in the metachronous first group; 73.8% and 73.8%, respectively, in the metachronous second group; and 68.1% and 56.7%, respectively, in the synchronous tumor group. There was no difference in the survival rate among the 3 groups before 2 years, but after 2 years, a shorter OS rate was observed in the metachronous first group than in the metachronous second group and synchronous tumor group. For all patients, age >60 years, male sex, and ⩾3 involved nodal sites were considered independent prognostic factors associated with survival. Conclusions: OS time was shorter in patients with NHL who developed a second tumor than in those who were diagnosed with solid cancer synchronously and second neoplasm after previous solid tumors. Long-term follow-up and effective treatment should be provided to these patients.

Personalized treatment of genetically stratified subgroups has the potential to improve outcomes in many malignant tumors. This study distills clinically meaningful prognostic/predictive genomic marker for cervical adenocarcinoma using signature genomic aberrations and single-point nonsynonymous mutation-specific droplet digital PCR (ddPCR). Mutations in PIK3CA E542K, E545K, or H1047R were detected in 41.7% of tumors. PIK3CA mutation detected in the patient’s circulating DNA collected before treatment or during follow-up was significantly associated with decreased progression-free survival or overall survival. PIK3CA mutation in the circulating DNA during follow-up after treatment predicted recurrence with 100% sensitivity and 64.29% specificity. It is the first indication of the predictive power of PIK3CA mutations in cervical adenocarcinoma. The work contributes to the development of liquid biopsies for follow up surveillance and a possibility of tailoring management of this particular women’s cancer.

Chondrosarcomas are malignant tumors that rarely grow inside the spinal canal. Prognosis depends on histological features, patient's age and surgical margins free from tumor. Response to radio and chemotherapy is poor. Ideal treatment consists of total " en-block" resection, not always achievable due to limitation of location, compromise of stability and risk of inducing neurological deficits. Two cases of spinal chondrosarcoma causing cord compression are reported, located in the cervical and thoracic spine. Microsurgical technique consisted of initial debulking followed by removal of margins until limits free from tumor were obtained. Total resection was accomplished and neurological function improved in both cases. Follow-up has been seven and one year respectively, with no evidence of recurrence and preserved neurological functions. Association between chondrosarcoma and estrogen-dependent tumor has been confirmed in this report. Although " en-block" resection of a chondrosarcoma should be tried whenever possible, tumor fragmentation should be considered in difficult cases, as in the present report, in which a long period free from recurrence with good quality of life can be obtained.

A 48-year-old woman was admitted to our hospital with a lump in her left breast. She was diagnosed with synchronous papillary thyroid carcinoma and breast ductal carcinoma. The patient underwent four cycles of neoadjuvant chemotherapy with epirubicin and cyclophosphamide, and one cycle of docetaxel. She then underwent left breast mastectomy and radical resection of thyroid cancer (total thyroidectomy and bilateral central group [levels VI and VII] lymph node dissection) at the same time. She was administered three cycles of chemotherapy with docetaxel and radiotherapy. The patient had no metastasis in the follow-up period. A literature search was performed to characterize the epidemiology, etiology, management, and prognosis of this condition. We speculate that hormone treatment could be a probable pathogenesis of synchronous breast and thyroid cancers.

An increasing number of patients with multiple primary cancers are encountered due to improved cancer detection, widespread cancer screening, and better cancer treatment. Here, we report such a patient without a family history of malignancies or a known genetic predisposition developing three primary malignancies of the rectum, breast, and kidney. A 63-year-old female who underwent an anterior resection for rectal cancer was detected to have an elevated carcinoembryonic antigen (CEA) level during a routine follow-up, 8 years after the initial surgery. Clinical examination revealed a left breast lump which was confirmed as invasive ductal carcinoma (pT3 pN0 cM0). Imaging and colonoscopy excluded a local recurrence. However, a right renal lesion suggestive of a renal cell carcinoma was detected (pT1a). She underwent left mastectomy with a sentinel lymph node biopsy and a right partial nephrectomy with a curative intent. Postoperatively, CEA levels declined to normal limits. Management of multiple primary malignancies poses a major challenge. A multidisciplinary approach and tailored decision-making for the individual patient help with the optimum outcome.

In recent years, several disorders to mental health have been observed, such as mental and behavioral disorders, and the use of psychotropic drugs has increased considerably. Thus, the study of the use of psychotropic drugs becomes important because it allows the elucidation of the prevalence of use, as well as, promote the rational use of medications, besides causing in the provision of data that allow the creation of public policies, related to medicines targeted to the population. However, there is a significant shortage of studies that address this issue. Thus, the objective of this study was to investigate the profile of psychotropic medication use of users of the Center for Psychosocial Care (CAPS) of Cuité-PB. This was a transversal, descriptive, exploratory and analytical study of a quantitative nature, from a non-probabilistic population for convenience. Data collection was performed using a semi-structured questionnaire and data were compiled in the Microsoft Excel 2013 programs and the Statistical Package for Social Science (SPSS). The profile of users was predominantly of individuals with 20-29 years of age (34.2%), male (59.2%), low schooling (46%) and family income of 1 to 3 minimum wages (65.8%). In addition, CAPS patients had regular medical follow-up (93.4%) and were accompanied by friends / family during consultations (65.8%). Regarding the diagnosis and treatment, it was observed that most of the studied population suffered from schizophrenia (35%) and the most prescribed drugs were antipsychotic (78.3%), being swallowed mainly with water (84.2%). Thus, these data can help in the organization of the mental health services offered to the community, besides highlighting the importance of CAPS in strengthening comprehensive treatment and for greater adherence and improvement of the quality of life of users.
